Pulpitis is a painful inflammatory condition of the dental pulp, commonly triggered by bacterial infection, trauma, or repeated dental procedures. Patients often experience heightened sensitivity to temperature changes, spontaneous pain, and, in severe cases, necrosis of the pulp tissue, leading to tooth loss if left untreated. Traditional treatment primarily involves root canal therapy, which removes infected pulp and seals the canal with gutta-percha; however, this approach lacks antimicrobial properties and does not support tissue regeneration. To address these limitations, this study investigates an injectable hydrogel system composed of hyaluronic acid (HA) and carboxymethyl chitosan (CMCs), chemically crosslinked with 1,4-butanediol diglycidyl ether (BDDE). The hydrogel demonstrated excellent injectability, self-healing ability, and mechanical stability. In vitro and in vivo assessments confirmed its biocompatibility, biodegradability, and capability for controlled release of therapeutic agents. Notably, amoxicillin-loaded hydrogel (HACM/AX) showed significant antibacterial properties; while erythropoietin-loaded hydrogel (HACM/EPO) significantly enhanced endothelial cell proliferation and migration, facilitating angiogenesis for tissue regeneration. These findings highlight the potential of HACM hydrogel as a dual-function biomaterial with antimicrobial and pro-angiogenic properties, offering a promising alternative for pulpitis treatment and dental tissue engineering.

Summary Pulpitis suppressed the level of let-7c-5p that promotes osteogenesis and bone formation by repressing HMGA2. In the current study, the function of let-7c-5p in the inflammation and osteogenesis in dental pulp stem cells (DPSCs) was explored. The level of let-7c-5p in DPSCs was up-regulated, and the cells were subjected to lipopolysaccharide (LPS) to induce inflammation. The effect of let-7c-5p on cell proliferation potential, osteogenic differentiation potential, and activity of HMGA2/PI3K/Akt pathway was detected. The administration of LPS suppressed the cell proliferation of DPSCs and suppressed calcium deposition, activity of alkaline phosphatase (ALP), and levels of OCN, OPN, OSX, MSX2, and RUNX2 in inflamed DPSCs. The impaired osteogenic differentiation of inflamed DPSCs was associated with the increased levels of HMGA2, p-PI3K, and p-Akt. In let-7c-5p-overexpressed inflamed DPSCs, the proliferation and osteogenic differentiation potential of DPSCs were restored, and the activation of HMGA2/PI3K/Akt signalling was inhibited. In rat pulpitis models, the injection of let-7c-5p agomir restored the osteogenic differentiation potential of dental pulp cells and inhibited HMGA2/PI3K/Akt signalling. The findings demonstrated the anti-inflammation and pro-osteogenesis effect of let-7c-5p during the attack of pulpitis, which depended on the inhibition of HMGA2/PI3K/Akt signalling.

Continuous decompression and drainage are a vital surgical strategy for managing severe tissue infections. In vital pulp therapy (VPT) for irreversible pulpitis, there is a clinical demand for advanced biomaterials capable of effectively sealing the pulp cavity, alleviating pulpal hypertension, preventing bacterial infiltration, and resolving acute pulp inflammation in bacteria-rich environments. In this study, we developed a polydopamine@zinc oxide nanoparticle (ZnO-NP)-coated polytetrafluoroethylene (PTFE) membrane with tunable Zn content ranging from 2.51 to 7.45 wt%. The polydopamine enhanced ZnO-NP adhesion to the PTFE membrane, enabling superior fluid permeability and robust antibacterial efficacy against E. faecalis and S. mutans , while maintaining excellent biocompatibility. In a minipig pulpitis model, the ZnO-coated membrane significantly outperformed iRootBP PLUS by promoting faster dentine bridge formation (934.0 ± 91.3 μm vs. 116.3 ± 45.4 μm), preserving the integrity of the underlying pulp tissue and inducing M2 macrophage polarization. These findings deomstrate that ZnO-functionalized fibrous membrane can address key challenges in VPT by alleviating pulp hypertension, preventing microbial invasion, and simultaneously promoting pulp tissue regeneration. This approach offers a promising strategy to enhance tharepeutic outcomes of vital pulp therapy.

The preservation of vital pulps is crucial for maintaining the physiological functions of teeth; however, vital pulp therapy (VPT) of pulpitis teeth remains a substantial challenge due to uncontrolled infection, excessive inflammation, and limited regenerative potential. Current pulp capping agents have restricted effects in the infectious and inflammatory microenvironment. To address this, a multifunctional hydrogel (TGH/DM) with antibacterial, immunomodulatory, and mineralization-promoting effects was designed. The antimicrobial peptide (AMP) and demineralized dentin matrix were incorporated into the hydrogel, achieving sustainable delivery of AMP and a cocktail of growth factors. In vitro results showed that TGH/DM could kill endodontic microbiota, ameliorate inflammatory responses of human dental pulp stem cells (hDPSCs), and prompt odontogenic differentiation of inflammatory hDPSCs via activation of peroxisome proliferator-activated receptor gamma. In vivo results suggested that TGH/DM was capable of inducing M2 phenotype transformation of macrophages in mice and fostering the regeneration of the dentin-pulp complex in inflamed pulps of beagle dogs. Overall, this study firstly proposed the synergistic regulation of AMP and tissue-specific extracellular matrix for the treatment of pulpitis, and the advanced hydrogel provided a facile and effective way for VPT. Preserving pulp viability and promoting pulp regeneration in pulpitis have attracted widespread attention. Restricted by the oxidative stress microenvironment of dental pulpitis, excessive reactive oxygen and nitrogen species (RONS) trigger uncontrolled inflammation and exacerbate pulp tissue destruction. However, modulating redox homeostasis in inflamed pulp tissue to promote pulp regeneration remains a great challenge. Herein, this work proposes an effective antioxidative system (C-NZ/GelMA) consisting of carbon dot nanozymes (C-NZ) with gelatin methacryloyl (GelMA) to modulate the pulpitis microenvironment for dental pulp regeneration by utilizing the antioxidant properties of C-NZ and the mechanical support of an injectable GelMA hydrogel. This system effectively scavenges RONS to normalize intracellular redox homeostasis, relieving oxidative stress damage. Impressively, it can dramatically enhance the polarization of regenerative M2 macrophages. This study revealed that the C-NZ/GelMA hydrogel promoted pulp regeneration and dentin repair through its outstanding antioxidant, antiapoptotic, and anti-inflammatory effects, suggesting that the C-NZ/GelMA hydrogel is highly valuable for pulpitis treatment.

Background Pulpitis is an oral disease primarily triggered by microbial infections. Current therapeutic strategies lack specific agents targeting the underlying pathogenic mechanisms. Non-coding RNAs (ncRNAs) and their competitive endogenous RNA (ceRNA) networks have emerged as critical regulators of diverse biological processes, offering novel insights into the pathogenesis of pulpitis and the identification of potential therapeutic targets. Methods RNA sequencing was performed on pulp tissues from healthy individuals and pulpitis patients. Bioinformatics tools were employed to analyse sequencing data, identify differentially expressed messenger RNAs (mRNAs), and construct protein-protein interaction (PPI) networks to pinpoint hub genes. A murine pulpitis model was established to investigate the effects of neutrophil extracellular trap (NET) inhibitors on disease progression. A NET-associated ceRNA regulatory network was systematically constructed based on ceRNA theory. In vitro experiments validated the expression patterns of key ncRNAs and mRNAs in pulpitis. Results Differentially expressed mRNAs, long non-coding RNAs (lncRNAs), and Circular RNAs (circRNAs) were identified in pulpitis. Bioinformatics analysis revealed significant activation of NET-related pathways, with sialic acid-binding immunoglobulin-like lectin-9 (SIGLEC9), complement C3 (C3), H2A clustered histone 14 (H2AC14), and H4 clustered histone 11 (H4C11) identified as core regulatory genes. In vivo experiments demonstrated that NET inhibition alleviated dental pulpitis and necrosis. Furthermore, a novel NET-associated ceRNA regulatory network was established, revealing four critical regulatory axes: LINC00466 / hsa-miR-27a-3p / SIGLEC9, hsa_circDNAH11_003 / hsa-miR-877-5p / C3, hsa_circSLC15A4_001 / hsa-miR-30d-3p / H2AC14, and hsa_circGABBR2_009 / hsa-miR-193b-3p / H4C11. These ncRNAs exhibited significant upregulation in inflamed pulp tissues. Conclusion By constructing a comprehensive NET-associated ceRNA network, we elucidate molecular mechanisms underlying dental pulpitis, providing novel evidence for understanding the disease progression. These findings establish a theoretical foundation for developing targeted therapeutic strategies against pulpitis.
