Mesh infection is a major complication of hernia surgery after polypropylene (PP) mesh implantation. Modifying the PP mesh with antibacterial drugs is an effective way to reduce the chance of infection, but the hydrophobic characteristic of PP fibers has obstructed the drug adhesion. Therefore, to prepare antimicrobial PP mesh with a stable drug coating layer and to slow the drug release property during the hernia repair process has a great practical meaning. In this work, PP meshes were coated by bio-inspired polydopamine (PDA), which can load and release levofloxacin. PP meshes were activated with cold oxygen plasma and then plasma activated PP fibers were coated with PDA. The PDA coated meshes were further soaked in levofloxacin. The levofloxacin loaded PP meshes demonstrate excellent antimicrobial properties for 6 days and the drug release has lasted for at least 24 h. Moreover, a control PP mesh sample without plasma treatment was also prepared, after coating with PDA and loading levofloxacin. The antimicrobial property was sustained only for two days. The maximum inhibition zone of PDA coated meshes with and without plasma treatment was 12.5 and 9 mm, respectively. On all accounts, the modification strategy can facilely lead to long-term property of infection prevention. Keywords:polypropylene;hernia meshes;antibacterial;drug release;polydopamine

Polypropylene (PP) mesh is commonly used in repairing abdominal wall hernia (AWH). However, the use of synthetic prosthesis comes with the risk of developing a prosthetic infection, resulting in delayed healing, secondary surgery, and potentially increased mortality. To address these issues, a facile surface functionalization strategy for PP mesh based on phytic acid (PA) and polyhexamethylene guanidine (PHMG) was constructed through a one-step co-deposition process, referred to as the PA/PHMG coating. The development of PA/PHMG coating is mainly attributed to the surface affinity of PA and the electrostatic interactions between PA and PHMG. The PA/PHMG coating could be completed within 4 h under mild conditions. The prepared PA/PHMG coatings on PP mesh surfaces exhibited desirable biocompatibility toward mammalian cells and excellent antibacterial properties against the notorious “superbug” methicillin-resistant Staphylococcus aureus (MRSA) and tetracycline-resistant Escherichia coli (TRE). The PA/PHMG-coated PP meshes showed killing ratios of over 99% against MRSA in an infected abdominal wall hernia repair model. Furthermore, histological and immunohistochemical analysis revealed a significantly attenuated degree of neutrophil infiltration in the PA/PHMG coating group, attributed to the decreased bacterial numbers alleviating the inflammatory response at the implant sites. Meanwhile, the pristine PP and PA/PHMG-coated meshes showed effective tissue repair, with the PA/PHMG coating group exhibiting enhanced angiogenesis compared with pristine PP meshes, suggesting superior tissue restoration. Additionally, PP meshes with the highest PHMG weight ratio (PA/PHMG(3)) exhibited excellent long-term robustness under phosphate-buffered saline (PBS) immersion with a killing ratio against MRSA still exceeding 95% after 60 days of PBS immersion. The present work provides a facile and promising approach for developing antibacterial implants.

Hernia typically does not heal spontaneously. Large-pore patches, most notably polypropylene patches (PP patches), are the gold standard in hernia repair surgery. However, a single patch is insufficient for both anti-adhesion and tissue fusion, leading to complications such as organ adhesions. In this study, a chemically stable and biocompatible modified fluorinated poly(aryl ether) (FPAE-F) was prepared by grafting perfluoroalkyl groups onto a fluorinated poly(aryl ether) via nucleophilic aromatic substitution. A porous FPAE-F fiber film (eFPAE-F) was fabricated by electrospinning and combined with a PP patch to produce a modified fluorinated poly(aryl ether)/polypropylene (FPAE-F/PP) composite patch. The eFPAE-F layer of the composite patch, which faces the abdominal viscera, exhibits a water contact angle of 151.3 ± 1.2°. This superhydrophobic surface prevents protein adhesion, thereby inhibiting rapid fibroblast proliferation. The small pore size (3.22 ± 1.25 μm) of the eFPAE-F layer effectively impedes fibroblast infiltration while permitting the transport and metabolism of nutrients. In vivo experiments have demonstrated that the composite patch is a viable anti-adhesion material, resulting in no adhesions and low inflammation levels after 2 weeks. Due to its outstanding anti-adhesion properties, eFPAE-F/PP is expected to be applied in the field of hernia repair.

Polypropylene (PP) hernia repair mesh has gained much attention due to its low cost, cost-efficiency, stable chemical properties and excellent properties. However, its lack of biocompatibility has greatly limited its clinical application, and different complications such as chronic pain and foreign body reaction are usually found after implantation. Chitosan (CS) and sodium alginate (SA) are among the most exploited naturally derived biomaterials with excellent biocompatibility. Therefore, plasma-treated PP meshes were combined with CS or SA to address these issues in this study. The results demonstrated that the hydrophilicity of the coated meshes exhibited a significant increase. Moreover, the two-component hernia repair meshes created a favorable environment for cell attachment with higher cell proliferation than PP mesh. This work provides insights on the use of naturally biocompatible materials as a potential coating on PP meshes to provide favorable conditions for cell adhesion and growth.

In recent years, the utilization of medical devices has gradually increased and implantation procedures have become common treatments. However, patients are susceptible to the risk of implant infections. This study utilized chemical grafting to immobilize polyethylenimine (QPEI) and hyaluronic acid (HA) on the surface of the mesh to improve biocompatibility while being able to achieve antifouling antimicrobial effects. From the in vitro testing, PP–PDA-Q-HA exhibited a high antibacterial ratio of 93% against S. aureus, 93% against E. coli, and 85% against C. albicans. In addition, after five rounds of antimicrobial testing, the coating continued to exhibit excellent antimicrobial properties; PP–PDA-Q-HA also inhibits the formation of bacterial biofilms. In addition, PP–PDA-Q-HA has good hemocompatibility and cytocompatibility. In vivo studies in animal implantation infection models also demonstrated the excellent antimicrobial properties of PP–PDA-Q-HA. Our study provides a promising strategy for the development of antimicrobial surface medical materials with excellent biocompatibility.

Abdominal wall defect caused by surgical trauma, congenital rupture, or tumor resection may result in hernia formation or even death. Tension-free abdominal wall defect repair by using patches is the gold standard to solve such problems. However, adhesions following patch implantation remain one of the most challenging issues in surgical practice. The development of new kinds of barriers is key to addressing peritoneal adhesions and repairing abdominal wall defects. It is already well recognized that ideal barrier materials need to have good resistance to nonspecific protein adsorption, cell adhesion, and bacterial colonization for preventing the initial development of adhesion. Herein, electrospun poly(4-hydroxybutyrate) (P4HB) membranes infused with perfluorocarbon oil are used as physical barriers. The oil-infused P4HB membranes can greatly prevent protein attachment and reduce blood cell adhesion in vitro. It is further shown that the perfluorocarbon oil-infused P4HB membranes can reduce bacterial colonization. The in vivo study reveals that perfluoro(decahydronaphthalene)-infused P4HB membranes can significantly prevent peritoneal adhesions in the classic abdominal wall defects’ model and accelerate defect repair, as evidenced by gross examination and histological evaluation. This work provides a safe fluorinated lubricant-impregnated P4HB physical barrier to inhibit the formation of postoperative peritoneal adhesions and efficiently repair soft-tissue defects.

Polypropylene (PP) mesh is the most widely used prosthetic material in hernia repair. However, the efficacy of implanted PP mesh is often compromised by adhesion between viscera and PP mesh. Thus, there is a recognized need for developing an anti-adhesive PP mesh. Here, a composite hydrogel coated PP mesh with the prevention of adhesion after hernia repair was designed. The composite hydrogel coating was prepared from polyvinyl alcohol (PVA) and hyaluronic acid (HA) by using the freezing-thawing (FT) method. To overcome the shortcoming of the long time of the traditional freezing-thawing method, a small molecule 3,4-dihydroxyphenylacetic acid (DHPA) was introduced to promote the formation of composite hydrogel. The as-prepared composite hydrogel coating displayed modulus more closely resembling that of native abdominal wall tissue. In vitro studies illustrated that the resulting meshes showed excellent coating stability, hemocompatibility, and non-cytotoxicity. In vivo experiments using a rat abdominal wall defect model demonstrated that the composite hydrogel coated PP mesh could prevent the formation of adhesion, alleviate the inflammatory response, and reduce the deposition of collagen around the damaged tissue. These disclosed results manifested that the PP mesh coated with HA/PVA composite hydrogel might be a promising application in preventing adhesion for hernia repair.

Infection-associated complications and repair failures and antibiotic resistance have emerged as a formidable challenge in hernia repair surgery. Consequently, the development of antibiotic-free antibacterial patches for hernia repair has become an exigent clinical necessity. Herein, a GBC/Gel/LL37 biological patch (biopatch) with exceptional antibacterial properties is fabricated by grafting 2-Methacryloyloxyethyl trimethylammonium chloride (METAC), a unique quaternary ammonium salt with vinyl, onto bacterial cellulose (GBC), followed by compounding with gelatin (Gel) and LL37. The GBC/Gel/LL37 biopatch exhibits stable swelling capacity, remarkable mechanical properties, flexibility, and favorable biocompatibility. The synergistic effect of METAC and LL37 confers upon the GBC/Gel/LL37 biopatch excellent antibacterial efficacy against Staphylococcus aureus and Escherichia coli , effectively eliminating invading bacteria without the aid of exogenous antibiotics in vivo while significantly reducing local acute inflammation caused by infection. Furthermore, the practical efficacy of the GBC/Gel/LL37 biopatch is evaluated in an infected ventral hernia model, revealing that the GBC/Gel/LL37 biopatch can prevent the formation of visceral adhesions, facilitate the repair of infected ventral hernia, and effectively mitigate chronic inflammation. The prepared antibacterial GBC/Gel/LL37 biopatch is very effective in dealing with the risk of infection in hernia repair surgery and offers potential clinical opportunities for other soft injuries, exhibiting considerable clinical application prospects.

Polypropylene (PP) mesh has been widely used in hernia repair as prosthesis material owing to its excellent balanced biocompatibility and mechanical properties . However, abdominal adhesion between the visceral and PP mesh is still a major problem. Therefore, anti-adhesive PP mesh was designed with poly(vinyl alcohol) (PVA) hydrogel and liposomes drug delivery system. First, PVA hydrogel coating was formed on the surface of PP mesh with freezing-thawing processing cycles (FTP). Subsequently, the lyophilized PVA 10 - c -PP was immersed in rapamycin (RPM)-loaded liposome solution until swelling equilibrated to obtain the anti-adhesion mesh [email protected] /PVA 10 - c -PP. It was demonstrated that the hydrogel coating can stably fix on the surface of PP mesh even after immersed in PBS solution at 37 °C or 40 °C for up to 30 days. In vitro cell tests revealed the excellent cytocompatibility and the potential to inhibit cell adhesion of the modified PP mesh. Moreover, the anti-adhesive effects of the [email protected] /PVA 10 - c -PP mesh was evaluated through in vivo experiments. The [email protected] /PVA 10 - c -PP mesh exhibited less adhesion than original PP mesh throughout the duration of implantation. At 30 days, the adhesion score of [email protected] /PVA 10 - c -PP mesh was 1.37 ± 0.75, however the original PP was 3 ± 0.71. Furthermore, the results of H&E and Masson trichrome staining proved that the [email protected] /PVA 10 - c -PP mesh showed slighter inflammation response and significant looser fibrous tissue surrounded the PP filaments as compared to the native PP. The current findings manifested that this type of [email protected] /PVA 10 - c -PP might be a potential candidate for anti-adhesion treatment. Aiming at the serious complications elicited by the inflammatory response to the polypropylene (PP) hernia mesh, we decorated the PP mesh with the immunomodulatory chondroitin sulfate (CS, a sulfated glycosaminoglycan present in extracellular matrixes) in the form of chrysanthemum–like hierarchitectures through the oxidative co-deposition with tannic acid (TA, a natural polyphenol compound). The mass ratio (MR) of TA to CS was modulated to obtain a hybrid colloidal particle with the balanced intermolecular interactions as the precursor of the co-deposition coating (TC). The interactions also protected the phenolic hydroxyls of TA from excessive oxidation and facilitated TA deposition, ultimately strengthening the antioxidant ability of TC. Relying on the immunomodulatory and antioxidant properties, the PP mesh coated with TC exhibited excellent anti-inflammatory effects in vitro. Moreover, TC had good stability without any adverse influence on the original mesh properties (the pore structure and compliance), which holds a great potential in the biocompatible modification of implants.

Polypropylene (PP) mesh is widely used in hernioplasty, but it is prone to contamination by pathogenic bacteria. Here, we present an infection microenvironment-responsive metal–phenolic network (MPN) coating, which is made up of Cu2+ and tannic acid (TA) (referred to as CT coating), and is fabricated on PP meshes by layer-by-layer (LbL) assembly. The CT coating provided a robust protection for the PP mesh from pathogenic bacterial infection in a pH-responsive manner due to the pH-responsive disassembly kinetics of MPN complexes. Moreover, the PP meshes with ten CT coating cycles (PP-CT(10)) exhibited excellent stability in a physiological environment, with the killing ratio against “superbug” methicillin-resistant Staphylococcus aureus (MRSA) at pH 5.5 exceeding 99% even after 28 days of PBS (pH 7.4) immersion. In addition, the PP-CT(10) exhibited excellent in vivo anti-infective ability in a rodent subcutaneous implant MRSA infection model, and the results of histological and immunohistochemical analyses demonstrated that the reduced bacterial number alleviated the inflammatory response at implant sites. This study revealed that MPN coating is a promising strategy, which could provide a self-defensive ability for various implants to combat post-surgical infections in a pH-responsive manner.

Hernia incidence remains a high level worldwide as witnessed by the fact that more than 20 million hernias are estimated to be repaired each year. Mesh-based hernioplasty allowing wound defects to close more easily and low recurrence rate is considered as the gold standard option for hernia repair. However, clinical polypropylene mesh (PPM) is still lack of effective treatments for the unavoidable bacterial infection, postoperative adhesion and foreign body reaction. Herein, a stimulus-responsive gel was constructed on the PPM by a simple photo-initiated polymerization. The homogeneous gel-modified mesh could spontaneously transform to a Janus mesh locally under endogenous stimulations evoked from inflammatory response or bacterial infection, and demonstrate asymmetric performances. One side of the Janus mesh facing the abdominal viscera could maintain intact gel structures and form a hydration layer to hinder the stick of proteins and cells, while the other side adjacent to the hernia defect site where bacteria prefer to colonize would accelerate degradation due to bacterial growth and release the bactericidal agent for combating the bacterial infection. The in vivo experiments’ results demonstrated that no adhesions were detectable on the Janus mesh treated group on the 14th day and the level of inflammatory response in histological evaluation was lower as compared to the commercial PPM. Overall, the endogenous stimulation-driven Janus gel network in abdominal wall wound provides a new approach for soft tissue repair biomaterials.
