The guided tissue regeneration (GTR) procedure has become an important process for the treatment of periodontal tissue regeneration, using the membrane as a mechanical barrier to generate a distance across the defect that subsequently allowing bone regeneration. Due to the complexity of periodontal microenvironment caused by bacterial infections, inflammation, and fibrous tissue penetration, the guided tissue regeneration membrane is required to have antibacterial and barrier functions. We developed a loaded methylene blue @ ZIF-8 and hydroxyapatite gradient thermoplastic polyurethane Janus membrane for antibacterial osteogenic and barrier. This Janus membrane with a porous gradient structure was prepared to use phase-conversion method. The rough layer facilitated cell growth and promoted periodontal bone regeneration, whereas the smooth layer isolated the growing fiber tissue. The rapid release of reactive oxygen species in the early stage and continuous release of zinc ions by methylene blue @ZIF-8 can achieve long-term synergistic antibacterial effect, with antibacterial rates of 84 % and 72 % against Escherichia coli and Staphylococcus aureus, respectively, which can effectively reduce the local inflammation caused by bacteria. In summary, we applied the Janus membrane to a rat periodontal bone defect model, and the membrane showed good surgical performance, barrier properties, osteogenic properties, and antibacterial effects, which could have broad application prospects in the treatment of periodontal bone defects.

The primary pathology of periodontitis involves the gradual deterioration of periodontal tissues resulting from the inflammatory reaction triggered by bacterial infection. In this study, a novel drug for periodontal pocket injection, known as the Shed-Cu-HA hydrogel, was developed by incorporating copper ions (Cu2+) and Shed-derived exosomes (Shed-exo) inside the hyaluronic acid (HA) hydrogel. Suitable concentrations of Cu2+ and Shed-exo released from Shed-Cu-HA enhanced cell viability and cell proliferation of human periodontal ligament stem cells. Additionally, the Shed-Cu-HA demonstrated remarkable antibacterial effects against the key periodontal pathogen (Aa) owing to the synergistic effect of Cu2+ and HA. Furthermore, the material effectively suppressed macrophage inflammatory response via the IL-6/JAK2/STAT3 pathway. Moreover, the Shed-Cu-HA, combining the inflammation-regulating properties of HA with the synergistic osteogenic activity of Shed-exo and Cu2+, effectively upregulated the expression of genes and proteins associated with osteogenic differentiation. The experimental findings from a mouse periodontitis model demonstrated that the administration of Shed-Cu-HA effectively reduced the extent of inflammatory cell infiltration and bacterial infections in gingival tissues and facilitated the regeneration of periodontal bone tissues and collagen after 2 and 4 weeks of injection. Consequently, it holds significant prospects for future applications in periodontitis treatment.

The main goal of periodontal therapy is to fully regenerate the bone that supports teeth while preventing non-osteogenic tissues from interfering with bone regeneration . There is no double-functional repair material in the existing treatment method. Hydroxyapatite (HAp) has been used as a bone substitute in biomedical applications. Polylactic acid (PLA) can effectively inhibit the growth of adherent cells and play a protective role as a barrier. It is possible to combine these two methods to prepare a guided regeneration membrane for guiding periodontal bone regeneration. In this study, HAp microtubules were prepared through hydrothermal synthesis , and bilayer films were prepared through self-evaporation with PLA. The HAp microtubules/PLA membrane had better tensile and breaking strengths and lower fracture productivity than the PLA membrane. The expansion rate reached 34% after 2 h, and the degradation rate exceeded 25% after 40 days. The results showed that the membrane composite had the clinically quoted mechanical requirements. The results of the cell experiments also showed that the membrane had no cytotoxicity. The HAp microtubule side can promote the expression of genes and proteins related to osteogenesis, while the PLA side can inhibit cell adhesion . The HAp microtubules/PLA membrane has stability and good mechanical properties. The double-layer structure has the function of a biological barrier and promotes bone formation.

Background Bone defects in the maxillofacial region restrict the integrity of dental function, posing challenges in clinical treatment. Bone tissue engineering (BTE) with stem cell implants is an effective method. Nanobiomaterials can effectively enhance the resistance of implanted stem cells to the harsh microenvironment of bone defect areas by promoting cell differentiation. Graphene oxide quantum dots (GOQDs) are zero-dimensional nanoscale derivatives of graphene oxide with excellent biological activity. In the present study, we aimed to explore the effects of GOQDs prepared by two methods (Y-GOQDs and B-GOQDs) on the osteogenic differentiation of human periodontal ligament stem cells (hPDLSCs), as well as the effect of gelatin methacryloyl (GelMA)-encapsulated GOQD-induced hPDLSC sheets on the repair of mandibular periodontal defects in rats. We also explored the molecular biological mechanism through which GOQD promotes bone differentiation. Results There were significant differences in oxygen-containing functional groups, particle size and morphology between Y-GOQDs and B-GOQDs. Y-GOQDs promoted the osteogenic differentiation of hPDLSCs more effectively than did B-GOQDs. In addition, GelMA hydrogel-encapsulated Y-GOQD-induced hPDLSC cell sheet fragments not only exhibited good growth and osteogenic differentiation in vitro but also promoted the repair of mandibular periodontal bone defects in vivo. Furthermore, the greater effectiveness of Y-GOQDs than B-GOQDs in promoting osteogenic differentiation is due to the regulation of hPDLSC mitochondrial dynamics, namely, the promotion of fusion and inhibition of fission. Conclusion sOverall, Y-GOQDs are more effective than B-GOQDs at promoting the osteogenic differentiation of hPDLSCs by regulating mitochondrial dynamics, which ultimately contributes to bone regeneration via the aid of the GelMA hydrogels in vivo.Graphical Abstract

The use of oxidoreductase nanozymes to regulate reactive oxygen species (ROS) has gradually emerged in periodontology treatments. However, current nanozymes for treating periodontitis eliminate ROS extensively and non-specifically, ignoring the physiological functions of ROS under normal conditions, which may result in uncontrolled side effects. Herein, using the MIL-47(V)-F (MVF) nanozyme, which mimics the function of glutathione peroxidase (GPx), we propose that ROS can be properly regulated by specifically eliminating H 2 O 2 , the most prominent ROS. Through H 2 O 2 elimination, MVF contributes to limiting inflammation, regulating immune microenvironment, and promoting periodontal regeneration. Moreover, MVF stimulates osteogenic differentiation of periodontal stem cells directly, further promoting regeneration due to the vanadium in MVF. Mechanistically, MVF regulates ROS by activating the Nrf2/HO-1 pathway and promotes osteogenic differentiation directly through the PI3K/Akt pathway. We present a promising periodontitis therapy strategy using GPx-mimicking nanozymes through their triple effects of antioxidation, immunomodulation, and bone remodeling regulation, making nanozymes an excellent tool for developing precision medicine. The osteogenic potential of mesenchymal stem cells (MSCs) is severely impaired under persistent inflammation of periodontitis. A highly efficient way to promote or rescue osteogenic potential of MSCs under inflammation remains an unmet goal. Herein, metformin carbon dots (MCDs) with excellent biocompatibility are prepared from metformin hydrochloride and citric acid via a hydrothermal method. The MCDs can more effectively enhance the alkaline phosphatase (ALP) activity, calcium deposition nodules formation, expression of osteogenic genes and proteins in rat bone marrow mesenchymal stem cells (rBMSCs) than metformin under both inflammatory and normal conditions. Moreover, a novel pathway of extracellular signal-regulated kinases (ERK)/AMP-activated protein kinase (AMPK) signaling is involved in the MCDs-induced osteogenesis. In periodontitis rats, MCDs can effectively regenerate the lost alveolar bone, but not the metformin. Taken together, MCDs can be the promising candidate nanomaterial for periodontitis treatment. This work may provide a new pharmacological target of ERK/AMPK pathway for treating bone loss and also give additional insights into developing nanodrugs from the numerous medications.

Commercial mammalian collagen-based membranes used for guided tissue regeneration (GTR) in periodontal defect repair still face significant challenges, including ethical concerns, cost-effectiveness, and limited capacity for periodontal bone regeneration. Herein, an enhanced biomimetic mineralized hydroxyapatite (HAp)–fish-scale collagen (FCOL)/chitosan (CS) nanofibrous membrane was developed. Specifically, eco-friendly and biocompatible collagen extracted from grass carp fish scales was co-electrospun with CS to produce a biomimetic extracellular matrix membrane. An enhanced biomimetic mineralized HAp coating provided abundant active calcium and phosphate sites, which promoted cell osteogenic differentiation, and showed greater in vivo absorption. In vitro experiments demonstrated that the HAp-FCOL/CS membranes exhibited desirable properties with no cytotoxicity, provided a mimetic microenvironment for stem cell recruitment, and induced periodontal ligament cell osteogenic differentiation. In rat periodontal defects, HAp-FCOL/CS membranes significantly promoted new periodontal bone formation and regeneration. The results of this study indicate that low-cost, eco-friendly, and biomimetic HAp-FCOL/CS membranes could be promising alternatives to GTR membranes for periodontal regeneration in the clinic.

The development of biomimetic bone graft materials for periodontal tissue engineering is a field of topical interest. In this study, we designed a dual-functionalized apatite nanocomposite, which could integrate multiple molecular cues for manipulating the fate of periodontal ligament stem cells (PDLSCs). Briefly, inspired by mussels, a biomimetic nanohydroxyapatite was fabricated using a polydopamine structure as a template (named as tHA) and then surface-modified with bone-forming peptide-1 (BFP-1) and vascular endothelial growth factor-mimicking peptide (QK) via a single step of catechol chemistry. Our study showed that the biofunctions of tethered peptides were not compromised on the surface of apatite nanoparticles. Because of the synergistic effect of BFP-1 and QK peptides, the dual-functionalized apatite nanocomposite showed improved cytocompatibility compared to controls. Moreover, it can boost the proliferation and osteogenic differentiation of PDLSCs, indicating excellent bioactivity of tHA–BFP/QK nanoparticles on cell fate decision. More importantly, animal experiments showed that dual-functionalized apatite nanocomposites could dramatically promote the regeneration of periodontal bone. It is concluded that our work provides an instructive insight into the design of biomimetic apatite nanocomposites, which holds a great potential for applications in periodontal bone repair.

A variety of novel biomaterials have recently been developed to promote bone regeneration. However, the current biomaterials cannot accurately and effectively resist bacterial invasion. In this study, we constructed microspheres that mimic certain functions of macrophages as additives to bone repair materials, which can be manipulated as demanded to resist bacteria effectively and protect bone defect healing. Firstly, we prepared gelatin microspheres (GMSs) by an emulsion-crosslinking method, which were subsequently coated with polydopamine (PDA). Then, amino antibacterial nanoparticles obtained by a nanoprecipitation-self-assembly method and commercial amino magnetic nanoparticles were modified onto these PDA-coated GMSs to construct the functionalized microspheres (FMSs). The results showed that the FMSs possessed a rough topography and could be manipulated by a 100–400 mT static magnetic field to migrate directionally in unsolidified hydrogels. Moreover, in vitro experiments with near-infrared (NIR) showed that the FMSs had a sensitive and recyclable photothermal performance and could capture and kill Porphyromonas gingivalis by releasing reactive oxygen species. Finally, the FMSs were mixed with osteogenic hydrogel precursor, injected into the Sprague-Dawley rat periodontal bone defect of maxillary first molar (M1), and subsequently driven by magnetism to the cervical surface of M1 and the outer surface of the gel system for targeted sterilization under NIR, thus protecting the bone defect healing. In conclusion, the FMSs had excellent manipulation and antimicrobial performances. This provided us with a promising strategy to construct light-magnetism-responsive antibacterial materials to build a beneficial environment for bone defect healing.

Traditional fibrous membranes employed in guided tissue regeneration (GTR) in the treatment of periodontitis have limitations of bioactive and immunomodulatory properties. We fabricated a novel nTPG/PLGA/PCL fibrous membrane by electrospinning which exhibit excellent hydrophilicity, mechanical properties and biocompatibility. In addition, we investigated its regulatory effect on polarization of macrophages and facilitating the regeneration of periodontal tissue both in vivo and in vitro. These findings showed the 0.5%TPG/PLGA/PCL may inhibit the polarization of RAW 264.7 into M1 phenotype by suppressing the PI3K/AKT and NF-κB signaling pathways. Furthermore, it directly up-regulated the expression of cementoblastic differentiation markers (CEMP-1 and CAP) in periodontal ligament stem cells (hPDLSCs), and indirectly up-regulated the expression of cementoblastic (CEMP-1 and CAP) and osteoblastic (ALP, RUNX2, COL-1, and OCN) differentiation markers by inhibiting the polarization of M1 macrophage. Upon implantation into a periodontal bone defect rats model, histological assessment revealed that the 0.5%TPG/PLGA/PCL membrane could regenerate oriented collagen fibers and structurally intact epithelium. Micro-CT (BV/TV) and the expression of immunohistochemical markers (OCN, RUNX-2, COL-1, and BMP-2) ultimately exhibited satisfactory regeneration of alveolar bone, periodontal ligament. Overall, 0.5%TPG/PLGA/PCL did not only directly promote osteogenic effects on hPDLSCs, but also indirectly facilitated cementoblastic and osteogenic differentiation through its immunomodulatory effects on macrophages. These findings provide a novel perspective for the development of materials for periodontal tissue regeneration.
