This work aims to investigate dental root filling custom gutta-percha (GP) properties based on melt extrusion 3D printing for periapical periodontal disease treatment. Zinc oxide nanoparticles are incorporated into polyurethane/gutta-percha (PGB) composites. A polyurethane/gutta-percha/ZnO (PGB-Zn) biocomposite including BaSO4, polyurethane (PU), GP, and ZnO nanoparticles is designed and fabricated. To improve the dispersion of ZnO nanoparticles, rosin is chosen to modify the surface of the ZnO nanoparticles. The PGB-Zn biocomposite exhibits a maximum tensile strength of 26.795 ± 1.173 MPa and a flexural strength of 6.037 ± 0.37 MPa. The chemical interaction between the nanoparticles and the matrix is found in the physical and mechanical characterization of the polyurethane/gutta-percha/ZnO (PGB-Zn) biocomposite. The inhibition zone test and absorbance photometry show that the composite material inhibits and kills Escherichia coli (E. coli) and Staphylococcus aureus (S. aureus). The CCK-8 experimental results show that the PGB-Zn biocomposite has a cytotoxicity level of 0-I. The obtained PGB-Zn biocomposite in this work can be used for accurate root canal therapy as a filling biocomposite by a fused extrusion method, which effectively decreases the potential risk of the clinical implementation of periapical periodontal disease treatment.

The utilization of intracanal medicaments is an indispensable procedure in root canal treatment(RCT). However, the conventional intracanal medicaments still need improvement regarding antimicrobial efficacy and ease of clinical operation. To address the above issues, OCT/PECT@OCT+ALK composite hydrogel characterized by programming sequential release of dual antimicrobial agents has been proposed. Thanks to the self-assemble ability of amphiphilic copolymer poly(ε-caprolactone-co-1,4,8-trioxa [4.6]spiro -9-undecanone)-poly(ethylene glycol)-poly(ε-caprolactone-co-1,4,8-trioxa [4.6]spiro-9- undecanone) (PECT), dual hydrophilic and hydrophobic antimicrobial agents could be easily encapsulated in the hydrogel system, and tailored for sequential drug release for a better antibiofilm effect. The hydrophilic octenidine (Octenidine dihydrochloride, OCT-HCl) is encapsulated in the hydrophilic part of hydrogel for instantaneous elevating the drug concentration through bursting release, and the hydrophobic octenidine(Octenidine, OCT) is further loaded into the PECT nanoparticles to achieve a slower and sustained release profile. Additionally, calcium hydroxide (Ca(OH)2) was incorporated into the system and evenly dispersed among PECT nanoparticles to create an alkaline (ALK) environment, synergistically enhancing the antibiofilm effect with higher efficiency and prolonged duration. The antibiofilm effect has been demonstrated in root-canal models and apical periodontitis rats, exhibiting superior performance compared to clinically used Ca(OH)2 paste. This study demonstrates that OCT/PECT@OCT+ALK composite thermosensitive hydrogel is a potential intracanal medicament with excellent antibiofilm effect and clinical operability.

Low monomer–polymer conversion is the key factor leading to cytotoxicity for resin-containing restorative materials. This paper provides a new root canal filling system based on self-curing injectable polyurethane which can achieve high conversion in a short time. Traced FTIR spectra show more than 90% NCO group participated in the curing reaction after 4 h, and only about 5% remained after 24 h. The calculated data also testified the curing process supports a third-order reaction, and this efficient and sufficient reaction is postulated to weaken the toxic stimulation. By culturing with L929 murine fibroblasts, the PU sealer is shown to be favorable for cell attachment and proliferation. Then physicochemical properties of the injectable PU-based sealer were evaluated according to the Standard [ISO 6876:2001 (E)] for clinical application. A series of physicochemical properties of PU sealer have been tested comparing with AH Plus and Apexit Plus. And the results present that the self-curing PU sealer could not only match the clinic requirements, but even has better properties than the other two commercial sealers. We expect the high conversion PU sealer has a tremendous potential in the field of root canal filling after further biological evaluation.

Lyotropic liquid crystals (LLC) have received increasing attention as a drug delivery system. In this study, a novel intra-canal disinfectant based on the glycerol monooleate (GMO) LLC precursor incorporation with chlorhexidine (CHX) and silver nanoparticles (Ag-NPs) was designed and evaluated. The LLC precursor with excellent fluidity was able to penetrate deeply into the complex tiny collateral branch root canals. The transformation of cubic LLC in root canals upon coming into contact with water provided long-lasting disinfection against multidrug-resistant bacteria to avoid the endodontic reinfection and follow-up visits. The GMO–ethanol–water (48% : 12% : 40%, w/w) formulation containing 0.5% CHX and 0.02% Ag-NPs was selected for further studies. The low viscosity of the precursor presented excellent injectability and flowabilities. From the in vitro release test, the release behaviours were found to be influenced by CHX and Ag-NP contents, allowing the optimized precursor to obtain a 28-day release profile. The CHX-Ag-NP containing LLC precursor exhibited an excellent and sustained sterilization effect on Enterococcus faecalis for more than one month with a bacterial inactivation rate of ≥98.5%, which was far more than the minimum clinical requirement (7 days). Furthermore, no in vitro toxicity was observed in the cytotoxicity evaluation. The CHX-Ag-NP containing LLC precursor was proved to be a promising intra-canal disinfectant in our study.

The validity and biocompatibility of irrigating agents are imperative for the success of root canal therapy. The imperfections in the currently available irrigants highlight the fact that more advanced technologies and strategies are required for complete disinfection in endodontic treatments. In the present study, a Fenton reaction-enhanced antimicrobial sonodynamic therapy (SDT) platform was fabricated for root canal disinfection. Firstly, mesoporous silica nanoparticles (MSNs) were synthesized, grafted with an amino group and then conjugated with sonosensitizer protoporphyrin IX (PpIX). Iron ions were then anchored (M@P-Fe) to initiate a Fenton reaction. Nanoparticle characterization by size and zeta potential measurements, scanning electron microscopy, transmission electron microscopy and thermogravimetric analysis confirmed that the platform was successfully developed. Reactive oxygen species (ROS) generation assessment, methylene blue degradation and electron spin resonance assays illustrated upon ultrasound (US) irradiation, that augmented ROS, can be produced by US activated PpIX and iron mediated Fenton reactions from low concentration H2O2 (0.01%). In vitro anti-Enterococcus faecalis efficacy was demonstrated by growth curve and colony forming unit measurements. Confocal laser scanning microscopy and scanning electron microscopy observations illustrated the effectiveness of the platform on in situ biofilm eradication in root canal. Owing to the stronger oxidizing capability and short lifetime of ROS, the Fenton reaction-enhanced SDT can induce detrimental oxidative damage to bacteria upon activation of US while avoiding nonspecific toxicity to cells, which was verified by cytotoxicity evaluations using CCK-8 assay and morphology observation of MC3T3-E1 cells. Compared to commonly used NaClO, this nanoplatform displayed desirable anti-bacterial, anti-biofilm abilities and better biocompatibility. These results highlight that the integrated M@P-Fe + US + H2O2 platform is a promising candidate for US enhanced root canal irrigation and disinfection.

A root canal sealer with antibacterial activity can be efficacious in preventing reinfection that results from residual microorganisms and/or the leakage of microorganisms. In the present study, a series of injectable, self-curing polyurethane (PU)-based antibacterial sealers with different concentrations of silver phosphate (Ag3PO4) were fabricated. Subsequently, their physicochemical properties, antibacterial abilities, and preliminary cytocompatibilities were evaluated. The results indicated that the fabricated PU-based sealers can achieve a high conversion rate in a short amount of time. More than 95% of the isocyanate group of PU sealers with 3 wt% (PU3) and 5 wt% (PU5) concentrations of Ag3PO4 were included in the curing reaction after 7 hours. With the exception of those for film thickness for PU5, the results of setting time, film thickness, and solubility were able to meet the requirements of the International Organization for Standardization. The antibacterial tests showed that PU3 and PU5 exhibit stronger antimicrobial effects than that achieved with 1 wt% Ag3PO4 (PU1) and AH Plus (positive control) against Streptococcus mutans. The cytocompatibility evaluation revealed that the PU1 and PU3 sealers possess good cytocompatibility and low cytotoxicity. These results demonstrate that the PU3 sealer offers good physicochemical and antimicrobial properties along with cytocompatibility, which may hold great application potential in the field of root canal fillings.

Background/purpose The root fracture resistance of endodontically treated teeth is decreased significantly, and it is more likely to fracture. This study aimed to evaluate the effect of a novel root canal sealer based on bioactive glass (BG) on root fracture resistance and explore its mechanism. Materials and methods The BG-based root canal sealer (BG Sealer) was prepared by mixing a kind of bioactive glass (10.8% P 2 O 5 , 54.2% SiO 2 , 35% CaO, mol.%, named PSC), zirconia (ZrO 2 ) powder, sodium alginate (SA) and phosphate solution (PS). A pH meter was used to measure the pH of simulated body fluid (SBF) after immersion with BG Sealer at different time. After preparing the samples of BG sealer with a diameter of 4 mm and a height of 6 mm, the compressive strength was tested by a universal testing machine. The scanning electron microscopy (SEM) and energy dispersive X-ray spectroscopy (EDS) were used to detect and analyze the mineral status of root canal systems filled with BG Sealer. The push out test was used to measure the push out bond strength of BG Sealer. The fracture resistance of root canals filled with BG Sealer was detected by the compressive loading test. Bioceramic root canal sealer iRoot SP was set as the control group. Results (1) Physicochemical properties: The pH value of SBF immersed with BG Sealer increased slightly up to 7.68, while the pH of SBF immersed with iRoot SP increased to 12.08. The compressive strength of the novel BG Sealer was 4.62 ± 1.70 MPa, which was lower than that of iRoot SP ( P < 0.05). (2) Mineralization: The hydroxyapatite layers were observed on the surface of BG Sealer and iRoot SP after being immersed in SBF for 4 weeks. BG Sealer and iRoot SP were both able to penetrate into the dentin tubules, duplicate the morphology of root canals well, and form a layer of hydroxyapatite. (3) Adhesion to dentin: There was no significant difference between the push out bond strength of the novel BG Sealer and iRoot SP ( P > 0.05). (4) Fracture resistance: After immersion in SBF for 4 weeks, the fracture resistance of roots filled with BG Sealer and iRoot SP was 454.16 ± 155.39 N and 445.50 ± 164.73 N, respectively, both of which were not statistically different from that of the roots unprepared and unfilled (394.07 ± 62.12 N) ( P > 0.05), whereas higher than that of the roots prepared and unfilled (235.36 ± 83.80 N) ( P < 0.05). Conclusion The novel BG Sealer has good adhesion to the root dentin, can penetrate into the dentin tubules to generate minerals, and meanwhile can improve the fracture resistance of the roots after root canal treatment. It is expected to be a bioactive root canal sealer with good clinical application prospects.

Enterococcus faecalis ( E. faecalis ) is one of the dominant bacteria in infected root canals, and it is tolerant to commonly used root canal disinfectants. Ag + has a certain antibacterial effect on E. faecalis , but the instability and potential toxicity of Ag + limit its application. Metal azolate framework-7 (MAF-7) is a typical metal–organic framework that has been used for drug delivery due to its high encapsulation efficiency, tunable release profile, and low cytotoxicity. In this study, we used a simple method for the synthesis of Ag + /MAF-7 nanocomposites. After characterization via several techniques, the antibacterial activity of this nanocomposite against E. faecalis was investigated. Ag + was found to be uniformly bound to MAF-7, and the resulting Ag + /MAF-7 nanocomposite showed significant antibacterial activity against E. faecalis . This study demonstrates the potential application of nanocomposite antibacterial materials in treating infected root canals.

Elimination of microorganisms from the root canal system and the prevention of subsequent reinfection are of importance for long-term endodontic treatment. The application of a sealer with antibacterial properties may reduce the reinfection and improve the success rate of the root filling treatment. The aim of this paper is to evaluate the antibacterial properties of a novel root canal sealer based on injectable self-curing polyurethane with silver phosphate (PU/Ag3PO4). The antibacterial abilities were assessed by direct contact test, anti-bacterial adhesion assay and bacteriostatic rate test. The results show that the fabricated PU/Ag3PO4 sealer can completely inhibit the bacterial growth and prevent bacterial adhension effectively. The bacteriostatic rate is 93.6% and 98.1% when the bacteria incubated with PU/Ag3PO4 sealer for 7 and 24h respectively. The strong antibacterial abilities suggest that PU/Ag3PO4 sealer has great application potential in the field of root canal filling.Key words: Root canal sealer, polyurethane, silver phosphate, antibacterial properties, direct contact test

Clinical root canal therapy which takes place through mechanical and chemical strategies is faced with challenges in eliminating bacteria owing to the intricate and curved nature of the root canal system. Moreover, the plaque biofilm within the root canal hinders drug penetration and limits treatment efficacy. Hence, efficient root canal therapy hinges on penetrating into the root canal and overcoming the barriers presented by the plaque biofilms. To penetrate and eradicate biofilms effectively at the root canal, we designed a novel magnetic nanoparticle (MN)-based nanoplatform which was synthesized by the self-polymerization of dopamine on the surface of Fe 3 O 4 MNs, and then loaded minocycline through the electrostatic interaction. The therapeutic efficacy of minocycline-loaded magnetic nanoparticles (FDM MNs) under a magnetostatic field was observed by various antibacterial experiments. The synthesized FDM MNs exhibited favorable biocompatibility and robust anti-biofilm efficacy. The designed nanoparticles could effectively navigate biofilms to eradicate bacteria residing deep with the assistance of magnetic force. Furthermore, FDM MNs penetrated into dentin tubules under a magnetic field, effectively disrupting biofilms for deep sterilization. The significant results offered valuable experimental evidence to support the potential clinical utility of magnetic nanoparticles for managing pulpitis and periapical inflammation.

To solve the high instances of failure caused by endodontic reinfection, herein, an improved root filling material was produced to meet the multi-functional demand of sealers for root canal therapy. In this study, polyurethane (PU)-based nanocomposites were prepared by loading bismuth oxide, hydroxyapatite and antibacterial agents, namely Ag3PO4 and ZnO nanoparticles, which were named CP-Ag and CP-Zn sealers, respectively. A parallel biological evaluation at bacterial and cellular levels was performed to determine the fate of the different components of the PU-based sealers. Furthermore, the composition of sealers was quantified by screening their antibacterial activity and apoptotic factors, considering the potential toxicity of the nanoparticles and high dosage of metals. The in vitro optimization investigation was conducted systematically against Streptococcus mutans and Staphylococcus aureus, including bacteriostatic and dynamic tests, and the expression of the B-cell lymphoma-2 gene family and caspase proteases in the mitochondria-mediated apoptotic pathway was evaluated using the commercial AH Plus® and Apexit® Plus sealers for comparison. Additionally, the physical properties and sealing ability of sealers were assessed. The results showed that all PU-based sealers could meet the requirements of ISO 6876:2012 for root canal sealing materials. Based on the evaluation system, CP-Zn sealers expressed longer lasting antibacterial activity and lower toxic effect on cells compared to CP-Ag sealers. Especially, the CP-Zn5 sealer exhibited selective antimicrobial efficacy and hypo-toxicity, which were better than that of the two commercial sealers. According to the two-dimensional and three-dimensional methods, the good sealing ability of the CP-Zn5 sealer is the same as the excellent filling characters of AH Plus, which adapts to irregular root canals.

Sealing the therapeutic agents in the root canal is considered to be an essential step in root canal therapy. The lyotropic liquid crystalline precursor (LLCP) incorporated with chlorhexidine (CHX) and silver nanoparticles (Ag-NPs) has been confirmed as a promising candidate for root canal therapy in the previous study. Importantly, the stability of the LLCP system was a significant determinant for its therapeutic effect and further application. The objective of this study was to comprehensively investigate the stability of the LLCP incorporated with CHX and Ag-NPs. The oil-water partition coefficient of CHX and Ag-NPs was measured. The water absorption and the physical stability of drug-loaded LLCP solution were studied. Stability under high temperature, high humidity, and strong light irradiation was also investigated. The results demonstrated that CHX and Ag-NPs could be entrapped in the water channel of LLCP, indicating the low tendency of drugs leakage. The drug-loaded LLCP was a pseudoplastic fluid and it showed an excellent physical stability with a sedimentation rate of 0.981 and a settling time of 26~28 h. The payload of LLCP was confirmed to weaken the water absorption behavior, which facilitated its transformation to cubic liquid crystal. The stress testing under high temperature, high humidity, and strong light irradiation also manifested that the LLCP was stable when stored under moisture-proof condition. In conclusion, the developed LLCP incorporated with CHX and Ag-NPs was highly stable during storage and qualified for further application.
