Polypropylene (PP) mesh is commonly used in abdominal wall repair due to its ability to reduce the risk of organ damage, infections and other complications. However, the PP mesh often leads to adhesion formation and does not promote functional tissue repair. In this study, we synthesized one kind of aldehyde Bletilla striata polysaccharide (BSPA) modified chitosan (CS) hydrogel based on Schiff base reaction. The hydrogel exhibited a porous network structure, a highly hydrophilic surface and good biocompatibility. We wrapped the PP mesh inside the hydrogel and evaluated the performance of the resulting composites in a bilateral 1 × 1.5 cm abdominal wall defect model in rats. The results of gross observation, histological staining and immunohistochemical staining demonstrated the positive impact of the CS hydrogel on anti-adhesion and wound healing effects. Notably, the addition of BSPA to the CS hydrogel further improved the performance of the composites in vivo, promoting wound healing by enhancing collagen deposition and capillary rearrangement. This study suggested that the BSPA-modified CS hydrogel significantly promoted the anti-adhesion, anti-inflammatory and pro-angiogenesis properties of PP meshes during the healing process. Overall, this work offers a novel approach to the design of abdominal wall repair patches.

Intestinal perforation is a serious medical emergency, and traditional surgery often causes adhesion and other complications. Innovative hydrogels improve postoperative care and rehabilitation with their anti-adhesion, antibacterial, and hemostatic properties. We have developed an advanced anti-adhesion hydrogel, AA-A30, composed of polypyrrole-modified gelatin (PPy-GelMA), carboxymethyl chitosan (CMCS), and NHS-functionalized polyethylene glycol (PEG-NHS). This hydrogel is specifically tailored for intestinal perforations. Upon hydrolysis, PEG-NHS forms a protective barrier that effectively prevents adhesion to surrounding normal tissues. Furthermore, the integration of PPy-GelMA significantly extends the degradation duration of the hydrogel, from 24 to 48 h. In a mouse model of intestinal perforation, the AA-A30 hydrogel demonstrated remarkable efficacy in inhibiting inflammation and preventing tissue adhesion by modulating the expression of both inflammatory and tissue adhesion-related factors, such as IL-1β, TNF-α, and the ratio of tPA to PAI-1. These findings underscore the considerable potential of AA-A30 for the therapeutic management of intestinal perforations.

Abdominal adhesion to polypropylene (PP) mesh remains one of the major complications in hernia repair. Thus, a challenge exists to endow PP mesh with powerful anti-adhesion properties in hernia repair. To investigate potential options, the assembled PP mesh was developed with effective anti-adhesion properties through an in situ coating of the mesh surface with self-fixable and biodegradable mussel-inspired hydrogels. Through mixing oxidized-carboxymethylcellulose functionalized with dopamine (OCMC-DA) with carboxymethylchitosan (CMCS), a layer of hydrogel (OCMC-DA/CMCS) can be formed in situ on the PP mesh without the addition of crosslinking agents; the dopamine then acts as an immobilization group to fix these hydrogels to the PP mesh and the tissue surface. In this way, the assembled PP mesh (OCMC-DA/CMCS/PP) was obtained. The properties of the OCMC-DA/CMCS hydrogels were optimized, and the OCMC-DA4/CMCS hydrogel was selected to construct the assembled PP mesh. The lap-shear test revealed that OCMC-DA4/CMCS has tissue-adhesive properties. In vitro cell tests proved the excellent biocompatibility of the hydrogel. An optimized bioabsorption time and significant anti-adhesion properties were demonstrated through an in vivo test with a rat model. The adhesion area and tenacity of the OCMC-DA4/CMCS/PP group were more than 80% lower than those of the native PP mesh group and created a slightly inflammatory reaction.

Electrospun fibrous membranes have been used frequently in biomedical applications, but their simultaneous use as antibacterial agents and in the prevention of cell adhesion on repaired tendons after injury has not been investigated. In this study, silver-nanoparticle (SN)-loaded poly( L -lactide) (PLLA) fibrous membranes were prepared by the electrospinning of SNs into PLLA fibers. Micrograph results showed that these membranes were composed of electrospun fibers and that the fibers were incorporated with SNs. From the results of X-ray diffraction and thermogravimetry, we concluded that the SNs were physically mixed into the fibers at the desired content. The mechanical properties were not significantly changed. The preliminary antibacterial effects on Staphylococcus epidermidis and Staphylococcus aureus and the synergistic antiproliferative effects of the SN-loaded PLLA fibrous membranes were observed. Taken together, these results demonstrate that SNs can be directly loaded onto a biodegradable PLLA fibrous membrane via electrospinning to achieve proper material properties with preliminary potential as antibacterial antiadhesion barriers for tendon injury. © 2013 Wiley Periodicals, Inc. The prevention and treatment of post-traumatic peritendinous adhesion (PA) have always been a great difficulty for orthopedic surgeons. Current treatments include resecting surgery, non-steroidal anti-inflammatory drugs (NSAIDs) usage and implantable membranes, often target single disease pathogenic processes, resulting in unfavorable therapeutic outcomes. Here a polylactic acid (PLA)-dicumarol conjugates-electrospun nanofiber membrane (ENM) (PCD) is generated, which can achieve spatial accuracy and temporal sustainability in drug release. It is further demonstrated that PCD possesses a significantly higher and more sustainable drug release profile than traditional drug-loading ENM. By providing a physical barrier and continuous releasing of dicumarol, PCD implantation significantly reduces tissue adhesion by 25%, decreases fibroblasts activity and inhibits key fibrogenic cytokine transforming growth factor beta (TGF β ) production by 30%, and improves the biomechanical tendon property by 14.69%. Mechanistically, PCD potently inhibits the connexin43 (Cx43) and thereby tunes down the fibroblastic TGF β /Smad3 signaling pathway. Thus, this approach leverages the anti-adhesion effect of dicumarol and drug release properties of grafted copolymer ENM by esters to provide a promising therapeutic strategy for patients who suffer from PA.

Janus hydrogels have recently emerged as promising bioadhesives for efficient wet-tissue adhesion and anti-postoperative adhesion. However, existing Janus hydrogel adhesives normally need varied chemical designs of different layers to achieve asymmetric adhesive/anti-adhesive properties on either side. Here, we present a new strategy to construct an adhesive/anti-adhesive Janus hydrogel tissue patch accomplished by switching the charge-balance of the hydrogel layers with similar compositions (anionic carboxyl polymer and cationic ε-polylysine, EPL). The bottom layer (AL) is formed under acidic condition (pH 2.85), featuring abundant -COOH and -NH 3 + residues, which provide rapid & robust adhesion to diverse wet tissues (up to 100.4 kPa) with high bursting pressure (362.5 mmHg), while the top layer (MLT) is formed under neutral condition, achieving a balanced charge between -COOH/-NH 2 and -COO − /−NH 3 + groups, which mimic the overall electroneutral structure of zwitterionic materials for efficient anti-postoperative tissue adhesion (up to 6 weeks). Further in vivo studies validated that the integrated AL/MLT hydrogel patch is biodegradable (within 10 weeks), exhibits broad-spectrum antibacterial activity (up to 99.8 %), and outperforms the commercial fibrin gel in sutureless wound sealing, rat gastric tissue repair, and anti-postoperative adhesion. This strategy may open a new avenue to develop adhesive/anti-adhesive Janus bioadhesives for efficient non-invasive internal tissue sealing and promoted wound healing.

Electrospun membranes of poly(p-dioxanone-co- l -phenylalanine) (PDPA) hold potential as an anti-adhesion material. Since adjustable degradation properties are important for anti-adhesion materials, in this study, the in vitro and in vivo degradation processes of PDPA electrospun membranes were investigated in detail. The morphological analysis of these membranes revealed the main degradation conditions of PDPA membranes. The weight remaining and molecular weight variation showed that the overall degradation rate of the membranes could be adjusted by modulating the molecular structure of the PDPAs. Especially, α-chymotrypsin could catalyze the degradation process of PDPAs. Based on these results, the in vitro degradation mechanism was demonstrated, and confirmed by 1 H NMR of the hydrolysis products. Finally, the in vivo degradation and biocompatibility of different PDPAs were investigated. The kinetic study showed that the in vitro and in vivo molecular weight loss of PDPAs have the first-order characteristics. The in vivo degradation rate of the most Phe-containing PDPA-3 is the slowest, and this result relates to the biocompatibilities of PDPAs. © 2016 Wiley Periodicals, Inc. Exudate management is of significant clinical value for the treatment of acute wound. Various wound dressings have been developed to restore the function of injured tissues and promote wound healing, but proper exploiting the healing factors inside exudate and achieving anti-adhesion wound care remains a challenge. Herein, we present a novel multi-functional composite dressing (MCD) by coupling supernatant lyophilized powder of mesenchymal stem cells (MSC-SLP) with a sandwich-structured wound dressing (SWD). The developed MCDs demonstrated unique unidirectional drainage capability, stable anti-adhesion characteristics, and improved wound healing performance. The designed SWD with both superhydrophobic inner surface and liquid-absorption ability of mid layer enables the dressings exhibit desired anti-adhesion property to neoformative granulation tissues, favorable shielding effect to exogenous bacteria, as well as appropriate exudate-retaining capability and unidirectional exudate-absorption property. The introduction of MSC-SLP in SWD was demonstrated to further improve wound healing quality. Compared to medical gauze, the synergic effect of SWD and MSC-SLP significantly accelerates wound healing rate by over 30%, avoids tissue avulsion when changing dressings, and produces a flat-smooth closure surface. More importantly, the wound treated with MCDs presents more skin accessory organs and blood vessels in regenerated tissues than other groups. In vivo/vitro biocompatibility evaluations indicated little toxicity, demonstrating the biosecurity of the developed dressings. The proposed method offers great potential in clinical applications particularly for chronic wound treatments.

To solve the problem of tendon adhesion after an operation, an anti-adhesion membrane is designed, which can inhibit the exogenous healing of tendons and promote endogenous healing. Here, poly[3(S)-methyl-morpholine-2,5-dione- co -lactic acid] P(MMD- co -LA) containing alanine units is obtained by melt ring-opening polymerization (ROP). It can effectively reduce the production of acidic degradation products that cause aseptic inflammation. Then a kind of oriented P(MMD- co -LA)/deferoxamine (DFO) anti-adhesion membrane with good mechanical properties, cell adhesion properties, and induced macrophage polarization properties is constructed by electrospinning. In vitro and in vivo studies have shown that oriented fibrous membranes can down-regulate the expression of inflammatory cytokines. In addition, the fibrous membrane can up-regulate the expression of angiogenesis-related genes, namely HIF1-α, SDF-1α, and VEGF, by releasing DFO in situ, thus promoting the revascularization of tendon defects and providing nutritional support for endogenous tendon healing. This method provides a new barrier strategy to reduce tendon adhesion through anti-inflammation combined with vascularization to inhibit exogenous tendon healing while promoting tendon endogenous healing. The results provide essential insights into the corresponding regulation of the tendon healing microenvironment.

Peptide hydrogels were believed to be potential biomaterials with wide application in the biomedical field because of their good biocompatibility, injectability, and three-dimensional (3D) printability. Most of the previously reported polypeptide hydrogels are composed of L-peptides, while the hydrogels formed by self-assembly of D-peptides are rarely reported. Herein, we reported a peptide hydrogel constructed by D-J-1, which was the all-D-enantiomer of antimicrobial peptide Jelleine-1. Field emission scanning electron microscope (FE-SEM) and rheologic study were performed to characterize the hydrogel. Antimicrobial, hemostatic, and anti-adhesion studies were carried out to evaluate its biofunction. Our results showed that D-J-1 hydrogel was formed by self-assembly and cross-linking driven by hydrogen bonding, hydrophobic interaction, and π-π stacking force of aromatic ring in the structure of D-J-1. It exhibited promising antimicrobial activity, hemostatic activity, and anti-adhesion efficiency in a rat sidewall defect-cecum abrasion model. In addition, it also exhibited good biocompatibility. Notably, D-J-1 hydrogel showed improved in vitro and in vivo stability when compared with its L-enantiomer J-1 hydrogel. Therefore, the present study would provide new insight into the application of D-peptide hydrogel, and provide a new peptide hydrogel with antibacterial, hemostatic, and anti-adhesion efficacy for clinical use. An electrospun poly(lactide- co -glycolide) (PLGA) membrane was prepared and used to perform the anti-adhesion of Achilles tendon. Throughout the experiments, the membrane showed an appropriate degradation rate, and the pH values of degradation media were maintained at around 7.4. Simultaneously, the excellent biocompatibility of the membrane in vitro and in vivo was confirmed by live/dead and histopathological analyses. Meanwhile, the membrane can reduce tendon adhesion significantly and promote functional recovery effectively. The encouraging results were further demonstrated by hematoxylin and eosin (H&E), and Masson’s trichrome stainings, and type I collagen immunohistochemical analysis. It was concluded that the model treated with the electrospun PLGA membrane was significantly better with respect to the adhesion prevention and tissue repair than that without treatment. Considering the results of degradation and adhesion prevention efficacy, the electrospun PLGA membrane would be a great candidate for the prevention of postoperative tendon adhesion.

Inferior healing and peritendinous adhesions are the major clinical problems following Achilles tendon injury, leading to impaired motor function and an increased risk of re-rupture. These complications are presumed to be inextricably linked to inflammation and fibroscar formation. Here, microRNA29a is identified as a promising therapeutic target for tendon injury through the cross-regulation of the immune response and matrix remodeling. MiR29a-LNPs were successfully prepared by microfluidic technology. They are then loaded into the core-shell nanofibers to achieve local delivery in the injured tendon , where the shell layer is composed of PELA for anti-adhesion. Our studies reveal that miR29a regulates collagen synthesis and NF-κB activation in tenocytes , and promotes macrophage polarization by inhibiting the inflammasome pathway. In vivo studies of the Achilles tendon-rupture model indicate the best repair in the miR29a group, as evidenced by superior collagen composition and alignment, higher mechanical strength , and better functional recovery. In conclusion, a functionalized anti-adhesive membrane that promotes nascent tendon matrix remodeling and improves the regenerative immune microenvironment is developed for the treatment of tendon injury.
