Hydroxyapatite/calcium alginate composite particles for hemostasis and alveolar bone regeneration in tooth extraction wounds

Gang He,Zhihui Chen,Luyuan Chen,Huajun Lin,Chengcheng Yu,Tingting Zhao,Zhengwen Luo,Yuan Zhou,Siyang Chen,Tianjiao Yang,Guixian He,Wen Sui,Yonglong Hong,Jianjiang Zhao

Tooth extractions can lead to complications such as post-extraction bleeding and bone resorption, which may result in unfavorable outcomes for implant restoration afterwards. To strive for an optimal condition for further restoration procedures, appropriate strategies, such as hemostasis or bone regeneration, are encouraged to be employed. However, the existing products are failed to meet both needs. As a widely employed tissue engineering materials, hydroxyapatite and calcium alginate both have demonstrated excellent performance in osteogenesis. However, their inferior mechanical strength poses a major limitation to their use in supporting the contracted extraction socket, which can easily lead to alveolar crest atrophy and barely achieve satisfying results. Calcium alginate improves the mechanical strength of hydroxyapatite, enabling the formation of new bone tissue and degradable in vivo . In this study, we demonstrated the biocompatibility and haemostatic ability of nCA particles on a rat tooth extraction model. In addition, long-term observation has revealed minimal inflammation and bone tissue regeneration. Our findings suggest a promising insight for clinical applications in hemostasis and bone regeneration after tooth extraction.

Nano-hydroxyapatite mineralized silk fibroin porous scaffold for tooth extraction site preservation

Ling Nie,He Zhang,Aishu Ren,Yuzhou Li,Gang Fu,Richard David Cannon,Ping Ji,Xiaohong Wu,Sheng Yang

Objective To fabricate a novel nano-hydroxyapatite mineralized silk fibroin (MSF) scaffold in order to diminish the resorption of alveolar ridge and accelerate new bone formation within tooth sockets. Also, to investigate the biocompatibility and osteogenic ability of the MSF in vitro , and the effect of site preservation of the MSF graft in post-extractive sockets in vivo . Methods SEM, EDX, FTIR and XRD were used to analyze the mineral crystals deposited on the silk fibroin (SF) surface. Pre-osteoblasts (MC3T3-E1) were seeded on SF and MSF scaffolds. Cell viability, distribution and differentiation were examined using a live-dead assay, histological analysis and Alizarin Red S staining. Furthermore, prepared grafts (SF or MSF scaffold) were implanted into the maxillary right first molar sockets of Sprague Dawley rats for 6 weeks and newly formed bone tissue was analyzed by micro-CT and histological examination. Results The SEM, EDX, FTIR and XRD analysis demonstrated that granulate nano-hydroxyapatite (nHA) crystals were uniformly distributed on the SF scaffold. In addition, the MSF hydrophilicity measured by water contact angle and swelling ratio was superior to plain SF scaffold. The effect of nHA inorganic crystals on osteogenic differentiation of MC3T3-E1 cells indicated the MSF scaffolds improved osteogenesis. Furthermore, MSF grafts induced more bone formation and reduced the height of alveolar bone resorption after tooth extraction. Significance The MSF scaffold partially simulated the structure and composition of natural bone matrix. It induced osteogenic differentiation of MC3T3-E1 cells in vitro , and also promoted new bone regeneration in tooth extraction sockets in vivo , indicating it is a biomaterial with great potential for tooth extraction site preservation.

An asymmetric Janus membrane with anti-bacteria adhesion and rapid hemostasis properties for wound healing

Zihe Hu,Gaoying Hong,Mumian Chen,Haiyan Wu,Weiying Lu,Yuewei Chen,Zhijian Xie,Changyu Shao,Jue Shi

Currently, commercial intraoral dressings have limitations as they only serve a single purpose and fail to meet various needs, such as hemostasis, wound protection, and dressing fixation. Here, we successfully fabricated a multifunctional polycaprolactone-chitosan Janus membrane (PCJM) with simultaneous hydrophobic and hydrophilic properties. The hydrophobic layer consists of a dense and disordered polycaprolactone, preventing bacterial adhesion, resisting fouling, and enhancing mechanical strength. The hydrophilic layer is a highly porous chitosan sponge, which facilitates cell and protein adhesion, as well as rapid absorption of blood and activation of the coagulation process. These results are confirmed by hemostasis experiments, which demonstrate that the PCJM had superior coagulation ability compared to commercial gelatin sponges and gauze, and by an early gingival healing model of beagle tooth extraction wounds, which exhibited excellent performance in promoting wound healing. Our work demonstrates the multifunctional PCJM wound dressing not only protects wounds from bacterial infections but also facilitates rapid hemostasis and early healing of beagle gingival soft tissue, which holds significant potential for clinical translation and large-scale production.

An anti-bacterial porous shape memory self-adaptive stiffened polymer for alveolar bone regeneration after tooth extraction

Weijun Zhang,Meilin Yu,Yongqiang Cao,Zihan Zhuang,Kunxi Zhang,Dong Chen,Wenguang Liu,Jingbo Yin

The regeneration of alveolar bone after tooth extraction is critical for the placement of dental implants. Developing a rigid porous scaffold with defect shape adaptability is of great importance but challenging for alveolar bone regeneration. Herein, we design and synthesize a biocompatible poly( l -glutamic acid)- g -poly( ε -caprolactone) (PLGA- g -PCL) porous shape memory (SM) polymer. The PLGA- g -PCL is then copolymerized with acryloyl chloride grafted poly( ω -pentadecalactone) (PPDLDA) having a higher phase transition temperature than shape recovery temperature to maintain stiffness after shape recovery to resist chewing force. The hybrid polydopamine/silver/hydroxyapatite (PDA/Ag/HA) is coated to the surface of (PLGA- g -PCL)-PPDL scaffold to afford the anti-bacterial activity. The porous SM scaffold can be deformed into a compact size and administered into the socket cavity in a minimally invasive mode, and recover its original shape with a high stiffness at body temperature, fitting well in the socket defect. The SM scaffold exhibits robust antibacterial activity against Staphylococcus aureus ( S. aureus ). The porous microstructure and cytocompatibility of PLGA allow for the ingrowth and proliferation of stem cells, thus facilitating osteogenic differentiation. The micro-CT and histological analyses demonstrate that the scaffold boosts efficient new bone regeneration in the socket of rabbit mandibular first premolar. This porous shape memory self-adaptive stiffened polymer opens up a new avenue for alveolar bone regeneration.

Electrospun Nanofibers Loaded with Concentrated Growth Factors and Nanohydroxyapatite for the Healing of Alveolar Bone in Tooth Extraction Wounds

Qihang Huang,Linzi Han,Ruishi Wang,Qunli Guan,Hang Wang,Yinhui Yang,Dejiang Du,Yujia Wang,Yichen Shi,Jiaxuan Chai,Yaozhong Wang,Dechao Li

The healing of tooth extraction wounds is significantly influenced by various factors, including interference from the oral microenvironment, invasion of gingival tissue, and inflammation of the alveolar socket, all of which contribute to the substantial loss of bone tissue in the alveolar socket. This study employed electrospinning technology to fabricate polyvinyl alcohol (PVA)/sodium alginate (SA) nanofiber scaffolds infused with freeze-dried concentrated growth factor (CGF) and nanohydroxyapatite (nHA). The objective was to investigate the repair mechanism of the PVA/SA/CGF/nHA nanofibers for oral alveolar bone defects, thereby offering novel treatment strategies for bone defect repair. In vitro experimental results demonstrate that the addition of nHA significantly enhances both the degradation rate and swelling ratio of the PVA/SA/CGF/nHA fiber membrane. Additionally, the scaffold exhibits favorable microstructural properties and biocompatibility. The sustained release of fibrin in CGF suggests that the fibrous membrane maintains a stable three-dimensional structure, facilitating the slow and sustained release of CGF. Alkaline phosphatase (ALP) and alizarin red staining indicate a significant promotion of osteogenic differentiation of human bone marrow stem cells (hBMSCs). Furthermore, qRT-PCR results reveal increased expression levels of the ALP, collagen type-1 (Col1), runt-related transcription factor 2 (Runx2), osteocalcin (OCN), and osteopontin (OPN) genes. To further investigate the impact of the local application of the nanofiber membrane on bone regeneration, a rat tooth extraction wound model was established. These results confirm that the electrospun PVA/SA/CGF/nHA nanofiber membrane significantly promotes the proliferation, migration, and osteogenic differentiation of hBMSCs. This finding offers a novel treatment for oral alveolar bone defects and has potential clinical applications in the oral cavity.

Reversibly Adhesive, Anti-Swelling, and Antibacterial Hydrogels for Tooth-Extraction Wound Healing

Siyu Li,Lunhao Zhi,Qin Chen,Weifeng Zhao,Changsheng Zhao

Oral wound treatment faces challenges due to the complex oral environment, thus, sealing the wound quickly becomes necessary. Although some materials have achieved adhesion and sterilization, how to effectively solve the contradiction between strong adhesion and on-demand removal remains a challenge. Herein, a reversibly adhesive hydrogel is designed by free radical copolymerization of cationic monomer [2-(acryloyloxy) ethyl] trimethylammonium chloride (ATAC), hydrophobic monomer ethylene glycol phenyl ether acrylate (PEA) and N -isopropylacrylamide (NIPAAm). The cationic quaternary ammonium salts provide electrostatic interactions, the hydrophobic groups provide hydrophobic interactions, and the PNIPAAm chain segments provide hydrogen bonding, leading to strong adhesion. Therefore, the hydrogel obtains an adhesion strength of 18.67 KPa to oral mucosa and can seal wounds fast within 10 s. Furthermore, unlike pure PNIPAAm, the hydrogel has a lower critical solution temperature of 40.3 °C due to the contribution of ATAC and PEA, enabling rapid removal with 40 °C water after treatment. In addition, the hydrogel realizes excellent anti-swelling ratio (≈80%) and antibacterial efficiency (over 90%). Animal experiments prove that the hydrogel effectively reduces inflammation infiltration, promotes collagen deposition and vascular regeneration. Thus, hydrogel as a multi-functional dressing has great application prospects in oral wound management.
