Uncontrolled hemorrhage poses a severe life-threatening situation. However, traditional hemostats still have various limitations. It is urgent to develop a material with excellent biocompatibility and hemostatic ability. Evidence has shown that carboxymethyl chitosan (CMCS) has hemostatic properties and good compatibility. Herein, we develop an expandable hemostatic sponge by modifying CMCS with cellulose nanofibrils (CNFs) through the CO-NH cross-linking method. We verified its potential as a hemostatic agent both in vivo and in vitro . The results demonstrated that the prepared carboxymethyl chitosan/cellulose nanofiber composite (CNF-CMCS) sponges could absorb blood, quickly expand to exert pressure in the wound, and exhibit an excellent coagulation ability. The CNF-CMCS sponges significantly decreased the bleeding time and blood loss in several hemorrhage models and possessed a significant advantage in treating the deep penetrating injury hemorrhage. Therefore, the sponges provide a unique application prospect and potential as a penetrating trauma hemostatic agent.

Developing porous hemostatic sponges that are both biosafe and multifunctional remains a complex challenge. Conventional hemostatic techniques often fall short in managing bleeding effectively, leading to severe critical cases such as suboptimal hemostasis, increased infection risk, and complications arising from profuse bleeding. To address these deficits, our study introduces a novel multifunctional nanocomposite sponge that synergistically incorporates chitosan (CS), cellulose (Cel), graphene oxide (GO), and silver (Ag) nanoparticles. The resulting CS/Cel/GO/Ag developed demonstrates a swelling rate exceeding 3000 %, an absorption rate of over 2100 %, and the lowest stress surpassing 20 kPa at an initial 80 % strain. In vitro analyses reveal that the CS/Cel/GO/Ag sponge has excellent cytocompatibility, non-hemolytic nature, and competence in blood cell adherence and bacterial inhibition. In vivo evaluations further demonstrate that compared to conventional hemostatic methods, the sponge substantially enhances hemostatic efficacy, as evidenced by the marked reductions in clotting times and diminished blood loss compared to conventional hemostatic methods. Specifically, the test results of the CS/Cel/GO/Ag sponge across three different models are as follows: for the rat tail amputation model, the clotting time was 99 s, while blood loss was 222 mg; for the rat liver injury model, the clotting time was 129 s. while blood loss was 812 mg; for the rat femoral artery laceration model, the clotting time was 96 s, while blood loss was 758 mg. The compelling attributes of the CS/Cel/GO/Ag sponges position them as a promising solution for the acute management of bleeding. Their excellent performance indicates they have potential role to play in trauma care.

It is important to control immediate hemorrhage and prevent infection simultaneously in the wound management. However, most of hemostatic materials are associated with low efficiency of hemostasis, poor biocompatibility and lack of antimicrobial properties. A kind of starch-based macroporous sponges (KR-Sps) immobilized covalently with antimicrobial peptide KR12 using highly efficient thiol-ene photo click reaction were developed. The physical properties of these sponges could be fine-tuned by varying the ratio of modified starch/HS-PEG-SH and the polymer concentration. The in vitro and vivo results demonstrated that KR-Sps induced thrombosis, shortened clotting time and reduced the blood loss at bleeding site. Besides, KR12 immobilized sponge exhibited inherent antimicrobial properties against Gram (+) and (−) bacteria and methicillin-resistant Staphylococcus aureus ( MRSA ), which could maintain at least 5 days. Therefore, KR-Sps were believed to be an excellent candidate as hemostatic and antimicrobial product for the intraoperative wound management.

Fish skin collagen has attracted a wide range of attention in the field of hemostatic materials due to its excellent biocompatibility and hemostatic performance. In this work, the hemostatic cotton based on poly(vinyl alcohol)/soluble starch-fish skin collagen (PVA/SS-FSC) composites was prepared by solvent displacement followed by lyophilization. The experimental results showed that the PVA/SS enabled the composite material to have 3-D porous structures. When compounding with FSC, PVA/SS-FSC(1) composite cotton (mass ratio 1:1:1) possessed 3-D porous structures, good water absorption, and no cytotoxicity or hemolytic properties. In vitro and in vivo experiments demonstrated that the PVA/SS-FSC(1) hemostatic cotton accelerated the coagulation rate, reduced blood loss, and improved the hemostatic efficiency. Furthermore, arterial hemostasis tests in rats confirmed that the PVA/SS-FSC(1) hemostatic cotton afforded a shorter hemostasis time (64 ± 7 s) and lower blood loss (323 ± 38 mg) compared with commercial hemostatic gauze (210 ± 12 s and 840 ± 41 mg, respectively), thus indicating promising applications in rapid wound hemostasis.

Clay minerals have attracted wide attention as biomedical materials due to the unique crystal structure, abundant morphology and good biocompatibility. However, the relevant studies on the abundant natural mixed clay deposits were scarcely reported. Herein, the hemostatic performance of natural mixed-dimensional attapulgite clay (MDAPT) composed of one-dimensional attapulgite and multiple two-dimensional clay were systematically investigated based on the structural evolution using oxalic acid for different time. The results of hemostatic evaluation showed that MDAPT leached by oxalic acid with 1 h presented the shortest clotting time (134 ± 12.17 s), a 15.09 % and 41.74 % reduction of relative hemoglobin absorbance at 180 s and 120 s when compared with the control group, respectively, and an increase of 19.45 % of the blood clotting index in vitro , as well as MDAPT obtained the shortest bleeding time (158.5 ± 6.9 s), nearly 66 % and 31 % reduction blood loss as compared to the blank group and the YNBY group in vivo . This improvement was primarily ascribed to the synergistic effect of lamellar non-expandable illite, and nano rod-like attapulgite. Furthermore, the rapid hemostasis of MDAPT was also due to the joint effect of superhydrophobic property toward blood, minimizing blood loss, surface negative charge, metal ions from MDAPT structural skeleton, promoting an average increase of 21 % for platelet activation. The results suggested that MDAPT could be served as a promising efficient inorganic hemostatic materials, which provided a feasible strategy to realize the high-valued utilization of natural mixed clay resources.

In emergencies, uncontrolled severe bleeding can result in undesired complications and even death of the injured. Designing advanced hemostatic agents is a potential solution for emergency hemostasis, yet it remains challenging to realize the persistent adhesion in a wet wound environment. In this study, based on dynamic reversible Schiff base bond and photo-initiated double-bond polymerization, a novel injectable hemostatic hydrogel ( L -COC) consisting of methacrylated carboxymethyl chitosan (CMCSMA), oxidized konjac glucomannan (OKGM) and (+)-catechin hydrate (CH) was synthesized for emergency hemostasis. To our delight, the incorporated CH imparted enhanced blood procoagulantion to the L -COC hydrogel by intensifying the hydrogel-red blood cell interactions. As a result, the hemostatic effect of the engineered L -COC hydrogel was significantly superior to that of fluid gelatin Surgiflo TM for liver bleeding wounds in rats (Blood loss: 0.62 ± 0.11 g ( L -COC), 0.90 ± 0.08 g (Surgiflo TM ); hemostasis time: 69.0 ± 2.9 s ( L -COC), 84.0 ± 2.2 s (Surgiflo TM )). With the favorable antioxidant and antibacterial activities, as well as multifunctional properties, the bio-adhesive L -COC hydrogel and the underlying design principles may facilitate further development of practical hemostatic hydrogels.
