Uncontrolled bleeding in emergency situations is a great threat to both military and civilian lives, and an ideal hemostat for effectively controlling prehospital hemorrhage is urgently needed but still lacking. Although hemostatic hydrogels are promising for emergency hemostasis, they are currently challenged by either the mutual exclusion between a short gelation time and strong adhesive network or the insufficient functionality of ingredients and complicated operations for in situ curing. Herein, an extracellular matrix biopolymer-based and multifunctional hemostatic hydrogel that simultaneously integrates rapid thermoresponsive gelation, robust wet adhesion, and ease of use in emergencies is rationally engineered. This hydrogel can be conveniently used via simple injection and achieves instant sol–gel phase transition at body temperature. Its comprehensive performance could be facilely regulated by tuning the proportions of components, and the optimal performance (gelation time 6–8 s, adhesion strength 125 ± 3.6 kPa, burst pressure 282 ± 4.1 mmHg) is established due to the coordinated enhancement of the photo-cross-linking pretreatment and the hydrophilic–hydrophobic balance among various interactions in the hydrogel system. Additionally, it exhibits significant coagulation effect in vitro and enables effective hemostasis and wound healing in vivo. This work provides a promising platform for versatile applications of hydrogel-based materials, including emergency hemostasis.

An asymmetrical wound dressing functions akin to human skin by serving as a protective barrier between a wound and its immediate environment. However, significant challenges persist concerning the robust adhesion and asymmetrical adhesion properties of hydrogels, particularly when applied in emergency hemostasis and wound healing contexts. Herein, the study has successfully synthesized hydrogel patches with Janus asymmetric-adhesion, denoted as HGO-C, exclusively comprised of natural polymers. This achievement is realized through the assembly of adhesive hydrogel (HGO) and non-adhesive hydrogel (CGC), thereby amalgamating their distinct functionalities. The non-adhesive hydrogel component served as a physical shield and safeguarding the wound against contamination, while the adhesive hydrogel, when in contacted with the wound surface, firmly adhered to it, swiftly arresting bleeding and facilitating wound healing. Cytocompatibility tests, hemolysis tests, antibacterial assays, and coagulation assays demonstrated excellent biocompatibility, antibacterial, and hemostatic properties of HGO-C. Finally, the in vivo experiments, including a liver hemorrhage assay and a wound healing assay, unequivocally showed the rapid hemostatic and enhanced wound healing capabilities of HGO-C. Consequently, these distinctive hydrogel patches, derived from natural polymers and characterized by their asymmetric adhesion properties, may have great potential for real-life usage in clinical patients.

Uncontrollable bleeding caused by accidental severe trauma and organ operation severely affects people's life safety, even leading to death. Therefore, it is important to cease the bleeding quickly and effectively. Here, we report a rapid photocrosslinking hydrogel (DP-F127 hydrogel) based on F127 and acrylate functional motifs for in situ emergency hemostasis. The acrylate groups in DP-F127 can be cross-linked within 10 s by UV irradiation. The resulted hydrogels exhibit excellent mechanical properties, biocompatibility, and rapid hemostatic ability. The in vivo studies on the DP-F127 hydrogel demonstrated that liver bleeding could be rapidly repaired and show significantly less blood loss than commercially available dressing (Gelatin sponge, GS). This study provides a new paradigm for developing a hemostatic hydrogel with the rapid speed of crosslinking for in situ emergency hemostasis.

Realizing the potential application of chitosan as an effective biomedical hemostatic agent has become an emerging research hotspot. However, fabricating a flexible chitosan-based hemostatic bio-hydrogel with self-adhesion feature in humid conditions and rapid hemostasis capability remains a challenge. Herein, we reported the development of chitosan-based hydrogels (DCS-PEGSH gels) with typical multilevel pore structures , which were cross-linked by 3-(3,4-dihydroxyphenyl) propionic acid-modified chitosan (DCS) and sebacic acid-terminated polyethylene glycol modified by p-hydroxybenzaldehyde (PEGSH). By precisely regulating the proportion of PEGSH, the fabricated bio-hydrogels displayed favorable cytocompatibility, suitable stretchability (∼780%), and blood absorbability (1300% ± 50%). Moreover, the strong adhesion (∼68.5 kPa) of the assembled bio-hydrogel ensured its firm adherence on pigskin and on bleeding wound in both static and dynamic humid environments without shedding, thus providing a long service life. The fabricated hydrogels exhibited shorter blood clotting time (50 s) and lower blood clotting index (BCI, 41) than the commercial chitosan sponge (288 s, BCI 65). Notably, the amount of blood loss from the liver in mice was reduced by almost 90% as compared to that for the control group. This study paves a solid way for developing a chitosan-based hydrogel with self-adhesive, self-healing, stretchability, biocompatibility, and antibacterial and antioxidant properties through molecular design and structural regulation, which will enable the biomedical application of chitosan in emergency hemostasis, particularly in joints and extremities. Statement of significance The design and preparation of multifunctional integrated green adhesive bio-hydrogels while avoiding the use of organic solvents and toxic chemical reagents has been an emerging challenge. Herein, a flexible chitosan-based hemostatic bio-hydrogel that integrates multifunctional properties was successfully synthesized. The bio-hydrogel displayed suitable stretchability (780%) and blood absorbability (1300% ± 50%). Moreover, the strong adhesion (68.5 kPa) ensured firm adherence of the assembled hydrogel on pigskin and on the bleeding wound site in both static and dynamic humid environments without shedding, thus providing a long service life. In addition, the designed hydrogel showed good compatibility and antibacterial performance. The dynamic Schiff base endowed the bio-hydrogel with excellent self-healing performance without any external stimuli.

Powder hemostatic particles have no limitation on wound type and a wide application prospect in the treatment of wound hemostasis. In this study, bioactive glass-based membrane-like structure camouflage composite particles ( [email protected] /CS) were prepared by Layer-by-layer (LBL) method to coat bovine serum albumin (BSA) and chitosan (CS) on the surface of the synthesized macroporous and mesoporous bioactive glass (MBG). The results showed that [email protected] /CS particles significantly improved the coagulation effect of MBG and could activated both endogenous and exogenous physiological coagulation pathways. [email protected] /CS had the excellent water absorption quality and could immediately release Ca 2+ to participate in the coagulation pathway. The main hemostatic mechanism of [email protected] /CS was that the membrane-like of BSA/CS could form a fibrin network by aggregating erythrocytes and activating platelets, thus rapidly aggregating coagulation factors. In addition, [email protected] /CS particles had a significant hemostatic effect on surface bleeding (tail-amputation model) and internal bleeding (liver injury model) in SD rats, which could shorten the bleeding time and reduce the amount of blood loss effectively. This research work stated that the MBG-based membrane-like structure camouflage composite particles might be with the potential applications as a hemostasis biomaterial for the emergency bleeding.

In emergencies, uncontrolled severe bleeding can result in undesired complications and even death of the injured. Designing advanced hemostatic agents is a potential solution for emergency hemostasis, yet it remains challenging to realize the persistent adhesion in a wet wound environment. In this study, based on dynamic reversible Schiff base bond and photo-initiated double-bond polymerization, a novel injectable hemostatic hydrogel ( L -COC) consisting of methacrylated carboxymethyl chitosan (CMCSMA), oxidized konjac glucomannan (OKGM) and (+)-catechin hydrate (CH) was synthesized for emergency hemostasis. To our delight, the incorporated CH imparted enhanced blood procoagulantion to the L -COC hydrogel by intensifying the hydrogel-red blood cell interactions. As a result, the hemostatic effect of the engineered L -COC hydrogel was significantly superior to that of fluid gelatin Surgiflo TM for liver bleeding wounds in rats (Blood loss: 0.62 ± 0.11 g ( L -COC), 0.90 ± 0.08 g (Surgiflo TM ); hemostasis time: 69.0 ± 2.9 s ( L -COC), 84.0 ± 2.2 s (Surgiflo TM )). With the favorable antioxidant and antibacterial activities, as well as multifunctional properties, the bio-adhesive L -COC hydrogel and the underlying design principles may facilitate further development of practical hemostatic hydrogels.
