A feasible strategy of on-demand drug delivery for the treatment of dermal inflammation under low-pH conditions is proposed, employing zeolitic imidazolate framework-8 (ZIF-8) as a pH-responsive nanoparticle and curcumin (CCM) as a model drug. To overcome the low bioavailability of topically treated drug, a microneedle (MN) form is used to incorporate CCM and ZIF-8. Taking advantage of the fact that ZIF-8 degrades under acidic conditions, CCM is embedded in porous ZIF-8 nanoparticles such that CCM is released when ZIF-8 comes into contact with an acidic dermal fluid at the inflammation site, and this CCM-encapsulated ZIF-8 (CCMZIF) is incorporated into water-dissolvable poly(vinyl pyrrolidone) MN. The ZIF-8 shows a high loading capacity (∼40.5%) of CCM through chemical bonding and physical adsorption. From in vitro tests with both a buffered solution and porcine skin, CCM from the CCMZIF MN is released in a higher amount at pH 5.0 than at pH 7.4, demonstrating the capability of the pH-responsive release of the drug when needed at inflammatory sites. The analytical investigation conducted here reveals that an acidic environment triggers the structural degradation of ZIF-8, allowing the release of the chemically bonded CCM. Cytotoxicity and stability tests demonstrate the good biocompatibility and bioavailability of ZIF-8. This study highlights the analytical discussion of the encapsulation and release mechanism of CCM in a ZIF-8-implemented MN drug delivery platform. The results demonstrate an advanced on-demand therapeutic strategy for skin disorder treatment.

Cutibacterium acnes is the most abundant bacterium of the human skin microbiome since adolescence, participating in both skin homeostasis and diseases. Here, we demonstrate individual and niche heterogeneity of C. acnes from 1,234 isolate genomes. Skin disease (atopic dermatitis and acne) and body site shape genomic differences of C. acnes , stemming from horizontal gene transfer and selection pressure. C. acnes harbors characteristic metabolic functions, fewer antibiotic resistance genes and virulence factors, and a more stable genome compared with Staphylococcus epidermidis . Integrated genome, transcriptome, and metabolome analysis at the strain level unveils the functional characteristics of C. acnes . Consistent with the transcriptome signature, C. acnes in a sebum-rich environment induces toxic and pro-inflammatory effects on keratinocytes. L-carnosine, an anti-oxidative stress metabolite, is up-regulated in the C. acnes metabolome from atopic dermatitis and attenuates skin inflammation. Collectively, our study reveals the joint impact of genes and the microenvironment on C. acnes function.

Psoriasis is a chronic inflammatory skin disease. Topical medicines are the preferred treatment for mild to moderate psoriasis, but the effect of excipients used in semi-solid preparations on psoriasis-like skin inflammation is not fully understood. In the present study, we investigated the effect of stearyl alcohol, a commonly used excipient, on imiquimod (IMQ)-induced psoriasis-like skin inflammation in mice. Psoriasis-like skin inflammation was induced by topical IMQ treatment on the back of mice. Skin lesion severity was evaluated by using psoriasis area and severity index (PASI) scores. The skin sections were stained by haematoxylin–eosin and immunohistochemistry. Stearyl alcohol (20% in vaseline) treatment significantly reduced the IMQ-induced increase of PASI scores and epidermal thickness in mice. IMQ treatment increased the number of Ki67- and proliferating cell nuclear antigen (PCNA)-positive cells in the skin, and the increases were inhibited by stearyl alcohol (20% in vaseline) treatment. Stearyl alcohol treatment (1%, 5%, 10% in vaseline) dose-dependently ameliorated IMQ-induced increase of PASI scores and epidermal thickness in mice. Hexadecanol (20% in vaseline), stearic acid (20% in vaseline) and vaseline treatment had no significant effect on IMQ-induced psoriasis-like skin inflammation in mice. In conclusion, stearyl alcohol has the effect of improving IMQ-induced psoriasis-like skin inflammation in mice.

The benefits of emollients for eczematous dermatitis and psoriasis have been thought to be due to the improvements in epidermal function, including epidermal permeability barrier, stratum corneum hydration, and stratum corneum pH. We determined here whether emollient can direct inhibit cutaneous inflammation. Ear inflammation was induced by topical application of either 12-O-tetradecanoylphorbol-13-acetate (TPA) or 1-fluoro-2,4-dinitrofluorobenzene (DNFB). Either 1% hydrocortisone cream or the novel emollient was applied to the right ear of the mice 45 min and 2 hours after TPA or DNFB application. The untreated left ear served as untreated controls. Both ear weight and ear thickness were measured 24 hours after TPA and DNFB application. Topical applications of either hydrocortisone cream or emollient significantly decreased both ear thickness and ear weight in comparison to untreated controls. In DNFB model, hydrocortisone significantly lowered expression levels of mRNA for IL-1α, IL-1β, and TNFα, while the emollient markedly decreased expression levels of IL-1α and TNFα mRNA. In TPA model, both hydrocortisone and emollient significantly decreased expression levels of IL-1α, IL-1β, IL-6, and TNFα mRNA. In parallel, inflammatory infiltration was also reduced by topical applications of either hydrocortisone or emollient. These results demonstrate that this novel emollient can directly inhibit cutaneous inflammation in murine models of both acute irritant contact dermatitis and acute allergic contact dermatitis. However, whether this emollient could also alleviate eczematous dermatitis in humans remains to be explored.

Psoriasis is a complex autoimmune disease that is closely associated with the disorganized pro-inflammatory polarization of macrophages and the activation of inflammatory signalling pathways. Nanoparticles (NPs) have shown their potential in immune response regulation and the related treatment of inflammatory diseases. Herein, we report the modulation of the skin immune system for amelioration of psoriasis-like skin inflammation using mung bean-derived NPs (MBNs), which exhibit high antioxidant activity to reduce reactive oxygen species (ROS) and modulate the immune microenvironment. For imiquimod (IMQ)-stimulated psoriasis-like skin, topical administration of MBNs can achieve the homeostasis of polarized macrophages and antagonize the activation of the nuclear factor kappa B (NF-κB) signalling pathway, which result in the alleviation of skin inflammation. The transcutaneous delivery of MBNs provides a promising approach for the treatment of psoriasis and other inflammatory skin diseases.

Disrupted N6-methyladenosine (m6A) modification modulates various inflammatory disorders. However, the role of m6A in regulating cutaneous inflammation remains elusive. Here, we reveal that the m6A and its methyltransferase METTL3 are down-regulated in keratinocytes in inflammatory skin diseases. Inducible deletion of Mettl3 in murine keratinocytes results in spontaneous skin inflammation and increases susceptibility to cutaneous inflammation with activation of neutrophil recruitment. Therapeutically, restoration of m6A alleviates the disease phenotypes in mice and suppresses inflammation in human biopsy specimens. We support a model in which m6A modification stabilizes the mRNA of the lipid-metabolizing enzyme ELOVL6 via the m6A reader IGF2BP3, leading to a rewiring of fatty acid metabolism with a reduction in palmitic acid accumulation and, consequently, suppressing neutrophil chemotaxis in cutaneous inflammation. Our findings highlight a previously unrecognized epithelial-intrinsic m6A modification–lipid metabolism pathway that is essential for maintaining epidermal and immune homeostasis and lay the basis for potential therapeutic targeting of m6A modulators to attenuate inflammatory skin diseases.

Concurrent imaging of skin inflammation and neovascularization is crucial for diagnosing and monitoring skin conditions, especially in flap transplantation. However, current imaging modalities in the clinic are often non-intuitive, have low resolution, or lack the ability to specifically target skin inflammation. Given that albumin can serve as a biomarker for the disruption of skin-vessel barrier (SVB), probes targeting skin inflammation typically need to specifically bind to endogenous albumin, which often results in high background signals. In this study, we screen a series of near-infrared (NIR) dyes for their in vivo covalent binding capabilities with endogenous albumin, and identify the optimal dye for achieving high-contrast imaging of skin inflammation in models of SVB disruption, with minimal interference from other tissues or organs (e.g., skin and muscle). Moreover, by utilizing an albumin-targeting dye with another albumin-escaping NIR-II dye with a non-overlapping emission wavelength, this work explores the concurrent imaging of skin inflammation and neovascularization after flap transplantation, affording to simultaneously assess skin inflammation and the restoration of blood supply.

Psoriasis is a highly prevalent and chronic inflammatory skin disease, the initiation and progression of psoriasis is closely associated with immune activation and abnormal proliferation of keratinocytes. Estradiol (E2) playing a significant role in psoriasis pathogenesis, exhibits favorable anti-inflammatory effects and could suppresses the inflammation characteristic of psoriasis. However, the transdermal efficiency of current E2 formulations is generally low, and alternative administration routes may be associated with adverse effects. Here, we present a transdermal delivery system of liposome gel designed to facilitate the transdermal delivery of E2 to ameliorate psoriatic skin inflammation. The phospholipids within the liposomes interact with the lipids of the stratum corneum, permeating dense skin structure and thereby increasing the intradermal retention of the drug. The gel matrix provides structural support, which reduces fluidity of the system, enhances liposome stability, and effectively prevents the rapid clearance of the drug from skin surface. Extensive in vitro and in vivo studies were conducted to evaluate the therapeutic efficacy of E2 liposome gel in treating psoriasis. The results showed that E2 liposome gel could effectively penetrate the skin barrier and deliver E2 to the site of inflammation. This led to a significant inhibition of the production of interleukin-1β (IL-1β), interleukin-23 (IL-23), and interleukin-17A (IL-17A), effectively suppressed the abnormal proliferation of keratinocytes, thereby ameliorating psoriatic skin inflammation. In conclusion, this study provides a safer and more efficient strategy for the sustained and controlled release of E2. This not only offers a more promising therapeutic approach for psoriasis, but also provides new insights for exploring the role of E2 in other inflammatory diseases.

Electrical stimulation (ES) is widely used in physiological and medical sciences, while its application to treat inflammatory skin diseases (ISDs) remains a challenge owing to their natural pathological cuticle barrier and lack of an effective combination with chemotherapy to achieve specific immunomodulation. Here, a wearable, battery-free, multi-component drug-loaded electronic microneedle (mD-eMN) system is developed by integrating remodeled metal microneedles loaded with multi-component chemical drugs and flexible triboelectric nanogenerators (TENGs). The system can rapidly release drugs into the site of ISDs and then realize an efficient penetration into cell body and specific immunomodulation under the synergism of pulsed electrons originating from the TENG. Also, the pulsed electrons can promote skin tissue homeostasis reconstruction to alleviate the inflammatory process of ISDs. Sufficient evidence shows that a significant skin inflammation regression of psoriasis (a typical ISDs model) is achieved using the mD-eMN system compared to traditional ES or chemotherapy alone. This innovative wearable mD-eMN system provides an effective flexible electronic and chemical drug joint technological platform for the treatment of ISDs, which is not only suitable for the treatment of psoriasis in this study but also maybe for other ISDs such as diabetic ulcers and skin tumors.

Solar skin damage is one of the most common diseases among outdoor workers. An important cause for the damage is the ultraviolet and infrared rays in sunlight, which are absorbed by the skin in large amounts, leading to severe skin inflammation and oxidative stress. Therefore, physical prevention by shielding the light from harmful wavelengths can be an effective method of skin protection from radiation. However, for existing skin lesions, prompt treatment is essential to avoid the aggravation of the injury and promote repair. Therefore, to improve the therapeutic effect on sun-damaged skin, we attempted to design a system with a dual purpose of eliminating toxic free radicals and modulating tissue inflammatory response. Here, we designed and synthesized a poly-acryloyl lysine (P-Ac-Lys) and polyvinyl alcohol-dihydroxyphenylalanine (PVA-DOPA) composite hydrogel ( [email protected] Hydrogel) loaded with lactate and pyruvate, that exhibites a good free radical scavenging activity and an excellent ability to modulate the inflammatory response. Experimental results showe that this hydrogel film could effectively reduce the UV-induced skin inflammation response, alleviate pathological damage and promote the recovery of the damaged skin.
