Periodontitis is a chronic inflammatory condition mainly caused by the interaction between the host immune system and periodontal tissue pathogens, and may lead to consequences, such as alveolar bone defects and tooth loss. Incomplete bacterial removal, persistent inflammation and high reactive oxygen species (ROS) environment are the main challenges for periodontal tissue repair and alveolar bone regeneration. In this study, an injectable composite microgel (Gelatin methacryloyl-Phenylboronic acid/Hydroxyadamantane, GPH) loaded with antimicrobial peptide (AMP) and cerium dioxide (CeO 2 ) microspheres was developed to achieve a synergistic function of bacteriostasis, immunomodulation, and ROS removal. In vitro studies had shown that the composite microgel had an inhibitory rate of >99 % against E. coli , S. aureus and P. gingivalis and scavenged DPPH with a rate of 87.1 % ± 2.0 %, exhibiting excellent antibacterial and antioxidant properties. In addition, it was able to promote macrophage phenotypic shift from M1-type to M2-type and reduce ROS levels, and also had excellent biocompatibility. Mechanistically, the composite microgel was subjected to transcriptome sequencing analysis of gene expression levels and signaling pathways in BMSC cells, and the results showed that the microgel played an important role in regulating the PI3K-Akt and chemokine signaling pathways, which in turn inhibited the expression of inflammatory factors. In vivo the effect of composite microgel on periodontal tissue repair and alveolar bone regeneration was verified by infected alveolar bone defect model. The results showed that after 4 weeks of using the composite microgel, the bone volume per unit of tissue volume of the alveolar bone, the thickness of bone trabeculae, and the bone mineral density reached 25.94 % ± 0.03 %, 0.14 mm ± 0.01 mm, and 0.442 g/cm 3 ± 0.003 g/cm 3 , respectively, while those of the control group were 22.3 % ± 0.29 %, 0.07 mm ± 0.02 mm, and 0.236 g/cm 3 ± 0.059 g/cm 3 , respectively, and the use of the composite microgel resulted in significantly better repair results than the control group. In addition, pro-inflammatory factors were significantly suppressed and inflammation levels were significantly reduced. Overall, this composite microgel showed great potential in reducing inflammation levels and promoting alveolar bone regeneration, providing an innovative approach to the treatment of periodontitis.

Immunomodulation has emerged as a promising strategy for promoting bone regeneration. However, designing osteoimmunomodulatory biomaterial that can respond to mechanical stress in the unique microenvironment of alveolar bone under continuous occlusal stress remains a significant challenge. Herein, a wireless piezoelectric stimulation system, namely, piezoelectric hydrogel incorporating BaTiO 3 nanoparticles (BTO NPs), is successfully developed to generate piezoelectric potentials for modulating macrophage reprogramming. The piezoelectric stimulation reprograms macrophages towards the M2 phenotype, which subsequently induces osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs). RNA sequencing analysis reveals that piezoelectricity-modulated macrophage M2 polarization is closely associated with metabolic reprogramming, including increased amino acid biosynthesis and fatty acid oxidation. The composite hydrogel with excellent biocompatibility exhibits immunomodulatory and osteoinductive activities. In a rat model of alveolar bone defects, the piezoelectric hydrogel effectively promotes endogenous bone regeneration at the load-bearing sites. The piezoelectric-driven osteoimmunomodulation proposed in this study not only broadens understanding of the mechanism underlying piezoelectric biomaterials for tissue regeneration but also provides new insights into the design and development of next-generation immunomodulatory biomaterials.

In this study, a scaffold was designed using 3-Matic software 12.0 (Materialise, Leuven, Belgium) and fabricated via Digital Light Processing (DLP) 3D printing technology, followed by a mechanical property evaluation. The scaffold was bilaterally implanted into mandibular bone defect models in four Beagle dogs to facilitate guided alveolar bone regeneration. After 12 weeks, samples were harvested from two dogs for radiographic and histopathological evaluations. In the remaining two dogs, two dental implants were placed into the scaffold sites. After an additional 12 weeks, samples were harvested for further radiographic and histopathological assessments. (1) Compression testing of the scaffold demonstrated a compressive strength of 24.77 ± 2.36 MPa. (2) Three of the implantation sites exhibited poor wound healing and exposure of the bone grafts early post-surgery (4 weeks), with an exposure rate of 37.5%. (3) Micro-CT imaging revealed a uniform distribution of newly formed bone within the scaffold, with an average bone height of 4.05 ± 0.55 mm and a bone volume fraction of 43.93 ± 4.68%. Histopathological analysis demonstrated the presence of vascularized tissue, non-calcified bone, and newly calcified bone within the scaffold. Additionally, newly formed calcified bone and vascularized tissue were observed at the interface between the implant and the scaffold. These findings suggest that DLP 3D-printed A-W bioactive glass scaffolds represent a promising approach for guided alveolar bone regeneration in dental implant applications.

The excess production of reactive oxygen species (ROS) will delay tooth extraction socket (TES) healing. In this study, we developed an injectable thermosensitive hydrogel (NBP@BP@CS) used to treat TES healing. The hydrogel formulation incorporated black phosphorus (BP) nanoflakes, recognized for their accelerated alveolar bone regeneration and ROS-scavenging properties, and dl-3-n-butylphthalide (NBP), a vasodilator aimed at enhancing angiogenesis. In vivo investigations strongly demonstrated that NBP@BP@CS improved TES healing due to antioxidation and promotion of alveolar bone regeneration by BP nanoflakes. The sustained release of NBP from the hydrogel promoted neovascularization and vascular remodeling. Our results demonstrated that the designed thermosensitive hydrogel provided great opportunity not only for ROS elimination but also for the promotion of osteogenesis and angiogenesis, reflecting the “three birds with one stone” concept, and has tremendous potential for rapid TES healing.

There has been increasing interest in adjunctive use of anti-inflammatory drugs to control periodontitis. This study was performed to examine the effects of pirfenidone (PFD) on alveolar bone loss in ligature-induced periodontitis in mice and identify the relevant mechanisms. Experimental periodontitis was established by ligating the unilateral maxillary second molar for 7 days in mice (n = 8 per group), and PFD was administered daily via intraperitoneal injection. The micro-computed tomography and histology analyses were performed to determine changes in the alveolar bone following the PFD administration. For in vitro analysis, bone marrow macrophages (BMMs) were isolated from mice and cultured with PFD in the presence of RANKL or LPS. The effectiveness of PFD on osteoclastogenesis, inflammatory cytokine expression, and NF-κB activation was determined with RT-PCR, Western blot, and immunofluorescence analyses. PFD treatment significantly inhibited the ligature-induced alveolar bone loss, with decreases in TRAP-positive osteoclasts and expression of inflammatory cytokines in mice. In cultured BMM cells, PFD also inhibited RANKL-induced osteoclast differentiation and LPS-induced proinflammatory cytokine (IL-1β, IL-6, TNF-a) expression via suppressing the NF-κB signal pathway. These results suggest that PFD can suppress periodontitis progression by inhibiting osteoclastogenesis and inflammatory cytokine production via inhibiting the NF-κB signal pathway, and it may be a promising candidate for controlling periodontitis. Keywords:pirfenidone;periodontitis;osteoclast differentiation;alveolar bone loss;inflammation;NF-κB pathway

Paeonol is a natural phenolic compound in Moutan Cortex with multiple biological functions, such as anti-inflammatory and anti-oxidant activity. Recent evidence has proven that persistent inflammation, oxidative stress , along with nuclear factor E2-related factor 2 (Nrf2) signaling dysfunction in periodontium are the possible causes of alveolar bone resorption , and ultimately lead to periodontitis. The present study was designed to explore the protective effects of paeonol on ligation-induced periodontitis in rats, and investigate the possible mechanism. We found that treatment with paeonol (40, 80 mg/kg, intraperitoneal injection) for 7 days remarkably decreased the expression of receptor activator of nuclear factor kappa-B ligand increased the expression of osteoprotegrin and inhibited the formation of osteoclasts . This function of paeonol might be correlated with its ability to reduce inflammatory factors (IL-1β, IL-6 and TNF-α) and alleviate oxidative stress (SOD, MDA, GSH and ROS) in gingival tissues. Besides, paeonol increased Nrf2 activity. Silence of Nrf2 using specific siRNA diminished the inhibitory effect of paeonol on NF-κB p65 activation and downstream expression, suggesting that Nrf2 was essential for protective effect of paeonol. These results showed that paeonol protected against periodontitis-aggravated osteoclastogenesis and alveolar bone lesion via regulating Nrf2/NF-κB/NFATc1 signaling pathway .

Background and Objective Periodontitis is intimately associated with the development of various systemic diseases, among which type 2 diabetes mellitus (T2DM) has a bidirectional relationship with the pathogenesis of periodontitis. The objective of the present work was to investigate the role of berberine (BBR) in periodontitis with T2DM and related mechanisms. Methods The mRNA expression of macrophage polarization-related factors in the microenvironment of periodontal inflammation was detected using real-time quantitative PCR (RT-qPCR). The experimental periodontitis model was constructed in wild-type (WT) and T2DM ( db/db ) mice, which were administered BBR after 7 days of modeling. Alveolar bone loss (ABL) in each group of mice was measured utilizing micro-computed tomography images. RT-qPCR was performed to analyze the levels of macrophage polarization-related factors in mouse gingiva. Lastly, using western blotting and RT-qPCR, the signaling pathway of BBR affecting macrophage polarization in the microenvironment of periodontitis was explored. Results BBR inhibited M1 polarization and stimulated M2 polarization in the periodontitis microenvironment. BBR decreased ABL in the WT and T2DM periodontitis models. And BBR reduced the production of proinflammatory cytokines and increased anti-inflammatory cytokine expression in the gingiva of WT and T2DM model mice. Ultimately, BBR mediates its anti-inflammatory effects on periodontitis through inhibition of the NF-κB pathway. Conclusions BBR had a therapeutic effect on T2DM-associated periodontitis via inhibiting the NF-κB pathway to affect macrophage polarization, which may have implications for the new pharmacological treatment of T2DM-associated periodontitis.

Repairing alveolar bone defects with chronic periodontitis is a major clinical challenge, since the periodontitis causes the intermittent bacteria invasion to irreversibly aggravates the degeneration of alveolar bones. To deal with the challenge, a kind of novel microcarriers with excellent abilities of responding to alkaline phosphatase (ALP) to intelligently eliminate the bacteria and promoting the recruitment and osteogenic differentiation of endogenous stem cells are developed. Mesoporous silica nanocarriers that chemically fix the polyethylene glycol-modified amoxicillin (PEG-AML) through the phosphoester groups and electrostatically adsorb the aspirin (ASP) are first prepared. The nanocarriers are then dispersed in a chitosan solution to prepare the bacteria-responsive, cell-recruitable, and osteoinductive nanocomposite microcarriers. When being stimulated by ALP that are secreted by the bacteria, phosphoester groups of microcarriers are cleaved, and PEG-AML are delivered on demand to well achieve the goal of intelligent bacteriostasis. The microcarriers slowly release the ASP to promote the recruitment and osteogenic differentiation of endogenous stem cells. At 8 weeks post-implantation, the microcarriers facilitate the repair of alveolar bone defects with periodontitis. The blood vessels in the neo-generated alveolar bones grow well. Due to the appropriate degradation time and excellent antibacterial ability of microcarriers, the microcarriers exhibit a better therapeutic efficacy than the standard clinical products (Geistlich Bio-Oss®), providing a new insight into the cure of alveolar bone defects with periodontitis.

Alveolar bone resorption is a major cause of teeth loss and jeopardizes the osseointegration of dental implants, greatly affecting patient's quality of life and health. It is still a great challenge to completely regenerate the alveolar bone defect through traditional guided bone regeneration (GBR) membranes due to their limited bioactivity and regeneration potential. Herein, a new hierarchical-structured mineralized nanofiber (HMF) scaffold, which is combined with both anisotropic and isotropic nanofibrous surface topography and the mineralized particles, is fabricated via a simple template-assisted electrospinning technology and in situ mineralization method. This HMF scaffold can not only directly induce osteogenic differentiation of bone mesenchymal stem cells (osteoinduction), but also stimulate macrophage toward pro-healing (M2) phenotype-polarization with an elevated secretion of the pro-healing cytokines, eventually enhancing the osteogenesis (osteoimmunomodulation). The results of in vivo rat alveolar bone defect repair experiments demonstrate that as compared with the combination of commercial Bio-Gide and Bio-Oss, the single HMF scaffold shows comparable or even superior bone repair effect, with better tissue-integration and more suitable degradation time and accompanied by a simplified operation.

Introduction: Periodontitis and inflammatory bowel disease are chronic inflammatory diseases with shared epidemiological, biological, and therapeutic associations. Given the similarities in their pathogenic factors, this study hypothesized that mesalazine, a key treatment agent for inflammatory bowel disease, could also be effective in managing periodontitis. Methods: The antimicrobial effect of mesalazine on Porphyromonas gingivalis was investigated in vitro, including observations of morphological changes on the surface of P. gingivalis. Additionally, the impact of mesalazine on both the formation and established plaque biofilms was examined. The antimicrobial mechanism was elucidated by assessing the expression of P. gingivalis virulence genes and by determining the disruptive effect on P. gingivalis cell membranes. An in vivo rat model of periodontitis was constructed to evaluate mesalazine’s efficacy and its influence on the periodontal bacterial flora in the context of periodontitis. Results and discussion: Our results demonstrated that mesalazine concentrations ranging from 0.5 to 2 mg/mL significantly inhibited P. gingivalis proliferation over 72 h. Flow cytometry revealed a marked reduction in the number of viable cells following mesalazine treatment. At the nanometer scale, mesalazine induced crumpling and rupture of the P. gingivalis surface, compromising cell membrane integrity. Mesalazine not only suppressed the formation of plaque biofilms by P. gingivalis and polymicrobial communities but also disrupted pre-existing biofilms. The data also suggested that mesalazine could disrupt the integrity of the P. gingivalis cell membrane and inhibit the expression of virulence factors. An animal model of periodontitis in rats was successfully constructed in vivo. Mesalazine treatment inhibited alveolar bone resorption, alleviated inflammation of periodontal tissues, and improved the composition of the periodontal flora to a healthier state. This study establishes that mesalazine can treat periodontitis through modulation of the periodontal flora and its anti-inflammatory properties, thus broadening its classical therapeutic applications.

Background Dental follicle stem cells (DFSCs) show mesenchymal stem cell properties with the potential for alveolar bone regeneration. Stem cell properties can be impaired by reactive oxygen species (ROS), prompting us to examine the importance of scavenging ROS for stem cell-based tissue regeneration. This study aimed to investigate the effect and mechanism of N-acetylcysteine (NAC), a promising antioxidant, on the properties of DFSCs and DFSC-based alveolar bone regeneration. Methods DFSCs were cultured in media supplemented with different concentrations of NAC (0–10 mM). Cytologic experiments, RNA-sequencing and antioxidant assays were performed in vitro in human DFSCs (hDFSCs). Rat maxillary first molar extraction models were constructed, histological and radiological examinations were performed at day 7 post-surgery to investigate alveolar bone regeneration in tooth extraction sockets after local transplantation of NAC, rat DFSCs (rDFSCs) or NAC-treated rDFSCs.Results 5 mM NAC-treated hDFSCs exhibited better proliferation, less senescent rate, higher stem cell-specific marker and immune-related factor expression with the strongest osteogenic differentiation; other concentrations were also beneficial for maintaining stem cell properties. RNA-sequencing identified 803 differentially expressed genes between hDFSCs with and without 5 mM NAC. “Developmental process (GO:0032502)” was prominent, bioinformatic analysis of 394 involved genes revealed functional and pathway enrichment of ossification and PI3K/AKT pathway, respectively. Furthermore, after NAC treatment, the reduction of ROS levels (ROS, superoxide, hydrogen peroxide), the induction of antioxidant levels (glutathione, catalase, superoxide dismutase), the upregulation of PI3K/AKT signaling (PI3K-p110, PI3K-p85, AKT, phosphorylated-PI3K-p85, phosphorylated-AKT) and the rebound of ROS level upon PI3K/AKT inhibition were showed. Local transplantation of NAC, rDFSCs or NAC-treated rDFSCs was safe and promoted oral socket bone formation after tooth extraction, with application of NAC-treated rDFSCs possessing the best effect. Conclusion sThe proper concentration of NAC enhances DFSC properties, especially osteogenesis, via PI3K/AKT/ROS signaling, and offers clinical potential for stem cell-based alveolar bone regeneration.
