Type 1 diabetes mellitus (T1DM) are characterized by blood glucose elevation with pancreatic β cells deficiency. As a safe alternative to frequent subcutaneous insulin injection, pancreatic β cell transplantation provides a promising therapeutic option for blood glucose control in T1DM. However, pancreatic β cell transplantation faces intractable challenges of the poor viability and severe host immune rejection. Therefore, a novel approach capable of improving the poor oxygen/nutrients supply and severe host immune rejection is highly desired. Herein, a novel biohybrid artificial pancreas, presenting glucose-dependent insulin release behavior, is constructed via pancreatic β cells encapsulating in a hydrogel scaffold. The hydrogel scaffold is made of the commixture of sodium alginate (SA), chitosan (CS) and sulfated bacterial cellulose (SBC). The biocompatible three-dimensional (3D) hydrogels protected pancreatic β cells from immune response but also allowed the exchange of nutrients and insulin. As a result of the synergistic effect, the biohybrid artificial pancreas can reverse the hyperglycemia and achieve sustained glycemic control for at least 30 days in diabetic mice. Collectively, we consider that this biohybrid artificial pancreas with an elaborate structure could provide an effective option for the treatment of type 1 diabetes.

Core-shell multicompartmental microparticles have gained increasing attention as their architecture resembles the multicompartmental composition of eukaryotic cells. However, challenges remain in the fabrication of biocompatible multicompartmental microparticles at a sufficiently high scale. Herein, all-aqueous ‘mangosteen-like’ core–shell multicompartmental microparticles were created by an oil-free gas-shearing fabrication approach. Both the shell thickness and core size of thus obtained mangosteen-like microparticles can be easily finetuned. Mangosteen-like particles have well segregated (up to eight) compartments which are attractive to precisely control enzymatic cascade reactions. To illustrate the application potential, insulin releasing mangosteen-like microparticles (‘artificial pancreatic beta cells’) were developed.

In order to improve the living standards of diabetes patients and reduce the negative health effects of this disease, the medical community has been actively searching for more effective treatments. In recent years, an artificial pancreas has emerged as an important approach to managing diabetes. Despite these recent advances, meeting the requirements for miniaturized size, accurate sensing and large-volume pumping capability remains a great challenge. Here, we present a novel miniaturized artificial pancreas based on a long microtube sensor integrated with an ultrasonic pump. Our device meets the requirements of achieving both accurate sensing and high pumping capacity. The artificial pancreas is constructed based on a long microtube that is low cost, painless and simple to operate, where the exterior of the microtube is fabricated as a glucose sensor for detecting diabetes and the interior of the microtube is used as a channel for delivering insulin through an ultrasonic pump. This work successfully achieved closed-loop control of blood glucose and treatment of diabetes in rats. It is expected that this work can open up new methodologies for the development of microsystems, and advance the management approach for diabetes patients.

Background: Precise and dynamic blood glucose regulation is paramount for both diagnosing and managing diabetes. Continuous glucose monitoring (CGM) coupled with insulin pumps forms an artificial pancreas, enabling closed-loop control of blood glucose levels. Indeed, this integration necessitates advanced micro-nano fabrication techniques to miniaturize and combine sensing and delivery modules on a single electrode. While microneedle technology can mitigate discomfort, concerns remain regarding infection risk and potential sensitivity limitations due to their short needle length. Methods: This study presents the development of an integrated electronic/fluidic microneedle patch (IEFMN) designed for both glucose sensing and insulin delivery. The use of minimally invasive microneedles mitigates nerve contact and reduces infection risks. The incorporation of wired enzymes addresses the issue of "oxygen deprivation" during glucose detection by decreasing the reliance on oxygen. The glucose-sensing electrodes employ wired enzyme functionalization to achieve lower operating voltages and enhanced resilience to sensor interference. The hollow microneedles' inner channel facilitates precise drug delivery for blood glucose regulation. Results: Our IEFMN-based system demonstrated high sensitivity, selectivity, and a wide response range in glucose detection at relatively low voltages. This effectively reduced interference from both external and internal active substances. The microneedle array ensured painless and minimally invasive skin penetration, while wired enzyme functionalization not only lowered sensing potential but also improved glucose detection accuracy. In vivo, experiments conducted in rats showed that the device could track subcutaneous glucose fluctuations in real-time and deliver insulin to regulate blood glucose levels. Conclusions: Our work suggests that the IEFMN-based system, developed for glucose sensing and insulin delivery, exhibits good performance during in vivo glucose detection and drug delivery. It holds the potential to contribute to real-time, intelligent, and controllable diabetes management.

The closed-loop delivery of insulin in response to change of the blood glucose level and long-term supply of insulin are both important for diabetes patients. However, combination of these two goals in a chemically controlled implantable system is still challenging yet highly desirable. The purpose of the present study is to design a synthetic artificial pancreas by integration of chitosan hydrogels with insulin-loaded glucose-responsive microspheres to deliver insulin in a close-looped and long-term way for diabetes care. Glucose-responsive insulin-loaded microspheres were firstly fabricated via a high-speed shear-emulsion based crosslinking method and then embedded into chitosan hydrogels to make a scaffold-based synthetic artificial pancreas. In vitro experiments indicated the scaffold exhibited a longer insulin supply as well as a lower burst release compared with free microspheres, and could keep the glucose-responsive insulin release property inherited from the corresponding microspheres even after 12 day-release. The released insulin was proved to remain active, and the culture of HDF cells on the scaffold showed good cell proliferation during 7 days incubation. These results suggested the scaffold-based synthetic artificial pancreas have great promise in the application of insulin delivery.

Transplantation of microencapsulated islet cells remains a promising strategy for the normalization of glucose metabolism control in type 1 diabetes mellitus. However, vigorous host immunologic rejection, fibrotic overgrowth around the microcapsules and poor oxygen supply often lead to graft failure. Herein, a bioartificial pancreas is constructed that incorporates the “stealth effect” based on polyethylene glycol copolymers and the high oxygen-carrying performance of fluorinated nanoparticles. Polycationic poly(L-lysine)-grafted-poly(ethylene glycol) is successfully coated on the surface of alginate microcapsules through electrostatic interaction, which can not only resist fibrinogen adhesion and avoid excessive fibrosis around the microcapsules, but also isolate the host immune system from attacking, achieving a “stealth effect” of microencapsulated islet cells. Furthermore, the co-loading of fluoride-based O 2 nanocarriers gives them enhanced oxygen-carrying and continuous oxygen supply capabilities, thereby effectively prolonging the survival of islet cells. The intracapsular islet cells still display similar cell viability and almost normal insulin secretion function even in long-term culture under hypoxic conditions. Collectively, this work opens up a new approach for microencapsulated islets to efficiently evade host immune attack and improve oxygen supply, and provides a promising strategy for islet transplantation in type 1 diabetes mellitus. Islets transplantation is a promising method to treat type 1 diabetes. Unfortunately, current islet encapsulation and transplantation strategies are difficult to simultaneously achieve the minimally invasive transplantation, rapid mass transportation, accurate positioning, long-term stability and easy retrieval. Herein, we present a novel strategy for islet encapsulation and transplantation that integrates the advantages of microencapsulation and macroencapsulation. Specifically, islets are first encapsulated in methacrylated carboxymethyl chitosan (CMCSMA) microgels by droplet microfluidic technology, and then oxidized sodium alginate-succinimide ester (OSA-NHS) as the assembling agent is added to time-sequentially assemble the islets-laden microgels. The as-formed microgel assembly (MA-CHO/NHS), or namely bioartificial pancreas, is injectable before transplantation due to the quick formation of dynamic Schiff base bonds, and becomes quite stable after transplantation with the slow formation of stable amide bonds. In addition, the interconnected micropores inside the microgel assembly is beneficial for mass transportation, and the abundant aldehyde and succinimide ester groups on assembling agent ensures good tissue adhesion. This injectable, tissue-adhesive, microporous, positionable and retrievable bioartificial pancreas can reverse the blood glucose levels of diabetic mice from hyperglycemia to normoglycemia for at least 90 days, and thus shows a broad application prospect in the field of diabetes treatment.

Islets transplantation is a promising treatment for type 1 diabetes mellitus. However, severe host immune rejection and poor oxygen/nutrients supply due to the lack of surrounding capillary network often lead to transplantation failure. Herein, a novel bioartificial pancreas is constructed via islets microencapsulation in core-shell microgels and macroencapsulation in a hydrogel scaffold prevascularized in vivo. Specifically, a hydrogel scaffold containing methacrylated gelatin (GelMA), methacrylated heparin (HepMA) and vascular endothelial growth factor (VEGF) is fabricated, which can delivery VEGF in a sustained style and thus induce subcutaneous angiogenesis. In addition, islets-laden core-shell microgels using methacrylated hyaluronic acid (HAMA) as microgel core and poly(ethylene glycol) diacrylate (PEGDA)/carboxybetaine methacrylate (CBMA) as shell layer are prepared, which provide a favorable microenvironment for islets and simultaneously the inhibition of host immune rejection via anti-adhesion of proteins and immunocytes. As a result of the synergistic effect between anti-adhesive core-shell microgels and prevascularized hydrogel scaffold, the bioartificial pancreas can reverse the blood glucose levels of diabetic mice from hyperglycemia to normoglycemia for at least 90 days. We believe this bioartificial pancreas and relevant fabrication method provide a new strategy to treat type 1 diabetes, and also has broad potential applications in other cell therapies.
