Intervertebral disc degeneration is characterized by a localized, chronic inflammatory response leading to a synthesis/catabolism imbalance within the nucleus pulposus (NP) and progressive functional impairment within the NP. Polyphenol molecules have been widely used in anti-inflammatory therapies in recent years; therefore, we designed an injectable, temperature-sensitive hydrogel PLGA-PEG-PLGA-based drug delivery system for local and sustained delivery of two drugs tannic acid (TA) and resveratrol (Res), with the hydrogel carrying TA directly and Res indirectly (carried directly by inflammation-responsive nanoparticles). The delivery system presents good injectability at room temperature and forms a gel in situ upon entering the intervertebral disc. The delivery system can rapidly release TA and sustain Res release. In vitro and in vivo experiments have shown that this hydrogel drug delivery system is effective in anti-inflammation of degenerated intervertebral discs and promotes the regeneration of extracellular matrix in the NP.

Rapid integration into the host tissue is critical for long-term patency after small diameter tissue engineering vascular grafts (sdTEVGs) transplantation. Neural recognition may be required for host integration and functionalization of the graft. However, immune rejection and inflammation hinder nerve regeneration of sdTEVGs. Here, a CRISPR/dCas9-nanocarrier was used for targeted programming of regulatory T cells (Treg cells) in situ to promote nerve regeneration of sdTEVGs by preventing excessive inflammation. Treg cells and (C-C chemokine receptor) CCR2+ macrophage recruitment occurred after transplantation. The nanodelivery system upregulated ten eleven translocation (TET2) in Treg cells in vitro. Reprogrammed Treg cells upregulated anti-inflammatory cytokines and decreased the proportion of CCR2+ macrophages. IL-6 concentrations decreased to the levels required for nerve regeneration. Implantation of CRISPR/dCas9 nanodelivery system-modified sdTEVGs in rats resulted in Treg cell editing, control of excessive inflammation, and promoted nerve regeneration. After 3 months, nerve regeneration was similar to that observed in normal blood vessels; good immune homeostasis, consistency of hemodynamics, and matrix regeneration were observed. Neural recognition promotes further integration of the graft into the host, with unobstructed blood vessels without intimal hyperplasia. Our findings provide new insights into vascular implant functionalization by the host.

Suture-free corneal repair strategy has become a focus in the field of corneal injury treatment due to its minimal invasiveness and high efficiency. This study introduces a dual-layer convex patch for suture-free repair of ocular trauma, comprising a thermosensitive gelatin/alginate adhesive layer and a convex collagen substrate layer. This design addresses critical limitations of conventional adhesives, particularly their difficulty in forming symmetric curvature during application and insufficient bonding strength under intraocular pressure fluctuations. In vitro studies demonstrated that the patch possessed superior transmittance (above 80 %) and high burst pressure (above 200 mmHg). This dual-layer convex patch can reduce surgical time by two-thirds compared to the suture group. In a lamellar keratoplasty model, the patch promoted epithelial healing, enhanced stroma regeneration, and inhibited the formation of stromal scars. For irregular large-sized corneal penetrating injuries, the patch group achieved rapid wound sealing, promoted epithelial healing, and showed more symmetrical corneal curvature and 90 % reduction in neovascularization compared to fibrin. The superior performance of the dual-layer convex patch makes it a promising alternative for the suture-free treatment of corneal trauma, achieving both corneal stroma defects repair and irregular penetrating injuries healing.

Intervertebral disc degeneration (IDD) progresses due to local inflammatory response, gradually unbalanced anabolic/catabolic activity, and progressive functional impairment within the nucleus pulposus. Antagomir-21, a cholesterol-modified miRNA-21 inhibitor, has potential extracellular matrix (ECM) regenerative ability , but its application for IDD is limited by inadequate local delivery systems. An injectable hydrogel gene delivery system encapsulating a modified tannic acid nanoparticles (TA NPs) vector was engineered for on-demand and sustained delivery of antagomir-21 into the nucleus pulposus. After nucleus pulposus cell uptake, antagomir-21 was released from TA NPs and regulated the ECM metabolic balance by inhibiting the MAPK/ERK signaling pathway . TA NPs scavenged intracellular ROS and reduced inflammation by downregulating TNF-α expression. In vivo , synergistic anti-inflammatory effects and ECM regeneration effectively promoted therapeutic efficacy against IDD. This hydrogel gene delivery system represents a creative, promising strategy for IDD repair.

Cutaneous injuries, a prevalent clinical challenge, often face delayed healing due to suboptimal wound management strategies. Conventional hydrogel dressings are limited by inadequate mechanical properties, poor bioactivity, and insufficient therapeutic efficacy. Here, we developed an innovative in situ forming hydrogel dressing by integrating silk fibroin (SF)—a natural biopolymer with tunable structural and bioactive properties—with polyvinyl butyral (PVB), which provides an ideal system for the delivery of active substances for wound healing and has broad application prospects in skin wound management. Upon ethanol evaporation, the SF/PVB composite rapidly formed a hydrogel film with enhanced mechanical strength, optimal breathability, and waterproofness. The SF-based dressing (LD-SF) demonstrated multifunctional wound-healing capabilities, including rapid hemostasis, antioxidative activity, and anti-inflammatory modulation. Notably, molecular-weight (MW)-dependent bioactivity was observed: low-MW SF (45 kDa) significantly promoted fibroblast proliferation and migration, while high-MW SF (72 kDa) exhibited superior immunomodulatory effects by polarizing macrophages toward pro-resolving phenotypes. In a murine full-thickness wound model, LD-SF accelerated re-epithelialization, enhanced angiogenesis, and stimulated collagen remodeling. Mechanistically, LD-SF facilitated extracellular matrix regeneration via β -sheet-driven structural stability and amino acid-mediated metabolic support. This dual-action system synergistically orchestrates immunomodulation—through macrophage phenotype regulation and cytokine balance—and robust extracellular matrix regeneration, offering a transformative approach to acute wound repair with minimized scarring and accelerated functional recovery. The study provides a clinically translatable solution for acute wound care by integrating rapid in situ film formation, molecular-weight-tunable bioactivity, and scar-minimizing outcomes, thereby addressing critical gaps in current wound management technologies.

Metabolic reprogramming is fundamental to synovium remodeling with drug delivery for osteoarthritis (OA) therapy. Mitochonic acid 5-MASM7@MnTBAP nanoparticles (MM@MT NPs) with various physicochemical properties and biological activities may be developed as a supramolecular nano-drug delivering to articulus for regulating mitochondrial metabolism of synovium. This study aims to explore the feasibility, efficacy, and mechanism of MM@MT NPs, which possibly excavates a novel perspective for OA therapy. Herein, for feasibility, MM@MT NPs has been indicated to possess excellent photothermal, reactive oxygen species (ROS) response, and oxygen release performances. For efficacy, MM@MT NPs has been confirmed to promote extracellular matrix (ECM) regeneration. For mechanism, MM@MT NPs has been illustrated to restore the mitochondrial membrane potential and recover the mitochondrial dynamics, which is beneficial for maintaining mitochondrial homeostasis. Moreover, MM@MT NPs has been demonstrated to stimulate the tricarboxylic acid (TCA) cycle and oxidative phosphorylation (OXPHOS) in mitochondria as well as enhance antioxidant capacity and eliminate oxidative stress, which is reflected in regulating the adenosine triphosphate (ATP) and ROS metabolism. Therefore, MM@MT NPs can remodel the homeostasis of mitochondria via reprogramming metabolism in synovium, which achieves the symptomatic and etiological treatments of OA.

Electrical stimulation (ES) is proposed as a therapeutic solution for managing chronic wounds. However, its widespread clinical adoption is limited by the requirement of additional extracorporeal devices to power ES-based wound dressings. In this study, a novel sandwich-structured photovoltaic microcurrent hydrogel dressing (PMH dressing) is designed for treating diabetic wounds. This innovative dressing comprises flexible organic photovoltaic (OPV) cells, a flexible micro–electro–mechanical systems (MEMS) electrode, and a multifunctional hydrogel serving as an electrode–tissue interface. The PMH dressing is engineered to administer ES, mimicking the physiological injury current occurring naturally in wounds when exposed to light; thus, facilitating wound healing. In vitro experiments are performed to validate the PMH dressing's exceptional biocompatibility and robust antibacterial properties. In vivo experiments and proteomic analysis reveal that the proposed PMH dressing significantly accelerates the healing of infected diabetic wounds by enhancing extracellular matrix regeneration, eliminating bacteria, regulating inflammatory responses, and modulating vascular functions. Therefore, the PMH dressing is a potent, versatile, and effective solution for diabetic wound care, paving the way for advancements in wireless ES wound dressings.
