Background China's significant population affected by HIV poses a substantial threat to blood transfusion safety. Despite advancements in blood testing techniques, a residual risk of HIV transmission persists. Accurately assessing HIV epidemic and the residual risk is vital for monitoring blood supply safety and evaluating the effectiveness of new screening tests. Methods We conducted a retrospective analysis of HIV detection results among voluntary blood donors from 2003 to 2022. The study included data on HIV-confirmed positive donors, HIV prevalence, infection risk factors, and an incidence-window period mathematical model to estimate the residual risk of HIV. Results Between 2003 and 2022, HIV prevalence among blood donors in Guangzhou showed a peak-shaped trend, initially increasing before declining. The overall HIV prevalence was 18.9 infections per 100,000 donations. Male donors had a significantly higher prevalence compared with female donors. Donors aged 26–35 years had the highest prevalence. Ethnic minority donors had a higher prevalence compared with Han donors. Repeat donors had a lower prevalence compared with first-time donors. Donors from other provinces had a higher prevalence compared with local donors. During the period of 2003 to 2022, the residual risk of HIV in Guangzhou steadily decreased, reaching a notable 1 in 526,316 donations in the past two years. Conclusion The HIV epidemic among blood donors in Guangzhou remains severe, but the residual risk of HIV is decreasing. Novel detection methods have proven advantageous in reducing this residual risk. Implementing additional effective measures is imperative to ensure blood safety and curb the spread of HIV.

Wastewater treatment plants (WWTPs) have been proposed as significant sources of microplastics (MPs) in freshwater and estuarine environments. WWTPs, even those with high removal efficiencies, release millions of MPs per plant daily. China is the largest plastic producer worldwide, but only a few studies of MP pollution from WWTPs have been carried out in China. In this work, we present a comprehensive report concerning the MPs in effluent from six WWTPs in Guangzhou, which is the third largest city in China. The six WWTPs employ different treatment processes and serve different populations and types of factories. The average abundance of MPs in the effluents of all six WWTPs was 1.719 ± 1.035 MP/L. Fiber was the most common type of MP in the effluent, accounting for 39.48 ± 6.37% of all MPs. Fourier transform infrared spectroscopy confirmed that 35.7% of the particles were plastics, including polyethylene terephthalate (31.9%), polypropylene (26.6%), and polyethylene (9.7%). The results showed that advanced or tertiary treatment technologies could substantially remove MPs and that the size of the population served was positively associated with the abundance of MPs. The number of textile factories was a key factor contributing to the total release of MPs. In addition, the MP shapes and polymer compositions showed that the occurrence of MP types is regional, varies regionally, and is related to the types of factories in the vicinity. More studies on the effects of specific industries are suggested in order to improve the management of wastewater discharge and reduce MPs presence in the natural environment.

Wastewater-based epidemiology (WBE) has been widely used as a complementary method for estimating consumption of illicit drugs in the population. Temporal drug consumption estimates derived from WBE can provide important information for law enforcement and public health authorities in understanding changes in supply and demand of illicit drugs, but currently lacking in China. In this study, influent wastewater samples from a municipal sewage treatment plant in Guangzhou, China were collected for 8 weeks to investigate the temporal change in consumption of six illicit drugs in the catchment. The results indicated that methamphetamine and ketamine were the dominant illicit drugs in Guangzhou with the per capita use of 14.7–470.7 mg/day/1000 people and 64.9–673.7 mg/day/1000 people, respectively. No distinct weekly patterns were observed for illicit drug consumption in Guangzhou, indicating that drug users are likely to be regular ones. Further assessment about the impact of public holidays on the consumption behavior of drugs showed little impact for ketamine ( p = 0.689), but higher consumptions of methamphetamine ( p = 0.003) and cocaine ( p = 0.027) were observed during public holidays than the control period. The considerable decrease in drug consumption observed in October 2017 compared with January and May 2017 was possibly the consequence of law enforcement action.

Purpose Human exposure to particulate matter emitted from on-road motor vehicles includes complex mixtures of heavy metals from tyres, brakes, part wear, and resuspended road sediment. The purpose of this study was to determine the concentrations of 14 platinum-group and other traffic-related heavy metals in road sediment within the metropolitan area of Guangzhou, China, with a view to identifying their sources and assessing the extent of anthropogenic influence on heavy metal contamination of road sediment. Materials and methods Thirty-five samples of road sediment were collected. The concentrations of Cr, Mn, Ni, Cu, Zn, La, Ce, Mo, Cd, Pb, Ba, and Rh were measured by inductively coupled plasma–mass spectrometry. Pt and Pd were analyzed by isotopic dilution–inductively coupled plasma–mass spectrometry. Multivariate statistical analysis and enrichment factor methods were employed to identify the sources of these heavy metals and to assess anthropogenic influences on their occurrence. Results and discussion The mean concentrations of Pt, Pd, Rh, Cr, Mn, Ni, Cu, Zn, La, Ce, Mo, Cd, Pb, and Ba in the road sediment samples were 68.24, 93.15, 23.85, 147.5, 712.3, 47.24, 177.5, 1254, 47.50, 96.62, 4.91, 3.00, 198.1, and 641.3 ng g −1 , respectively. Very weak to significant linear positive correlations were found among the various heavy metals. The elemental composition of road sediment was dominated by five principal components. Three clusters were identified through cluster analysis, and enrichment factors were calculated relative to soils in China. The sources and degree of contamination of the heavy metals are discussed based on the results. Conclusions The mean concentrations of heavy metals are higher than background values, especially for Pt, Pd, Rh, Cd, and Zn. Four main sources are identified: (1) Pt, Pd, and Rh were derived from traffic sources; (2) La, Ce, Mn, and Ba were derived mainly from natural sources; (3) Cr, Ni, Cu, Mo, Cd, and Pb showed mixed traffic-industry sources; and (4) Zn originated mainly from industrial sources. Enrichment factor analysis supported this source identification and further indicated that contamination of road sediment in Guangzhou is extremely high for Pt, Pd, and Rh; moderate to very high for Cd, Zn, Pb, Cu, and Mo; and minimal for Cr, Ni, La, Ce, and Ba.

The mass load of pharmaceuticals in the municipal wastewater based on wastewater-based epidemiology (WBE) is a good indication of population consumption in the catchment. After successful application of illicit drugs’ estimation, this method holds the potential to measure the geographical and temporal consumption of prescription medicines. In this study, we investigated the occurrence of four non-steroidal anti-inflammatory drugs (NSAIDs), acetaminophen (ACM), diclofenac (DCF), ibuprofen (IBU) and naproxen (NPX), in two wastewater treatment plants in Guangzhou City, China and compared the spatial and temporal consumption variation of them. Over a period of 28 days’ sampling, the detection frequency of ACM, DCF, IBU, and NPX in the influent of two wastewater treatment plants (WWTPs) in Guangzhou City were 91%, 66%, 100%, and 95%, and their concentrations were up to 128, 131, 372, and 324 ng/L, respectively. No significant inter-catchment difference was observed regarding the per capita mass load in the two WWTPs investigated. A literature review which covered 160 WWTPs in 18 countries was conducted to compare the population normalized mass load of four commonly used NSAIDs. ACM had the highest population normalized mass loads (29–17,430 mg/d/1000 inhabitants) and DCF had the lowest population normalized mass load (6.5–628 mg/d/1000 inhabitants) in the catchments located in 18 countries. The mass loads of selected NSAIDs in China were lower than those in European and North American. ACM and IBU consumptions were at least 2 times higher in winter than that in summer, in contrast, DCF and NPX consumptions had no significant seasonal variation.

Polycyclic aromatic hydrocarbons (PAHs) in PM 2.5 pose potentially serious threats to human health. In this study, the distribution characteristics of 16 priority controlled, fine PM (PM 2.5 )-bound PAHs in the ambient air of Guangzhou city were analysed from 2016 to 2019. Four high-molecular-weight PAHs with the highest annual average concentrations were benzo[ghi]perylene (BghiP; 0.757 ng/m 3 ), indeno(1,2,3-cd)pyrene (IcdP; 0.627 ng/m 3 ), benzo[ b ]fluoranthene (BbF, 0.519 ng/m 3 ) and 3,4-benzopyrene (BaP; 0.426 ng/m 3 ). Increasing concentrations of BghiP, IcdP, BbF and BaP were associated with increasing numbers of outpatient visits for respiratory diseases, indicating that exposure to these PAHs potentially causes acute respiratory injury in residents. Acute exposure of the human bronchial epithelial cell line BEAS-2B cells to BghiP, IcdP, BbF and BaP in vitro resulted in acute inflammation, DNA damage and apoptosis. Further bioinformatic analysis indicated that nuclear receptor subfamily 1 group D member 1 ( NR1D1 ) may be a key target gene involved in mediating the toxic effects of BghiP. Collectively, our results suggest that BghiP and the other PAHs represented by it can damage the respiratory system and induce lung cancer. This study provides valuable evidence regarding the potential health risks posed by local ambient PAHs pollution.
