Catheter-associated infections (CAIs) are serious issues in clinics, significantly affecting patient morbidity, mortality, and incurring substantial treatment costs. Hence, the development of a novel strategy to endow surfaces with antibacterial function is particularly important for catheter materials. In this work, Tannic acid (TA) and various concentrations of copper (Cu) were used to construct metal-phenolic network (MPN) coatings to impart silicone catheters antibacterial surfaces (TA/Cu 5 , TA/Cu 5 and TA/Cu 25 ). The prepared copper-phenolic coatings exhibited desirable biocompatibility towards mammalian cells and remarkable antibacterial properties against Staphylococcus aureus ( S. aureus ), Escherichia coli ( E. coli ) and even “superbug” such as methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus (VRE). The antibacterial properties of the MPN coatings were attributed to the combined effects of Cu and TA. Among all MPN coated silicone catheters, TA/Cu 25 (10 mM TA and 250 mM CuCl 2 ·2H 2 O) exhibited the highest antibacterial activity with killing ratios of 99.84%, 98.67%, 99.85%, and 99.90% against S. aureus, E. Coli , MRSA and VRE, respectively. Thus, the MPN has significant potential for developing antibacterial surface coatings for silicone catheters, which can effectively mitigate CAIs.

Ureteral stricture is a common urological condition caused by the proliferation of scar tissue at the site of the stenosis. Effectively inhibition of scar tissue proliferation at ureter is key to preventing stenosis after ureteral injury. Retention of an anti-fibrotic-loaded ureteral stent tube that gradually releases the drug can inhibit scar tissue proliferation, thus preventing and treating ureteral stricture. In this study, double layers drug release system was used to carry pirfenidone to create ureteral stent tubes for preventing ureteral stricture. Long-term sustained release of pirfenidone was assessed, and the anti-stenosis drug was continuously released by the stent, which also provided drainage support. Drug release data showed that the stent tube experiments rapidly released pirfenidone over the first 5 days, with the release becoming stable and gradual over the subsequent 9 days. In vitro data showed that the stent tube had a sustained inhibitory effect on human ureteral smooth muscle cells, a significant inhibitory effect on macrophages and an anti-inflammatory factor release effect. More importantly, in vivo data showed that the stent tube had a significant inhibitory effect on ureteral stricture formation in a model of porcine ureteral stricture. These results suggested that the proposed stent tube, a novel pirfenidone sustained-release stent, can effectively inhibit the formation and progression of ureteral stenosis in vitro and in vivo .

Urinary catheters necessitate a surface with antifouling and lubricating properties in order to mitigate bacterial infection, calculus formation, and tissue trauma. Hydrophilic coating is a promising strategy but suffers from the complications of the manufacturing method as well as the lack of longevity. Herein, we report an antifouling and lubricating hydrophilic coating based on zwitterionic copolymer poly(2-Methacryloyloxyethyl phosphatidylcholine- co -hydroxyethyl methacrylate-co-(Benzoyl)phenyl methacrylate (poly(MPC- co -HEMA- co -BP)) through pre-gel solution spin-coating followed by UV-curing. The poly(MPC- co -HEMA- co -BP (PMHB) coatings possess hygroscopic properties that facilitate the formation of a hydrated layer, thereby leading to friction reduction. Moreover, both antibacterial and protein resistance tests substantiate the exceptional antifouling characteristics exhibited by the PMHB coating. Furthermore, after 7 days of coincubation, the antibacterial rates of PMHB coatings against Escherichia coli (E. coli) , Staphylococcus aureus (S. aureus) , and Proteus mirabilis (P. mirabilis) are 72.98 %, 75.11 %, and 88.23 %, respectively. Subsequently, PMHB coating reduced the attachment of encrustation in a 7 days artificial urine cycling experiment. This study brings new perspective on designing urinary catheter surfaces with lubricating and antifouling properties.

Biofilm formation on indwelling medical devices is a major cause of hospital-acquired infections. Monofunctional antibacterial surfaces have been developed to resist the formation of biofilms by killing bacteria on contact, but the adsorption of killed bacterial cells and debris gradually undermines the function of these surfaces. Here, we report a facile approach to produce an antibacterial surface that can regenerate its function after contamination. The self-regenerating surface was achieved by sequential deposition of alternating antibacterial and biodegradable layers of coating using a solvent-free initiated chemical vapor deposition method. As the top antibacterial layer gradually loses its killing ability due to the accumulation of debris, the underlying biodegradable layer degrades, shedding off the top surface layers and exposing another fresh antibacterial surface. Urinary catheters coated with monofunctional and self-regenerating antibacterial coatings both showed more than 99% bacterial killing ability at the initial antibacterial test, but the monofunctional surface lost its killing ability after continued exposure to concentrated bacterial solution, whereas the self-regenerating surfaces regained strong bacterial killing ability after prolonged exposure. Employing poly(methacrylic anhydride) and its copolymers with varied composition as the degrading layer, the degradation kinetics can be well-tailored and the self-regeneration duration spanned from minutes to days. The designed self-regenerating antibacterial surfaces could provide an effective approach to resist biofilm formation and extend the service life of indwelling medical devices.

To minimize friction when silicone-based medical devices interact with soft tissues, coating their hydrophobic surfaces with lubricants has become a widely adopted method. In this study, a vesicle-like polysaccharide derivative biolubricant (HPC-PSBMA) is synthesized by combining hydrophilic monomers with hydroxypropyl cellulose (HPC) as the main chain. HPC-PSBMA exhibits good dispersibility in water, and it can be adsorbed onto the surface of silicone rubber. By integrating molecular dynamics simulations with experimental data, the adsorption and film-forming mechanisms are uncovered. Furthermore, HPC-PSBMA showed a lower coefficient of friction (COF∼0.02) on the surface of urinary catheters when compared with commercially available lubricants (COF∼0.05). Consequently, this work offers a novel approach to improving surface lubrication of hydrophobic medical devices with broad potential applications.

Silicone urinary catheters are broadly employed in medical practice. However, they are susceptible to inducing catheter-associated urinary tract infections (CAUTIs) due to bacterial adherence to the catheter’s surface, and they exhibit a high friction coefficient, which can greatly affect their effectiveness and functionality. Thus, the development of a silicone urinary catheter with antibacterial properties and lubricity is in strong demand. We hereby developed a poly(vinyl acetate) carrier coating to load chlorhexidine acetate and applied a hydrogel coating primarily composed of polyvinylpyrrolidone (PVP) and poly(ethylene glycol) diacrylate (PEGDA), which was then coated onto the silicone urinary catheters and cured through a thermal curing process and could provide lubricity. Subsequently, we analyzed its surface characteristics and assessed the antibacterial property, lubricity, cytotoxicity, and potential for vaginal irritation. The findings from the Fourier transform infrared spectrometer (FTIR), scanning electron microscope (SEM), water contact angle (WCA), inhibition zone measurements, and friction coefficient analysis confirmed the successful modification of the silicone urinary catheter. Additionally, the outcomes from the cytotoxicity and vaginal irritation assessments demonstrated that the dual-function hydrogel coating-coated silicone urinary catheters exhibit outstanding biocompatibility. This study illustrates that the prepared silicone urinary catheters possess durable antibacterial properties and lubricity, which thus gives them broad clinical application prospects.

Prevention of encrustation on the surface has always been the biggest challenge for urological implants. In the field of ureteral stent design, biodegradability has attracted much attention in recent years, because biodegradable ureteral stents not only avoid secondary intervention, but also prevent encrustation due to surface renewal by degradation process. Furthermore, researches have focus on some surface parameters to provide guidance for the development of stent materials, such as hydrophilicity or surface charge. In this work, we synthesized two types of poly(ester-carbonate)s, poly(L-lactide-co-5-amino-1,3-dioxan-2-one) (P(LA-co-AC)) containing amino, and poly (L-lactide-co-5-methyl-5-carboxyl-1,3-dioxan-2-one) (P(LA-co-MCC)) containing carboxyl. Blending P(LA-co-AC) and P(LA-co-MCC) with poly(L-lactide-co-Ɛ-caprolactone) (PLACL) respectively, two types of ureteral stent materials were prepared. Due to the influence of ions formed by the dissociation of amino and carboxyl, two types of materials show differences in surface charge analyses. We further developed a dynamic urinary extracorporeal circulation (DUEC) system to assess in vitro encrustation of materials with different surface charges. The results of this comparative study identified that the materials with strong negative surface charge were most favorable for use as ureteral stent, and provided a new approach to surmount the problems faced by urological surgery which complied with the future trend of biodegradable ureteral stent design.

Commercial ureteral stents frequently encounter clinical issues, such as mucosal injury due to friction, bacterial colonization resulting in infections, and mineral encrustation compromising long-term use. To effectively address these challenges, we developed a bridge-like copolymer coating, namely, P(DMA-bMPC-bDMA) (PDMD), which was synthesized via reversible addition–fragmentation chain transfer polymerization. The PDMD architecture strategically positioned dopamine methacrylamide groups at both ends, generating a “molecular clamp” that provided robust dual-end self-adhesion and ensured uniform, stable anchoring to the polyurethane substrate. Compared to conventional single-end adhesive coatings, this “molecular clamp” approach significantly enhanced the integrity and durability under dynamic physiological conditions. In addition, the bridge-like structure also optimally introduced the central segment of 2-methacryloyloxyethyl phosphorylcholine, forming a highly effective hydration layer that dramatically reduced friction by 92% (coefficient of friction ∼ 0.032) and acted as a physical barrier to inhibit bacterial adhesion. Consequently, the PDMD-modified polyurethane samples inhibited the adhesion of common uropathogens such as Escherichia coli (>98.8%), Staphylococcus aureus (>92.2%), and Proteus mirabilis (>98.5%) and effectively prevented the formation of encrustation in dynamic in vitro and in vivo urinary tract infection models. Overall, this multifunctional surface treatment combining dual-end self-adhesion stability with both superlubrication and antifouling performances provides a substantial improvement over previous traditional coatings, enhancing biocompatibility, reducing complications, and markedly improving patient comfort and clinical outcomes for commercial ureteral stents.

Renal fibrosis is the most common pathway for the development of end-stage renal disease (ESRD) in various kidney diseases. Currently, the treatment options for renal fibrosis are limited. Ferroptosis is iron-mediated lipid peroxidation, triggered mainly by iron deposition and ROS generation. Notably, the kidney is the most sensitive of all tissues to iron-dependent ferroptosis, and the inhibition of ferroptosis is an effective therapeutic strategy for the treatment of kidney fibrosis. Nonetheless, the pathways involved in ferroptosis in renal fibrosis are still unclear. Bergapten, a natural coumarin derivative, is mainly found in bergapten essential oil, grapefruit juice, and other commonly used plants, and it has various pharmacological effects. However, the role that ferroptosis plays in renal fibrosis and the potential therapeutic benefits of bergapten remain unclear. In this study, we investigated the therapeutic effects of bergapten on renal fibrosis and its mechanisms. We investigated the anti-fibrotic effects of bergapten in in vivo and in vitro models of renal fibrosis. Initially, network pharmacological analysis was employed to predict the potential therapeutic impact of bergapten on renal fibrosis. We then explored the potential therapeutic role of bergapten in obstructive nephropathy, which is due to unilateral ureteral obstruction (UUO). Furthermore, RNA-Seq was conducted to investigate the possible mechanism of bergapten against renal fibrosis. Additionally, Bergapten demonstrated a significant improvement in TGF-β1-induced fibrosis and RSL3-induced renal tubular epithelial cell ferroptosis; these findings are consistent with those of the in vivo studies. Our findings indicate that bergapten is a potential treatment for renal fibrosis. Treatment with bergapten significantly reduced the expression of fibronectin and α-SMA in the damaged kidneys of UUO mice, thereby improving fibrosis. Meanwhile, bergapten protected against fibrosis caused by TGF-β1 and ferroptosis induced by glutathione peroxidase 4 (GPX4) inhibitor RSL3. Significantly, bergapten therapy alleviated renal fibrosis by modulating ferroptosis. We found that bergapten inhibited PI3K phosphorylation and indirectly restored GPX4 expression. In conclusion, we have revealed the nephroprotective effect of bergapten, whose mechanism of action is related to the inhibition of ferroptosis, and it is expected that it will be developed as a therapeutic agent for the treatment of renal fibrosis. This study aims to explore the effect of bergapten on renal fibrosis ferroptosis. Collectively, these results demonstrate that bergapten is an inhibitor of ferroptosis and provides a new treatment strategy for diseases associated with ferroptosis.

Silicone is a common elastomer used in indwelling urinary catheters, and catheters are widely used in various medical applications due to their exceptional biocompatibility, hypoallergenic properties, and flexibility. However, silicones exhibit hydrophobic characteristics, lack inherent biolubrication, and are susceptible to nonspecific biosubstance adsorption, resulting in complications including but not limited to tissue trauma, postoperative pain, and urinary tract infections (UTIs). The development of effective surface designs for biomedical catheters to mitigate invasive damage and UITs has been a longstanding challenge. Herein, we present a novel approach to prepare a mucus mimic hydrogel coating. A thin layer of hydrogel containing xylitol is fabricated via photopolymerization. The surface modification technique and the interface-initiated hydrogel polymerization method ensure robust interfacial coherence. The resultant coating exhibits a low friction coefficient (CoF ≈ 0.1) for urinary catheter applications. Benefiting from the hydration layer and the antifouling of the xylitol unit, the xylitol hydrogel-coated surfaces (pAAAMXA) demonstrate outstanding antibiofouling properties against proteins (98.9% reduction relative to pristine polydimethylsiloxane (PDMS)). Furthermore, the pAAAMXA shows general adhesion resistance against bacteria primarily responsible for UITs (Escherichia coli (E. coli), Staphylococcus aureus (S. aureus), Methicillin-resistant strains of Staphylococcus aureus (MRSA), and Staphylococcus epidermidis (S. epidermidis)) without compromising biotoxicity (cell viability 98%). In vivo, catheters coated with the mucus mimic hydrogel displayed excellent biocompatibility, resistance to adhesion of bio substance, and anti-inflammatory characteristics. This work describes a promising alternative to conventional silicone catheters, offering potential for clinical interventional procedures with minimized complications.

Ureteral stents are prone to encrustation due to urinary crystallization and bacterial infection, leading to severe complications and restricting long-term use. In this study, a multifunctional hydrogel-based coating was developed, which integrates the crosslinking of polyacrylic acid coupled with zinc ions (PAA(Zn 2+ )) possessing antibacterial properties and polyvinylpyrrolidone (PVP) with hydrophilic lubrication properties on the PU stent (PPAZ/PU). This novel PPAZ/PU coating was devised to mitigate the challenges of bacterial infectious encrustation associated with stent placement in clinical settings. The coating was synthesized via complex decomposition and addition polymerization, forming a stable zinc-ion-rich crosslinked network. The intertwining of PVP and PAA(Zn²⁺) molecular chains, along with hydrogen bond formation, enhanced the hydrogel's cohesion and peel strength up to 1000 N/m. The hydrophilicity and lubricity of the PPAZ/PU-coated stent was markedly enhanced for clinical applications, with a reduction in contact angle and friction coefficient by a factor of 3.25 and 2.62, respectively. In vitro antibacterial assays and simulated infectious encrustation experiments revealed that the PPAZ/PU-coated stent had an antibacterial efficacy of over 99% and halved the surface encrustation per unit area. This PPAZ/PU hydrogel-based coating strategy presents an efficacious solution to the anti-encrustation demands of ureteral stents, particularly in severe urinary tract infection scenarios.

Background Renal fibrosis is a hallmark of chronic kidney disease (CKD). Smad3 serves as the principal transcription factor mediating the pro-fibrosis effects of TGF-β signaling in renal fibrosis. Biochanin A (BCA), a natural isoflavone, has been shown to attenuate renal fibrosis by inhibiting TGF-β signaling but the detailed mechanisms remain unresolved. This study aimed to elucidate the specific mechanisms by which BCA modulates TGF-β signaling. Methods Renal fibrosis models were established both in vitro , using TGF-β1-stimulated mouse renal tubular TCMK1 cells, and in vivo , employing mice with unilateral ureter obstruction (UUO). RNA-seq was conducted to identify BCA-regulated genes. The AnimalTFDB4.0 database was utilized to predict transcription factors with potential binding to Smad3 promoter. The activities of TGF-β signaling and the cloned mouse Smad3 promoter were assessed using luciferase reporter assays. Plasmid transfection was performed using polyethylenimine in TCMK1 cells or ultrasound microbubbles in UUO kidneys. Gene expression was analyzed by RT-PCR, western blot, and immunohistochemistry assays. Results BCA significantly inhibited TGF-β signaling activity and suppressed TGF-β1-induced fibrotic gene expression in TCMK1 cells. RNA-seq and in silico analyses identified Smad3 as the key gene downregulated by BCA, while leaving Smad2 unaffected. This selective transcriptional suppression of Smad3 by BCA was validated by luciferase reporter assays using the cloned Smad3 promoter. Furthermore, transcription factor binding prediction identified that Klf6, a transcription factor downregulated by BCA, has binding potential to the Smad3 promoter and promotes Smad3 transcription. Klf6 expression was induced in TGF-β1-stimulated TCMK1 cells and UUO kidneys, but this induction was abolished upon BCA treatment. Importantly, Klf6 overexpression restored Smad3 expression and counteracted the anti-fibrosis effects of BCA in both TGF-β1-stimulated TCMK1 cells and UUO kidneys. Conclusion TGF-β-responsive Klf6 transcriptionally transactivates Smad3 expression. BCA exerts anti-renal fibrosis effects by inhibiting the Klf6-Smad3 signaling axis, underscoring its therapeutic potential in the treatment of CKD.
