Biodegradable Anti-Stenosis Microsurgical Implantable Hydrogel Stent for Efficient Vas Deferens Recanalization

Yaoqi Chen,Hua Zhang,Jie Yao,Xu Cao,Yang Luo,Qiguang Wang,Yi Yu,Rui Su,Yudong Yao,Gonghui Li,Jun Fu,Gordon G. Wallace,Jun Chen,Kerong Wu

Vasovasostomy is a surgical procedure aimed at restoring male fertility, whereas low anastomotic efficiency, delayed healing, and postoperative stenosis are key problems encountered during vas deferens recanalization. Here, a directly coaxial-printable and robust PCB hydrogel stent is designed to enhance the effectiveness of vasovasostomy by photo-polymerizing host–guest complexations between poly(ethylene glycol) methyl ether acrylate and α-cyclodextrin acrylate, using N, N’-bis(acryloyl)cystamine as a degradable crosslinker. The PCB hydrogel exhibits continuous micro-stent fabrication capabilities and exceptional anti-deformation mechanical properties (elastic modulus, fracture strength, and toughness of 1.3 MPa, 2.5 MPa, and 480.4 kJ·m −3 , respectively) due to the formation of a polypseudorotaxane threading network integrated with nano-self-assembled structures. Rabbit microsurgical vasovasostomy experiments demonstrate that the time required for tissue anastomosis is reduced by more than half with the support of the PCB hydrogel. Moreover, the stent's excellent resistance to cell adhesion and complete degradability within 28 days in vivo effectively prevents stenosis and promotes healing of the vas deferens, achieving a 100% patency rate. This high-performance PCB stent presents a promising solution for effective vas deferens-related treatment and various other applications.

Ultrasound-responsive theranostic platform for the timely monitoring and efficient thrombolysis in thrombi of tPA resistance

Lin Lin,Ba Zhaojing,Tian Hao,Qin Haoxiang,Chen Xi,Zhou Xin,Zhao Shanlan,Li Lang,Xue Fangchao,Li Hong,He Lang,Li Xiaochen,Du Jiahui,Zhou Zhenhua,Zeng Wen

There is no effective and noninvasive solution for thrombolysis because the mechanism by which certain thrombi become tissue plasminogen activator (tPA)-resistant remains obscure. Endovascular thrombectomy is the last option for these tPA-resistant thrombi, thus a new noninvasive strategy is urgently needed. Through an examination of thrombi retrieved from stroke patients, we found that neutrophil extracellular traps (NETs), ε-(γ-glutamyl) lysine isopeptide bonds and fibrin scaffolds jointly comprise the key chain in tPA resistance. A theranostic platform is designed to combine sonodynamic and mechanical thrombolysis under the guidance of ultrasonic imaging. Breakdown of the key chain leads to a recanalization rate of more than 90% in male rat tPA-resistant occlusion model. Vascular reconstruction is observed one month after recanalization, during which there was no thrombosis recurrence. The system also demonstrates noninvasive theranostic capabilities in managing pigs’ long thrombi (>8 mm) and in revascularizing thrombosis-susceptible tissue-engineered vascular grafts, indicating its potential for clinical application. Overall, this noninvasive theranostic platform provides a new strategy for treating tPA-resistant thrombi.

Repair of Large-Scale Rib Defects Based on Steel-Reinforced Concrete-Designed Biomimetic 3D-Printed Scaffolds with Bone-Mineralized Microenvironments

Baoshuai Bai,Junxiang Hao,Mengjie Hou,Tao Wang,Xiaodi Wu,Yanhan Liu,Yiyang Wang,Chengxiang Dai,Yujie Hua,Guangyu Ji,Guangdong Zhou

Tissue engineering technology provides a promising approach for large-scale bone reconstruction in cases of extensive chest wall defects. However, previous studies did not consider meticulous scaffold design specific to large-scale rib regeneration in terms of three-dimensional (3D) shape, proper porous structures, enough mechanical strength, and osteogenic microenvironments. Thus, there is an urgent need to develop an appropriate bone biomimetic scaffold (BBS) to address this problem. In this study, a BBS with controllable 3D morphology, appropriate mechanical properties, good biocompatibility and biodegradability, porous structure suitable for cell loading, and a biomimetic osteogenic inorganic salt (OIS) microenvironment was successfully prepared by integrating computer-aided design, 3D-printing, cast-molding, and freeze-drying technologies. The addition of the OIS in the scaffold substantially promoted ectopic bone regeneration in vivo, which might be attributed to the activation of osteogenic and angiogenic signaling pathways as well as upregulated expression of osteogenic genes. More importantly, dual long rib defects could be successfully repaired and medullary cavity recanalized by the rib-shaped mature cortical bone, which might be mediated by the activation of osteoclast signaling pathways. Thus, this paper presents a reliable BBS and proposes a new strategy for the repair of large-scale bone defects.
