Novel triphosphorylation polyurethane nanoparticles for blood-contacting biomaterials' coating

Jun Zhang,Zhijun Sun,Haomiao Zhu,Qiaochu Guo,Chengai He,Ao Xia,Hong Mo,Xiaohua Huang,Jian Shen

Improving hemocompatibility of biomaterials and devices contacting the human blood has been the subject of intensive research. In this study, we synthesized a novel excellent blood compatible polyurethane/sodium triphosphate nanoparticle (PU/STPP). Characterization of polyurethane/sodium triphosphate (PU/STPP) nanoparticles was carried out by transmission electron microscopy (TEM), scanning electron microscopy (SEM), dynamic light scattering (DLS), nuclear magnetic resonance (NMR), and energy dispersive spectroscopy (EDS). Blood compatibility assessment of PU/STPP nanoparticles was performed by in vitro coagulation time, plasma clotting time, hemolysis rate, and red blood cell morphology tests. Cell compatibility evaluations of PU/STPP nanoparticles were obtained by MTT cell viability tests. The PU/STPP nanoparticles also were used to modify vascular prostheses with cosedimentation. Platelet adhesion tests showed that blood compatibility of vascular prostheses coated with PU/STPP nanoparticles is better than that of pure vascular prostheses.

Self-Lubricating and Self-Healing Polyurethane Elastomer as a Meniscal Prosthesis to Delay Osteoarthritis Progression

Yi Zhou,Xia Jiang,Xiaolong Yang,Huan Liang,Xing Xie,Weili Fu

The absence of a meniscal prosthesis that fully accommodates the complex motion demands of the human knee joint remains a challenge in clinical practice. This study synthesized a polyurethane elastomer with enhanced mechanical properties, featuring a dynamic dimethyldioxime-urethane structure that provides self-healing capabilities. Additionally, adamantane-grafted hyaluronic acid has been integrated into polyurethane containing cyclodextrin, imparting self-lubricating properties through supramolecular host-guest interactions between adamantane and cyclodextrin. This innovative polyurethane elastomer is utilized to fabricate a meniscal prosthesis (DPU@HA). In vitro experiments demonstrated DPU@HA's exceptional cytocompatibility with chondrocytes. In vivo studies revealed that DPU@HA significantly protected articular cartilage following meniscus removal by reducing the high mechanical stress within the cartilage, thereby inhibiting Piezo1 overactivation and subsequent chondrocyte apoptosis. With its self-healing and self-lubricating properties, the meniscal prosthesis (DPU@HA) holds promise for clinical applications in preventing the progression of osteoarthritis after meniscectomy.

Electrospun borneol/PVP antibacterial coating for postoperative management of keratoprosthesis implantation

Yan Luo,Tengxiang Wang,Ke Yang,Taixiang Liu,Xue Bai

Keratoprosthesis (K-Pro) implantation and postoperative recovery often lead to severe ocular infections called keratoprosthesis-associated infections (KPAI). Antibiotics are commonly used; however, prolonged antibiotic use can lead to bacterial resistance and disrupt microbial balance in the eye. Borneol provides possibilities for developing antibiotic-independent antimicrobial K-Pro. Therefore, the development of an antibiotic-independent self-antimicrobial K-Pro is crucial for postoperative management. Herein, electrospinning technology was employed to fabricate a soluble borneol-based antibacterial coating on K-Pro surfaces for postoperative management of keratoprosthesis implantation. The modified K-Pro exhibited a distinctive nanofibrous morphology (100% borneol release ratio within 125 min), surface antibacterial properties (Escherichia coli: 2%, Staphylococcus aureus: 6%), biocompatibility, and negligible cytotoxicity towards epithelial cells. This study aimed to explore the potential application of camphor-based electrospun coatings in the field of K-Pro infection resistance, thereby providing novel strategies and material foundations for the effective postoperative management of KPAI.

The Topical Tranexamic Acid Have Potential Hazard of Promoting Biofilm Formation of Staphylococcus aureus in Microenvironment of the Prosthetic Joint

Feiyang Zhang,Wenjun Dong,Fengyan Wang,Jinlong Yu,Feng Jiang,Jin Tang,Yan Qian,Hao Shen

Background. Perioperative topical tranexamic acid as antifibrinolytic agent is often used for total joint replacement to reduce bleeding currently. Staphylococcus aureus was the most common isolates from perioperative infection of prosthetic joint. The influence of topical application with tranexamic acid on the incidence of acute prosthetic joint infection of Staphylococcus aureus has not been clarified. Methods. Mouse model of Staphylococcus aureus knee prosthesis infection was constructed. Tranexamic acid was intra-articular injected during the perioperative period. CFU counting from tissue and implant sample was evaluated 3 days and 7 days after inoculating of Staphylococcus aureus. Bacterial growth curve, biofilm formation, aggregation, and plasmin inhibition of Staphylococcus aureus were tested with tranexamic acid added to the synovial culture medium. Results. There were no significant differences of CFU counting from tissue and implant samples in knee prosthesis infection after a single local injection of tranexamic acid at the postoperative 3 or 7 days. The amount of bacterial colonization on the surface of implant increased after 3 days’ continuous local injection of tranexamic acid. Tranexamic acid has no effect on bacterial growth at the concentration (10 mg/ml) of clinical application, but it can inhibit bacterial aggregation and mildly inhibit biofilm formation. Plasmin can significantly inhibit biofilm formation which can be revised by adding tranexamic acid. Conclusion. Although continuous local injection of tranexamic acid can promote the biofilm formation of Staphylococcus aureus on the surface of articular implant, it has clinical safety for using one single local injection of tranexamic acid during the perioperative period.

Diagnosis of prosthetic joint infection based on Cas12a-catalytic aggregation mediated by supramolecular chemistry

Yining Zhao,Ziye Liu,Ziting Liu,Juan Zhang,Jinghui Peng,Genxi Li

Prosthetic joint infection (PJI), which is still difficult to be diagnosing, is caused by bacterial infection. Rapid detection for trace amounts of bacteria should be a solution; however, the existing assays cannot serve for PJI diagnosis due to the inherent limitations. Herein, a strategy based on supramolecular chemistry mediated Cas12a catalytic aggregation (CCA) is explored in this work for ultrasensitive detection of universal bacteria. In the design, supramolecular chemistry is introduced to impede the cis-cleavage of activated Cas12a, resulting in its aggregation in the surrounding of bacteria with the linkage of CCA probe. Since Cas12a aggregation owns excellently enhanced catalytic efficiency, the CCA-based strategy can be used for bacterial analysis with a limit of detection as low as 1 CFU/mL. With high sensitivity, reproducibility, generalizability, and stability, the CCA-based strategy can be applied for the detection of clinical effusion samples. The remarkable performance of CCA-based strategy has also demonstrated its robust potential to be a simple, rapid, and highly sensitive bacteria diagnostic tool, paving the way for prompt diagnosis and management of PJI.

Melittin Treats Periprosthetic Osteolysis in a Rat Model by Inhibiting the NF-kB Pathway and Regulating the Ratio of Receptor Activator of Nuclear Factor Kappa B Ligand/Osteoprotegerin

Junqi Niu,Fanggang Bi,Qing Tian,Ke Tian

Background Aseptic loosening around the prosthesis is a common cause of failure in total joint arthroplasty. Polyethylene wear particles trigger the release of inflammatory factors by macrophages. Key mediators involved in osteoclastogenesis include interleukin-6, tumor necrosis factor-α, receptor activator of nuclear factor kappa B (RANK), RANK ligand (RANKL), and bone protection hormone (Osteoprotegerin [OPG]). The purpose of our experiment was to see whether melittin can slow down the release of inflammatory mediators through the NF-kB pathway, regulate the RANKL/OPG ratio, reduce osteoclast formation, and delay the onset of arthritis in rats. Methods A total of 20 male Sprague-Dawley rats (10 months, Specific Pathogen Free, 350 g ± 20 g) were randomly divided into 5 groups: sham group, model group, melittin concentration 1 group (0.2 mg/kg), concentration 2 group (0.4 mg/kg), and concentration 3 group (0.6 mg/kg). All rats were implanted with TA2 high-purity titanium rods. A drill was used to create a bone canal along the long axis of the femur in the intercondylar notch. The model group and experimental groups were exposed to polyethylene particles, while the sham group did not receive any particles. Results The melittin group exhibited significantly increased serum levels of serum P, calcium-phosphorus product, OPG, PINP, PINP/CTX-I, and OPG/RANKKL ( P < .05). In the experimental group, micro computed tomography scanning results revealed a decrease in the amount of bone defect around the prosthesis. Immunofluorescence analysis demonstrated a decrease in the expression of IKKα and P65, while the expression of OPG showed an upward trend. Both Hematoxylin-Eosin and Tartrate-Resistant Acid Phosphatase staining revealed less osteoclast and inflammatory cell infiltration in bone resorption pits. Conclusions Our study demonstrates that melittin has the ability to inhibit the NF-kB pathway in a rat model, and reduce the impact of RANKL/OPG, thereby delaying osteoclast activity and alleviating periprosthetic osteolysis.

Fused exosomal targeted therapy in periprosthetic osteolysis through regulation of bone metabolic homeostasis

Tianliang Ma,Qimeng Liu,Zheyu Zhang,Jiangyu Nan,Guanzhi Liu,Yute Yang,Yihe Hu,Jie Xie

The onset of periprosthetic osteolysis is mediated by wear particles following artificial arthroplasty. This manifests as a disturbed bone metabolism microenvironment, characterized by insufficient osteogenesis and angiogenesis, and enhanced osteoclastic activity. To target and remodel the homeostatic environment of bone metabolism in the sterile region around the prosthesis, we successfully pioneered the proposal and construction of a fused exosome (f-exo) system with M2 macrophage-derived exosomes (M2-exo) and urine-derived stem cell exosomes (USC-exo). The results demonstrate that f-exo effectively combines the osteolysis region-targeting capabilities of M2-exo with the bone metabolic homeostasis modulation effects of two exosomes (M2-exo and USC-exo), thereby achieving a significantly enhanced bone metabolic homeostasis targeting effect in the periprosthetic osteolysis region. The proteomic analysis of M2-exo, USC-exo, and f-exo revealed the potential mechanism of f-exo in targeting-regulation of bone metabolic homeostasis. Our study employs an innovative approach utilizing the fused exosome system for exosome targeted delivery, which offers a novel intervention strategy for the clinical management of periprosthetic osteolysis. Furthermore, it provides a novel conceptual framework for the development of exosome-based drug-targeting delivery systems.

Rapid differential diagnosis of periprosthetic joint infection by microdroplet reaction on portable biochip and indicator

Zheng Liu,Jimei Chi,Zewei Lian,Yaqi Yang,Lijun Cheng,Xiaoxue Lin,Xuwei He,Xiaojian Ji,Chenxuan Yang,Yunhao Tang,Minzhi Yang,Zhisen Gao,Wang Gu,Haoming An,Qingda Wei,Yanlin Song,Meng Su,Rui Li,Wei Chai

Periprosthetic joint infection (PJI) is a catastrophic complication after joint arthroplasty. A key step to treat PJI is accurate diagnosis. However, according to the existing guidelines, no biomarker has been used to diagnose PJI alone and autoimmune diseases such as rheumatoid arthritis (RA) often interfere with the diagnosis. Patients with PJI often still face issues such as misdiagnosis and delayed treatment after a comprehensive scoring diagnosis. Thus, a rapid quantitative detection method based on excellent biomarker is needed. Here, an all-printed photonic crystal (PC) biochip is reported. Through the fluorescence enhancement effect of the photonic crystals and the design of the hydrophilic and hydrophobic patterning structure, trace detection of lactoferrin (LTF) in a drop of viscous synovial fluid can be achieved within 10 min by the biochip. The detection limit is as low as 4.6 ng/mL. For more intuitive results, portable testing equipment is developed with indicator light displays of three colors (green = NPJI, yellow = RA, and red = PJI) to indicate the diagnosis depending on the biochip’s fluorescence intensity. Furthermore, 79 clinical synovial fluid samples are tested with PC biochips, and the PC results are in good agreement with the ELISA results. Due to the microdroplet reaction on PCs, biochips are suitable for the detection of the selected markers in the synovial fluid. The use of the three-color portable testing equipment avoids the need for professional testing devices. Overall, this strategy provides a new paradigm for the early and differential diagnoses of PJI.

Free-aldehyde neutralized and oligohyaluronan loaded bovine pericardium with improved anti-calcification and endothelialization for bioprosthetic heart valves

Yuhong Liu,Chunyang Chen,Ting Lu,Sixi Liu,Zhongshi Wu,Zhenjie Tang

The number of patients with valvular heart disease is increasing yearly, and valve replacement is the most effective treatment, during which bioprosthetic heart valves (BHVs) are the most widely used. Commercial BHVs are mainly prepared with glutaraldehyde (Glut) cross-linked bovine pericardial or porcine aortic valves, but the residual free aldehyde groups in these tissues can cause calcification and cytotoxicity. Moreover, insufficient glycosaminoglycans (GAGs) in tissues can further reduce biocompatibility and durability. However, the anti-calcification performance and biocompatibility might be improved by blocking the free aldehyde groups and increasing the GAGs content in Glut-crosslinked tissues. In our study, adipic dihydrazide (ADH) was used to neutralize the residual free aldehyde groups in tissues and provide sites to blind with oligohyaluronan (OHA) to increase the content of GAGs in tissues. The modified bovine pericardium was evaluated for its content of residual aldehyde groups, the amount of OHA loaded, physical/chemical characteristics, biomechanical properties, biocompatibility, and in vivo anticalcification assay and endothelialization effects in juvenile Sprague-Dawley rats. The results showed that ADH could completely neutralize the free aldehyde groups in the Glut-crosslinked bovine pericardium, the amount of OHA loaded increased and the cytotoxicity was reduced. Moreover, the in vivo results also showed that the level of calcification and inflammatory response in the modified pericardial tissue was significantly reduced in a rat subcutaneous implantation model, and the results from the rat abdominal aorta vascular patch repair model further demonstrated the improved capability of the modified pericardial tissues for endothelialization. Furthermore, more α-SMA + smooth muscle cells and fewer CD68 + macrophages infiltrated in the neointima of the modified pericardial patch. In summary, blocking free-aldehydes and loading OHA improved the anti-calcification, anti-inflammation and endothelialization properties of Glut-crosslinked BHVs and in particularly, this modified strategy may be a promising candidate for the next-generation of BHVs.

Photoreceptor-Mimetic Microflowers for Restoring Light Responses in Degenerative Retina through a 2D Nanopetal/Cell Biointerface

Gerile Oudeng,Seema Banerjee,Qin Wang,Ding Jiang,Yadi Fan,Honglian Wu,Feng Pan,Mo Yang

Retinitis pigmentosa is the main cause of inherited human blindness and is associated with dysfunctional photoreceptors (PRs). Compared with traditional methods, optoelectronic stimulation can better preserve the structural integrity and genetic content of the retina. However, enhancing the spatiotemporal accuracy of stimulation is challenging. Quantum dot-doped ZnIn 2 S 4 microflowers (MF) are utilized to construct a biomimetic photoelectric interface with a 0D/3D heterostructure, aiming to restore the light response in PR-degenerative mice. The MF bio interface has dimensions similar to those of natural PRs and can be distributed within the curved spatial region of the retina, mimicking cellular dispersion. The soft 2D nano petals of the MF provide a large specific surface area for photoelectric activation and simulate the flexibility interfacing between cells. This bio interface can selectively restore the light responses of seven types of retina ganglion cells that encode brightness. The distribution of responsive cells forms a pattern similar to that of normal mice, which may reflect the generation of the initial “neural code” in the degenerative retina. Patch-clamp recordings indicate that the bio interface can induce spiking and postsynaptic currents at the single-neuron level. The results will shed light on the development of a potential bionic subretinal prosthetic toolkit for visual function restoration.

Engineered Niobium Carbide MXenzyme-Integrated Self-Adaptive Coatings Inhibiting Periprosthetic Osteolysis by Orchestrating Osteogenesis–Osteoclastogenesis Balance

Shishuo Li,Xiaoqing Lu,Qihao Chai,Benzhao Huang,Shimin Dai,Peng Wang,Jianing Liu,Zhibo Zhao,Xiao Li,Bing Liu,Kangqing Zuo,Zhentao Man,Ningbo Li,Wei Li

Periprosthetic osteolysis induced by the ultrahigh-molecular-weight polyethylene (UHMWPE) wear particles is a major complication associated with the sustained service of artificial joint prostheses and often necessitates revision surgery. Therefore, a smart implant with direct prevention and repair abilities is urgently developed to avoid painful revision surgery. Herein, we fabricate a phosphatidylserine- and polyethylenimine-engineered niobium carbide (Nb2C) MXenzyme-coated micro/nanostructured titanium implant (PPN@MNTi) that inhibits UHMWPE particle-induced periprosthetic osteolysis. The specific mechanism by which PPN@MNTi operates involves the bioresponsive release of nanosheets from the MNTi substrate within an osteolysis microenvironment, initiated by the cleavage of a thioketal–dopamine molecule sensitive to reactive oxygen species (ROS). Subsequently, functionalized Nb2C MXenzyme could target macrophages and escape from lysosomes, effectively scavenging intracellular ROS through its antioxidant nanozyme-mimicking activities. This further achieves the suppression of osteoclastogenesis by inhibiting NF-κB/MAPK and autophagy signaling pathways. Simultaneously, based on the synergistic effect of MXenzyme-integrated coatings and micro/nanostructured topography, the designed implant promotes the osteogenic differentiation of bone mesenchymal stem cells to regulate bone homeostasis, further achieving advanced osseointegration and alleviable periprosthetic osteolysis in vivo. This study provides a precise prevention and repair strategy of periprosthetic osteolysis, offering a paradigm for the development of smart orthopedic implants.

Living joint prosthesis with in-situ tissue engineering for real-time and long-term osteoarticular reconstruction

Wei Sun,Hongwei Wu,Yiyang Yan,Xianzhu Zhang,Xudong Yao,Rui Li,Jingyi Zuo,Wenyue Li,Hongwei Ouyang

The reconstruction of large osteoarticular defects caused by tumor resection or severe trauma remains a clinical challenge. Current metal prostheses exhibit a lack of osteo-chondrogenic functionality and demonstrate poor integration with host tissues. This often results in complications such as abnormal bone absorption and prosthetic loosening, which may necessitate secondary revisions. Here, we propose a paradigm-shifting “living prosthesis” strategy that combines a customized 3D-printed hollow titanium humeral prosthesis with engineered bone marrow condensations presenting bone morphogenetic protein-2 (BMP-2) and transforming growth factor–β3 (TGF-β3) from encapsulated silk fibroin hydrogels. This innovative approach promotes in situ endochondral defect regeneration of the entire humeral head while simultaneously providing immediate mechanical support. In a rabbit model of total humerus resection, the designed “living prosthesis” achieved weight, macroscopic and microscopic morphologies that were comparable to those of undamaged native joints at 2 months post-implantation, with organized osteochondral tissues were regenerated both around and within the prosthesis. Notably, the “living prosthesis” displayed significantly higher osteo-integration than the blank metal prosthesis did, as evidenced by a 3-fold increase in bone ingrowth and a 2-fold increase in mechanical pull-out strength. Furthermore, the "living prosthesis" restored joint cartilage function, with rabbits exhibiting normal gait and weight-bearing capacity. The successful regeneration of fully functional humeral head tissue from a single implanted prosthesis represents technical advance in designing bioactive bone prosthesis, with promising implications for treating extreme-large osteochondral defects.
