Electro-Acupuncture Therapy Alleviates Post-Stroke Insomnia by Regulating Sirt1 and the Nrf2-ARE Pathway

Wang Yiming,Chen Yifei,Yang Jianbo,Sun Wei,Zhang Xiaoning

Post-stroke insomnia (PSI) is a common complication following stroke, which seriously affects patients’ life quality. Electro-acupuncture (EA) is an innovative form of traditional Chinese acupuncture that combines electricity with needles to achieve the prevention and treatment of diseases. However, there is limited understanding regarding the treatment mechanism of EA in PSI. In our study, we aimed to investigate the role of EA on PSI development. Our study findings indicated that the quality of sleep, levels of neurotransmitters 5-hydroxytryptamine (5-HT) and gamma-aminobutyric acid (γ-GABA), and antioxidant levels showed significant improvement following EA treatment in PSI clinical samples and rat models, while the levels of pro-inflammatory factor interleukin 6 (IL-6), tumor necrosis factor-alpha (TNF-α), and astrocyte damage were notably reduced. Furthermore, it was discovered that the levels of sirtuin 1 (Sirt1) were reduced in PSI, a condition that was significantly ameliorated by EA treatment. Additionally, the inhibition of Sirt1 caused a marked elevation in astrocyte apoptosis, inflammatory response, and oxidative stress. Besides, the nuclear factor E2-related factor 2 (Nrf2)-antioxidant response element (ARE) pathway was deactivated in the PSI rat model and Sirt1-silenced cells. However, the suppressive impact was successfully counteracted by EA or estazolam (ES), and the overexpression of Nrf2 partially alleviated the increase in apoptosis, inflammation, and oxidative stress caused by Sirt1 knockdown. Taken together, these findings indicated that EA improved sleep quality and silenced Sirt1-induced apoptosis, inflammation, and oxidative stress in PSI by activating the Nrf2-ARE pathway.

Anti-neuroinflammatory agent rhein lysinate-based self-assembled injectable hydrogel loaded with ZL006 for promoting post-stroke functional recovery

Jiamei Song,Jiaqi Zeng,Xi Chen,Jiayu Wang,Ying Zhang,Yuhao Gao,Ruiqi Wang,Nan Jiang,Yuhui Lin,Rui Li

The therapeutic agent-based self-assembled hydrogel is gaining interest for biomedical applications, because it overcomes the poor biodegradability and low therapeutic agent loading of conventional polymer gelator-based hydrogel. Here, we present rhein lysinate (RHL), a therapeutic agent that self-assembles to form a stable hydrogel through the π-π stacking and hydrogen bonding interactions, while also exerting anti-neuroinflammatory effect. As a small molecular hydrogelator, RHL has significantly improved water solubility and enhanced self-assembly and gelation capabilities compared to the natural anthraquinone rhein. The relaxed gel-forming conditions enhance the practical application potential of self-assembled hydrogel of RHL (RHL gel). The RHL gel can be loaded with the bioactive agents such as 5-Fluorouracil, temozolomide, edaravone, and ZL006, mainly based on efficient stacking between aromatic rings in the bioactive agents and anthraquinone rings in the hydrogel network structure. The pre-gelled RHL gel and ZL006-loaded RHL gel (ZL006-RHL gel) exhibit shear-thinning behavior, flowing like a liquid under high shear stress during injection. Once this shear stress is removal within the body, they rapidly recover to the initial solid-like state. When a single dose of ZL006-RHL gel is administrated to stroke cavity in the subacute phase of stroke, RHL gel matrix effectively reduces post-stroke neuroinflammation, creates a favorable environment for ZL006 to enhance neuroplasticity, and confers a sustained and stable action to ZL006, leading to a long-lasting improvement of motor performance. This study may provide a valuable strategy for therapeutic intervention to promote post-stroke functional recovery, for which there are no clinically available drugs.

A Soft On-Skin Sensor Simultaneously Capturing Bioelectrical and Biomechanical Signals for Muscle Spasticity Detection and Assessment

Zhixin Wang,Yuanpeng Fu,Tianzhao Bu,Yuqi Qiu,Bo Pang,Qingyang Zheng,Sen Zhou,Yangyang Li,Xiling Xiao,Zhouping Yin,Hao Wu

Quantitative and objective assessment of muscle spasticity grades in post-stroke patients plays a pivotal role for physicians and patients in adjusting rehabilitation plans and preventing secondary complications. However, the development of such methods for spasticity detection and assessment is hindered by the subjectivity of conventional assessment methods, the limited clinical applicability of digital-devices, and the structural complexity of sensors used for monitoring muscle strength. Here, a multimodal on-skin sensor is developed, enabling simultaneous acquisition of surface electromyography (sEMG) and triboelectric signals, through wavelet analysis, the signal from the sensor can comprehensively reflect the bioelectrical and biomechanical characteristics of human motion. The high-performance triboelectric materials (BPA NPs) with metal-organic–inorganic core-shell structures are synthesized to enhance the signal quality of the multimodal on-skin sensor by leveraging the self-polymerization ability and weak reducibility of dopamine. The sensitivity of the multimodal on-skin sensor is improved by designing a radially arranged micro-cone array with gradient heights. Additionally, a quantitative spasticity assessment strategy is developed by integrating muscle co-activation coefficients (Index 1) and antagonistic efficacy metrics (Index 2), which are strongly correlated with the Modified Ashworth Scale scores. The multimodal on-skin sensor with the proposed assessment strategy enables quantitative assessment of spasticity in 9 spasticity patients.
