In cases of deep skin defects, spontaneous tissue regeneration and excessive collagen deposition can result in the formation of proliferative scarring. Salvianolic acid B (SAB) demonstrates promising applications in the treatment of fibrotic diseases, including scarring. This study prepared biodegradable gelatin (Gel) and poly(lactic- co -ε-caprolactone) (PLCL) nanofibrous membranes using electrospinning technology and functionalized them with salvianolic acid B to create fibrous membrane dressings with anti-scarring properties. The resulting Gel/PLCL/SAB (GPS) fibrous membrane exhibited good mechanical properties and biodegradability. In vitro experiments demonstrated that the fibrous membranes of Gel/PLCL containing 0.5 wt% SAB (GPS 0.5 ) exhibited good cytocompatibility, regulated macrophage function and polarization, and inhibited fibrosis-related genes, including CD36, α-SMA, and collagen I. In a mouse model of full-thickness skin defects, the GPS 0.5 fibrous membrane effectively modulated the immune microenvironment and promoted vascular regeneration, thereby accelerating skin healing. Furthermore, the GPS 0.5 fibrous membrane reduced the expression of CD36 and TGF-β1, downregulated collagen I and III, and promoted the regeneration of hair follicles, sebaceous glands, and other appendages, ultimately reducing scar formation. Thus, SAB-loaded Gel/PLCL fibrous membranes hold potential as bifunctional skin dressings that promote wound healing and inhibit fibrosis.

Excessive fibrosis is the primary factor for the failure of glaucoma drainage device (GDD) implantation. Thus, strategies to suppress scar formation in GDD implantation are crucial. Although it is known that in implanted medical devices, microscale modification of the implant surface can modulate cell behavior and reduce the incidence of fibrosis, in the field of ophthalmic implants, especially the modification and effects of hydrogel micropatterns have rarely been reported. Here, we designed the patterned gelatin/acrylamide double network hydrogel and developed an innovative GDD with micropattern to suppress inflammatory and fibroblast activation after GDD implantation. Pattern topography suppressed F-actin expression and mitigated actin-dependent nuclear migration of myocardin-related transcription factor A (MRTF-A) during the proliferative phase after GDD implantation. Ultimately, the expression of α-smooth muscle actin (α-SMA), a key fibrosis-related gene product, was suppressed. Moreover, the modified GDD effectively controlled intraocular pressure (IOP), mitigated fibrous formation, and remodeled extracellular matrix (ECM) collagen distribution in vivo. Therefore, the novel GDD with surface patterning interventions provides a promising strategy to inhibit scar formation after GDD implantation and raise the efficacy of GDD implantation.

Excessive fibrosis is the major factor in the failure of glaucoma filtration surgery. So far, the dominant approach for inhibiting fibrosis is the use of an antimetabolite drug, but the complications it causes, such as filtering bleb leakage, bacterial endophthalmitis and ocular hypotony, are also inevitable. Herein, a multifunctional anti-scarring platform (PVA@rGO-Ag/5-Fu) integrated with outstanding photothermal, antibacterial and drug delivery abilities is developed. PVA@rGO-Ag shows favorable biocompatibility as well as an accurate regional photothermal killing ability on both conjunctival fibroblasts and bacteria under 808 nm near-infrared (NIR) irradiation. Furthermore, PVA@rGO-Ag/5-Fu improves bleb survival rates and results in the satisfactory reduction of intraocular pressure (IOP) by decreasing the fibrous reaction in vivo. In summary, PVA@rGO-Ag/5-Fu has promising potential as an efficacious and safe anti-scarring agent for filtering surgery.

A keloid is a benign tumor manifested as abnormal fibroplasia on the surface of the skin. Curing keloids has become a major clinical challenge, and searching for new treatments and medications has become critical. In this study, we developed a LA67 liposome-loaded thermo-sensitive hydrogel (LA67-RL-Gel) with active targeting for treating keloids via peritumoral injection and explored the anti-keloid mechanism. Firstly, Arg-Gly-Asp (RGD) peptide-modified liposomes (LA67-RL) loaded with LA67 were prepared with a particle size of 105.9 nm and a Zeta potential of −27.4 mV, and an encapsulation efficiency of 89.6 ± 3.7%. We then constructed a thermo-sensitive hydrogel loaded with LA67-RL by poloxamer 407 and 188. The formulation was optimized through the Box–Behnken design, where the impact of the proportion of the ingredients on the quality of the hydrogel was evaluated entirely. The optimal formulation was 20.7% P407 and 2.1% P188, and the gelation time at 37 °C was 9.5 s. LA67-RL-Gel slowly released 92.2 ± 0.8% of LA67 at pH 6.5 PBS for 72 h. LA67-RL-Gel increased adhesion with KF cells; increased uptake; promoted KF cells apoptosis; inhibited cell proliferation; reduced α-SMA content; decreased collagen I, collagen III, and fibronectin deposition; inhibited angiogenesis; and modulated the keloid microenvironment, ultimately exerting anti-keloid effects. In summary, this simple, low-cost, and highly effective anti-keloid liposome hydrogel provides a novel approach for treating keloids and deserves further development. Keywords:keloid;in situ hydrogel;pluronic;triptolide;Arg-Gly-Asp
