Human immunodeficiency virus 1 (HIV-1) attacks the immune system, making people susceptible to various diseases, thus increasing their risk of death. Comprehensive detection of major HIV-1 strains circulating in China is vital for effective HIV-1 infection prevention and treatment. HIV-1 nucleic acid detection is considered effective for HIV-1 diagnosis since traditional immunological testing may fail to detect HIV-1 infection during the window period. This work demonstrates a one-pot two-stage amplification assay (RT-RAP), a combination of reverse transcription recombinase (RT- RAA), and quantitative real-time polymerase chain reaction (qRT-PCR). The turn-around time of the assay is only 50 min and can be performed with commonly available laboratory equipment, the qPCR devices. The RT-RAP assay could detect approximately 5 and 14 copies/reaction of HIV-1 DNA and RNA using recombinant plasmids and standard reference strains, respectively. Additionally, we found that the clinical performance of RT-RAP (detected 169 samples out of 170 specimens) was consistent with that of qRT-PCR. The sensitivity and specificity of RT-RAP were 100.00% (99/99) and 98.59% (70/71), respectively, while its positive and negative predictive values were 99.00% (99/100) and 100.00% (70/70), respectively. The total coincidence rate of the RT-RAP was 99.41% (169/170), with a kappa value of 0.988 (p< 0.05). We demonstrated that RT-RAP could rapidly detect the common HIV-1 subtypes commonly circulating in China with comparable sensitivity and specificity to qRT-PCR.Keywords:HIV-1;RT-RAA;qRT-PCR;clinical detection

Background Monkeypox virus (MPXV) infection has garnered significant global attention due to its rising incidence and substantial public health implications. A rapid, sensitive, and accurate diagnostic method is urgently required to enable early intervention and effective management of MPXV outbreaks. Results In this study, we developed a novel one-step assay that integrates loop-mediated isothermal amplification (LAMP) with CRISPR/Cas12b in one-pot for the detection of MPXV. The entire detection process did not require opening the lid of the reaction tube and could be completed within 40 min using extracted viral nucleic acids, which is faster than real-time quantitative PCR (qPCR). And the results could be interpreted through either real-time fluorescence or naked-eye visualization. The limit of detection (LOD) of the assay was demonstrated to be 6.5 copies per reaction and no cross-reactivity with other pathogens such as HSV, EBV, CVA16, EV-A71, and MV was found. Furthermore, when evaluated using 113 clinical samples, the assay achieved 100% sensitivity (71/71) and 100% specificity (42/42) compared to the qPCR. Conclusions In resource-limited settings, our method requires only a portable heat block or water bath and a blue light or ultraviolet flashlight for visual detection of MPXV, making it highly accessible. The integration of LAMP and CRISPR/Cas12b provides a robust, user-friendly platform for point-of-care testing, with promising potential for the rapid molecular diagnosis of infectious diseases. The multiplex and simultaneous determination of blood glucose, creatinine and uric acid is essential for the early screening of chronic diseases or regular disease monitoring. Here, we report for the first time a biosensing array for the multiplex and simultaneous determination of plasma glucose, creatinine and uric acid. The sensing electrodes are fabricated on a PET surface, including three working electrodes, one reference electrode, and one counter electrode. Each specific enzyme is immobilized on its corresponding working electrode. The biosensing array can exhibit high sensitivity and selectivity for the simultaneous determination of blood glucose, creatinine and uric acid in real blood samples, and the measurement results are accurate and consistent with those from the clinical biochemistry analyzer in the hospital. It is expected that this work could provide new avenues for the fundamental study of biosensing device construction, as well as practical applications of the detection of biomarkers in chronic diseases.

An ESI-MS/MS method through direct infusion was validated for quantitative analysis of meropenem powder for injection. The validation parameters were established in a rapid analysis of 30 s. Drug stability was studied through the submission to stress testing, resulting on four degradation products. Under hydrolytic conditions, in acid, neutral and alkaline media, the major degradation product was formed through the cleavage of the β-lactam ring. Oxidation of the drug using H 2 O 2 (3%) showed the formation of two degradation products from a decarboxylation reaction and N-oxide formation. Under high humidity conditions, there was detected a dimer product. The stability of meropenem after reconstitution was studied in conditions that simulate its clinical use. In samples reconstituted and diluted in infusion fluids, an extensive degradation was observed. At room temperature meropenem maintained its content > 90% for up to 4 h when prepared in 5% glucose and for up to 12 h when prepared in 0.9% NaCl. Through ESI-MS/MS analyzes it was observed a degradation product formed by β-lactam ring cleavage, detected in all conditions studied. It was also identified a degradation product formed only in 5% glucose, generated by the hydrolysis of β-lactam followed by the attachment of a glucose molecule to the nitrogen of the pyrrolidine ring. In general, all the results obtained in the stability studies contribute to the knowledge about this antibiotic and future candidates of this class.

Designing and developing a simple and reliable method for detecting disease biomarkers is crucial for early disease prevention and diagnosis. Fe-functionalized carbon quantum dot (Fe-CQD), synthesized from natural dandelion biomass through a controlled process, were developed as nanozymes with outstanding peroxidase-like activity. Uricase decomposes uric acid to release hydrogen peroxide (H 2 O 2 ), which the Fe-CQD nanozymes catalyze to produce reactive oxygen species (ROS) such as singlet oxygen ( 1 O 2 ) and hydroxyl radicals (•OH). These ROS then oxidize the colorless substrate o -phenylenediamine (OPD) into yellow 2,3-diaminophenazine (DAP), enabling visual detection of uric acid. Building on this principle, we have developed, for the first time, a visual sensing platform using Fe-CQD nanozymes. This platform integrates absorption spectra, a microplate reader, and smartphone-assisted visible-light imaging to detect uric acid, achieving detection limits as low as 0.31, 0.36, and 0.83 μM, respectively. The platform demonstrated excellent accuracy in clinical blood sample analysis, yielding results highly consistent with standard hospital methods and the UA Assay Kit-based colorimetric method. It also exhibited robust anti-interference capabilities and practicality, making it a reliable tool for UA detection in real-world clinical settings. This innovative platform highlights the potential of nanozymes in biomarker detection and lays a theoretical foundation for integrating intelligent, portable sensing technologies into clinical diagnostics.

Luminescent copper nanoclusters (CuNCs) constitute a very active research topic due to their unique properties and lower cost than gold and silver NCs. In this study, we report a new, facile, and rapid top-down etching method for synthesizing luminescent CuNCs, using Cu nanoparticles (CuNPs) as the precursor and ammonia (NH3) as the etchant. The etching mechanism is systematically investigated and the optical and structural properties of the obtained CuNCs are carefully studied. The NH3-triggered etching process is very fast and the newly generated CuNCs can emit strong green fluorescence with a high quantum yield. Moreover, by coupling the urease-catalyzed hydrolysis of urea with the NH3-induced etching of CuNPs, we developed a novel fluorescence turn-on assay for urea. The linear range for urea detection is from 0.25 to 5 mM, and the limit of detection is 0.01 mM. This novel sensing approach, with good sensitivity and excellent selectivity, is then successfully utilized to detect urea in human serum samples, demonstrating its great potential in clinical diagnosis. In addition, the proposed coupling method can be extended to monitor other analytes that influence the size-focusing etching process, allowing metal NCs to be used to construct diverse chemosensors and biosensors.

Anlotinib is a small molecule of novel tyrosine kinase inhibitor initially approved to treat non-small cell lung cancer in China. Drug–drug interaction (DDI) is an extrinsic factor important for the appropriate use of anlotinib in clinical practice. In vitro experiments demonstrated that anlotinib is a substrate of cytochrome P450 (CYP) enzymes and moderate inhibitor of several common ones; however, no clinical DDI studies have been performed to investigate inhibitory effects of anlotinib on these CYP enzymes. Thus, its drug label recommends avoiding co-administration with substrates of these enzymes, which have narrow therapeutic windows. In this study, we performed a CYP450 inhibition study, followed by gathering in vitro and clinical pharmacokinetic data to build the first physiologically based pharmacokinetic (PBPK) model of anlotinib. The verified model was subsequently used to predict the DDI mediated by anlotinib. As a result, the marginal plasma exposure changes of typical CYP3A and CYP2C9 substrates were less than the bioequivalence threshold, indicating that anlotinib has a very low potential of causing clinically meaningful DDI through the inhibition of several major CYP enzymes. According to the FDA's latest guideline on DDI, the established model with the simulation results may support the revision of anlotinib labelling without further clinical studies, lifting unnecessary restrictions on anlotinib regimens.

As the primary fission product in spent nuclear fuel being generally filed into the high-level liquid waste, the 90 Sr recovered from the reprocessed nuclear fuel could be a significant source for producing 90 Y nuclide (the decay daughter). It presents as one favorable therapeutic radionuclide candidate for radiolabeling of pharmaceuticals in widespread clinical application. Under this situation, a novel silica-based adsorbent (HDEHP/SiO 2 -P) was developed for selective separation of Y 3+ from concentrated Sr 2+ in aqueous solution. Batch adsorption process was investigated under different parameters of acid type and strength, contact time, temperature, ionic concentration and molar ratio etc. It was confirmed that, the HDEHP/SiO 2 -P had highly selective properties toward Y 3+ over Sr 2+ in different acidic medium (i.e., hydrochloric, nitric and acetic acid), with maximum adsorption capacity 34.05 mg Y 3+ /g. Sorption isotherms and uptake kinetics, were better fitting for Langmuir monomolecular layer adsorption mode and the pseudo-second order (PSORE) kinetic model respectively. The sorbent shows high SF Y/Sr of 1.93 × 10 3 even at a Sr-Y molar ratio of 2 × 10 3 . Moreover, dynamic separation of trace amount of Y 3+ from the concentrated Sr 2+ aqueous solution was successfully validated by the column test, in which 100 % Y 3+ was recovered. The produced eluate is characterized by a sharp elution curve without Sr trailing under high acidic conditions of 6 M HCl. All these results would promisingly promote the application of the developed material and chromatographic approach to facilely obtain the adequate quantities of 90 Y with required radiochemical purity for its widespread clinical use.

Background: Homocysteine (Hcy) has been found to be closely related to the occurrence of diabetes mellitus (DM) and is considered as one of the risk factors of DM. However, Hcy alone is not enough as a factor to predict DM, and our study analyzed and determined the relationship between the main metabolites involved in the Hcy metabolic pathway and DM. Methods: A total of 48 clinical samples were collected, including 18 health control samples and 30 DM samples. All standards and samples were detected by LC-QTOF-MS. Multivariate statistical analysis and k-means cluster analysis were performed to screen and confirm the metabolites significantly correlated with DM. Results: A total of 13 metabolites of the Hcy metabolic pathway were detected in the samples. The content of Hcy, cysteine, taurine, pyridoxamine, methionine, and choline were significantly increased in the DM group (p< 0.05). Hcy, choline, cystathionine, methionine, and taurine contributed significantly to the probabilistic principal component analysis (PPCA) model. The odds ratios (OR) of Hcy, cysteine, taurine, methionine, and choline were all greater than one. K-means cluster analysis showed that the Hcy, taurine, methionine, and choline were significantly correlated with the distribution of glucose values (divided into four levels: 10.5–11.7 mmol/L, 7.7–9.7 mmol/L, 6.0–6.9 mmol/L, and 5.0–5.9 mmol/L, respectively). Conclusion: Hcy, taurine, methionine, and choline can be used as risk factors for diabetes diagnosis and are expected to be used for the assessment of diabetes severity. Keywords:homocysteine metabolism;diabetes;LC-MS;clinical diagnosis

Kynurenine (KYN), a metabolite of tryptophan, is currently receiving increasing attention as one of the biomarkers for clinical disease monitoring. To realize the determination of KYN in human plasma samples in the clinic, a specific monoclonal antibody (mAb) against KYN was prepared based on the structural characteristics of KYN. By optimizing the detection conditions, we obtained anti-KYN mAb 1G10 with a sensitivity of 143.66 ng/mL, and established a colloidal gold immunochromatographic assay (CGCA) platform for the qualitative and quantitative analysis of KYN in human plasma samples, using CGCA strips to meet the needs of clinical testing. The recoveries of the strips for the determination of KYN in plasma samples were in the range of 85.22%–99.72%, which were validated by liquid chromatography–tandem mass spectrometry (LC-MS/MS), and results were in good agreement with the LC-MS/MS method, indicating that the accuracy of the prepared CGCA strips was high. Based on the characteristics of low cost, high throughput and good specificity, the test strips will be widely applicable for clinical determination.

Formalin-fixed paraffin-embedded (FFPE) samples represent a vast and valuable resource of patient material, often linked to extensive clinical history and follow-up data. However, achieving single-cell or single-nucleus RNA (sc/snRNA) profiling in these archived tissues remains challenging. To address this, we have developed snRandom-seq, a droplet- and random primer–based single-nucleus total RNA sequencing technology specifically designed for FFPE tissues. This method captures total RNAs by using random primers and demonstrates a low doublet rate (0.3%), increased RNA coverage and enhanced detection of non-coding and nascent RNAs compared to state-of-the-art high-throughput sc/snRNA-seq technologies. This protocol provides a comprehensive guide to isolating single nuclei from FFPE samples; performing in situ DNA blocking, reverse transcription and dA tailing reactions; barcoding single-nucleus droplets; and preparing sequencing libraries. The entire snRandom-seq process can be completed in 4 d. This platform serves as a powerful tool for snRNA-seq of clinical specimens, with broad applications in studying complex biological systems.
