Oral ulcers, a prevalent mucosal disorder with high recurrence rates, remain challenging to treat due to the oral cavity’s dynamic environment and the multifactorial nature of ulcer progression. Here, we report a multifunctional microneedle (MN) patch loaded with astragalus polysaccharide (APS-MN) that integrates rapid mucosal penetration, reactive oxygen species (ROS) scavenging, and ferroptosis inhibition to accelerate ulcer healing. Unlike conventional MN systems reliant on synthetic drugs or prolonged dissolution, APS-MN leverages the natural antioxidant and immunomodulatory properties of APS-a traditional Chinese medicine-derived polysaccharide-enabling complete drug release within 2 minutes and targeted delivery to ulcer bases. In vitro and in vivo studies demonstrate that APS-MN promotes M2 macrophage polarization, suppresses pro-inflammatory cytokines (TNF-α, IL-1β), and inhibits ferroptosis via the Nrf2/HO-1/SLC7A11 axis, achieving 88.2% wound closure in rats within 6 days. Notably, this work combined herbal polysaccharides with MN technology for oral ulcer therapy, offering a biocompatible, non-invasive alternative to corticosteroids and antibiotics. The rapid dissolution, dual antioxidant/anti-ferroptotic action, and scalability of APS-MN position it as a transformative platform for managing recurrent oral ulcers and other redox-imbalanced mucosal diseases.

Oral ulcers, prevalent lesions of the oral mucosa, often stem from trauma, viral infections, and autoimmune diseases, necessitating effective treatment strategies to address both their physical and psychological impacts. Current treatments are often limited by issues such as poor adhesion and rapid dilution within the dynamic oral environment. To overcome these challenges, we have explored the application of thermosensitive hydrogels, specifically aqueous solutions of poly(ethylene glycol)–poly(trimethylene carbonate) (PEG–PTMC) copolymers. These copolymers are noted for their biocompatibility and temperature-responsive phase transition properties. We engineered a thermosensitive drug delivery system by fine-tuning the molecular weight and segment lengths of PEG–PTMC, which gels near 32 °C and precipitates at 37 °C. Utilizing the slight temperature differential between the oral mucosa and ambient air, the system forms a Janus structure with distinct gel and precipitate layers, ensuring sustained drug release and enhanced tissue adhesion. This innovative formulation not only acts as a protective, drug-laden dressing that promotes healing but is also designed for ease of use, allowing for self-administration. This study presents a significant advancement in the treatment of oral ulcers, offering a promising solution through the in situ formation of a bilayered, drug-loaded dressing, leveraging the unique properties of thermosensitive hydrogels.

Bioadhesives have potential advantages over sutures and staples for wound closure, hemostasis, and in vivo dressing applications. However, the tissue adhesives reported so far typically fail to integrate an in vivo adhesion fault-tolerant mechanism with autonomous therapeutic activity. Here, we present the unprecedented development of a bioadhesive based on the coenzyme factor lipoic acid (LA), featuring an upper critical solution temperature and adhesion fault-tolerance. The in-situ ring-opening polymerization of LA triggered by aggregation in the naturally occurring sodium queen bee acid (SQBA) nanomicelles, leads to nanomicelle self-crosslinking via strong ionic hydrogen bonds, resulting in the formation of a thermoresponsive bioadhesive (PolyLA-SQBA). PolyLA-SQBA exhibits thermally switchable and reusable adhesion, forming rapid and robust adhesion on oral mucosa at physiological temperature, and detaching from the adhered tissue without residue upon cooling. Leveraging the natural bioactivity of LA and SQBA, PolyLA-SQBA demonstrates excellent biocompatibility, potent antioxidant activity, and exhibits significant inhibitory effects against common oral pathogens. With its intelligent responsiveness, efficient adhesion, and sustained release of bioactive molecules, PolyLA-SQBA effectively delivers both LA and SQBA to oral ulcer sites, reducing inflammatory cytokine levels and significantly accelerating the healing of oral ulcers.

Fabrication of layered bioadhesives with asymmetric bioadhesion, on-demand detachment and inherent biological functions remains a great challenge. This work reports a novel and generalizable solvent volatilization-induced annealing (SVA) strategy to prepare a Janus film with an integrated dual layer structure, asymmetric adhesion, on-demand detachment and inherent biological functions. Depositing polyvinyl pyrrolidone/caffeic acid/lipoic acid (PVP/CA/LA) ethanol solutions onto an ethylcellulose (EC) layer and applying SVA strategy can integrate two layers in molecular-level to obtain the dual-layered Janus film. Porous PVP/p(CA-LA) surface pressed onto wet tissues can absorb interfacial water to form tight tissue contact, and their functional groups can form abundant bonds to induce robust bioadhesion. In contrast, dense EC surface limits water absorption and exhibits minimal adhesion of proteins, cells and tissues. Furthermore, the adhered Janus film can be detached by using a glutathione/sodium bicarbonate solution. Additionally, CA and LA provide the film with desired antibacterial, antioxidant, and anti-inflammatory properties. Finally, by providing the antibacterial and anti-inflammatory microenvironment, the Janus film promotes angiogenesis and significantly expedites the healing of the oral ulcers in rats. This work not only introduces a novel approach for preparing multi-layered and asymmetric materials, but also paving the way for developing adhesive materials with inherent biological functions.

Oral ulcer is a common oral mucosal injury disease that causes significant inconvenience and discomfort for patients. Traditional treatments, such as drugs, ointments, and patches, have limitations, including low local drug utilization, short residence time on the affected area, and the risk of secondary injury from manual application. To address these issues, this study developed a sprayable temperature-sensitive hydrogel (PEI/PAA/PF127-CHO/AA2G@MSN-NH2 hydrogel, PMAG) without manual contact on injuryed mucosa by combining aldehyde-modified Pluronic F-127 (PF127-CHO), ascorbic acid 2-glucoside-loaded amino-mesoporous silica (AA2G@MSN-NH2), polyethylenimine (PEI), and polyacrylic acid (PAA) through Schiff base reaction and physical crosslinking. When sprayed onto an oral ulcer, PMAG exhibits excellent temperature sensitivity and adhesion, rapidly adapting to oral temperature and forming a protective film that closely adheres to the ulcer surface. This creates a stable healing environment by isolating the wound from external stimuli. PMAG has demonstrated significant abilities in promoting epithelial cell proliferation and migration, as well as antioxidant, anti-inflammatory, and antibacterial properties. RNA sequencing revealed that the tissue repair-promoting effects of PMAG are primarily mediated by activating the nerve growth factor receptor/extracellular signal-regulated kinase (NGFR/ERK) signaling pathway. In summary, PMAG represents a significant advancement in improving the treatment of oral ulcers and offers a more convenient option for patients.

Diabetic oral ulcers pose a significant challenge in healing due to persistent inflammation. Despite local therapeutic interventions remaining the primary mode of treatment, the dynamic nature of the oral cavity, characterized by continuous muscular activity and salivary secretion, poses barriers to sustained drug retention and thereby limits therapeutic efficacy. To address this issue, an approach has been devised that aims to facilitate transdermal delivery of bioactive components to promote the healing of diabetic oral ulcers. A multifunctional soluble microneedle (MN) patch is prepared using γ-polyglutamic acid (γ-PGA), which is loaded with quercetin (Qu)-modified Mg−Zn layered hydroxide salt (LHS) nanosheets (LHSQ) with anti-inflammatory, antimicrobial and pro-angiogenic bioactivities to combat the diabetic oral ulcers. These findings demonstrate that the LHSQ-loaded MN (LHSQ-MN) patches are capable of effectively penetrating the oral mucosa. In a rat model of diabetic oral ulcers, the application of LHSQ-MN patches is found to facilitate the establishment of regeneration sites, regulate the inflammatory microenvironment of damaged tissues, exhibit potent antibacterial effects, expedite the reconstitution of the mucosal epithelium, and accelerate ulcer healing through the promotion of angiogenesis. These outcomes highlight the potential of LHSQ-MN patches as a promising therapeutic strategy for the management of oral ulcers in diabetic patients.

Oral ulcers severely affect the patient’s mood, diet, and language activities and are mainly treated by topical administration of a patch. However, the currently commercially available patches have a limited treatment effect because of the short action time in the oral cavity. In this study, we present a novel adhesive patch made from trifunctional polycaprolactone crosslinked waterborne polyurethane (CDCPU) loaded with curcumin for treating oral ulcers. The long-chain triol cross-linking points in the polyurethane can limit water entry, while stabilizing the curcumin loading in the emulsion. Meanwhile, the interaction between curcumin and polyurethane improved the anti-swelling (swelling rate of 37.7 ± 4.9 %), water resistance (water absorption rate of 66.6 ± 5.6 %), and mechanical properties of the patch. The adhesion strength of the CDCPU patch to porcine oral mucosa and skin was 11.3 ± 1.7 kPa and 14.7 ± 2.5 kPa, respectively. The addition of curcumin provides the CDCPU patch with effective antibacterial and antioxidant functions. Moreover, the CDCPU patch was effective in promoting mucosal epithelialization and ulcer healing in a rat oral ulcer model. The development of this adhesive patch provides a new strategy for the treatment of oral ulcers.

We identified Armillariella tabescens polysaccharide (PAT-W), a compound isolated from a Chinese medicinal mushroom , as a potential novel oral ulcer (OU) drug. In treating OU rats with PAT-W, especially in the high-dose group, oral mucous tissue TNF- α , IL-1 β , and IL-6 levels were markedly reduced, and pathological morphology and oxidative stress were effectively improved. Western blot analysis showed that the PAT-W channel ameliorated OU mucous tissue damage, which depends on the activation of the Nrf2/HO-1 antioxidant signaling pathway . Furthermore, high-throughput sequencing results showed that PAT-W regulated the maladjustment of the oral microbiota caused by OU. Therefore, based on the new viewpoint of activating the Nrf2/HO-1 pathway and regulating oral microbiota, PAT-W is expected to become a new natural drug for treating oral ulcers and improving patients' quality of life.

Recurrent aphthous ulceration (RAU) is a common oral condition that significantly affects patients' quality of life worldwide. Mucosal dressings for wound healing and drug delivery are essential in RAU management. The dynamic and moist environment of the oral cavity poses major obstacles to localized drug delivery, particularly in terms of patient self-administration and dressing retention. In this study, we present a universal oral mucosal drug delivery system based on sprayable rapid gelation powder (SRGP) composed of multigroup-modified alginate, which can be evenly sprayed onto ulcers in different locations within the oral cavity using a low-cost, commercially available plastic spray bottle. Upon contact with the wound, SRGP absorbs saliva and forms an adhesive hydrogel barrier in 2 s, which can continuously release the drug for over 12 h, even in the presence of saliva and oral activities. When loaded with drugs such as lidocaine hydrochloride and dexamethasone sodium phosphate, experiments in rat and dog models demonstrated that drug loaded SRGP significantly reduced inflammation at the ulcer site and reduced wound healing time by 2–3 days compared to commercial dressings. Furthermore, the powder formulation of SRGP can be stored a long time for 24 months, highlighting its potential as a clinical hydrogel dressing for oral ulcers.

Oral ulcers are prone to recurrence and often complicated by bacterial infections. Currently, antibiotics, glucocorticoids, and anesthetics are commonly employed in clinical practice to alleviate symptoms. However, these medications exhibit limited retention in the moist and dynamic environment of the oral cavity, and their long-term use may lead to various side effects or drug resistance. In this study, we designed a borate ester cross-linked oxidation-responsive hydrogel (AHQP hydrogel) for the treatment of oral ulcers. The AHQP hydrogel is composed of phenylboronic acid-functionalized hyaluronic acid, sodium alginate, and quaternized chitosan (QCS), and is further enhanced by the incorporation of proanthocyanidins (PA) to strengthen its borate ester cross-linked network. These natural raw materials comprising the AHQP hydrogel exhibit excellent biocompatibility and degradability, ensuring that the hydrogel is safe for use in the oral cavity. Once the AHQP hydrogel adheres to the wet wound tissue, it degrades in response to reactive oxygen species, thereby releasing QCS and PA to exert therapeutic effects. Our results demonstrated that the AHQP hydrogel exhibits significant antioxidant, anti-inflammatory, antibacterial, and pro-angiogenic properties, thereby facilitating the healing of oral ulcers. Overall, this borate ester cross-linked polysaccharide hydrogel reinforced by PA presents promising prospects for the treatment of oral ulcers.

Oral ulcers affect more than 25 % of the global population and are driven by complex pathological mechanisms, including bacterial infection, oxidative stress, and dysregulated inflammation, all of which impede healing. To address the limitations of therapies that target only a single aspect of ulcer pathology, this study developed a multifunctional electrospun film by combining 45S5 bioactive glass (BG) with hydroxypropyl methyl cellulose (HPMC). The BG/HPMC films, fabricated by electrospinning with 5 wt% BG incorporated into HPMC matrices, were thoroughly characterized for their structural and bioactive properties. In a rat oral ulcer model, the BG/HPMC films significantly accelerated re-epithelialization and reduced ulcer size compared to both control and chitosan-treated groups. The films also exhibited strong antibacterial activity against Staphylococcus aureus. In vitro, BG extracts promoted the viability and migration of human oral epithelial cells (HOECs) and human umbilical vein endothelial cells (HUVECs), mitigated hydrogen peroxide-induced oxidative stress, and suppressed the expression of pro-inflammatory cytokines—interleukin-1 alpha (IL-1α), tumor necrosis factor alpha (TNF-α), and interleukin-6 (IL-6)—in lipopolysaccharide-stimulated macrophages.

The liquid formulation of Periplaneta Americana extract (PAE) demonstrates significant efficacy in the treatment of oral ulcers. However, its limited ability to adhere to the oral cavity leads to a diminished therapeutic effect. This study aims to address this limitation by developing a Periplaneta Americana extract oral film (PAEOF) designed to extend the PAE ‘s duration of action through enhanced adhesion. Utilizing a one-factor analysis approach, the optimal oral film incorporating Periplaneta Americana extracts , based on polyvinyl alcohol, hydroxypropyl cellulose, and carbomer, was developed. The resulting oral film exhibited an elongation at break of 548.86 ± 279.78 % and an adhesive force of 2.34 ± 0.47 N/cm 2 . Furthermore, the mucoadhesion time of PAEOF was quantified at 89.00 ± 2.83 min, demonstrating favorable bioadhesion properties. Pharmacodynamic studies in Sprague-Dawley rats revealed that the high PAEOF group, containing 20 % Periplaneta Americana extract , exhibited the most significant healing effect. After 5 days of administration, ulcer areas decreased from 52.43 ± 10.53 mm 2 to 16.92 ± 2.33 mm 2 , and ulcer burning points decreased from 10.00 ± 0.00 to 4.00 ± 1.46. Additionally, levels of various biomarkers including SOD (superoxide dismutase), TNF-α (tumor necrosis factor alpha), IL-6 (interleukin 6), IL-2 (interleukin 2), and IFN-γ (interferon gamma) in the PAEOF-treated groups approached those found in the healthy group, indicating a satisfactory therapeutic effect. In conclusion, PAEOF presents a novel approach for the treatment of oral ulcer diseases, offering enhanced adhesion and prolonged therapeutic efficacy.
