Copper-phenolic coating constructed on silicone urinary catheters to prevent catheter-associated infections

Rui Ding,Haoqi Xu,Jiajun Zhang,Zixi Cai,Pandi Peng,Yunxiu Zhang,Peng Li

Catheter-associated infections (CAIs) are serious issues in clinics, significantly affecting patient morbidity, mortality, and incurring substantial treatment costs. Hence, the development of a novel strategy to endow surfaces with antibacterial function is particularly important for catheter materials. In this work, Tannic acid (TA) and various concentrations of copper (Cu) were used to construct metal-phenolic network (MPN) coatings to impart silicone catheters antibacterial surfaces (TA/Cu 5 , TA/Cu 5 and TA/Cu 25 ). The prepared copper-phenolic coatings exhibited desirable biocompatibility towards mammalian cells and remarkable antibacterial properties against Staphylococcus aureus ( S. aureus ), Escherichia coli ( E. coli ) and even “superbug” such as methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus (VRE). The antibacterial properties of the MPN coatings were attributed to the combined effects of Cu and TA. Among all MPN coated silicone catheters, TA/Cu 25 (10   mM TA and 250   mM CuCl 2 ·2H 2 O) exhibited the highest antibacterial activity with killing ratios of 99.84%, 98.67%, 99.85%, and 99.90% against S. aureus, E. Coli , MRSA and VRE, respectively. Thus, the MPN has significant potential for developing antibacterial surface coatings for silicone catheters, which can effectively mitigate CAIs.

Solvent-Free Fabrication of Self-Regenerating Antibacterial Surfaces Resisting Biofilm Formation

Cuicui Su,Yumin Ye,Haofeng Qiu,Yabin Zhu

Biofilm formation on indwelling medical devices is a major cause of hospital-acquired infections. Monofunctional antibacterial surfaces have been developed to resist the formation of biofilms by killing bacteria on contact, but the adsorption of killed bacterial cells and debris gradually undermines the function of these surfaces. Here, we report a facile approach to produce an antibacterial surface that can regenerate its function after contamination. The self-regenerating surface was achieved by sequential deposition of alternating antibacterial and biodegradable layers of coating using a solvent-free initiated chemical vapor deposition method. As the top antibacterial layer gradually loses its killing ability due to the accumulation of debris, the underlying biodegradable layer degrades, shedding off the top surface layers and exposing another fresh antibacterial surface. Urinary catheters coated with monofunctional and self-regenerating antibacterial coatings both showed more than 99% bacterial killing ability at the initial antibacterial test, but the monofunctional surface lost its killing ability after continued exposure to concentrated bacterial solution, whereas the self-regenerating surfaces regained strong bacterial killing ability after prolonged exposure. Employing poly(methacrylic anhydride) and its copolymers with varied composition as the degrading layer, the degradation kinetics can be well-tailored and the self-regeneration duration spanned from minutes to days. The designed self-regenerating antibacterial surfaces could provide an effective approach to resist biofilm formation and extend the service life of indwelling medical devices.

Dual-Function Hydrogel Coating on Silicone Urinary Catheters with Durable Antibacterial Property and Lubricity

Shuai Gao,Wei Zeng,Zheng Liu,Fanjun Zhang,Yunfeng Zhang,Xi Liu,Dimeng Wu,Yunbing Wang

Silicone urinary catheters are broadly employed in medical practice. However, they are susceptible to inducing catheter-associated urinary tract infections (CAUTIs) due to bacterial adherence to the catheter’s surface, and they exhibit a high friction coefficient, which can greatly affect their effectiveness and functionality. Thus, the development of a silicone urinary catheter with antibacterial properties and lubricity is in strong demand. We hereby developed a poly(vinyl acetate) carrier coating to load chlorhexidine acetate and applied a hydrogel coating primarily composed of polyvinylpyrrolidone (PVP) and poly(ethylene glycol) diacrylate (PEGDA), which was then coated onto the silicone urinary catheters and cured through a thermal curing process and could provide lubricity. Subsequently, we analyzed its surface characteristics and assessed the antibacterial property, lubricity, cytotoxicity, and potential for vaginal irritation. The findings from the Fourier transform infrared spectrometer (FTIR), scanning electron microscope (SEM), water contact angle (WCA), inhibition zone measurements, and friction coefficient analysis confirmed the successful modification of the silicone urinary catheter. Additionally, the outcomes from the cytotoxicity and vaginal irritation assessments demonstrated that the dual-function hydrogel coating-coated silicone urinary catheters exhibit outstanding biocompatibility. This study illustrates that the prepared silicone urinary catheters possess durable antibacterial properties and lubricity, which thus gives them broad clinical application prospects.

Mucus Mimic Hydrogel Coating for Lubricous, Antibiofouling, and Anti-Inflammatory Urinary Catheters

Sijia Zheng,Ying Liu,Jun Yao,Ruiying Zhu,Xing Yu,Zhihai Cao

Silicone is a common elastomer used in indwelling urinary catheters, and catheters are widely used in various medical applications due to their exceptional biocompatibility, hypoallergenic properties, and flexibility. However, silicones exhibit hydrophobic characteristics, lack inherent biolubrication, and are susceptible to nonspecific biosubstance adsorption, resulting in complications including but not limited to tissue trauma, postoperative pain, and urinary tract infections (UTIs). The development of effective surface designs for biomedical catheters to mitigate invasive damage and UITs has been a longstanding challenge. Herein, we present a novel approach to prepare a mucus mimic hydrogel coating. A thin layer of hydrogel containing xylitol is fabricated via photopolymerization. The surface modification technique and the interface-initiated hydrogel polymerization method ensure robust interfacial coherence. The resultant coating exhibits a low friction coefficient (CoF ≈ 0.1) for urinary catheter applications. Benefiting from the hydration layer and the antifouling of the xylitol unit, the xylitol hydrogel-coated surfaces (pAAAMXA) demonstrate outstanding antibiofouling properties against proteins (98.9% reduction relative to pristine polydimethylsiloxane (PDMS)). Furthermore, the pAAAMXA shows general adhesion resistance against bacteria primarily responsible for UITs (Escherichia coli (E. coli), Staphylococcus aureus (S. aureus), Methicillin-resistant strains of Staphylococcus aureus (MRSA), and Staphylococcus epidermidis (S. epidermidis)) without compromising biotoxicity (cell viability 98%). In vivo, catheters coated with the mucus mimic hydrogel displayed excellent biocompatibility, resistance to adhesion of bio substance, and anti-inflammatory characteristics. This work describes a promising alternative to conventional silicone catheters, offering potential for clinical interventional procedures with minimized complications.

A feasible strategy of Ag nanoparticles-sodium alginate-polyacrylamide-polyvinyl alcohol hydrogel coatings for preventing catheter-associated urinary tract infections

Hong Yang,Yao Chen,Yongwei Cai,Jianxiang Li,Lei Qiu,Yimeng Wang,Yuanju Li,Danni Qu,Ruru Wang,Qi Zhao

Catheter-associated urinary tract infections (CAUTIs) pose a significant clinical challenge, potentially resulting in complications such as bacteriuria and mortality. Developing coatings with antibacterial and antiadhesive properties for urinary catheters is essential to address these issues. In this study, a novel hydrogel coating composed of silver nanoparticles (Ag NPs), sodium alginate (SA), polyacrylamide (PAAm), and polyvinyl alcohol (PVA) was prepared on polydopamine (PDA)-modified latex urinary catheters using a simple dipping method. The Ag NPs were synthesized via an in-situ reduction of AgNO 3 with SA, eliminating the need for additional chemical reductants. The Ag NPs-SA-PAAm-PVA hydrogel-coated urinary catheter demonstrated superior antibacterial and antiadhesive characteristics compared to the bare urinary catheter. The antiadhesive properties of the hydrogel-coated urinary catheter were attributed to its hydrophilicity and lubricity, while the antibacterial effects were linked to the penetration of Ag NPs and Ag + ions into bacterial structures, disrupting bacterial metabolism through the generation of reactive oxygen species (ROS) and free radicals upon attachment of Ag + ions to cell walls and membranes. Furthermore, the hydrogel coatings exhibited remarkable mechanical properties, along with good biocompatibility and hemocompatibility. The exceptional antibacterial and antiadhesive properties of the hydrogel-coated urinary catheter hold promise for reducing CAUTIs.

Rectifying disorder of extracellular matrix to suppress urethral stricture by protein nanofilm-controlled drug delivery from urinary catheter

Tian Juanhua,Fu Delai,Liu Yongchun,Guan Yibing,Miao Shuting,Xue Yuquan,Chen Ke,Huang Shanlong,Zhang Yanfeng,Xue Li,Chong Tie,Yang Peng

Urethral stricture secondary to urethral injury, afflicting both patients and urologists, is initiated by excessive deposition of extracellular matrix in the submucosal and periurethral tissues. Although various anti-fibrotic drugs have been applied to urethral stricture by irrigation or submucosal injection, their clinical feasibility and effectiveness are limited. Here, to target the pathological state of the extracellular matrix, we design a protein-based nanofilm-controlled drug delivery system and assemble it on the catheter. This approach, which integrates excellent anti-biofilm properties with stable and controlled drug delivery for tens of days in one step, ensures optimal efficacy and negligible side effects while preventing biofilm-related infections. In a rabbit model of urethral injury, the anti-fibrotic catheter maintains extracellular matrix homeostasis by reducing fibroblast-derived collagen production and enhancing metalloproteinase 1-induced collagen degradation, resulting in a greater improvement in lumen stenosis than other topical therapies for urethral stricture prevention. Such facilely fabricated biocompatible coating with antibacterial contamination and sustained-drug-release functionality could not only benefit populations at high risk of urethral stricture but also serve as an advanced paradigm for a range of biomedical applications.

Antibacterial AgNPs-PAAm-CS-PVP nanocomposite hydrogel coating for urinary catheters

Yongwei Cai,Hong Yang,Jianxiang Li,Ronghua Gu,Yuhang Dong,Qi Zhao,Yao Chen,Yuanju Li,Ruru Wang

Catheter-associated urinary tract infections (CAUTI) account for 40% of all hospital-acquired infections and more than 1 million CAUTIs occur annually in the United States and Europe with increased morbidity and mortality. Although anti-bacterial coating is an effective way to solve this problem, so far only limitted progress has been made. In this paper we prepared a novel antibacterial silver nanoparticles poly(acrylamide)-chitosan-polyvinylpyrrolidone (AgNPs-PAAm-CS-PVP) composite hydrogel coating on latex urinary catheters by a simple dipping method. The average particle size, the size distribution as well as the polydispersity index (PDI) value of the AgNPs were examined by using TEM analysis. The surface morphology and roughness, the element contents, the functional groups, the static contact angles, and the surface energy of the coatings were characterized by using SEM, EDS, AFM, FTIR, XPS, and a video optical contact angle instrument, respectively. The amount of the total Ag released from the hydrogel was tested by ICP-MS. The antibacterial and antiadhesion efficacies against Gram-negative E. coli of the coated urinary catheters were evaluated with an inhibition zone test and fluorescence microscopy. A synergistic antibacterial mechanism of AgNPs and CS was also presented. The results showed that the synthesized AgNPs in the hydrogel had a spherical shape with an average diameter of 25.9 nm. The AgNPs-PAAm-CS-PVP hydrogel had a more durable release of the antibacterial agents of AgNPs and Ag + , which explains why the coated urinary catheters had long-term antibacterial activity. The mathematical modelling of Ag release kinetics supported Case I or Fickian diffusion mechanism for the AgNPs-PAAm-CS-PVP hydrogel. The hydrogel-coated urinary catheters became smoother and more hydrophilic which further explains why the coated urinary catheter decreased bacterial adhesion. More importantly, bacterial activity and bacterial adhesion assays showed that both the outer and inner surfaces of the AgNPs-PAAm-CS-PVP hydrogel-coated urinary catheters had strong antibacterial property against E. coli . Furthermore, the hydrogel-coated urinary catheters showed high hemocompatibility and cytocompatibility for promising biomedical applications. The AgNPs-PAAm-CS-PVP hydrogel-coated urinary catheters have great potential for decreasing CAUTIs.
