Rapid and accurate screening for urinary tract infections (UTIs) is crucial for effective treatment, yet traditional urine culture methods are slow and resource-intensive. This study presents NH 2 -TPE-SPy , a novel nitrite-responsive fluorescent probe designed for rapid UTI screening. By leveraging nitrite-mediated fluorescence quenching through diazonium salt formation, NH 2 -TPE-SPy demonstrates high sensitivity and selectivity for nitrite detection, even in complex biological matrices. Integrated into a smartphone-compatible point-of-care testing (POCT) platform, the probe enables rapid, cost-effective, and user-friendly UTI screening, yielding results within minutes. Evaluation using simulated and clinical urine samples revealed a sensitivity of 78.18% and a specificity of 86.02%, suggesting its potential as a reliable preliminary diagnostic tool, particularly for resource-limited settings. This POCT system facilitates prompt screening, informs timely clinical decisions, and may help mitigate the overuse of empirical antibiotics. Further research will explore the applicability of NH 2 -TPE-SPy to other diagnostic markers, broadening its clinical diagnostic potential.

Long-term indwelling catheters or stents often cause complications like infection, encrustation, hematuria, pain, and so on. The source of these problems is bacteria, which can form biofilms on the stents to reduce antibiotic sensitivity and produce urease to form encrustation by increasing the urine pH. Urinary tract infection (UTI) can aggravate the body damage and even seriously endanger lives, and the encrustation will block the stents, which can cause hydronephrosis and renal function damage. Therefore, the prevention of UTI and encrustation represents a great challenge in clinical ureteral stent uses. In this work, a clickable mussel-inspired peptide and antimicrobial peptide (AMP) were used to functionalize the commercial stents’ surfaces to inhibit long-term infection and encrustation caused by bacteria. Copper (Cu) ions were used to coordinate the mussel-inspired peptide to improve the stability. The AMP with an azido group was clicked to the mussel-inspired Cu-coordinated peptide coating through click chemistry. The bio-inspired antibacterial coating was constructed with excellent stability, bactericidal properties, and improved biological compatibility. In in vitro and in vivo experiments, it was further found that the coating showed bactericidal and encrustation reduction abilities. This study thus developed an effective, safe, and stable AMP coating on urinary stents/catheters capable of long-term antibacterial and encrustation inhibition.

Urinary tract infections (UTIs) have greatly affected human health in recent years. Accurate and rapid diagnosis of UTIs can enable a more effective treatment. Herein, we developed a multichannel sensor array for efficient identification of bacteria based on three antimicrobial agents (vancomycin, lysozyme, and bacitracin) functional gold nanoclusters (AuNCs). In this sensor, the fluorescence intensity of the three AuNCs was quenched to varying degrees by the bacterial species, providing a unique fingerprint for different bacteria. With this sensing platform, seven pathogenic bacteria, different concentrations of the same bacteria, and even bacterial mixtures were successfully differentiated. Furthermore, UTIs can be accurately identified with our sensors in ∼30 min with 100% classification accuracy. The proposed sensing systems offer a rapid, high-throughput, and reliable sensing platform for the diagnosis of UTIs.

Uropathogenic Escherichia coli (UPEC) is the most common causative agent of urinary tract infection (UTI). UPEC invades bladder epithelial cells (BECs) via fusiform vesicles, escapes into the cytosol, and establishes biofilm-like intracellular bacterial communities (IBCs). Nucleoside-diphosphate kinase (NDK) is secreted by pathogenic bacteria to enhance virulence. However, whether NDK is involved in UPEC pathogenesis remains unclear. Here, we find that the lack of ndk impairs the colonization of UPEC CFT073 in mouse bladders and kidneys owing to the impaired ability of UPEC to form IBCs. Furthermore, we demonstrate that NDK inhibits caspase-1-dependent pyroptosis by consuming extracellular ATP, preventing superficial BEC exfoliation, and promoting IBC formation. UPEC utilizes the reactive oxygen species (ROS) sensor OxyR to indirectly activate the regulator integration host factor, which then directly activates ndk expression in response to intracellular ROS. Here, we reveal a signaling transduction pathway that UPEC employs to inhibit superficial BEC exfoliation, thus facilitating acute UTI.

Urinary tract infections (UTIs) caused by uropathogenic Escherichia coli (UPEC) remain a matter of concern, as the clinical use of multiple antibiotics induces antibiotic resistance in bacteria, resulting in the failure of treatments. Despite the emergence of anti-adhesion strategies that can prevent the development of bacterial drug resistance, these strategies are mainly used for disease prevention rather than effective treatment. Photothermal therapy (PTT) has emerged as an efficient alternative for the elimination of bacteria. Nevertheless, high local temperatures related to PTT probably cause damage to surrounding healthy tissue. Herein, a biomimetic nonantibiotic nanoplatform for low-temperature photothermal treatment of UTIs is developed. The nanoplatform comprises polydopamine (PDA) photothermal core and biphenyl mannoside (Man) shell with multivalent high-affinity to UPEC. Scanning electron microscope (SEM) shows PDA-Man possessed ultra-strong targeting binding ability toward UPEC. It is the fact that this impulse UPEC to form a large bacterial cluster. Consequently, the high photothermal energy of the PDA-Man appears predominantly in the affected bacterial area, while the overall environment remains at a low temperature. The fabricated nanoplatform shows excellent photothermal bactericidal effects, approximately 100% in a UTI model. Overall, this low-temperature photothermal nanoplatform provides an appropriate strategy for the elimination of bacteria in clinical applications.

Urinary tract infections (UTIs) caused by uropathogenic Escherichia coli (UPEC) have become a matter of concern because of the clinical use of multiple antibiotics, which results in antibiotic resistance in bacteria and causes the failure of treatments. Photothermal therapy (PTT) has been a high-potential method in antibacterial therapy on account of its noninvasion and avoiding bacterial resistance. However, high temperatures and nonselectivity related to PTT would cause serious damage to the surrounding healthy tissue. Herein, a surface-adaptive gold-based nanoplatform (AuNPs@GalNAc/DMMA-Cys) by conjugating N-acetylgalactosaminoside (GalNAc) and maleimidized cysteine (DMMA-Cys) on gold nanoparticles for low-temperature photothermal antibacterial therapy was proposed. The AuNPs@GalNAc/DMMA-Cys was characterized by the pH-activated photothermal effect on account of the translation from negative property to zwitterionic structure of DMMA-Cys and high adhesion toward UPEC originating from the recognition of GalNAc to FmlH protein. It can kill UPEC under low photothermal temperature but perform poorly against S. aureus. This approach may develop a promising tool for low-temperature photothermal antibacterial treatment.

This study explored a method that can rapidly detect bacteria in urine samples for the auxiliary determination of urinary tract infections (UTIs). Urine samples from patients with UTIs (230 cases) were obtained using aseptic technique. The urine biochemical assay was then carried out using an automated urine analyzer for all the urine samples. Bacterial species were identified by a combination of bacterial culture technique , morphological observation and the BACT-IST microbial identification/susceptibility analysis system. The most common seven species of bacteria in the study included Escherichia coli , Klebsiella pneumoniae , Pseudomonas aeruginosa , Enterobacter cloacae , Enterococcus faecalis , Staphylococcus aureus and Staphylococcus epidermidis . Bacterial samples were suspended in sample buffer solutions and separated by the “three-plug-injection” method using capillary electrophoresis (CE). Each species of bacteria appeared as a bacterial peak. The mixture of the seven species also provided only one peak. Further analysis showed that the concentration limit for the “three-plug-injection” method is 10 6 colony forming units (CFU)/mL, and there is a good linear relationship between the peak height and bacterial concentration ( R 2 = 0.99). The effect of urine composition on CE results was also investigated. The results showed that urine composition, i.e., proteins, white blood cells (WBCs) and red blood cells (RBCs), affected the peak retention time but could not affect the separation of bacteria. The results demonstrated that the bacteria in urine samples can be detected within 10 min by the “three-plug-injection” method using CE. The “three-plug-injection” method is therefore suitable for the rapid detection of organisms in clinical urine samples from UTIs.

Catheter-associated urinary tract infections (CAUTIs) pose a significant clinical challenge, potentially resulting in complications such as bacteriuria and mortality. Developing coatings with antibacterial and antiadhesive properties for urinary catheters is essential to address these issues. In this study, a novel hydrogel coating composed of silver nanoparticles (Ag NPs), sodium alginate (SA), polyacrylamide (PAAm), and polyvinyl alcohol (PVA) was prepared on polydopamine (PDA)-modified latex urinary catheters using a simple dipping method. The Ag NPs were synthesized via an in-situ reduction of AgNO 3 with SA, eliminating the need for additional chemical reductants. The Ag NPs-SA-PAAm-PVA hydrogel-coated urinary catheter demonstrated superior antibacterial and antiadhesive characteristics compared to the bare urinary catheter. The antiadhesive properties of the hydrogel-coated urinary catheter were attributed to its hydrophilicity and lubricity, while the antibacterial effects were linked to the penetration of Ag NPs and Ag + ions into bacterial structures, disrupting bacterial metabolism through the generation of reactive oxygen species (ROS) and free radicals upon attachment of Ag + ions to cell walls and membranes. Furthermore, the hydrogel coatings exhibited remarkable mechanical properties, along with good biocompatibility and hemocompatibility. The exceptional antibacterial and antiadhesive properties of the hydrogel-coated urinary catheter hold promise for reducing CAUTIs.

Point-of-care testing (POCT) for urinary tract infection (UTI) holds significant importance in the field of disease prevention and control, as well as the advancement of personalized precision medicine. However, conventional methods for detecting UTIs continue to face challenges such as time-consuming and labor-intensive detection processes, and reliance on specialized equipment and personnel rendering them unsuitable for point-of-care applications, especially in resource-limited areas. Here, we propose a novel flexible programmable manually powered microfluidic (FPM) for rapid point-of-care diagnosis of UTIs. For the first time, the proposed FPMs was achieved through a combined strategy of laser printing, cutting, and laminating, with the entire process completed in under 15 min at a cost of less than $0.5, which effectively circumvent the traditionally time-consuming and labor-intensive soft lithography techniques. By incorporating a modular structure-based design concept, we successfully developed various types of portable FPMs with functionalities including parallel pumping, simultaneous releasing, quantitative dispensing, sequential releasing, cyclic motion of multiple liquids and concentration gradient generating. As a proof-of-concept demonstration, we initially employed a high-throughput parallel dispensing design to analyze six urinary biochemical markers within 1 min, presenting potential applicability for future at-home testing. We then integrated a manually powered concentration gradient generator with spatial confinement signal enhancement to enable rapid phenotypic antimicrobial susceptibility testing (AST) within three to 5 h, while achieving clinical diagnostic accuracy rates of up to 95.56%. Therefore, our proposed FPMs eliminate the need for external pumps or actuators and could serve as an affordable hand-held POCT tool for UTI diagnosis. Moreover, in resource-poor areas, they have potential utility as robust POCT devices addressing diverse rapid detection needs.

Catheter-associated urinary tract infection (CAUTI) is a prevalent type of hospital-acquired infection, affecting approximately 15% to 25% of patients with urinary catheters. Long-term use of the catheter can lead to colonization of microorganisms and biofilm formation, and may develop into bacterial CAUTI. However, the frequent replacement of catheters in clinical settings can result in tissue damage, inflammation, ulceration, and additional complications, causing discomfort and pain for patients. In light of these challenges, a novel nanodrug-supported hydrogel coating called NP-AM/FK@OMV-P/H has been developed in this study. Through in vitro experiments, it is confirmed that OMV nano-loaded liquid gel coating has an effective reaction against E.coli HAase and releases antibacterial drugs. This coating has also demonstrated strong inhibition of E.coli and has shown the ability to inhibit the formation of bacterial biofilm. These findings highlight the potential of the OMV nanoparticle gel coating in preventing and treating bacterial infections. Notably, NP-AM/FK@OMV-P/H has exhibited greater efficacy against multidrug-resistant E.coli associated with UTIs compared to coatings containing single antimicrobial peptides or antibiotics. Additionally, it has demonstrated good biosecurity. In conclusion, the NP-AM/FK@OMV-P/H coating holds great potential in providing benefits to patients with CAUTI.

Urinary tract infections (UTIs) are among the most common infectious diseases worldwide, characterized by high recurrence rates and increasing antibiotic resistance. Currently, urine culture remains the gold standard for pathogen diagnosis, yet it suffers from low sensitivity and prolonged turnaround times (16–18 h). To address these limitations, we developed a rapid and interference-free phosphorescence biosensor based on persistent luminescent nanoparticles (PLNPs), enabling accurate bacterial detection in complex clinical samples without autofluorescence or exogenous interference. Polymyxin B was conjugated with both PLNPs and magnetic nanoparticles (MNPs), forming a sandwich-structured detection system: MNPs facilitate magnetic enrichment and separation of pathogens, while PLNPs provide a stable phosphorescent signal for quantification. Using Escherichia coli as a model pathogen, the assay achieved a detection limit of 13 CFU mL −1 within 15 min. Clinical validation using 30 urine specimens showed excellent correlation with standard culture methods. This PLNP-based biosensor not only offers a promising alternative for rapid UTI diagnosis but also paves the way for personalized antimicrobial therapy management.

Background Urinary tract infection (UTI) is a significant global health issue. Urine bacterial culture is time-consuming, prone to misdiagnosis, and the existing sample pretreatment methods are cumbersome, relying on centralized laboratories and specialized equipment. Colorimetric sensing provides a faster alternative. However, its accuracy is often compromised by the variable color of the patient's urine samples. Therefore, it is clear that a rapid, simple, gentle, and environmentally friendly pretreatment method is needed that effectively eliminates color interference in colorimetric detection and is compatible with smartphones, enabling real-time, quantitative on-site monitoring of UTI. (90) Results In this study, we describe a unique smartphone-assisted diagnostic method for quantifying nitrite, leucocyte esterase (LE), and bacteria in urine samples. The method employs activated carbon composite hydrogel spheres for urine pretreatment, which effectively removes background color interference and improves sensor performance, and combined with a smartphone-based microplate analyzer independently developed by our group previously, allows for precise colorimetric examination. This will aid in the prompt detection and dynamic monitoring of urinary tract infections in clinical settings. Furthermore, the bacterial detection time was shortened to 2 h, enhancing the accuracy and timeliness of the diagnostic process. The detection limits of nitrite, LE, and E. coli were 0.0584 mg L-1, 0.0775 Leu μL-1, and 2.94 CFU mL-1, respectively, meeting clinical requirements. Validation of 147 clinical samples indicated high consistency with the Sysmex UC-3500 Automatic Urine Analyzer, demonstrating the sensor's durability and accuracy. (141) Significance Activated carbon hydrogel beads with high adsorption properties were used for the pretreatment of urine samples for the first time, effectively removing background color interference in colorimetric detection. Integration of a smartphone-based microplate analyzer with a hydrogel slow-release system enables quantitative detection of nitrite, LE, and bacteria. This strategy offers a portable, low-cost solution for home-based UTI monitoring, enabling more timely management and appropriate administration of therapy to improve patient outcomes. (71)
