Macro/nano topological modification of silk fibroin mesh with mimicked extracellular matrix structure and excellent biocompatibility

Pei Cai,Ying Mao,Xinmei Liu,Zhiwei Li,Jinfeng Wang,Wenxing Chen,Hongshi Zhao,Wangyang Lu

Synthetic surgical meshes have been widely used for repairing hernias, but their performances, such as nonabsorbability and insufficient mechanical strength, require further improvement due to postsurgical complications, including chronic pain and inflammation. In this work, naturally derived and bioresorbable silk fibroin meshes (SFM) with three knit patterns were optimized and modified by a combination of regenerated silk fibroin (RSF) and polydopamine (PDA), to endow SFM with mimicked extracellular matrix (ECM) structure and excellent biocompatibility. Our study confirmed that the modified meshes (SFM@PDA-RSF) exhibited ECM-like structure and good structural stability. Tensile testing results revealed that SFM substrate played a dominant role in mechanical properties, and SFM@PDA-RSF showed high tensile strength (49.58 N/cm transversely, 68.42 N/cm longitudinally), which could afford sufficient mechanical support for abdominal wall hernia (AWH) repair (16 N/cm). Moreover, SFM@PDA-RSF was found to be significantly antioxidant, non-hemolytic, and favorable for cell adhesion and growth, showing great potential for effective hernia repair.

3D printing/electrospinning of a bilayered composite patch with antibacterial and antiadhesive properties for repairing abdominal wall defects

Qingxi Hu,Yu Zhang,Yongteng Song,Hekai Shi,Dongchao Yang,Haiguang Zhang,Yan Gu

The application of patch methods for repairing abdominal wall wounds presents a variety of challenges, such as adhesion and limited mobility due to inadequate mechanical strength and nonabsorbable materials. Among these complications, postoperative visceral adhesion and wound infection are particularly serious. In this study, a bilayered composite patch with a gelatin methacryloyl (GelMA)/sodium alginate (SA)-vancomycin (Van)@polycaprolactone (PCL) (GelMA/SA-Van@PCL) antibacterial layer was prepared via coaxial 3D printing and a polycaprolactone (PCL)-silicon dioxide (SiO2) antiadhesive layer (PCL-SiO2) was prepared via electrospinning and electrostatic spray for hernia repair. The evaluation of the physicochemical properties revealed that the composite patch had outstanding tensile properties (16 N cm−1), excellent swelling (swelling rate of 243.81 ± 12.52%) and degradation (degradation rate of 53.14 ± 3.02%) properties. Furthermore, the composite patch containing the antibiotic Van exhibited good antibacterial and long-term drug release properties. Both in vivo and in vitro experiments indicated that the composite patch displayed outstanding biocompatibility and antiadhesive properties and could prevent postoperative infections. In summary, the bilayered composite patch can effectively prevent postoperative complications while promoting tissue growth and repair and holds significant application potential in hernia repair.

Filament-anchored hydrogel layer on polypropylene hernia mesh with robust anti-inflammatory effects

Yansha Qiao,Qian Zhang,Qian Wang,Yan Li,Lu Wang

The efficacy of implanted polypropylene (PP) hernia meshes is often compromised by an inflammatory response. Thus, engineering an anti-inflammatory mesh has significant implications for hernioplasty. Here, we report a facile strategy to develop a filament-anchored hydrogel layer (FAHL) on PP mesh (FAHL-P). The network of FAHL, made up of chondroitin sulfate and gelatin (CG), provided a biomimetic surface with immunoregulatory properties. The use of tannic acid (TA) as a crosslinker for CG additionally enhanced its anti-inflammatory properties. In addition, the fabrication protocol ensured that the hydrogel maintained the properties of the knitted mesh and the firmly adherent FAHL during general handling (dry state) and in the simulated body environment (wet state). CG/TA-PP killed 99.99% of S. aureus and retained 73% of its original antioxidant properties after 7 d. The FAHL durably performed with a controlled release of TA for 15 d. The strong anti-inflammatory effects of FAHL-P reduced collagen deposition and increased vascularization , which promoted native tissue generation. The fabrication strategy has potential applications in hernioplasty and may provide new insights into the design of other anti-inflammatory implants. Statement of significance A hydrogel layer with robust anti-inflammatory effects was anchored firmly on mesh filament for hernia repair. Requiring no drug loading, this chondroitin sulphate -gelatin (CG) based hydrogel itself could inhibit the immunological attack owing to the biomimetic microenvironment created by the CG. Moreover, the hydrogel's crosslinker (tannic acid) content served as an effective scavenger for reducing pro-inflammatory factors, significantly mitigating the inflammation. Interestingly, the antibacterial effect of such hydrogel layer was also observed. In terms of the synergistic outcome of the design, our mesh can remarkably attenuate inflammation and promote constructive tissue regeneration in vivo . Furthermore, considering the relatively simple and easily scaled up formulation process, our strategy may indeed have great potential in alleviating post-implantation outcomes.

Combination of Polypropylene Mesh and in Situ Injectable Mussel-Inspired Hydrogel in Laparoscopic Hernia Repair for Preventing Post-Surgical Adhesions in the Piglet Model

Wanjun Hu,Zhigang Zhang,Long Zhu,Yazhou Wen,Tianzhu Zhang,Pengfei Ren,Faming Wang,Zhenling Ji

Polypropylene (PP) mesh has been used successfully for a long time in clinical practice as an impressive prosthesis for ventral hernia repair. To utilize a physical barrier for separating mesh from viscera is a general approach for preventing adhesions in clinical practice. However, a serious abdominal adhesion between the mesh and viscera can possibly occur post-hernia, especially with the small intestine; this can lead to a series of complications, such as chronic pain, intestinal obstruction, and fistula. Thus, determining how to prevent abdominal adhesions between the mesh and viscera is still an urgent clinical problem. In this study, a dopamine-functionalized polysaccharide derivative (oxidized-carboxymethylcellulose-g-dopamine, OCMC-DA) was synthesized; this was blended with carboxymethylchitosan (CMCS) to form a hydrogel (OCMC-DA/CMCS) in situ at the appropriate time. The physical and chemical properties of the hydrogel were characterized successfully, and its excellent biocompatibility was presented by the in vitro cell test. The combination of this hydrogel and PP mesh was used in laparoscopic surgery for repairing the abdominal wall defect, where the hydrogel could become fixed in situ on the PP mesh to form an anti-adhesion gel-mesh. The results showed that the gel-mesh could prevent abdominal adhesions effectively in the piglet model. Moreover, the histology and immunohistochemical staining proved that the gel-mesh could effectively alleviate the inflammation reaction and deposition of collagen around the mesh, and it did not disturb the integration between mesh and abdominal wall. Thus, the gel-mesh has superior tissue compatibility.
