Intracranial pressure (ICP) during the interaction between blast wave and the head is a crucial evaluation criterion for blast-induced traumatic brain injury (bTBI). ICP variation is mainly induced by the blast wave transmission and skull deformation. However, how the skull deformation influences the ICP remains unclear, which is meaningful for mitigating bTBI. In this study, both experimental and numerical models are developed to elucidate the effect of skull deformation on ICP variation. Firstly, we performed the shock tube experiment of the high-fidelity surrogate head to measure the ICP, the blast overpressure, and the skull surface strain of specific positions. The results show that the ICP profiles of all measured points show oscillations with positive and negative change, and the variation is consistent with the skull surface strain. Further numerical analysis reveals that when the blast wave reaches the measured point, the peak overpressure transmits directly through the skull to the brain, forming the local positive ICP peak, and the impulse induces the local inward deformation of the skull. As the peak overpressure passes through, the blast impulse impacts the nearby skull supported by the soft and incompressible brain tissue and extrudes the skull outward in the initial position. The inward and outward skull deformation leads to the oscillation of ICP. These numerical analyses agree with experimental results, which explain the appearance of negative and positive ICP peaks and the synchronization of negative ICP with surface strain. The study has implications for medical injury diagnosis and protective equipment design.

Modern optical imaging techniques provide powerful tools for observing cortical structure and functions at high resolutions. Various skull windows have been established for different applications of cortical imaging, and each has its advantages and limitations. Most critical of the limitations, none of the current skull windows is suitable for observing the responses to some acute craniocerebral injuries on a large scale and at high resolution. Here, we developed a “Through-Intact-Skull (TIS) window” that enables the observation of an immune response on a bilateral cortical scale and at single-cell resolution after traumatic brain injury without affecting the pathological environment of the brain. The TIS window also has the advantages of craniotomy-freeness, centimeter-field of view, synaptic resolution, large imaging depth, long-term observation capability, and suitability for awake mice. Therefore, the TIS window is a promising new approach for intravital cortical microscopy in basic research in neuroscience.

Transcranial ultrasound imaging and therapy have gained significant attention due to their noninvasive nature, absence of ionizing radiation, and portability. However, the presence of the skull, which has a high acoustic impedance, presents a challenge for the penetration of ultrasound into intracranial tissue. This leads to a low transmission of ultrasound through the skull, hindering energy focusing and imaging quality. To address this challenge, we propose a novel approach that utilizes a flexible matching layer with gradual acoustic impedance to enhance ultrasound transmission through the skull. This matching layer is constructed using Poly(dimethylsiloxane) (PDMS)/tungsten powders as the structural component responsible for the gradual impedance, while agarose serves as the flexible matrix. Our simulation and experimental results demonstrate that the matching layer with an exponential gradual acoustic impedance significantly improves the ultrasound transmission coefficient across a wide frequency range compared to traditional quarter wavelength matching layers. Specifically, at 2 MHz, the maximum transmission coefficient reaches 49.5%, more than four times higher than that of the skull without a matching layer (only 11.7%). Additionally, the good flexibility of our matching layer ensures excellent adhesion to the curved surface of the skull, further enhancing its application potential in transcranial ultrasound imaging and therapy. The improved transmission performance allows for a lower ultrasound transmission power, effectively addressing overheating and safety issues.
