Implant-related infections (IRIs) pose a major challenge in orthopedic applications due to the persistence of biofilms, which are highly resistant to conventional antibiotics. This study introduces oxygen vacancy-engineered Zn–Fe spinel nanoparticles as microwave-responsive antibacterial agents. The oxygen vacancies in the spinel structure enhance reactive oxygen species (ROS) generation under microwave irradiation, providing a dual-mode antibacterial mechanism of thermal and oxidative stress. Zn–Fe spinel nanoparticles were synthesized using a sol–gel method and evaluated for their antibacterial efficacy against Staphylococcus aureus biofilms. Under microwave irradiation, the Zn–Fe spinel demonstrated significant biofilm disruption and bacterial eradication. Mechanistic studies revealed that oxygen vacancies promoted ROS generation, leading to bacterial membrane damage. In vivo experiments using a mouse infection model confirmed the material's antibacterial efficacy and biocompatibility, with no observed toxicity. This study highlights the potential of oxygen vacancy-enhanced Zn–Fe spinel as a microwave-assisted, antibiotic-free strategy for treating deep-seated infections associated with orthopedic implants.

Zirconia has become a new dental implant material because of its excellent esthetics, biocompatibility, and minimal ion release. However, biological inertness of zirconia surface results in weak integration with bone tissue in the early stage of osseointegration. To enhance the early osseointegration of zirconia implant, a homogeneous nanoporous tantalum (TaNS) coating (~30 nm in diameter and 1 μm in length) with good adhesion strength was prepared on zirconia surface via magnetron sputtering combined with anodization. The roughness and hydrophilicity in ZrO 2 /TaNS groups were significantly improved as compared to other groups. The protein adsorption, initial adhesion, spreading and proliferation of MC3T3-E1 cells were significantly enhanced in ZrO 2 /TaNS group as compared to ZrO 2 and ZrO 2 /Ta group. Moreover, the osteogenic differentiation of MC3T3-E1 cells was significantly improved in ZrO 2 /TaNS groups than other groups by up-regulating the expression of the osteogenic-related genes (RunX2, ALP , COL-1, OSX, OCN and OPG). Histological analysis displayed that more newly regenerated bone was deposited around ZrO 2 /TaNS group than other groups, and the bone-implant contact in ZrO 2 /TaNS group was enhanced correspondingly. In conclusion, the TaNS coating with good adhesion strength can be successfully prepared on zirconia surface by magnetron sputtering combined with anodization. This novel coating will be a promising clinical application in improving osseointegration of the zirconia implant.

Developing titanium-based dental implants with both excellent antibacterial properties and good osseointegration is crucial for the success of the implant operation and the long-term durability of the implant. In this study, a polydopamine-based coating was created by attaching TiO 2 nanosheets-cyanine composites onto the titanium surface, enabling the integration of effective photothermal antibacterial therapy with osseointegration. The exceptional dual-photothermal conversion abilities of polydopamine and cyanine in the coating resulted in outstanding photothermal antibacterial and antibiofilm therapy against four types of bacteria. Furthermore, TiO 2 nanosheets promoted the adhesion, proliferation and early osteogenic differentiation of osteoblasts. In an infected dental implant model in rats, the developed coating exhibited potent antibacterial activity and remarkable osteogenic differentiation in the bone, leading to increased bone formation around the implants. This innovative approach, combining photothermal therapy with osteogenic two-dimensional nanomaterials, presents a novel method for surface functionalization of implants to achieve effective antibacterial and osseointegration capabilities.

Polymeric elastomers are widely utilized in implantable biomedical devices. Nevertheless, the implantation of these elastomers can provoke a robust foreign body response (FBR), leading to the rejection of foreign implants and consequently reducing their effectiveness in vivo. Building effective anti-FBR coatings on those implants remains challenging. Herein, we introduce a coating-free elastomer with superior immunocompatibility. A super-hydrophilic anti-fouling zwitterionic layer can be generated in situ on the surface of the elastomer through a simple chemical trigger. This elastomer can repel the adsorption of proteins, as well as the adhesion of cells, platelets, and diverse microbes. The elastomer elicited negligible inflammatory responses after subcutaneous implantation in rodents for 2 weeks. No apparent fibrotic capsule formation was observed surrounding the elastomer after 6 months in rodents. Continuous subcutaneous insulin infusion (CSII) catheters constructed from the elastomer demonstrated prolonged longevity and performance compared to commercial catheters, indicating its great potential for enhancing and extending the performance of various implantable biomedical devices by effectively attenuating local immune responses. Statement of significance The foreign body response remains a significant challenge for implants. Complicated coating procedures are usually needed to construct anti-fibrotic coatings on implantable elastomers. Herein, a coating-free elastomer with superior immunocompatibility was achieved using a zwitterionic monomer derivative. A pure zwitterionic layer can be generated on the elastomer surface through a simple chemical trigger. This elastomer significantly reduces protein adsorption, cell and bacterial adhesion, and platelet activation, leading to minimal fibrotic capsule formation even after six months of subcutaneous implantation in rodents. CSII catheters constructed from the PQCBE-H elastomer demonstrated prolonged longevity and performance compared to commercial catheters, highlighting the significant potential of PQCBE-H elastomers for enhancing and extending the performance of various implantable biomedical devices.

In the last decades, to enhance success rates in assisted reproductive technology (ART) cycles, scientists have continually tried to optimize embryo culture and selection to increase clinical outcomes. In this scenario, the application of laser technology has increased considerably worldwide and is currently applied across ART in several ways: for assisted hatching (AH) or thinning of the zona pellucida (ZP), embryo biopsy, to immobilize and select the sperm during intracytoplasmic sperm injection, as well as to induce artificial blastocyst shrinkage before cryopreservation. Laser-AH has been suggested as a procedure to improve embryo implantation: the concept is that drilling holes through or thinning of the ZP could improve the hatching process and implantation. The artificial disruption of the ZP can be performed by different approaches: mechanically, chemically and with the laser, which is one of the most favourable and easy methods to remove part of the ZP and to augment the possibilities of implantation in patients defined as having a poor prognosis of success, or when the ZP is too thick. However, in the current literature, there is not sufficient evidence about the potential risk or impairment that laser utilization might induce on embryo development; therefore, the main aim of the current review is to provide an overview of the existing knowledge on the ZP and the mechanisms of manipulating it to improve the effectiveness of ART. Also, it emphasizes the positive aspect of laser application as a powerful tool that might increase the chance of pregnancy for infertile couples undergoing ART cycles.

Implanted materials with both osteogenic and antibacterial functions are promising for facilitating osteointegration and preventing infection for orthopedic applications. In this work, we synthesized flower-like molybdenum disulfide (fMD) submicro-spheres containing nanosheets, which were incorporated onto the microporous surface of polyimide (PI) via concentrated sulfuric acid, suspending fMD contents of 5 wt% (SPM1) and 10 wt% (SPM2). Compared with sulfonated polyimide (SPM0), both SPM1 and SPM2 with microporous surfaces containing fMD exhibited nano-submicro-microporous surfaces, which improved the surface roughness, wettability, and surface energy. Due to there being more fMD submicro-spheres on the microporous surface, SPM2 revealed a better antibacterial effect than SPM1. In addition, compared with SPM1 and SPM0, SPM2 with more fMD significantly promoted rat bone marrow-derived stromal cell response in vitro. Moreover, SPM2 remarkably enhanced new bone formation and osteointegration in vivo. In summary, the combination of fMD with the microporous surface of SPM2 resulted in a nano-submicro-microporous surface with optimized surface performance, which possessed not only osteogenic bioactivity but also an antibacterial effect. As a bone implant, SPM2 with osteogenic and antibacterial functions may have enormous potential as a bone tissue substitute.

Titanium (Ti) and its alloy are widely used in biomedical implant but the poor bioactivity of Ti restricts some direct applications. Polydopamine (PDA), a functional polymeric substance mimicking the mussel adhesive protein, is deposited on Ti surface to improve the bioactivity by means
of a mild and facile technique employing an alkaline dopamine solution. The PDA coated Ti implant accelerates nucleation and growth of hydroxyapatite (HAP), enhances osteoblast cell adhesion and proliferation, and improves the ALP activity thus having promising biomedical applications.

Copper, a frequently used additive of implant materials, can alter macrophage phenotype thus directing the fate of the implants. Exosomes, secreted by mammalian cells, can target to recipient cells and mediate their functions. However, whether exosomes derived from macrophages upon copper ion stimulation can modulate angiogenesis, a key index for implant osseointegration, is still unclear. Herein, the influence of copper ions on macrophage-derived exosome secretion, ingestion behavior by endothelial cells, and angiogenic-induction ability is investigated. The results show copper ions (0–100 μM) have little influence on the secretion of macrophage-derived exosomes. Endothelial cells can uptake the exosomes from all the groups in a time-dependent manner. The exosomes have little influence on endothelial adhesion and proliferation, but can upregulate angiogenic ability of endothelial cells in vitro and in vivo , which may be related to trafficking of integrin β1. The results provide insight into the effect of copper ions on immunomodulatory mechanism of macrophages, which is important for implant design from the perspective of material compositions.

Intratumoral implants have aroused great interests for local chemotherapy of cancer, however, how to efficiently control drug release from implants is still a great challenge. Herein, we designed and prepared a new hollow bullet-shaped implant with porous surface by 3D printing, loaded chemotherapeutic agent cytoxan (CTX) with tetradecyl alcohol or lecithin as matrix and coated it with poly (lactic acid) to obtain a CTX implant, which has a highly tuned drug release property with a drug release time from 4 h to more than 1 month. The drug release from the implant can be easily controlled by changing pore sizes, kinds of matrices, and coating thickness.

Multifarious strategies have been proposed to enhance the antibacterial ability of implant surfaces for preventing bacterial infection, however, developing facile and universal modification methods still remains extremely elusive. Herein, inspired by the fact that the electron transfer respiratory chain of bacteria is embedded in the membrane, we proposed a novel strategy of local alkalinity disturbing bacterial respiration to endow implant materials with antibacterial ability. As a demonstration, MgO was deposited on biomedical titanium via magnetron sputtering to regulate surface alkalinity. With the thickness of MgO films increasing, they exhibited an excellent antibacterial rate against both Gram-negative and positive bacteria. The antibacterial mechanism confirmed that the alkaline surface can disturb the bacterial respiration action via weakening the transmembrane proton concentration gradient, resulting in the blockage of energy metabolism and the increase of oxidative stress of bacteria. Cell experiments indicated that MgO films not only have no obvious cytotoxicity to osteoblast cells, but can also selectively kill bacteria and promote cell proliferation in the presence of both bacteria and cells. More importantly, the by-product of MgO was only the biocompatible Mg2+, reducing any concerns about potential toxic effects. Furthermore, sputtering alkaline MgO films was confirmed to work well on polyetheretherketone polymer and zirconia ceramic implants, which indicates that this strategy has broad prospects of clinical application for preventing implant-associated bacterial infection.

Infection associated with implants and devices is a problem in medicine, especially when bacterial colonization leads to the formation of biofilms on the implant surface. Here, we report an effective strategy to develop dual-functional coatings on titanium implants with antibacterial and antifouling properties to combat biofilm infections. Coating monomers with antibacterial and antifouling blocks were prepared by conjugating curcumin-derived curcumin carboxylic acid (Cur-COOH) with different chain lengths of allyl polyethylene glycol (APEG; MW 1200 or 2400). The coating monomer (APEG 1200/2400 -Cur) was grafted onto the surface of the titanium implant through azobisisobutyronitrile (ABIN)-induced radical polymerization to form dual-functional coatings with antibacterial and antifouling properties. Scanning electron microscopy & energy dispersive spectroscopy (SEM & EDS), contact angle (CA), and X-ray spectroscopy (XPS) confirmed successful grafting of coupling compounds. The coatings exhibited excellent antibacterial activity against Gram-negative bacteria ( Escherichia coli ) and Gram-positive bacteria ( Staphylococcus aureus ) and prevented biofilm formation. The coatings also reduced the adsorption of proteins and the adhesion of platelets, indicating their excellent antifouling properties. Notably, the coatings also exhibited favorable biocompatibility with mammalian erythrocytes and pre-osteoblasts. In summary, this dual-functional coating showed remarkable potential for application in preventing and treating infections caused by biofilms.

Implantable biomaterials and biosensors are integral components of modern medical systems but often encounter hindrances due to the foreign body response (FBR). Herein, we report an albumin coating strategy aimed at addressing this challenge. Using a facile and scalable silane coupling strategy, human serum albumin (HSA) is covalently grafted to the surface of polydimethylsiloxane (PDMS) implants. This covalently grafted albumin coating remains stable and resistant to displacement by other proteins. Notably, the PDMS with covalently grafted HSA strongly resists the fibrotic capsule formation following a 180-day subcutaneous implantation in C57BL/6 mice. Furthermore, the albumin coating led to reduced recruitment of macrophages and triggered a mild immune activation pattern. Exploration of albumin coatings sourced from various mammalian species has shown that only HSA exhibited a promising anti-FBR effect. The albumin coating method reported here holds the potential to improve and extend the function of silicone-based implants by mitigating the host responses to subcutaneously implanted biomaterials.
