Synergistic hydration lubrication of polyurethane meniscus based on zwitterionic structures and hyaluronic acid

Lingfeng Hu,Jinge Li,Jie Wang,Xiaoniu Yang

In this study, polyurethanes with graded sulfobetaine content (PU-SBs) were employed to resolve the high friction of artificial menisci. Under the simulated gait of knee joints , the coefficient of friction of PU-SB60 in hyaluronic acid (HA) aqueous solution was similar to that of the native meniscus. Importantly, this phenomenon was caused by synergistic hydration lubrication based on sulfobetaine groups and HA, that a thicker and stronger hydration layer was preferred to form quickly on the surfaces of HA-adsorbed PU-SB60 and was harder to be destroyed by the friction force . Furthermore, PU-SB60 exhibited both excellent modulus and long-term low cytotoxicity. Therefore, PU-SB60 has the potential to be applied for artificial menisci due to its overall performance.

Applying Electrospun Gelatin/Poly(lactic acid-co-glycolic acid) Bilayered Nanofibers to Fabrication of Meniscal Tissue Engineering Scaffold

Li Peng,Zhang Weiguo,Yu Hongquan,Zheng Lianjie,Yang Liang,Liu Gang,Sheng Chenchen,Gui Haoran,Ni Shuo,Li Pengsheng,Shi Feng

The menisci are fibrocartilaginous tissues composed primarily of an interlacing network of collagen fibers with nanoscale diameter. Electrospinning is a suitable process of producing nanoscale fibers that mimic collagen fibers. In this study, a bilayered scaffold (group B), which consists of a gelatin nanofiber mesh and a PLGA nanofiber mesh, has been fabricated through an electrospinning method. At the same time, we electrospun pure PLGA fibrous mesh (group A) and gelatin/PLGA composite fibrous mesh (group C) as control groups. In order to compare all scaffold morphologies, the scaffolds were imaged by SEM and some parameters were measured and analyzed as following: Diameters of fibrils are from the smallest of less than average 0.14 μm for group C to the biggest of nearly average 0.38 μm for group B. The scaffolds pore diameters are from average 4.9 μm for group A to average 11.2 μm for group B. Porosity rates show that the group B has the highest porosity rate at about 91%. The scaffolds’ properties were compared and analyzed, including hydrophilicity property (water contact angle) and mechanical properties (tensile strength). The results of water contact angle showed the group B is the most hydrophil among the groups. The results of tensile strength showed the tensile strength of group C is the weakest among the groups. All the results showed significant differences between the groups. Finally, in vitro, the meniscal cells derived from New Zealand white rabbits menisci were seeded in the scaffolds. We observed the cells proliferation behavior in the scaffolds. All above demonstrates that a bi-layered gelatin/PLGA scaffold reveals not only concurrent effects of mechanics and cytocompatibility in a fibrous context, but also a promising scaffold for future meniscal repair strategies.

Harnessing electrospun nanofibers to recapitulate hierarchical fibrous structures of meniscus

Xiaoyu Wang,Jingjing Zhu,Binbin Sun,Qiu Jin,Haiyan Li,Changlei Xia,Hongsheng Wang,Xiumei Mo,Jinglei Wu

The meniscus has complex biomechanical functions endowed by the hierarchical fibrous structure of its extracellular matrix (ECM), which plays a central role in protecting the knee joint. However, it is challenging to recapitulate the ECM structure of the meniscus. Herein, we used electrospinning to prepare various scaffolds with distinct nanofibrous structures to approximate that of the heterogeneous ultrastructure of meniscus ECM. Our results showed that adjusting the deposition manner of nanofibers during electrospinning could effectively manipulate the architectures of resulting scaffolds. This approach led to electrospun scaffolds with random or aligned fibrous structures that reassemble the surface or superficial layers of the meniscus. We also showed that assembly of electrospun nanofibers into yarn-like configurations replicates the circumferentially aligned fibrous bundles of the inner part of the meniscus. These scaffolds exhibited distinct fibrous anisotropies and mechanical properties. In vitro studies indicated good cytocompatibility of scaffolds, especially, the yarn scaffold supported considerable meniscus cell infiltration. The meniscus cells, in turn, enhanced the mechanical properties of scaffolds. Taken together, these data imply that electrospun scaffolds may have potential in enhancing meniscus repair and regeneration.

Self-Lubricating and Self-Healing Polyurethane Elastomer as a Meniscal Prosthesis to Delay Osteoarthritis Progression

Yi Zhou,Xia Jiang,Xiaolong Yang,Huan Liang,Xing Xie,Weili Fu

The absence of a meniscal prosthesis that fully accommodates the complex motion demands of the human knee joint remains a challenge in clinical practice. This study synthesized a polyurethane elastomer with enhanced mechanical properties, featuring a dynamic dimethyldioxime-urethane structure that provides self-healing capabilities. Additionally, adamantane-grafted hyaluronic acid has been integrated into polyurethane containing cyclodextrin, imparting self-lubricating properties through supramolecular host-guest interactions between adamantane and cyclodextrin. This innovative polyurethane elastomer is utilized to fabricate a meniscal prosthesis (DPU@HA). In vitro experiments demonstrated DPU@HA's exceptional cytocompatibility with chondrocytes. In vivo studies revealed that DPU@HA significantly protected articular cartilage following meniscus removal by reducing the high mechanical stress within the cartilage, thereby inhibiting Piezo1 overactivation and subsequent chondrocyte apoptosis. With its self-healing and self-lubricating properties, the meniscal prosthesis (DPU@HA) holds promise for clinical applications in preventing the progression of osteoarthritis after meniscectomy.

An electrospun fiber reinforced scaffold promotes total meniscus regeneration in rabbit meniscectomy model

Shuang Gao,Mingxue Chen,Pei Wang,Yan Li,Zhiguo Yuan,Weimin Guo,Zengzeng Zhang,Xueliang Zhang,Xiaoguang Jing,Xu Li,Shuyun Liu,Xiang Sui,Tingfei Xi,Quanyi Guo

Low vascularization in meniscus limits its regeneration ability after injury, and tissue engineering is the most promising method to achieve meniscus regeneration. In this study, we fabricated a kind of composite scaffold by decellularized meniscus extracellular matrix/polycaprolactone (DMECM/PCL) electrospinning fibers and porous DMECM, in which DMECM/PCL fibers were used as reinforcing component. The tensile modulus of the composite scaffold in longitudinal and crosswise directions were 8.5 ± 1.9 and 2.3 ± 0.3 MPa, respectively. Besides that, the DMECM/PCL electrospinning fibers enhanced suture resistance of the composite scaffold more than 5 times than DMECM scaffold effectively. In vitro cytocompatibility showed that the porous structure provided by DMECM component facilitated meniscus cells’ proliferation. DMECM was also the main component to regulate cell behaviors, which promoted meniscus cells expressing extracellular matrix related genes such as COL I, COL II, SOX9 and AGG . Rabbits with total meniscectomy were used as animal model to evaluated the composited scaffolds performance in vivo at 3 and 6 months. Results showed that rabbits with scaffold implanting could regenerate neo-menisci in both time points. The neo-menisci had similar histology structure and biochemical content with native menisci. Although neo-menisci had inferior tensile modulus than native ones, its modulus was improved with implanting time prolonging. MRI imaging showed the signal of neo-meniscus in the body is clear, and X-ray imaging of knee joints demonstrated the implantation of scaffolds could relief joint space narrowing. Moreover, rabbits with neo-menisci had better cartilage condition in femoral condyle and tibial plateau compared than meniscectomy group. Statement of Significance We fabricated the meniscus scaffold by combining porous decellularized meniscus extracellular matrix (DMECM) and DMECM/PCL electrospinning fibers together, which used the porous structure of DMECM, and the good tensile property of electrospinning fibers. We believe single material cannot satisfy increasing needs of scaffold. Therefore, we combined not only materials but also fabrication methods together to develop scaffold to make good use of each part. DMECM in electrospinning fibers also made these two components possible to be integrated through crosslinking. Compared to existing meniscus scaffold, the composite scaffold had (1) soft structure and extrusion would not happen after implantation, (2) ability to be trimmed to suitable shape during surgery, and (3) good resistance to suture.

3D printing stiff antibacterial hydrogels for meniscus replacement

Rong Yang,Chuanchuan Fan,Yiming Dou,Xiaoping Zhang,Ziyang Xu,Qian Zhang,Yage Sun,Qiang Yang,Wenguang Liu

3D printing of high-strength and stiff hydrogel is of great significance in customizing individualized load-bearing soft tissues, but faces a significant challenge. Herein, inspired by fugitive ink generally adopted in supporting bath printing, gelatin is employed as a sacrificial rheological modifier material to thicken the multiple hydrogen bonding monomer N -acryloylsemicarbazide (NASC), affording direct printability. Post-UV light irradiation-initiated polymerization of NASC results in robust hydrogel in spite of disturbance of hydrogen bonding caused by gelatin. Notably, PNASC H -bonding interactions is well reconstructed after high temperature dialyzing of gelatin, which resumes the tensile strength , Young's modulus , compressive modulus, of the printed hydrogels to 2.3 MPa, 26.25 MPa, 4.22 MPa, respectively. The NASC/gelatin can be printed into different architectures including meniscus, and the printed hydrogel does not exhibit noticeable wear to cartilage in simulation test of femoral wear. In addition, polydopamine coated-ZIF-8 is modified onto the surface of PNASC/gelatin hydrogels through one-pot synthesis method, thereby affording an antibacterial property without varying mechanical performance and wear property. This printed PNASC/gelatin scaffold with high strength , stiffness, better biocompatibility, low wear property, and built-in antibacterial activity hold potential as a meniscus substitute with a potent ability to minimize infection-induced implantation failure.

Characterization of decellularized scaffold derived from porcine meniscus for tissue engineering applications

Gao Shuang,Yuan Zhiguo,Xi Tingfei,Wei Xiaojuan,Guo Quanyi

Menisci are fundamental fibrocartilaginous organs in knee joints. The injury in meniscus can impair normal knee function and predisposes patients to osteoarthritis. This study prepared decellularized meniscus scaffolds using a 1% (w/w) sodium dodecyl sulfate solution and sufficient rinsing steps. Complete cell removal was verified by hematoxylin and eosin staining and DNA content assay. Decellularized menisci had accordant tension properties to intact ones, but with declined compression properties. This occurred because the collagen fiber was not damaged but glycosaminoglycans was significantly lost during the decellularization process, which was confirmed by biochemical assay and histology staining. In vitro cytotoxicity assay demonstrated that decellularized meniscus scaffolds have no toxicity on L929 murine fibroblasts and porcine chondrocytes. Further experiment showed that porcine chondrocytes could adhere and proliferate on the scaffold surface, and some cells even could infiltrate into the scaffold. All results showed the potential of this decellularized meniscus to be the scaffolds in tissue engineering.

A novel composite meniscus scaffold with shape memory and adhesive properties alleviates knee cartilage degeneration through minimally invasive and sutureless approach

Yan Wenqiang,Tian Feng,Zhao Lingyu,Wang Pengyi,Zhang Manwen,Gao Zeyuan,Huang Wei,Ao Yingfang,Yu Ran

Three-dimensionally (3D) printed engineered meniscus scaffolds offer a promising therapeutic strategy for post-meniscectomy defects. Nevertheless, existing scaffolds face critical limitations impeding clinical implementation. Primarily, the transplantation procedure necessitates larger incisions in skin and joint capsules due to insufficient scaffold deformability. This invasive approach risks iatrogenic tissue damage, prolongs recovery timelines, and compromises aesthetic outcomes. Secondly, conventional fixation methods involving sutures to peripheral meniscal remnants or capsular tissues introduce dual complications: mechanical perforation of scaffolds reduces structural integrity and accelerates degradation, while the demanding technical requirements for suture placement increase surgical complexity. To address these challenges, we engineered a composite meniscus scaffold featuring an elastomeric skeleton fabricated via digital light processing (DLP) 3D printing integrated with a hydrogel matrix. This biomimetic design recapitulates the native meniscus architecture comprising an aqueous extracellular matrix reinforced by collagen fibrils. The composite scaffold demonstrates three critical functional attributes: 1) biomechanical properties approximating native meniscus tissue, 2) exceptional shape recovery capabilities facilitating trans-arthroscopic delivery through minimally invasive portals, and 3) strong interfacial adhesion enabling suture-free fixation via covalent bonding to host tissues. In vivo evaluation using a rabbit meniscectomy model confirmed the scaffold’s chondroprotective efficacy. Notably, preclinical validation in an ex vivo ovine knee under arthroscopic guidance demonstrated successful scaffold deployment through subcentimeter incisions with stable adhesion to residual meniscal tissue. This innovative design eliminates suture-related complications while reducing surgical invasiveness, thereby presenting a clinically translatable solution for irreparable meniscal injuries.

Regional-Specific Decellularized Meniscus Extracellular Matrix Elastic Nanofiber Aerogels Regulate Meniscal Regeneration and Vascularization

Moran Huang,Yangfan Ding,Jize Dong,Guoqi Li,Guojian Fu,Jinhui Pang,Yaying Sun,Shanxing Zhang,Jinglei Wu,Jiwu Chen

The meniscus is a heterogeneous structure with spatial distribution of cells and vessels. Promoting meniscus healing remains challenging, especially in its avascular inner region. The ideal repair mode shall promote meniscus repair while maintaining local avascularity to prevent pathological changes from vascular invasion. Given the natural biochemical components of native meniscus, decellularized meniscus extracellular matrix (dmECM) shows promise for meniscus repair. Compared to homogeneous dmECM, regional-specific dmECM (RS-dmECM) appears to offer greater potential for constructing heterogeneous meniscus structures. Furthermore, there is currently no study on the effects of RS-dmECM on vascularization. Thus, RS-dmECM is prepared and found dmECM-Outer induced stem cells fibrochondrogenic differentiation, while dmECM-Inner induced the chondrogenic differentiation and inhibited angiogenesis through suppressing the peroxisome proliferators-activated receptors signaling pathway. Subsequently, regional-specific poly(lactic acid)/gelatin/dmECM elastic 3D nanofiber aerogels (PG-dmECM) are fabricated to repair meniscal avascular area defects in a rabbit model. The PG-Outer promotes meniscal fibrocartilage regeneration without a significant effect on vascular invasion. In contrast, the PG-Inner achieves meniscal cartilage regeneration and inhibits vascular invasion. Moreover, the compressive stress can reach 117 kPa, comparable to that of the avascular area. This PG-Inner may have the potential to promote meniscal cartilage regeneration and prevent pathological changes within the avascular area.

Photocurable hydrogel-elastomer hybrids as an adhesive patch for meniscus repair

Tao Lei,Yushuang Zhao,Xinrang Zhai,Shunxian Ji,Binghua Song,Wei Dong,Chong Teng,Wei Wei

Hydrogel adhesives are gaining popularity in tissue engineering and regeneration medicine. However, unsatisfactory anti-tension properties of traditional hydrogels limit their practical uses, particularly in sports medicine, which usually requires robust mechanical properties. Here, we design a tissue adhesive patch based on a bioadhesive hydrogel (HAMA-GelNB) and a tough elastomer (PDMS) that can atraumatically and rapidly repair tissue injuries such as meniscus tears. The strong and sutureless sealing capabilities of the hybrids are demonstrated using ex vivo porcine skin and stomach models. In vitro cell studies and in vivo subcutaneous implantation models are used and demonstrated the biocompatibility of the material. Finally, the efficiency of the tissue adhesive patch is demonstrated using a rabbit meniscus tear model which is challenging in clinic. The results of macroscopic images and histological analysis revealed that the surface of cartilage in the material-treated group was slightly worn, but the untreated group developed progressive cartilage degeneration. These findings demonstrate that the hydrogel-elastomer composite tissue adhesive could be an alternative to sutures for tissue repair and is promising in translational medicine.

Fabrication and characterization of electrospun nanofibers composed of decellularized meniscus extracellular matrix and polycaprolactone for meniscus tissue engineering

Shuang Gao,Weimin Guo,Mingxue Chen,Zhiguo Yuan,Mingjie Wang,Yu Zhang,Shuyun Liu,Tingfei Xi,Quanyi Guo

Many kinds of scaffolds have been produced in meniscus tissue engineering, but few have matched the mechanical properties of native meniscus, making it impossible for them to sustain large stress at initial implantation. In this study, we used a differential centrifugation method to obtain decellularized meniscus extracellular matrix (DMECM) and combined the DMECM with polycaprolactone (PCL) via electrospinning to fabricate random and aligned microfibers. The FTIR results and biochemical assays demonstrated the successful mixing of these two elements, and the addition of DMECM improved the hydrophilicity of the microfibers. The blending of DMECM also enhanced the tensile modulus of the microfibers, and aligned fibers had tensile moduli ranging from 132.27 to 331.40 MPa, which match that of human meniscus. In addition, we defined yield stress as the lose-efficacy point. The results showed that DMECM/PCL fibers had higher yield stresses than the pure PCL fibers, and the aligned fibers had higher yield stress values than the randomly oriented fibers. Nanoindentation results showed that adding DMECM had no significant impact on modulus and hardness with the exception of fibers containing 80% DMECM, which exhibited an obvious increase in modulus. In vitro assay demonstrated that the DMECM/PCL fibers had no hemolysis or cytotoxicity. Meniscus cells could attach and proliferate on the fibers, and the fiber orientation had a direct influence on cell arrangement. RT-PCR results showed that meniscus cells had higher gene expressions of aggrecan, collagen I, collagen II and Sox 9 when seeded on fibers with higher DMECM contents.

CHMP5 attenuates osteoarthritis via inhibiting chondrocyte apoptosis and extracellular matrix degradation: involvement of NF-κB pathway

Gao Weilu,Liu Rui,Huang Keke,Fu Wenhan,Wang Anquan,Du Gongwen,Tang Hao,Yin Li,Yin Zongsheng S.

Background Osteoarthritis (OA), the most common joint disease, is linked with chondrocyte apoptosis and extracellular matrix (ECM) degradation. Charged multivesicular body protein 5 (CHMP5), a member of the multivesicular body, has been reported to serve as an anti-apoptotic protein to participate in leukemia development. However, the effects of CHMP5 on apoptosis and ECM degradation in OA remain unclear. Methods In this study, quantitative proteomics was performed to analyze differential proteins between normal and OA patient articular cartilages. The OA mouse model was constructed by the destabilization of the medial meniscus (DMM). In vitro, interleukin-1 beta (IL-1β) was used to induce OA in human chondrocytes. CHMP5 overexpression and silencing vectors were created using an adenovirus system. The effects of CHMP5 on IL-1β-induced chondrocyte apoptosis were investigated by CCK-8, flow cytometry, and western blot. The effects on ECM degradation were examined by western blot and immunofluorescence. The potential mechanism was explored by western blot and Co-IP assays. Results Downregulated CHMP5 was identified by proteomics in OA patient cartilages, which was verified in human and mouse articular cartilages. CHMP5 overexpression repressed cell apoptosis and ECM degradation in OA chondrocytes. However, silencing CHMP5 exacerbated OA chondrocyte apoptosis and ECM degradation. Furthermore, we found that the protective effect of CHMP5 against OA was involved in nuclear factor kappa B (NF-κB) signaling pathway. Conclusion sThis study demonstrated that CHMP5 repressed IL-1β-induced chondrocyte apoptosis and ECM degradation and blocked NF-κB activation. It was shown that CHMP5 might be a novel potential therapeutic target for OA in the future.
