Fabrication of PLGA-chitosan-tranexamic acid composite sponge and its mechanism for rapid hemostasis

Yue Zhu,Teng Chen,Xiujuan Wan,Guiqin Zhu,Zhiyong Chen,Zhoujie Yang,Jing Yang,Tingting Feng,Zuhua Wang,Ying Zhou

Persistent intraoperative hemorrhage is the major cause of trauma-related mortality, emphasizing the need for hemostatic dressings with improved efficacy. In this study, we designed a PLGA-CTS-TXA composite sponge, which was fabricated by grafting of poly (lactic- co -glycol acid) (PLGA) and hemostatic small molecule tranexamic acid (TXA) onto chitosan (CTS), for rapid hemostasis. The sponge integrates the advantages of CTS, PLGA and TXA, showcasing superior porosity (nearly 85 %) and water absorption capacity (3197 ± 105 % within 30 s). It also demonstrates good biocompatibility (hemolysis <1.2 %, RCV > 125 %), low blood clotting index (12.4 %) and abbreviated clotting times (<30 s). The incorporation of TXA substantially augmented the hemostatic capability of the composite sponge. Compared to CTS alone, PLGA-CTS-TXA demonstrated a significantly reduced hemostatic time in both rat tail and mouse liver hemostatic models. Its hemostatic mechanism was further analyzed from the perspective of the actual interface created by the interaction between the material and the biological fluid (ie., protein corona). The results demonstrated that the swift hemostatic mechanism involves modulation of erythrocyte and platelet aggregation/adhesion and fibrin degradation, etc. , thereby influencing the coagulation cascade. This research extends the utility of TXA in hemostasis and offers novel idea into the development and hemostatic mechanism of new high-efficiency hemostatic dressings.

Preparation and characterization of tranexamic acid modified porous starch and its application as a hemostatic agent

Xinhong Zhao,Yunbo Sun,Zhiyun Meng,Zhiyuan Yang,Shan Fan,Tong Ye,Lei Yang,Tong Li,Ruolan Gu,Zhuona Wu,Hui Gan,Guifang Dou,Yongxiu Qi,Shuchen Liu

Effective bleeding control is essential for the reduction of traumatic deaths among civilians and military personnel, particularly for physical visceral and arteriovenous injuries. Materials with good hemostatic properties have recently attracted significant scientific attention. In this study, a novel material of tranexamic acid modified porous starch (TAMPS) was produced through esterification . The structure of the final product was characterized using Fourier-transform infrared spectroscopy, X-ray diffraction, scanning electron microscopy, and thermogravimetric analysis . The hemostatic effect of TAMPS was preliminarily analyzed via in vitro clotting time, mouse tail amputation model and liver injury model experiments. Hemostatic effect of TAMPS was found to be significantly better than that of the positive control Quickclean. Through the exploration of related hemostatic mechanisms , TAMPS can promote coagulation via rapid fluid absorption and high erythrocyte aggregation capacity. The in vitro cytotoxicity, acute toxicity, and hemolysis tests revealed that TAMPS is safe and nontoxic and has perfect blood compatibility. Therefore, the TAMPS has a great potential for future clinical application as a rapid and multitarget hemostatic material.

Transition from Procoagulation to Antiplatelet Effect: Application and Mechanism of Aspirin-Modified Chitosan in Small-Diameter Vascular Grafts

Si-Yuan Zhou,Lei Li,Jian-Hua Cao,Xiu-Bin Yang,Da-Yong Wu

Chitosan is generally considered to be a procoagulant effect, which may cause adverse phenomena such as blood clotting when used in small-diameter vascular grafts. However, it also shows good biocompatibility and anti-inflammatory properties, which can facilitate vascular reconstruction. Therefore, it is significant to transition the effect of chitosan from coagulation promotion to antiplatelet while still harnessing its bioactivity. The procoagulant mechanism of chitosan is primarily attributed to the presence of protonated amino groups in the molecular chain. If the number of amino groups in chitosan is reduced, the procoagulant effect will be diminished as well. Aspirin has a strong antiplatelet function, and its molecular structure contains numerous active carboxyl groups, which can couple with the amino groups in chitosan. Aspirin-modified chitosan retains the biological activity of chitosan while also imparting an antiplatelet effect. In our study, we used a heparinized electrospun graft as the substrate and coated it with aspirin-modified chitosan to create a functional vascular graft. The blood clotting index of the graft remained above 80% after 45 min, and the platelet activation degree was only 4.03%. Additionally, the graft maintained complete patency with stable blood flow after 4 weeks of implantation and the vascular structure was largely rebuilt.

A macroporous composite sponge with high water absorbency and active coagulation mechanism for traumatic hemostasis and anti-infection

Xinchen Du,Tongxing Zhang,Tong Li,Taosheng Sun,Yadong Liu

The development of a composite sponge with high water absorbency and active coagulation mechanism for traumatic hemostasis and anti-infection remains a challenge. Herein, we developed a composite sponge using gelation, swelling, and freeze-drying methods based on quaternized chitosan, succinimidyl-modified F127, and bioactive glass. The sponge exhibited macroporous structure, high porosity, and water absorbency. When exposed to blood, it strongly interacted with blood cells, promoting their adhesion, aggregation, and activation. Moreover, it activated the intrinsic coagulation pathway. The sponge/ powder demonstrated superior hemostatic capacity to commercial gauze, gelatin sponge, Yunnan Baiyao, and chitosan hemostatic powder in rat tail amputation, liver superficial injury, liver resection, and liver semi-perforation wound models. The sponge also presented robust anti-infection activity against methicillin-resistant Staphylococcus aureus and Escherichia coli. Additionally, the sponge showed low cytotoxicity, hemolysis activity, inflammation response, and systemic toxicity, demonstrating its favorable biocompatibility.

Preparation and Hemostatic Effect of Micro-Nanograded Porous Particles Doped with Dopamine-Based Water-Triggered Intelligent Composite Adhesives

Caiyun Zheng,Qian Gao,Yanxiao Quan,Que Bai,Fangfang Hu,Wenting Chen,Jinxi Liu,Yanni Zhang,Tingli Lu

The wet environment of water or tissue in bleeding wounds poses significant challenges to the adhesion performance of existing hemostatic adhesives. An intelligent composite adhesive prepared by doping starch-based silicate micro-nanograded porous particles (MBC@CMS) with dopamine-hyperbranched polymers (HPD, 7800 Mw) synthesized by the Michael addition reaction could be triggered by water to form a glue (MBC@CMS-HPD). The results indicated that MBC@CMS-HPD could still have adhesion properties under running water washing and water immersion and could effectively seal the water outlet. The results of the glue-forming mechanism showed that MBC@CMS-HPD had better wettability than water, which could eliminate water molecules at the wet adhesive surface. When contacted with water, the agglomeration of the HPD hydrophobic chain increases the exposure of the catechol group, and the relative atomic mass of the N element on the surface increases from 2.8 to 4.8%. The adhesion of MBC@CMS-HPD was enhanced and stable. MBC@CMS-HPD showed significant hemostasis effects in five injury bleeding models of Sprague–Dawley (SD) rats and New Zealand rabbits. Especially in the fatal femoral artery bleeding model of New Zealand rabbits, MBC@CMS-HPD reduced the amount of bleeding by 75% and shortened the bleeding time by 78% compared with the a-cyanoacrylate adhesives. The results of the coagulation mechanism showed that compared with HPD, MBC@CMS-HPD could activate both endogenous and exogenous coagulation pathways. Among them, after contact with blood, HPD formed a gel to close the blood outlet, and MBC@CMS entered the wound to activate the internal and external coagulation pathways. In addition, HPD and MBC@CMS had good histocompatibility and degradability, which has the potential to be applied to different wounds.
