It is challenging for traditional hemostatic sponges to meet the clinic demand for both uncontrolled and noncompressible hemorrhage. With the aim to develop a rapid shape recovery material with both active and passive hemostatic performance, a dual-functional hemostatic sponge (TRAP-Sp) with a macroporous structure and good mechanical properties for controlling massive and noncompressible hemorrhage was prepared by chemically immobilizing thrombin-receptor-agonist-peptide (TRAP) onto a starch/polyethylene glycol (PEG) sponge. The TRAP2-Sp1 showed the best hemostatic performance among all samples in both rat artery uncontrollable hemorrhage and liver defect noncompressible hemorrhage models. When analyzing the hemostatic mechanism of TRAP-Sp, the high water absorption capacity of the sponge contributed to absorbing plasma, concentrating blood cells, and enhancing blood coagulation . After absorbing water, the shape-fixed TRAP-Sp with sufficient mechanical strength and high resilience can rapidly expand and apply pressure to the wound. TRAP immobilized on the sponge could activate the adhered platelets in an active pathway. Additionally, evaluation of cytotoxicity, hemolysis, and histology further highlighted the adequate biocompatibility of TRAP-Sp. With excellent hemostatic performance and biosafety, this sponge could be a potential candidate as a topical hemostatic agent for uncontrolled and noncompressible hemorrhage. Statement of Significance There is a need for innovative hemostatic materials for both uncontrolled and noncompressible hemorrhage. This manuscript describes a rapid shape recovery hemostatic sponge with both active and passive hemostatic performances synthesized by foaming technique, cross-linking reaction, and chemical immobilization of thrombin-receptor-agonist-peptide (TRAP). On contact with blood, the shape-fixed sponge can not only rapidly recover its original shape and concentrate platelets and RBCs but also activate the adhered platelets efficiently. The dual-functional sponge has excellent hemostatic efficacy in rat femoral artery hemorrhage and can control noncompressible hemorrhage in penetrating liver wound. Thus, we believe that this sponge could be a potential candidate as a topical hemostatic agent for uncontrolled and noncompressible hemorrhage.

Chitosan (CHI) is a versatile biological material that is well known for its hemostatic properties. This preliminary study evaluated several self-assembling hydrophobically modified chitosan (HM–CHI) sponges to determine their efficacy on hemostasis . Fourier transform infrared (FT-IR) spectroscopy was used to determine the successful graft of dodecyl groups onto the nitrogen atoms of CHI molecules. A platelet aggregation assay revealed that HM–CHI accelerated the platelet aggregation rate. Fluorescence spectroscopy showed that the HM–CHI changed the structure of fibrinogen in blood. Activated partial thromboplastin time, prothrombin time, fibrinogen time, and thromboelastographic assays were used to explore the effect of HM–CHI on the autologous blood coagulation pathway. Finally, a hemostatic sponge was made with HM–CHI and freeze-dried zeolite composite film and was applied to the rat femoral artery hemostasis model. A hemostasis time of 86 ± 5 sec was achieved, which was significantly better than the one composed with pure CHI. The experimental results of the HM–CHI hemostatic materials are inspiring and will encourage the research and development of such materials. HM–CHI may be a strong candidate as a safe and effective hemostatic material. Copyright © 2017 John Wiley & Sons, Ltd.

The interactions between human serum albumin (HSA) and the hemostatic components of the Chinese medicine Sanguisorbae Radix (SR), specifically phenolic acid compounds such as caffeic acid (CA), ferulic acid (FA) and their 1:1 mixture (1:1) were studied to investigate the molecular mechanism underlying the hemostatic effect of SR. Network pharmacology combined with the experimental and computational data revealed that HSA is one of the hemostatic targets to SR phenolic acids. SDS-PAGE and multi-spectroscopy demonstrated that the phenolic acids bind to the Sudlow site I on HSA, altering its structure and influencing its migration velocity. There is an observed synergistic effect upon the mixture of CA and FA. Quantum chemistry, molecular docking, and molecular dynamics simulations indicate that the binding of phenolic acids to HSA is stable, and variations in binding efficiency are associated with the hydrophobicity of the substituent at the C3 position of the side chain, and also, the key amino acids and functional groups for hemostasis of SR were identified, along with the active sites that contribute to the synergistic enhancement by phenolic acids.

To address the clinical challenges of conventional cotton gauze (CG), including inadequate hemostatic performance and strong blood clot adhesion, which impeded dressing removal, this study developed hydrophilic kapok fiber gauze (H-KG) and Janus kapok fiber gauze (J-KG) via hydroentanglement and chemical modification of natural kapok fibers. Results demonstrated that the kapok-based gauze significantly reduced blood loss and hemostatic time compared to CG, indicating superior hemostatic efficacy. Additionally, J-KG leveraged asymmetric wettability to prevent blood clot accumulation at the wound-dressing interface, enabling facile dressing removal. This work proposed, for the first time, the hemostatic mechanism of kapok fibers: axially aligned hollow cavities, thin-walled mesopores, and nanoscale grooves collectively generate multiscale capillary forces, forming a three-dimensionally interconnected fluid transport network, which spatially confined and enriched coagulation components through size-sieving effects, thereby accelerating the coagulation cascade. The study pioneered the high-value biomedical application of kapok fiber—a traditional economic crop—as advanced hemostatic gauze.

Powder hemostatic particles have no limitation on wound type and a wide application prospect in the treatment of wound hemostasis. In this study, bioactive glass-based membrane-like structure camouflage composite particles ( [email protected] /CS) were prepared by Layer-by-layer (LBL) method to coat bovine serum albumin (BSA) and chitosan (CS) on the surface of the synthesized macroporous and mesoporous bioactive glass (MBG). The results showed that [email protected] /CS particles significantly improved the coagulation effect of MBG and could activated both endogenous and exogenous physiological coagulation pathways. [email protected] /CS had the excellent water absorption quality and could immediately release Ca 2+ to participate in the coagulation pathway. The main hemostatic mechanism of [email protected] /CS was that the membrane-like of BSA/CS could form a fibrin network by aggregating erythrocytes and activating platelets, thus rapidly aggregating coagulation factors. In addition, [email protected] /CS particles had a significant hemostatic effect on surface bleeding (tail-amputation model) and internal bleeding (liver injury model) in SD rats, which could shorten the bleeding time and reduce the amount of blood loss effectively. This research work stated that the MBG-based membrane-like structure camouflage composite particles might be with the potential applications as a hemostasis biomaterial for the emergency bleeding.
