Bioadhesive First-Aid Patch with Rapid Hemostasis and High Toughness Designed for Sutureless Sealing of Acute Bleeding Wounds

Ziyuan Zeng,Jiaming Zhang,Yige Gao,Yuanyuan Song,Luoming Liu,Miaomiao Zhu,Wenjing Ma,Jiajun Fu,Dongyang Miao,Chaobo Huang,Ranhua Xiong

The global military and civilian sectors express widespread concern over the significant hemorrhage associated with various acute wounds. Such bleedings lead to numerous casualties in military confrontations, traffic accidents, and surgical injuries. Consequently, the rapid control of the bleedings, particularly for extensive and pressurized wounds, is crucial in first-aid situations. In this work, a double-layered bioadhesive patch that combines a superabsorbent adhesive hydrogel with a highly tough antibacterial polyurethane film, which is called as Bio-Patch, is proposed. The Bio-Patch demonstrates superior mechanical strength and forms robust bioadhesion to acute bleeding wounds. Furthermore, the Bio-Patch enables protecting against external Gram-negative and Gram-positive bacteria. Thanks to the double-layered structures having synergistic functions of stable barrier and robust adhesion, the Bio-Patch provides optimal wound sealing (burst strength exceeding 310 mmHg) both in vitro and in vivo. It also demonstrates superior hemostatic effects (less than 30 s) in vivo. This offers promising opportunities for rapid control of extensive and pressurized hemorrhage in first-aid clinical scenarios.

Robust hemostatic bandages based on nanoclay electrospun membranes

Cui Yan,Huang Zongwang,Lei Li,Li Qinglin,Jiang Jinlong,Zeng Qinghai,Tang Aidong,Yang Huaming,Zhang Yi

Death from acute hemorrhage is a major problem in military conflicts, traffic accidents, and surgical procedures, et al. Achieving rapid effective hemostasis for pre-hospital care is essential to save lives in massive bleeding. An ideal hemostasis material should have those features such as safe, efficient, convenient, economical, which remains challenging and most of them cannot be achieved at the same time. In this work, we report a rapid effective nanoclay-based hemostatic membranes with nanoclay particles incorporate into polyvinylpyrrolidone (PVP) electrospun fibers. The nanoclay electrospun membrane (NEM) with 60 wt% kaolinite (KEM1.5) shows better and faster hemostatic performance in vitro and in vivo with good biocompatibility compared with most other NEMs and clay-based hemostats, benefiting from its enriched hemostatic functional sites, robust fluffy framework, and hydrophilic surface. The robust hemostatic bandages based on nanoclay electrospun membrane is an effective candidate hemostat in practical application.

Strong tissue adhesive polyelectrolyte complex powders based on low molecular weight chitosan for acute hemorrhage control

Mi Wu,Shun Mao,Xu Liu,Yunen Liu,Peifang Cong,Jianhua Lv,Huaqin Tian,Yan Zhao

Self-gelling and bioadhesive powders offered promising effective hemostats to suit irregularly shaped, complex and non-compressible wounds for clinical applications. In the current study, chitosan based polyelectrolyte complex coacervate were simply prepared by mixing high concentrations (10 %) of low molecular weight chitosan (CS) and polyacrylic acid (PAA) solutions. Obtained by lyophilization, the physical cross-linked polyelectrolyte complex powders would form a gel within 5 s upon hydration, which demonstrated excellent mechanical properties, significant antibacterial activities, strong and lasting adhesion on wet tissues in physiological environment. In vitro blood clotting assays showed that the CS/PAA powders could remarkably aggregate blood cells and accelerate blood clotting process. As studied by diverse hemorrhage models, including rat tail, liver and heart injuries and dog incision, CS/PAA powders significantly facilitated the decrease of blood loss as well as hemostatic time by creating robust physical barriers and promoting blood clot formation on the bleeding sites. These outstanding properties in terms of easy preparation, rapid self-gelling, strong wet adhesion, effective hemostasis and shape-adaptability endowed CS/PAA polyelectrolyte complex powders with great potential in managing acute hemorrhage of non-compressible trauma.

Naturally Derived Injectable Dual-Cross-Linked Adhesive Hydrogel for Acute Hemorrhage Control and Wound Healing

Ziran Zhu,Huijun Ye,Kaiwen Zhang,Gang He,Zheng Pan,Yiwen Xian,Yu Yang,Chong Zhang,Decheng Wu

Developing biocompatible injectable hydrogels with high mechanical strength and rapid strong tissue adhesion for hemostatic sealing of uncontrolled bleeding remains a prevailing challenge. Herein, we engineer an injectable and photo-cross-linkable hydrogel based on naturally derived gelatin methacrylate (GelMA) and N-hydroxysuccinimide-modified poly(γ-glutamic acid) (γPGA-NHS). The chemically dual-cross-linked hydrogel rapidly forms after UV light irradiation and covalently bonds to the underlying tissue to provide robust adhesion. We demonstrate a significantly improved hemostatic efficacy of the hydrogel using various injury models in rats compared to the commercially available fibrin glue. Notably, the hydrogel can achieve hemostasis in porcine liver and spleen incision, and femoral artery puncture models. Moreover, the hydrogel is used for sutureless repair of the liver defect in a rat model with a significantly suppressed inflammatory response, enhanced angiogenesis, and superior healing efficacy compared to fibrin glue. Together, this study offers a promising bioadhesive for treating severe bleeding and facilitating wound repair.

Double-network structure sponge with enhanced mechanical properties, procoagulant potential, and 3D printability for acute hemorrhage

Xiao Yang,Xiaoqin Si,Luqi Pan,Xujian Li,Zhixiao Ji,Changcan Shi

In pre-hospital care, achieving rapid and effective hemostasis for arterial rupture and visceral perforation wounds remains a critical challenge. Herein, we have developed a macroporous sponge with double-network structure using foaming technique, chemical and physical crosslinking reactions, and lyophilization. The prepared sponge not only demonstrates outstanding water absorption and water-triggered shape recovery capacity, but also exhibits significantly enhanced mechanical properties due to the construction of double-network structure. Simultaneously, the sponge shortens blood clotting time (from 1354.3 ± 41 s to 473.0 ± 28 s) by concentrating blood components and regulating coagulation pathways. Particularly, the sponge possesses excellent 3D printability and can be additively manufactured through DIW technique while maintaining favorable physical properties and procoagulant potential. Animal experiments have shown that sponges prepared by different processing method exhibit desirable hemostatic performance in SD rat/rabbit liver volume defect and artery complete transection hemorrhage. The construction of double-network provides promising avenue for developing high-performance sponge for massive and noncompressible hemorrhage, and the proposed DIW printing strategy for hemostatic sponge may revolutionize processing methods and provide a potentially feasible approach for designing and preparing customized sponges.
