Mussel-inspired hydrogel with injectable self-healing and antibacterial properties promotes wound healing in burn wound infection

Sun Ao,Hu Danrong,He Xinye,Ji Xiao,Li Tao,Wei Xiawei,Qian Zhiyong

Burn wound infections cause serious problems for public health. More than 180,000 patients die from burns every year worldwide. In addition, the difficulty of healing wounds and wound infections caused by burns affects the mental health of patients. Therefore, it is very important to develop a wound dressing that can promote wound repair and exhibits good antibacterial effects. Here, we used oxidized konjac glucomannan (OKGM), γ-poly(glutamic acid) modified with dopamine and L -cysteine (γ-PGA-DA-Cys) and ε-polylysine (ε-PL) to produce an OKGM/γ-PGA-DA-Cys/ε-PL (OKPP) hydrogel. This hydrogel was produced by thiol-aldehyde addition and Schiff-base reactions and has the ability to be injected and self-heal. The results showed that the hydrogel exhibits good antibacterial effects on Pseudomonas aeruginosa and Staphylococcus aureus and has antioxidant effects in vitro. Moreover, the hydrogel also exhibits good adhesion. In a burn wound infection model, the hydrogel promoted wound healing and reduced the production of inflammation. These results proved that the hydrogel has clinical potential as a wound dressing for burn wound infection.

Neutrophil-derived heparin binding protein triggers vascular leakage and synergizes with myeloperoxidase at the early stage of severe burns (With video)

Liu Lu,Shao Yiming,Zhang Yixuan,Yang Yunxi,Huang Jiamin,Li Linbin,Sun Ran,Zhou Yuying,Su Yicheng,Sun Bingwei

BackgroundBurn shock caused by vascular leakage is one of the main causes of high mortality in severe burn injury. However, the pathophysiological mechanism of vascular leakage is still unclear. The purpose of this study was to explore the molecular mechanism of vascular leakage in the early stage of severe burn and provide a new target for the treatment of severe burns.MethodsNeutrophils were isolated from human peripheral blood by magnetic beads sorting. ELISA was used to detect neutrophil-derived granule proteins and glycocalyx injury products in plasma. The vascular leakage and neutrophil movement were assessed by in vivo laser confocal imaging in mice, and high-quality video were provided. Adhesion-related molecules were investigated by qRT-PCR. The damage to glycocalyx of mice vascular endothelial cells was observed by transmission electron microscope and scanning electron microscope. Proteomic analysis, flow cytometry and immunofluorescence were used to further study the relationship between human peripheral blood neutrophil-derived hypochlorite (HOCl) and CD44 of human vascular endothelial cells.ResultsIn this study, we found that rapidly increasing activated neutrophils secrete heparin binding protein (HBP) and myeloperoxidase (MPO) after severe burn injury. Increased HBP triggers vascular leakage with synergy of MPO, results in systemic edema and burn shock. Furthermore, we found that the MPO catalytic product HOCl but not MPO triggers CD44 extracellular domain shedding from vascular endothelial cells to damage the glycocalyx. Damage to the glycocalyx results in firm adhesion of neutrophils and increases vascular leakage. However, MPO inhibitors partially protect the glycocalyx of vascular endothelial cells. The combination of HBP and MPO inhibitors markedly reduces vascular leakage and systemic edema in the early stage of severe burns.ConclusionsTaken together, these data reveal that neutrophil-derived HBP and MPO play an important synergies role in triggering vascular leakage at the early stage of severe burns. Targeted intervention in these two biomolecules may introduce new strategies for helping to reduce large amount of fluid loss and subsequent burn shock.

Multi-Functional Fibrous Dressings for Burn Injury Treatment with Pain and Swelling Relief and Scarless Wound Healing

Yuan Zhengchao,Zhao Yue,Shafiq Muhammad,Song Jiahui,Hou Jing,Liang Yuqing,Yu Xiao,Chen Yujie,Yu Fan,EL-Newehy Mohamed,EL-Hamshary Hany,Morsi Yosry,Jiang Shichao,Zheng Hui,Mo Xiumei

Abstract As one of the most common forms of skin injuries, skin burns are often accompanied by edema pain, suppuration of infection, slow tissue regeneration, and severe scar formation, which significantly delay wound healing as well as affect the quality of life. We prepared multifunctional electrospun poly(L-lactide-co-glycolide)/gelatin (P/G)-based dressings to synergistically harness the therapeutic benefits of peppermint essential oil (T), burn ointment (B), and Oregano essential oil (O) (P/G@TBO) for skin regeneration in punch and burn injury models. The P/G@TBO can afford the sustained release of bioactive cues for up to 72 h as well as remarkably promote cell migration ( ca. P/G@TBO, 89% vs. control group, 51%) at 24 h. The P/G@TBO membranes also showed significant angiogenic effect as well as antibacterial and anti-inflammatory properties than that of the control group in vitro. Moreover, P/G@TBO dressings enabled fast wound healing ( ca. P/G@TBO, 100% wound closure vs. control group, 95%) in a full-thickness excisional defect model up to 14 days in rats. Further evaluation of membranes in different animal models, including tail wagging model, facial itch model, and hot burn injury model showed significant pain relieve effect as well as itching and swelling relief functions during earlier stages of wound healing. Membranes were next transplanted into a scalded wound model in rats and an ear punch wound model in rabbits, which manifested antibacterial and anti-inflammatory properties and promoted re-epithelialization to achieve scarless wound healing percentage wound closure at day 28: P/G@TBO, 96% vs. control group 66%. Taken together our approach of simultaneously harnessing T, B, and O to enable multifunctionality to fibrous dressings may hold great promise for burn wound healing applications and other related disciplines. Chitosan-Gentamicin Conjugate Hydrogel Promoting Skin Scald Repair

Tingting Yan,Songzhi Kong,Qianqian Ouyang,Chengpeng Li,Tingting Hou,Yu Chen,Sidong Li

Our earlier research indicated that chitosan-gentamicin conjugate (CS-GT) possesses superior antimicrobial activity and good water solubility. To develop CS-GT-based scald dressings, the antibacterial properties of CS-GT were further studied, and the biosafety of CS-GT and the healing mechanism of CS-GT hydrogel was systematically explored in this article. It was found that cell viability shows a declined inclination with the prolonged culture time and the increased concentration of CS-GT. After three day’s culture, the cell viability could still remain at 79.72% when CS-GT concentration was as high as 1000 μg/mL. On the other hand, the hemolysis rate of CS-GT was lower than 5% when its concentration is 800 μg/mL. Therefore CS-GT has good cytocompatibility and hemocompatibility. A wound-healing experiment has shown that the skin healing rate of CS-GT hydrogel was the highest at 99.61%, followed by the positive control (wet burn ointment) 94.98%, GT hydrogel 87.50%, and matrix 77.39%. The blank control group, however, possessed the lowest healing rate of 75.45%. Further analysis indicated that CS-GT hydrogel could promote the synthesis of total protein (TP) in skin granulation tissue, resulting in the enhanced hydroxyproline (HYP) content, which facilitated collagen fibrogenesis, reduced cytokine expression in an inflammatory response, and, ultimately, accelerated wound healing. To sum up, CS-GT hydrogel is a promising scald dressing.Keywords:chitosan-gentamicin conjugate;antimicrobial;anti-inflammatory;scald repair

Microneedle Patch for Transdermal Sequential Delivery of KGF-2 and aFGF to Enhance Burn Wound Therapy

Huacheng He,Wen Huang,Shihui Zhang,Jie Li,Jian Zhang,Bingxin Li,Jie Xu,Yuting Luo,Huiling Shi,Yue Li,Jian Xiao,Odinaka Cassandra Ezekiel,Xiaokun Li,Jiang Wu

Severe burn wounds usually destroy key cells’ functions of the skin resulting in delayed re-epithelization and wound regeneration. Promoting key cells’ activities is crucial for burn wound repair. It is well known that keratinocyte growth factor-2 (KGF-2) participates in the proliferation and morphogenesis of epithelial cells while acidic fibroblast growth factor (aFGF) is a key mediator for fibroblast and endothelial cell growth and differentiation. However, thick eschar and the harsh environment of a burn wound often decrease the delivery efficiency of fibroblast growth factor (FGF) to the wound site. Therefore, herein a novel microneedle patch for sequential transdermal delivery of KGF-2 and aFGF is fabricated to enhance burn wound therapy. aFGF is first loaded in the nanoparticle (NP aFGF ) and then encapsulated NP aFGF with KGF-2 in the microneedle patch (KGF-2/NP aFGF @MN). The result shows that KGF-2/NP aFGF @MN can successfully get across the eschar and sequentially release KGF-2 and aFGF. Additional data demonstrated that KGF-2/NP aFGF @MN achieved a quicker wound closure rate with reduced necrotic tissues, faster re-epithelialization, enhanced collagen deposition, and increased neo-vascularization. Further evidence suggests that improved wound healing is regulated by significantly elevated expressions of hypoxia-inducible factor-1 alpha (HIF-1ɑ) and heat shock protein 90 (Hsp90) in burn wounds. All these data proved that KGF-2/NP aFGF @MN is an effective treatment for wound healing of burns.

Injectable deferoxamine-loaded microsphere hydrogels for inhibition of ferroptosis and promotion of third-degree burn wound healing

Langjie Chai,Jianglong Huang,Min Wang,Yihui Huang,Zhuo Huang,Ruiyu Zhang,Lu He,Haijie Wang,Danyang Chen,Yifeng Lei,Liang Guo

In chronic third-degree burn wounds, high levels of inflammatory factors such as IL-1β lead to ferroptosis in surrounding cells, thereby delaying the healing process of the burn wounds. To promote the healing of such wounds, an injectable DFO@GM-H hydrogel was developed. This hydrogel was prepared by loading deferoxamine (DFO) into gelatin microspheres (GMs), and further crosslinking in a mild reaction. The resulting DFO@GM-H hydrogels demonstrated effective self-healing and injectability, rendering them well-suited for application in irregular burn wounds. DFO@GM-H hydrogels not only maintained a moist healing environment for the burn wound healing, but also exhibited the ability to slowly release DFO, therefore overcoming the limitation of the short half-life of DFO in vivo . With sustained release and enhanced stability of DFO from the injectable hydrogels, DFO@GM-H hydrogels demonstrated excellent biocompatibility, effectively promoted cell proliferation, migration and angiogenesis, showed antibacterial activity, and effectively inhibited cellular ferroptosis both in vitro and in vivo . Consequently, DFO@GM-H hydrogels accelerated the healing of burn wounds in rats via ferroptosis inhibition and enhanced cellular response. Therefore, DFO@GM-H hydrogel is anticipated to emerge as a novel bioactive dressing for the management of burn wounds.

Improved Cooling Performance of Hydrogel Wound Dressings via Integrating Thermal Conductivity and Heat Storage Capacity for Burn Therapy

Wenbin Shi,Nijia Song,Yanping Huang,Chao He,Man Zhang,Weifeng Zhao,Changsheng Zhao

Burn injury has become a crucial public health issue worldwide. It is necessary to explore new methods to reduce heat damage and improve healing efficiency during burn injury treatment. In this study, a kind of hydrogel combining heat storage capacity and thermal conductivity was fabricated via a one-pot method for burn therapy. The novel hydrogel was easily prepared by in situ cross-linking polymerization, using poly(ethylene glycol) (PEG) derivatives, oligo(ethylene glycol) methacrylate and 2-(2-methoxyethoxy) ethyl methacrylate, as thermally responsive base materials and hydroxylated multiwall carbon nanotubes (CNT-OH) as thermally conductive fillers. By dispersing CNT-OH, a thermally conductive network was formed in the hydrogel, leading to an increase in the thermal conductivity. The cooling performance, thermal conductivity, heat storage property, swelling performance, rheological and mechanical properties, biocompatibility, in vivo cooling effect, and wound healing properties of the prepared hydrogel were systematically investigated. The hydrogel consisted of thermally responsive PEG derivatives, and CNT-OH performed a function of rapid heat absorption, further reduced thermal damage, and promoted wound healing. The improved cooling performance of the hydrogel was ascribed to the improved thermal conductivity, enhanced heat storage capacity, and good adhesive ability. Thus, the hydrogel has great potential to be practically applied in burn therapy, laser treatment, cooling fabrics, heat-protective clothing, and other emergency scenarios.

The effects of aqueous extract of Labisia Pumila (Blume) Fern.-Vill. Var. Alata on wound contraction, hydroxyproline content and histological assessments in superficial partial thickness of second-degree burn model

Ibrahim Nurul ‘Izzah,Mohamed Isa Naina,Mohamed Norazlina,Mohd Ramli Elvy Suhana,Shuid Ahmad Nazrun

Background: Burns are considered a critical care problem in emergency medicine, resulting in physical, psychological, and chronic disabilities. Silver sulfadiazine is the gold standard in topical burn treatment but was associated with toxicity to keratinocytes and fibroblasts, which may delay wound healing. In discovering potential alternative treatments for burn wound healing, this study was performed to determine the effect of Labisia Pumila (Blume) Fern.-Vill. Var. Alata (LPVa) extract on thermal-burn wounds in rats. Methods: A total of 50 Sprague-Dawley male rats were categorized into five groups. There were three control groups; normal control (left untreated), negative control (given ointment base) and positive control (given silver sulfadiazine). Meanwhile, the two intervention groups were given with 2% LPVa leaf and root extracts, respectively. Burn wounds were inflicted on the loin region of the rat by applying a heated steel rod at 80°C for 10 s. On days 3, 7, 14, and 21, wounds were measured macroscopically using a digital calliper and one animals of each group were sacrificed, and the wounded skin were excised for histomorphological assessments. The wounds were excised for hydroxyproline content on Day 14 of treatment.Result: For wound contraction percentage, both the leaf and root extracts of LPVa showed a significant reduction in burn wound size on Day 7 onwards, when compared to other groups. For hydroxyproline content, only the leaf extract of LPVa produced significantly higher content compared to both negative and normal control groups. In terms of histological examination, the leaf extract group demonstrated a superior healing effect than the root extract group. Conclusion: Both leaf and root extracts of LPVa could promote wound healing in the thermal-burn wound rat model, with leaf extract being superior to root extract.

Acceleration of skin regeneration in full-thickness burns by incorporation of bFGF-loaded alginate microspheres into a CMCS–PVA hydrogel

Quan Liu,Yuchen Huang,Yong Lan,Qinhua Zuo,Chenghua Li,Yi Zhang,Rui Guo,Wei Xue

Two important issues in skin tissue engineering are the vascularization and regeneration of the dermis. Basic fibroblast growth factor (bFGF) is known to promote angiogenesis and accelerate wound healing. Direct delivery of bFGF to the wound area, however, would lead to a loss of bioactivity. To this end, bFGF-loaded alginate microspheres (Ms) were fabricated and incorporated into carboxymethyl chitosan (CMCS)–poly(vinyl alcohol) (PVA) to form a composite hydrogel. Scanning electron microscopy (SEM) results indicated that the incorporation of Ms does not significantly affect the inner structure of CMCS–PVA. In an in vitro study, the release of bFGF from Ms–CMCS–PVA in a sustained manner retained higher bioactivity over a 2-week period. Full-thickness burn wounds were created in the dorsal area of rats for in vivo evaluation of skin regeneration treated with CMCS–PVA hydrogel, with and without bFGF. Compared with the control, CMCS–PVA and bFGF–CMCS–PVA groups, the bFGF/Ms–CMCS–PVA group revealed significantly faster wound recovery rates, with re-epithelialization and regeneration of the dermis. Moreover, the bFGF/Ms–CMCS–PVA group had the highest density of newly formed and mature blood vessels during the 2 mweek treatment period. The ability of the bFGF/Ms–CMCS–PVA hydrogel to accelerate wound healing in a full-thickness burn model suggests its potential for use in dermal tissue regeneration. Dual antibacterial polypeptide-coated PCL@ZIF-8 nanofiber reduces infection and inflammation in burn wounds

Bai Xiao-Han,Zhang Jun,Cheng Guo-Ting,Liu Xiao-Fei,Sun Tian-Cai,Yang Jun,Cao Zhi-Kai,Ramakrishna Seeram,Long Yun-Ze

Burns on human skin often leads to slow recovery because of continuous wound infections caused by the loss of skin protection. In this study, a seed-free soaking method is put forward to prepare PCL@ZIF-8/εPL nanofibers as a dressing for burn wounds. This ZIF-8-coated polycaprolactone (PCL) fiber exhibits increased specific surface area, significantly increasing the polypeptide loading of epsilon polylysine (εPL) from 12 to 20 wt.%. The antibacterial experiment found that ZIF-8 and εPL exhibited dual antibacterial properties, and after εPL was loaded to ZIF-8, the nanofiber demonstrated stronger antibacterial properties that can better kill the remaining tenacious bacteria. This high-efficiency bactericidal property, combined with the ability to protect wounds from common infections by bacteria in the air, would promote wound healing, shortening the healing time period from 24 to 17 days. Further tissue sections confirm that the shielding effect and dual antibacterial properties of the composite fiber membrane are the reasons behind the acceleration of the healing of burns and scalds. Shielding and antibacterial material are helpful for the treatment of burns and scalds, and it may also guide the treatment of other types of wounds.

Enhanced thermal conductive and moisturizing hydrogels by constructing 3D networks of BN-OH and CNT-OH in alignment for burn therapy

Wenbin Shi,Nijia Song,Yanping Huang,Wenjie Wang,Chao He,Weifeng Zhao,Changsheng Zhao

Hydrogel treatment as a common cooling method could effectively ease pain and promote wounds healing by reducing thermal injury to the skin. However, the inherent low thermal conductivity of polymers limits the improvement of hydrogels’ cooling performance. The present study reports a simple method by combining thermal conductive fillers 1D hydroxylated multiwall carbon nanotubes (CNT-OH) and 2D hydroxylated hexagonal boron nitride (BN-OH) to construct 3D conductive networks in hydrogels for cooling treatment and burn therapy. It is interesting that the thermal conductive fillers are aligned via varying the mass ratios of CNT-OH to BN-OH. Hence, the utilization efficiency of the thermal conductive fillers is improved; and the thermal conductivity of the hydrogels is enhanced up to 1.31 W*m −1 *K −1 , which is 226 % to the pristine hydrogel of 0.58 W*m −1 *K −1 . Consequently, the optimized hydrogel exhibits rapid cooling abilities that the heated porcine skin only takes 168 s to decrease 30 °C with the treatment of the hydrogel. More importantly, the thermal conductivity enhanced hydrogel exhibits the capacities of rapid cooling, reducing heat injury and pain relieving, and promoting wound healing in vivo tests, which prove that the enhanced thermal conductive and moisturizing hydrogel can be a good alternative for burn therapy.

Composite antibacterial hydrogels based on two natural products pullulan and ε-poly-l-lysine for burn wound healing

Wenzhuang Cui,Chu Gong,Yujie Liu,Ying Yue,Jun Wang,Zhizhou Yang,Junli Yang

Antibacterial hydrogels as burn wound dressings are capable of efficaciously defending against bacterial infection and accelerating burn wound healing. Thus far, a large plethora of antibacterial hydrogels have adopted numerous components and intricate preparation processes, yet restricting their practical industrialization applications. Simple and effective preparation methods of antibacterial hydrogels are hence urgently needed. Herein, an easy but efficacious strategy with the employment of two natural products pullulan and ε-poly- l -lysine (ε-PL) was designed to fabricate composite antibacterial hydrogels for burn wound healing for the first time. The hydrogel crosslinking networks were formed through amidation reactions between carboxylated pullulan derivative (CP) and ε-poly- l -lysine hydrochloride (ε-PL·HCl). The resulting hydrogels possessed high transparency, porous structures, tunable gelation time and gel content, relatively low swelling ratios, appropriate self-degradability, proper mechanical properties, strong in vitro bacteriostatic activities, non-cytotoxicity, capacities of facilitating cell migration and excellent hemocompatibility. In the infected burn model of mice, the hydrogels were observed to display prominent in vivo antibacterial activities and enable the acceleration of burn wound healing. We opine the simply and effectively prepared antibacterial hydrogels as promising dressings for burn wound recovery have broad industrialization prospects.
