Application of Zirconia Ceramic Biomaterial Implants in the Restoration of Tooth Loss Caused by Periodontal Disease

Wei Zhili,Wei Li

Zirconia ceramics (ZiC) has become a biomaterial with good clinical application prospects due to its good biocompatibility, stability, and excellent mechanical properties. In this study, bioactive glass (BioS) was undertaken as the modifier, and the ZiC precursor solution was penetrated on the surface of the ZiC body or the ZiC particles were coated with the BioS precursor solution, so as to obtain the ZiC with biological activity by introducing the BioS phase. While the material was characterized, its mechanical properties and biological activity were analyzed by simulating the mineralization of the body fluid, including the cell adhesion, proliferation, and differentiation of the material. The ZiC was applied as an implant for restoration of patients with teeth loss caused by periodontal disease. In the test, after surface infiltration of BioS sol or coating of ZiC particles, a very thin BioS phase can be formed on the surface of ZiC after high temperature sintering. The in vitro mineralization test showed that the activation of the grain boundary promoted the deposition of calcium and phosphorus layers on the surface of the sample. The mechanical test revealed that the relative density, flexural strength, and fracture toughness of the material decreased with the infiltration and coating of BioS. However, under proper preparation conditions, the modified ZiC still showed good mechanical properties and can promote the spreading, adhesion, and differentiation of mouse bone marrow mesenchymal stem cells (mBMSCs). Clinical trials proved that after the modified ZiC implant material was implanted in patients with teeth loss, the structure was not loose or broken, and the intraoral prosthesis could function normally. After treatment, the level of IL-1β in the gingival crevicular fluid of the patient decreased obviously (P <0.05). In addition, the probing depth (PD), clinical attachment loss (CAL), and other indicators decreased greatly (P <0.05).

Research Note: A novel method for preparation of egg yolk immunoglobulin Y against Porphyromonas gingivalis

Han Su,Kemin Wei,Minlei Zhao,Xin Li,Yibo Zhang

ABSTRACT Porphyromonas gingivalis ( P. gingivalis, P. g ) is the main pathogen of periodontal disease, which is treated with egg yolk immunoglobulin Y ( IgY ) against P. gingivalis . In order to quickly obtain IgY, 30 hens were immunized with inactivated P. gingivalis . The purification of IgY was carried out by the oleic acid ( OA ) method and the classical method ( AS ), respectively. The IgY antibody characteristics and antibacterial effects in HPDLF cells were detected by SDS-PAGE, indirect ELISA, Western blot and viability/toxicity assays. SDS-PAGE and Western blot analysis showed that IgY molecules which were rapidly purified by OA method were complete and specific to P. gingivalis . In addition, the results of crystal violet staining and bacterial staining indicated that IgY could agglutinate with P. gingivalis , inhibiting bacterial invasion of host cells. This study is the first to rapidly and efficiently purify IgY by OA method, and the purified IgY is expected to be used in the detection and treatment of P. gingivalis .

Detection of Periodontal Pathogens Based on an Integrated Continuous Flow PCR and Capillary Electrophoresis Microfluidic Chip

Bo Yang,Jiaxin Huang,Chunxian Tao,Zhenqing Li,Dawei Zhang,Yoshinori Yamaguchi

Continuous-flow PCR (CF-PCR) can realize rapid DNA amplification because of the high temperature variation rate. However, off-line detection methods for PCR may induce cross contamination. To overcome this problem, we herein fabricated an integrated CF-PCR and electrophoresis microfluidic chip. The optimal voltage applied in the electrophoresis part of the microfluidic chip was achieved by simulation in COMSOL. Coating the inside wall of the microchannel can inhibit electroosmotic flow and improve the resolution for DNA fragments. The temperature distribution of the serpentine part can meet the PCR and has no obvious suppressive effect on sample separation. Finally, we have performed the amplification of target genes forPorphyromonas gingivalis,Tannerella forsythia, andTreponema denticolaand detected the corresponding PCR products in the microfluidic chip within 11 min. Such work provides a new method for the rapid detection of bacteria. Keywords:capillary electrophoresis;periodontal pathogens;continuous flow PCR;microfluidic chip;fluorescence detection

Facile minocycline deployment in gingiva using a dissolvable microneedle patch for the adjunctive treatment of periodontal disease

Huimin Li,Xueyu Wen,Xinyi Gong,Yange Wu,Puxuan Zhao,Yun Zhang,Zhuomin Sha,Hao Chang,Xuepeng Chen

Minocycline is a commonly used drug for adjunctive therapy in periodontal disease. However, the current mainstream local medications primarily rely on intra-pocket administration, which, while avoiding the side effects of traditional systemic drugs, presents challenges such as inconvenience, discomfort, and the need for professional assistance, thus affecting patient compliance. Herein, we introduce a minocycline-loaded dissolvable microneedle (Mino-DMN) patch that allows for local and efficient delivery of minocycline to gingiva for the treatment of periodontitis. A two-step casting micro-molding process involving vacuum drying and freeze drying is employed to concentrate minocycline in the microneedle part and limit its diffusion into the patch backing. The resulting Mino-DMN patch features an array of minocycline-enriched gelatin MNs with a porous HA patch backing. The microneedles can penetrate into gingiva with enough mechanical strength and quickly release minocycline into the gingival tissue, ensuring prolonged local residence of the drug and minimizing its loss to saliva. In vivo experiments show Mino-DMN inhibits pro-inflammatory factors, promotes anti-inflammatory factors, and stimulates bone formation, surpassing topical application and comparable to the inconvenient and discomfort administration of Periocline®. This proposed Mino-DMN offers a simple, efficient, user-friendly strategy for the adjunctive treatment of periodontal disease.

Au@Ag Nanorods-PDMS Wearable Mouthguard as a Visualized Detection Platform for Screening Dental Caries and Periodontal Diseases

Dongxu Ma,Baiqi Chen,Yuanfang Li,Xueyuan Pang,Quanying Fu,Zihan Xiao,Zhonghong Shi,Xiaolei Li,Chongdai Luo,Zhang-kai Zhou,Yin Chen,Jianhua Zhou

The development of easy-to-use, low-cost, and visualized detection platforms for screening human dental caries and periodontal diseases is in urgent demand. In this work, a Au@Ag nanorods-poly(dimethylsiloxane) (Au@Ag NRs-PDMS) wearable mouthguard, which can visualize the tooth lesion sites through the color change of it at the corresponding locations, is presented. The Au@Ag NRs-PDMS composite exhibits a distinct color response to hydrogen sulfide (H 2 S) gas generated by bacterial decay at the lesion sites. Moreover, the Au@Ag NRs-PDMS mouthguard is demonstrated to own desired mechanical properties, excellent chemical stability, as well as good biocompatibility, and can accurately locate the lesion sites in human oral cavity. These findings suggest that the mouthguard has the potential to be utilized on a large scale to help people self-monitor their oral health in daily life, and treat oral diseases locally.

Zoledronic acid affects the process of Porphyromonas gingivalis infecting oral mucosal epithelial barrier: An in-vivo and in-vitro study

Sun Hanyu,Li Pugeng,Kong Qingci,Deng Feilong,Yu Xiaolin

Zoledronic acid (ZA), one of the commonly used bisphosphonates, is mainly used for bone-metabolic diseases. Studies proved that ZA has adverse effects on oral soft tissues. As the first line of innate immunity, the gingival epithelium could be infected by periodontal pathogens, which is a key process of the initiation of periodontal diseases. Yet, how ZA affects the periodontal pathogens infecting the epithelial barrier remains unclear. This study aimed to investigate the influences of ZA on the process of Porphyromonas gingivalis (P. gingivalis) infecting the gingival epithelial barrier via in-vitro and in-vivo experiments. In the in-vitro experiments, under the condition of different concentrations of ZA (0, 1, 10, and 100 μM), P. gingivalis was used to infect human gingival epithelial cells (HGECs). The infections were detected by transmission electron microscope and confocal laser scanning microscope. Besides, the internalization assay was applied to quantify the P. gingivalis, which infected the HGECs, in the different groups. To evaluate the expression levels of pro-inflammatory cytokines, including interleukin (IL)-1β, IL-6, and IL-8, by infected HGECs, real-time quantitative reverse transcription-polymerase chain reactions were applied. In the in-vivo experiments, rats were given ZA solution (ZA group) or saline (control group) by tail intravenous injection for 8 weeks. Subsequently, we put ligatures around the maxillary second molars of all the rats and inoculated P. gingivalis to the gingiva every other day from day 1 to day 13. The rats were sacrificed on days 3, 7, and 14 for micro-CT and histological analyses. The in-vitro results manifested that the quantity of P. gingivalis that had infected HGECs increased with the ZA concentrations. Pro-inflammatory cytokines expression by HGECs were significantly increased by 100 μM ZA. In the in-vivo study, compared to the control group, more P. gingivalis was detected in the superficial layer of gingival epithelium in the ZA group. Besides, ZA significantly increased the expression level of IL-1β on day 14 and IL-6 on days 7 and 14 in gingival tissues. These findings suggest that the oral epithelial tissues of patients who receive high-dose ZA treatment may be more susceptible to periodontal infections, resulting in severe inflammatory conditions.

Grafting resveratrol onto mesoporous silica nanoparticles towards efficient sustainable immunoregulation and insulin resistance alleviation for diabetic periodontitis therapy

Yujie Tan,Jing Feng,Yu Xiao,Chongyun Bao

Host-modulation therapy is generally accepted as a novel promising method for diabetic periodontitis (DP) treatment and screening an appropriate drug model is the key to success. Resveratrol (RSV), because of its viable antioxidative and anti-inflammatory properties and its ability to control glucose metabolism, is considered a potential candidate. However, poor water solubility, rapid decomposition and short serum half-life period significantly limit its application. Therefore, in this study, we designed a RSV-grafted mesoporous silica nanoparticle (MSN-RSV) drug carrier system to enhance RSV's stability effectively and prolong its duration. Further analyses have verified the indispensable role of MSNs in improving the bioavailability of RSV, which could result in a more favorable therapeutic efficacy in DP related to regulating the polarization of the macrophage. The reason for this could be explained by activating the SIRT1/AMPK signaling pathway and inhibiting the NF-κB signaling pathway. This study also focused on the auxiliary effect of MSN-RSV on alleviating insulin resistance (IR) and controlling glucose metabolism. In brief, the study has provided a potential alternative strategy for DP therapy. It is also helpful for future intensive research topics like the immunoregulatory mechanisms in the bidirectional relationship between diabetes and periodontitis.

Multifunctional chitin-based barrier membrane with antibacterial and osteogenic activities for the treatment of periodontal disease

Tao Wu,Lin Huang,Jing Sun,Jiahui Sun,Qi Yan,Bo Duan,Lina Zhang,Bin Shi

The guided tissue regeneration technique is an effective approach to repair periodontal defect. However, collagen barrier membranes used clinically lose stability easily, leading to soft tissue invasion, surgical site infection, and failure of osteogenesis. An ideal barrier membrane should possess proper antibacterial, osteoconductive activities, and favorable biodegradation. In this study, zinc oxide nanoparticles were homogeneously incorporated into the chitin hydrogel (ChT-1%ZnO) through one-step dissolution and regeneration method from alkaline/urea solution the first time. The remaining weights of ChT-1%ZnO in 150 μg/mL lysozyme solution was 52% after 5 weeks soaking. ChT-1%ZnO showed statistical antibacterial activities for P. gingivalis and S. aureus at 6 h, 12 h, and 24 h. Moreover, ChT-1%ZnO exhibits osteogenesis promotion in vitro , and it was further evaluated with rat periodontal defect model in vivo . The cemento-enamel junction value in ChT-1%ZnO group is 1.608 mm, presenting a statistical difference compared with no-membrane (1.825 mm) and ChT group (1.685 mm) after 8 weeks postoperatively.

NIR responsive nitric oxide nanogenerator for enhanced biofilm eradication and inflammation immunotherapy against periodontal diseases

Manlin Qi,Xuan Ren,Wen Li,Yue Sun,Xiaolin Sun,Chunyan Li,Siyao Yu,Lin Xu,Yanmin Zhou,Shuyan Song,Biao Dong,Lin Wang

Periodontal disease is one of the most common oral infectious diseases and closely related to many systemic diseases which is in urgent need of novel and effective drugs. In particular, the problems of deep lesion location and synchronization of antibacterial and anti-inflammatory needs to be addressed. Herein, near infrared (NIR) light triggered nanoplatform with antibacterial and anti-inflammatory dual-functions by antibacterial photodynamic therapy (aPDT), photothermal therapy (PTT) and gas therapy was realized by loading S-nitrosothiols (SNO) and ICG into mesoporous silica-coated gold nanorods . Using a single 808 nm NIR light, the excellent antibacterial efficacy can be achieved not only by aPDT-induced bacteria killing but also by PTT-triggered biofilm dispersal. Furthermore, SNO molecules would also be thermally activated to release nitric oxide (NO) gas molecules at the site of periodontal disease, exerting inflammation inhibition by suppressing the assembly of pro-inflammatory factors and NLRP3 inflammasome. Additionally, such nanoplatform could validly combat pathogenic bacterial biofilm, and modulate the inflammatory immunity, leading to favorable therapeutics on animal models with periodontal diseases. Hence, this design provides an effective solution to solve the problems of deep tissue treatment along with simultaneously antibacterial/anti-inflammatory capabilities, which provides a bright prospect for clinical treatment of periodontal diseases, even extended to other refractory infections in deep sites.

Injectable hydrogels activated with copper sulfide nanoparticles for enhancing spatiotemporal sterilization and osteogenesis in periodontal therapy

Yuting Yang,Chunbin Xu,Shengqian Xu,Yan Li,Ke'er Chen,Tao Yang,Jiaqi Bao,Yajing Xu,Jingyao Chen,Chuanbin Mao,Lili Chen,Weilian Sun

Developing biomaterials capable of promoting bone regeneration in bacteria-infected sites is of utmost urgency for periodontal disease therapies. Here we produce a hybrid hydrogel by integrating CuS nanoparticles (CuSNPs), which could kill bacteria through photothermal therapy (PTT) triggered by a near infrared (NIR) light, and a gelatin methacryloyl (GelMA) hydrogel, which is injectable and biocompatible. Specifically, CuSNPs were precipitated by chitosan (CS) firstly, then grafted with methacrylic anhydride (MA) to form CuSNP@CS-MA, which was photo-crosslinked with GelMA to synthesize hybrid hydrogels (GelMA/CuSNP). The hybrid hydrogels exhibited a broad-spectrum antibacterial property that could be spatiotemprorally manipulated through applying a NIR light. Their mechanical properties were adjustable by controlling the concentration of CuSNPs, enabling the hydrogels to become more adapted to the oral diseases. Meanwhile, the hybrid hydrogels showed good cytocompatibility in vitro and improved hemostasis in vivo. Moreover, they accelerated alveolar osteogenesis and vascular genesis, successfully treating periodontis in four weeks in a rat model. GelMA/CuSNP hydrogels showed a broad-spectrum sterilization ability via PTT in vitro and outstanding antibacterial property in vivo, suggesting that the hybrid hydrogels could function in the challenging, bacteria-rich, oral environment. Such injectable hybrid hydrogels, capable of achieving both facilitated osteogenesis and NIR-inducible sterilization, represent a new biomaterial for treating periodontitis.
