Compatibility and aerosol characteristics of beclomethasone mixed with N-acetylcysteine.

Xiufeng Wu,Zhaoliu Cao,Jiang Ni,Xiaoyuan Zheng,Lizhen Zhu,Xin wang,Juan Lv,Shijun Zhou,Yongjuan Ding,Ruirong Wu

Mixing different kind inhalation medications for simultaneous inhalation is widely used in the treatment of chronic respiratory diseases, and it can minimize the administration time and improve patient adherence. To our knowledge, it is unclear whether beclomethasone (BDP, Clenil®) can bemixed with acetylcysteine (NAC, Fluimucil®), because the in vitro physico-chemical compatibility and aerosol characteristics of the mixture are unknown. In this study, we investigated physical compatibility, including the appearance, pH, osmotic pressure and chemical stability, as well as aerosol characteristics, including particle size corresponding to 10%/50%/90% of the cumulative percentage of total particle volume ( X 10/ X 50/ X 90), volume median droplet diameter (VMD), mass median aerodynamic diameter (MMAD), fine particle fraction (FPF), fine particle dose (FPD) and geometric standard deviation (GSD), delivery rate, and total delivery of the above solutions. After mixing, there were no significant changes in visual appearance, pH, osmolality and drug content of the mixtures at room temperature for 12 h. The FDP of BDP in the mixture decreased by 16.49%, whereas the NAC increased by 10.85%. The delivery rates of BDP and NAC in the mixture decreased by 66.05% and 45.54%, and total delivery increased by 13.20% and 25.29%, respectively. However, the MMAD, FPF, particle size and GSD of the mixture were almost unchanged. We demonstrated that these admixtures are physico-chemically compatible but that coadministration of beclomethasone with acetylcysteine can markedly affect output and aerosol characteristics.

Aerosol Characteristics and Physico-Chemical Compatibility of Combivent® (Containing Salbutamol and Ipratropium Bromide) Mixed with Three Other Inhalants: Budesonide, Beclomethasone or N-Acetylcysteine

Rui Zhang,Junhua Hu,Liangjun Deng,Sha Li,Xi Chen,Fei Liu,Shanping Wang,Khaja Shameem Mohammed Abdul,Huimin Beng,Wen Tan

Inhalation therapy with a nebulizer is widely used in chronic respiratory disease. Mixing inhalation solutions/suspensions for simultaneous inhalation is more convenient and might simplify the administration procedure. However, there are no data available to address the in vitro aerosol characteristics and physico-chemical compatibility of Combivent®(containing Salbutamol and Ipratropium bromide) with other inhalation solutions/suspensions. In order to investigate the in vitro aerosol characteristics and physico-chemical compatibility of Combivent®with Budesonide, Beclomethasone, andN-acetylcysteine, the appearance, pH, osmotic pressure, chemical stability, mass median aerodynamic diameter (MMAD), fine particles fraction (FPF), particle size corresponding to X50 (particle size, which accounts for 50% of the total cumulative percentage of volume of all particles), delivery rate, and total delivery of the mixed inhalation solution/suspension were tested. There was no change in the appearance such as a change in color or precipitation formation at room temperature. The pH, osmolality, and chemicals of the mixtures were stable for 24 h after mixing. There were no significant differences between Combivent®, Budesonide, Beclomethasone,N-acetylcysteine, and the mixtures in MMAD, FPF, X50, the delivery rate, and the total delivery. This indicates that the mixtures were physically and chemically compatible. The mixing did not influence the particle size, distribution, or delivery compatibility of the mixtures.Keywords:inhalation;nebulizer;physico-chemical;particle size and distribution;delivery compatibility;Combivent®;Budesonide;Beclomethasone;N-acetylcysteine

HPLC-UV/VIS for Determination of Ipratropium Bromide Mixed with Salbuterol, Beclomethasone Propionate and Budesonide using Dual Wavelength-Detection Method

Guo Haihua,Zhang Rui,Deng Liangjun,Li Meng,Li Sha,Zhao Suqing

Background Inhalation preparation involves liquid or solid raw materials for delivering to lungs as aerosol or vapor. The liquid preparation for nebulizer is effective for convenient use and patient compliance and it has been extensively used in the treatment of clinical lung diseases. Clinical staff often mixes the compound ipratropium bromide with beclomethasone propionate and budesonide inhaler but reference values of inhalants for clinical use need to be established for simplifying the operation procedure. The high-performance liquid chromatography (HPLC) method of compound ipratropium bromide solution, beclomethasone propionate suspension and budesonide suspension after mixed atomization was studied. Methods The specificity, linearity, recovery (accuracy), precision and stability of compound ipratropium bromide, beclomethasone propionate and budesonide were tested to verify the developed liquid phase method. Results The developed liquid phase method had high specificity, linear R2≥0,999, recovery (accuracy) RSD (relative standard deviation) less than 2%, precision RSD less than 2,0%, and stability RSD less than 2,0%. Conclusion The liquid phase methodology developed in this study can be used for the determination of compound ipratropium bromide mixed with beclomethasone propionate and budesonide. The current methodology can also be used to provide a reference for the determination of its content after mixing, and further data support for its clinical medication.

Preparation of high-specificity monoclonal antibody based on rational hapten design and development of immunoassay for accurate detection of dexamethasone in milk and animal tissues

Hongfei Yang,Liangni Qin,Jiaxu Xiao,Xiaoming Pan,Shiyun Han,Linwei Zhang,Jixiang Liang,Yu Si,Hao Wen,Long Li,Xingyu Wang,Xiaojuan Zhang,Dapeng Peng

Dexamethasone, a long-acting glucocorticoid, exhibits considerable structural similarity to other glucocorticoids. To avoid false-positive results in immunoassays due to cross-reactivity of antibodies with structural analogues, the preparation of highly specific antibodies is crucial. In this study, a novel hapten DEX-GA was designed and synthesized, and the monoclonal antibody (mAb) 3D1 was successfully prepared based on the hapten. The mAb 3D1 has the 50 % inhibitory concentration of 0.38 ng/mL for dexamethasone, with cross-reactivity below 7.45 % against twelve glucocorticoids including betamethasone, prednisone, triamcinolone and beclomethasone. Based on this antibody, indirect competitive enzyme-linked immunosorbent assay (ic-ELISA) and colloidal gold immunochromatography assay (GICA) were developed for accurate detection of dexamethasone in milk and animal tissues. The limits of detection of ic-ELISA in milk and animal tissues ranged from 0.132 to 0.215 μg/kg, which was a 1-3-fold increase in sensitivity compared with the previously reported ic-ELISA method. The GICA method established in this study had a visual limit of detection of 0.3 μg/L in milk. Notably, this constitutes the first application of GICA in animal tissues, demonstrating a visual detection limit of 0.2 μg/kg. The accuracy and reliability of both ic-ELISA and GICA were validated using liquid chromatography-tandem mass spectrometry.
