Adenosine triphosphate (ATP)-activated prodrug approaches demonstrate potential in antibacterial uses. However, their efficacy frequently faces obstacles due to uncontrolled premature activation and spatiotemporal distribution differences under physiological circumstances. Herein, we present an endogenous ATP-activated prodrug system (termed ISD3) consisting of nanoparticles (indole-3-acetic acid/zeolitic imidazolate framework-8@polydopamine@platinum, IZPP) embedded in a silk fibroin-based hydrogel, aimed at treating multidrug-resistant (MDR) bacteria-infected pressure ulcers. Initially, an ultraviolet-triggered adhesive ISD3 barrier is formed over the pressure ulcer wound by a simple local injection. Subsequently, the bacteria-secreted ATP prompts the degradation of IZPP, allowing the loaded IAA prodrug and nanozyme to encounter spatiotemporally on a single carrier, thereby efficiently generating reactive oxygen species (ROS). Exposure to 808 nm near-infrared light enhances the catalytic reaction speed, boosting ROS levels for stronger antibacterial action. Once optimal antibacterial action is reached, ISD3 switches to a dormant state, halting any further ROS production. Moreover, the bioactive components in ISD3 can exert anti-inflammatory functions, aiding in pressure ulcer recovery. Overall, our research introduces a hydrogel prodrug strategy activated by bacterial endogenous ATP, which precisely manages ROS generation and accelerates the recovery of MDR bacteria-infected pressure ulcers.

On the basis of the mice pressure ulcers (PU) model, the protective effect and potential mechanism of sodium Danshensu (SDSS) cream against PU were investigated. The mice were randomly divided into three groups: the negative control group (cream without 0.5 g SDSS), the SDSS group (cream containing 0.5 g SDSS), and the positive group (0.5 g Hirudoid®). After 7 and 14 days of ointment application, the wound-healing rate of the SDSS and positive groups was significantly higher than that of the control group (p< 0.05). The results of hematoxylin–eosin staining also indicated that SDSS has the potential to promote the healing of PU. In addition, the serum IL-6, IL-1β, TNF-α, and MDA levels decreased significantly (p< 0.01) after 14 days of SDSS treatment, while the SOD, CAT, and GSH-Px activities increased significantly (p< 0.01). In addition, SDSS cream was able to significantly increase the expression of Nrf2, HO-1, GCLM, NQO1, NF-κB p65, NF-κB p50, IKKα, and IKKβ while decreasing the expression of Keap1 and IκBαin the Nrf2/HO-1 and NF-κB pathways. Our research will provide a foundation for the future clinical prevention and treatment of PU with SDSS cream. Keywords:pressure ulcers;sodium Danshensu cream;ischemia/reperfusion injury;Nrf2/HO-1 pathway;NF-κB pathway

Approximately 4.5% to 7.0% of hospitalized patients suffer from pressure ulcers. Mitigating risks for pressure ulcers through sensors remains a challenge and a high requirement. A simple, low-cost, battery-free, multi-parameter passive wireless flexible sensor (MPWFS) for all-around pressure and temperature monitoring to prevent pressure ulcers is developed. The pressure sensing unit is fabricated with functional gradient-structured balsa wood and has high sensitivity of 0.34 kPa −1 with a wide detection range of 0–20 kPa. The temperature sensing unit, which is 0.4 mm × 0.2 mm, is embedded in the surface of the pressure sensing unit, enabling temperature monitoring with a resolution of 0.1 °C. The flexible Radio Frequency energy-harvesting unit, data acquisition, and processing, as well as Bluetooth-Low-Energy wireless transmission, are integrated within a 20 mm × 20 mm unit. It acquires continuous temperature and pressure data without a battery at any position more than 1 m away from the power transmitter. Moreover, the combination of the sensor array design with a mobile terminal provides the MPWFS's various benefits, including tracking changes in the supine posture, warning about pressure ulcers, and monitoring falls out of bed. This study presents a new method for long-term bedridden patient care.

Bedridden patients are at high-pressure injuries risk, and timely turning or moving a bedridden patient is crucial to release localized pressure and reduce pressure injury risk. Pressure-sensing electronics can collect real-time pressure signals reflecting the movement of a patient and help carers evaluate pressure injury risk early and intervene timely. However, most pressure-sensing electronics cannot be seamlessly mounted on the curved surface of the skin, resulting in signal instability and inaccuracy. Besides, detecting small pressures over a wide range is challenging. Herein, a lamellar shape-memory conductive (LSMC) aerogel is developed to assemble 3D wireless point-of-care electronics for assisting pressure injury prevention. The lamellar structure makes the LSMC aerogel highly compressible, endowing the assembled electronics with high sensitivity and low detection limit. Moreover, the excellent shape-memory effect allows the fabrication of 3D electronics on certain curved surfaces for adaptive wearing and provides sensitive and stable signals. Furthermore, a point-of-care monitoring system that integrates the LSMC aerogel-based 3D electronics with wireless technology is developed. The system records real-time pressure signals at certain locations (e.g., foot), which facilitates carers to analyze an individual's bedridden time and body movements, assess pressure injury risk, and intervene timely; this greatly benefits pressure injury prevention.

Background Pressure ulcer is common in the bedridden elderly with high mortality and lack of effective treatment. In this study, human-adipose-derived-stem-cells-hyaluronic acid gel (hADSCs-HA gel) was developed and applied topically to treat pressure ulcers, of which efficacy and paracrine mechanisms were investigated through in vivo and in vitro experiments. Methods Pressure ulcers were established on the backs of C57BL/6 mice and treated topically with hADSCs-HA gel, hADSCs, hyaluronic acid, and normal saline respectively. The rate of wound closure was observed continuously during the following 14 days and the wound samples were obtained for Western blot, histopathology, immunohistochemistry, and proteomic analysis. Human dermal fibroblasts (HDFs) and human venous endothelial cells (HUVECs) under normal or hypoxic conditions were treated with conditioned medium of human ADSCs (ADSC-CM), then CCK-8, scratch test, tube formation, and Western blot were conducted to evaluate the paracrine effects of hADSCs and to explore the underlying mechanism. Results The in vivo data demonstrated that hADSCs-HA gel significantly accelerated the healing of pressure ulcers by enhancing collagen expression, angiogenesis, and skin proliferation. The in vitro data revealed that hADSCs strengthened the proliferation and wound healing capabilities of HDFs and HUVECs, meanwhile promoted collagen secretion and tube formation through paracrine mode. ADSC-CM was also proved to exert protective effects on hypoxic HDFs and HUVECs. Besides, the results of proteomic analysis and Western blot elucidated that lipid metabolism and PPARβ/δ pathway mediated the healing effect of hADSCs-HA gel on pressure ulcers. Conclusion Our research showed that topical application of hADSCs-HA gel played an important role in dermal regeneration and angiogenesis. Therefore, hADSCs-HA gel exhibited the potential as a novel stem-cell-based therapeutic strategy of treating pressure ulcers in clinical practices.

Due to its highly insidious and rapid progression, deep tissue pressure injury (DTPI) is a clinical challenge. Our previous study found that DTPI may be a skeletal muscle injury dominated by macrophage immune dysfunction due to excessive iron accumulation. Decellularized skeletal muscle matrix (dECM) hydrogel promotes skeletal muscle injury repair. However, its role in polarizing macrophages and regulating iron metabolism in DTPI remains unclear. Here, porcine dECM hydrogel was prepared, and its therapeutic function and mechanism in repairing DTPI were investigated. The stimulus of dECM hydrogel towards RAW264.7 cells resulted in a significantly higher percentage of CD206+ macrophages and notably decreased intracellular divalent iron levels. In mice DTPI model, dECM hydrogel treatment promoted M1 to M2 macrophage conversion, improved iron metabolism, and reduced oxidative stress in the early stage of DTPI. In the remodeling phase, the dECM hydrogel remarkably enhanced revascularization and accelerated skeletal muscle repair. Furthermore, the immunomodulation of dECM hydrogels in vivo was mainly involved in the P13k/Akt signaling pathway, as revealed by GO and KEGG pathway analysis, which may ameliorate the iron deposition and promote the healing of DTPI. Our findings indicate that dECM hydrogel is promising in skeletal muscle repair, inflammation resolution, and tissue injury healing by effectively restoring macrophage immune homeostasis and normalizing iron metabolism.

Pressure ulcers have emerged as a substantial burden on individuals and society. The introduction of innovative dressings that facilitate the healing of pressure ulcer wounds represents a cost-effective alternative for treatment. In this study, we focus on the preparation of Carthamus tinctorius L. polysaccharide (CTLP) as hydrogel microspheres, which are then encapsulated within a hydrogel matrix crosslinked with phenylboronic acid gelatin (Gelatin-PBA) and ε-polylysine-grafted catechol (ε-PL-Cat) to enable sustained release for promoting pressure ulcer healing. The presented GPL/CTLP-MPs hydrogel demonstrated outstanding self-healing properties. In addition, in vitro experiments revealed that the hydrogel exhibited remarkable antibacterial activity, excellent biocompatibility. And it showed the capacity to promote vascular formation, effectively scavenge reactive oxygen species (ROS), and facilitate macrophage polarization from the M1 to M2 phenotype. In vivo wound healing of mice pressure ulcers indicated that the prepared GPL/CTLP-MPs hydrogel effectively accelerated the formation of granulation tissue and facilitated the healing of the wounds. In summary, in vivo and in vitro experiments consistently highlight the therapeutic potential of GPL/CTLP-MPs hydrogel in facilitating the healing process of pressure ulcers.

Current approaches to treating infected pressure ulcers (IPUs) tend to lack specificity. Such injuries frequently exhibit extended inflammation and slow tissue regeneration, largely due to a surplus of M1 macrophages in the impacted regions. While immunomodulatory hydrogels offer potential for targeted IPU care, facilitating a safe and efficient shift from M1 to M2 macrophages continues to be a considerable challenge. The wound's harsh microenvironment—marked by unchecked infections and reactive oxygen species—often undermines the effectiveness of these hydrogels. We unveil a custom-engineered hydrogel dressing specifically designed to have immunomodulatory properties for expediting the healing of IPUs. Consisting of intertwined polymer networks, this hydrogel incorporates gallium (Ga) ions-triggered self-assembled glycyrrhizic acid (GA). Crucially, both in vitro and in vivo examinations, along with RNA sequencing assessments, demonstrate that the Ga/GA combination successfully reduces the M1/M2 macrophage ratio under inflamed circumstances, thereby contributing to improved IPU wound recovery. In summary, our proposed immunomodulatory strategy provides a simple, secure, and highly efficacious method for managing IPUs.

Pressure ulcer dressings with different functions can enhance wound healing ability to varying degrees; however, pressure ulcer dressings that integrate various functions and break the resistance of bacteria to traditional antibiotics have not been widely studied. We proposed a self-pumping/super-absorbent/transportable drug dressing (PLD-SLD), polycaprolactone (PCL)/sodium alginate (SA) was used to load platelet-derived growth factor (PDGF) and lidocaine hydrochloride (LID) by Janus electrospinning and self-assembly technology, and Ɛ-polylysine was used as a biological bacteriostatic agent to prepare a multi-layer dressing. SEM showed that the dressing had a fluffy structure. The dressing can pump the exudate to the SA layer away from the skin. The swelling ratio reached 1378.667 ± 44.752 %. Coagulate blood in 5 min. On the 8th day, the unclosed area rate of the PLD-SLD dressing group was 16.112 ± 0.088 % lower than that of the model group. Importantly, the dressing can induce the expression of CD31, VEGF, α-SMA, and reduce the expression of CD68, thereby giving priority to wound healing. There was no scar formation after healing. In this study, a new dressing preparation method was proposed for the problems of exudate management, infection control, pain relief and healing promotion of stage 3–4 pressure ulcer healing.

Chronic wound healing is a major challenge in clinical practice. Secondary dressing damage and antibiotic resistance are the main obstacles for traditional wound dressings. Resina draconis (RD), a natural resin traditionally used in powder form for wound care, is now considered unsuitable due to the lack of gas permeability and moist environment required for wound healing. Here, RD is incorporated in situ by constructing a 3D coiled fibrous scaffold with polycaprolactone/polyethylene oxide. Due to the high porosity of 3D scaffold, the RD-3D dressings have a favorable swelling capacity, providing permeability and moisture for wound repair. Meanwhile, the transformation of RD powder into 3D dressings fully demonstrates capabilities of RD in rapid hemostasis, bactericidal, and inflammation-regulating activities. In vivo evaluations using pressure ulcer and infected wound models confirm the high efficacy of RD-3D dressing in early wound healing, particularly beneficial in the infected wound model compared to recombinant bovine FGF-basic. Further biological analysis shows that resveratrol, loureirin A, and loureirin B, as potentially bioactive components of RD, individually contribute to different aspects of wound healing. Collectively, RD-3D integrated dressings represent a simple, cost-effective, and safe approach to wound healing, providing an alternative therapy for translating medical dressings from bench to bedside.

Pressure ulcers (PU) as a major clinical concern are disproportionately affecting the older adult population and resulting in a lot of medical expenses. The issues of preventing the incidence of PU and treating chronic wounds are urgent to be solved at the present stage. Here, a 3D hybrid scaffold with conductive and antibacterial properties is designed to monitor and repair chronic wounds. The 3D hybrid scaffold is prepared by encapsulating nanofiber yarns (NFY) network into the injectable hydrogel which is cross-linked by Schiff base . The aligned NFY networks assembled by weaving nanofiber yarns consisting of polyacrylonitrile (PAN) and reduced graphene oxide (rGO) can induce cellular elongation and alignment on their 3D peripheral surface. The injectable hydrogels outside not only provide the protection for the cells on the surfaces of the NFYs, but also mimic a native extracellular matrix (ECM) to supply a much more suitable 3D microenvironment for the cells proliferation and arrangement. The addition of ions within the hydrogel endows the hydrogels with excellent conductivity , making the hydrogels the potential flexible strain sensor to exactly monitor the wound pressure. The 3D hybrid scaffold with aligned nanofiber yarns embedded in injectable hydrogels displays favorable biocompatibility, self-healing and antibacterial properties, showing promising medical applications especially in tissue engineering and wearable electronics .

The treatment of infected pressure ulcers (IUPs) requires addressing diverse microenvironments. A pressing challenge is to effectively enhance the regenerative microenvironment at different stages of the healing process, tailoring interventions as needed. Here, a dual enzyme mimetic and bacterial responsive self-activating antimicrobial hydrogel designed to enhance IPUs healing is introduced. This hydrogel incorporates pH-responsive dual enzyme-active nanoplatforms (HNTs-Fe-Ag) encapsulated within a methacrylate-modified silk fibroin (SFMA) and dopamine methacrylamide (DMA) matrix. This composite hydrogel exhibits adaptive microenvironment regulation capabilities. Under the low pH microenvironment of bacterial infection, it has excellent antimicrobial activity by self-activating the •OH generation in conjunction with photothermal effects. Under the neutral and alkaline microenvironment of chronic inflammation, it catalyzes the decomposition of hydrogen peroxide (H 2 O 2 ) to produce oxygen (O 2 ), thereby alleviating hypoxia and scavenging reactive oxygen species (ROS), which in turn remodulates the phenotype of macrophages. The composite hydrogel demonstrates on-demand therapeutic effects in the microenvironment of infected wounds, significantly enhancing the regenerative microenvironment of IUPs by promoting wound closure, inflammation regulation, and collagen deposition through self-activated antimicrobial action during infection and adaptive hypoxia relief during recovery. This approach offers a novel strategy for developing smart wound dressings.
