Hypoxia, as an inevitable characteristic of solid tumors, has been regarded as a noticeably causative factor to therapeutic resistance and metastatic variants. Exploring novel theranostics to realize the accurate diagnosis of hypoxia and the simultaneous implementation of effective therapy is a promising prospect for the successful treatment of tumors. In the present study, we designed and synthesized a multifunctional rattle-structured nanotheranostic, with the inner core coated by hollow mesoporous silica for chemical drug Doxorubicin (DOX) storage and hypoxia-sensitive MnO 2 enrichment. In various acidic micro-environments caused by hypoxia, MnO 2 nanosheets could be degraded into manganese ions (Mn 2+ ), which were chelated by the modified Tetraxetanum (DOTA) ligands for real-time T 1 -magnetic resonance imaging (T 1 -MRI), with on-demand DOX release to realize both normoxia and hypoxia-sensitive chemotherapy by overcoming hypoxia. Nanotheranostics integrating hypoxia-driven T 1 -MRI with synergetic chemotherapy have tremendous potential in the intelligent diagnosis, personalized treatment and excellent prognosis of solid tumors in the future.

Surgery is the main treatment for liver cancer in clinic owing to its low sensitivity to chemotherapy and radiotherapy, but this results in high mortality, recurrence, and metastasis rates. It is a feasible strategy to construct tumor microenvironments activated by nanotheranostics agents for the diagnosis and therapy of liver cancer. This study reports on a nanotheranostic agent (MONs@PDA-ICG) with manganese oxide nanoflowers (MONs) as core and polydopamine (PDA) as shell loading, with ICG as a photosensitizer and photothermal agent. MONs@PDA-ICG can not only produce ROS to kill cancer cells but also exhibit good photothermal performance for photothermal therapy (PTT). Importantly, O2 generated by MONs decomposition can relieve the tumor hypoxia and further enhance the treatment effects of photodynamic therapy (PDT). In addition, the released Mn2+ ions make MONs@PDA-ICG serve as tumor microenvironments responsive to MRI contrast for highly sensitive and specific liver cancer diagnosis.

Multifunctional nanosystems that can transport therapeutic and diagnostic agents into tumor sites and activate their respective functions via tumor-microenvironment recognition are highly desirable for clinical applications. We fabricated pH and redox dual-activatable self-assembled nanotheranostics (named as DA-SNs) via coordination-driven self-assembly of chlorin e6 (Ce6) disulfide-linked pH sensitive polymer ligand, poly (isobutylene- alt -maleic anhydride- graft -methoxy-poly (ethyleneglycol)- graft -imidazole-graft-Cystamine-Ce6) [PIMA-mPEG-API-SS-Ce6], and gadolinium ions (Gd 3+ ). DA-SNs exhibited uniform particle size of ~48 nm, excellent stability, and inherent biosafety. Negatively charged DA-SNs could prolong blood circulation time (t 1/2 = 2.91 h) and improve tumor accumulation. Moreover, DA-SNs could undergo surface charge switch from negative charge to positive one in a slightly acidic tumor extracellular environment (pH 6.8), thus enhancing cellular uptake. After entering tumor cells, fluorescence, photodynamic therapeutic activity, and T 1 MR contrast from DA-SNs could be activated within this intracellular environment with lowered pH and high level of GSH. Importantly, human tumors implanted in mice could be successfully visualized via distinct pH and redox dual-sensitive T 1 MR contrast and fluorescence imaging, indicating that DA-SNs could serve as a dual-modal MR/fluorescence imaging probe for tumor-targeting diagnosis. In addition, DA-SNs exhibited superior photodynamic therapeutic efficiency with negligible side effects. Therefore, this DA-SN shows great promise for synergistic photodynamic therapy and diagnostic imaging.

A simple method is presented for the preparation of novel europium complexes-grafted-oxidative dopamine (ECOD) nanoparticles with uniform sizes (ca. 220 nm), as a high-performance nanotheranostic agent guided by X-ray computed tomography (CT) and photoluminescence (PL). Here, an oxidative dopamine precursor with a rough surface is first prepared by copper (II) ionic catalysis. Then, europium complexes are successfully grafted onto the oxidative dopamine precursor by surface-initiated atom transfer radical polymerization (ATRP). Interestingly, as the additional amount of europium (III) complexes increases, the roughening extent of the ECOD nanoparticles is increased, and the roughening surface expands to a porous network structure. This nanoagent with a rough surface exhibits not only excellent X-ray attenuation properties, but also prominent eminent near-infrared (NIR) absorbance and strong optical imaging efficiency. In addition, ECOD can provide an effective photothermal therapy (PTT) effect, and 39% photothermal conversion efficiency can be achieved. Furthermore, there is no widely detectable toxicity induced in vivo/vitro , and blood circulation can last for a long time in the presence of ECOD nanoparticles, leading to efficient accumulation at the tumor site by a persistently of enhanced permeability and retention (EPR) effect. With the combination of NIR laser irradiation (1.5 W/cm 2 ) for 10 min at 808 nm, effective photothermal ablation of a tumor in vivo can be achieved. Thus, ECOD nanoparticles can be used for effective antitumor treatment and may further exhibit potential as a nanotheranostic agent for clinical diagnosis and treatment.

The combination of photodynamic therapy (PDT) and photothermal therapy (PTT) has attracted much interest in recent years, but non-specific distribution of photosensitizers and intrinsic tumor hypoxic microenvironment have continued to limit its therapeutic efficiency. We herein report a nano-theranostic system, denoted as Ce6-CuS/MSN@PDA@MnO 2 -FA NPs, which combines PDT, PTT, magnetic resonance (MR) imaging with hypoxia-relieving and tumor-targeting functionalities. Central to this design is the use of mussel-inspired polydopamine (PDA) coating to encapsulate the chlorin e6 (Ce6) and copper sulfide nanoparticles (CuS NPs) loaded mesoporous silica nanoparticle (MSN) core. The PDA coating not only acts as pH sensitive gatekeeper to prevent the premature release of Ce6 under non-acidic tumor microenvironment (TME), but also facilitates post-functionalization so that hypoxia-relieving MnO 2 nano-sheets and tumor-targeting ligand folic acid-PEG-thiol (FA-PEG-SH) can be decorated on the outer part of the drug system. In vitro and in vivo measurements clearly demonstrated that all these functionalities worked synergistically as expected. The system, having a low dark cytotoxicity, can be effectively internalized by 4T1 cells and decrease the cell viability to 2% upon 660 nm/808 nm laser irradiation. Tumors in 4T1 tumor-bearing mice can almost be completely destroyed in 2 weeks via combined PDT/PTT. Together with the TME-sensitive MR imaging performance demonstrated, Ce6-CuS/MSN@PDA@MnO 2 -FA NPs represent a multifunctional prototype which holds great potential to be developed into clinical theranostics.

Designing a smart nanotheranostic system has recently attracted tremendous attention and is highly desirable for realizing targeted cancer therapy and early diagnosis. Herein we report the fabrication of smart nanotheranostic system using multiresponsive gatekeeping protocol of mesoporous silica nanoparticles (MSN). Acid, oxidative stress and redox sensitive manganese oxide (MnOx) coated superparamagnetic iron oxide nanoparticle (SPION) were employed as nanolids to regulate the camptothecin drug release from the channels of mesoporous silica and achieve responsive dual-mode MRI contrast. The nonvehicle showed high magnetization and T2 contrast in magnetic resonance imaging (MRI) due to the significant density of SPION onto the surface of MSN, and at the same time the MnOx shell degradation release Mn2+ which enhanced the T1MRI visualization. The efficacy of responsive drug delivery system was investigated on pancreatic cancer cells and tumor-bearing mice, and results reinforced that MnOx-SPION@MSN@CPT nonvehicle is efficacious against cancer cells. We envision that our unique and multiresponsive nanoplatform may find applications in effective delivering of imaging and therapeutic agents to wide range of diseases besides cancer.

We report a computed tomography/magnetic resonance (CT/MR) imaging-guided X-ray induced dynamic therapy (XDT) nanotheranostic platform based on Ti x O y /Ru reaction units. A new metal–organic nanostructure (TiZrRu-MON) was prepared via a solvothermal reaction of [Ti 8 Zr 2 O 12 (COO) 16 ] clusters and bipyridine-(COOH) 2 /Ru(II)-bipyridine-(COOH) 2 co-organic ligands. We demonstrate that the cluster can absorb X-ray and transform the energy to Ru(II)-bipyridine-(COOH) 2 with effective reactive oxygen species (ROS) production, which is five times more than TiZr-MON. In addition, Gd(III) ions were anchored to the bipyridine of MON for concurrent CT/MR imaging. Notably, the TiZrRuGd-MON possesses ultrahigh longitudinal relaxivity ( r 1 value up to 53.09 mM −1 ·s −1 at 1.5 T, 7-fold higher than commercial Gd-DTPA). With the guidance of CT/MR dual-mode imaging that provides comprehensive and complementary contrast information, the TiZrRuGd-MON is monitored to accumulate in tumor sites and eliminate tumor with high efficiency under X-ray irradiation. The TiZrRu-MON-based XDT strategy opens a pathway for clinical translational research of cancer theranostics.

The development of intelligent nanotheranostic technology that integrates diagnostic and therapeutic functions holds great promise for personalized nanomedicine. However, most of the nanotheranostic agents exhibit “always-on” properties and do not involve an amplification step, which may largely limit imaging contrast and restrict therapeutic efficacy. Herein, we construct a novel nanotheranostic platform (Hemin/DHPs/PDA@CuS nanocomposite) by assembling DNA hairpin probes (DHPs) and hemin on the surface of PDA@CuS nanosheets that enables amplified fluorescence imaging and activatable chemodynamic therapy (CDT) of tumors. The cancer-relevant APE1 triggers nucleic acid amplification with DHPs to generate activatable and amplified fluorescence signals for discriminating cancer cells from normal cells. Meanwhile, excessive G-quadruplex/hemin-based DNAzyme are also activated, and they function as Fenton-like catalysts to catalyze the production of highly toxic hydroxyl radicals (•OH) for CDT. Moreover, owing to the excellent photothermal conversion efficiency in the near-infrared-II (NIR-II) window, the PDA@CuS not only improves the catalytic performance of CDT but also furnishes PTT. A remarkable antitumor therapeutic effect is demonstrated both in vitro and in vivo. Therefore, the Hemin/DHPs/PDA@CuS nanocomposite is expected to provide a promising avenue for precise imaging-guided antitumor therapy.

Manganese dioxide (MnO 2 )-based nanoparticles are a promising tumor microenvironment-responsive nanotheranostic carrier for targeted magnetic resonance imaging (MRI) and for alleviating tumor hypoxia. However, the complexity and potential toxicity of the present common synthesis methods limit their clinical application. Herein, multifunctional hyaluronic acid-MnO 2 nanoparticles (HA-MnO 2 NPs) are synthesized in a simple way by directly mixing sodium permanganate with HA aqueous solutions, which serve as both a reducing agent and a surface-coating material. The obtained HA-MnO 2 NPs show an improved water-dispersibility, fine colloidal stability, low toxicity, and responsiveness to the tumor microenvironment (high H 2 O 2 and high glutathione, low pH). After intravenous injection, HA-MnO 2 NPs exhibit a high imaging sensitivity for detecting rat intracranial glioma with MRI for a prolonged period of up to 3 d. These nanoparticles also effectively alleviate the tumor hypoxia in a rat model of intracranial glioma. The downregulation of VEGF and HIF-1α expression in intracranial glioma validates the sustained attenuation effect of HA-MnO 2 NPs on tumor hypoxia. These results show that HA-MnO 2 NPs can be used for sensitive, targeted MRI detection of gliomas and simultaneous attenuation of tumor hypoxia.

Nanotheranostics, which can provide great insight into cancer therapy, has been deemed as a promising technology to settle the unmet medical needs. The rational design of high performance nanotheranostics with multiple complementary imaging features and satisfactory therapeutic efficacy is particularly valuable. Herein, versatile nanotheranostic agents DPPB-Gd-I NPs were fabricated by using gadolinium-diethylenetriaminepentaacetic acid chelates and an iodine-decorated copolymer as encapsulation matrixes to encapsulate a polymer DPPB through one-step nanoprecipitation. We have demonstrated that such nanoagents are able to efficiently damage tumors under single dose injection and NIR laser illumination conditions due to the enhanced photodynamic therapy and enhanced photothermal therapy (the tumor inhibition rate was as high as 94.5%). Moreover, these nanoagents can be utilized as dual-modal NIR-II fluorescence/magnetic resonance imaging probes for tumor diagnosis with high sensitivity, deep tissue penetration, and excellent spatial resolution. Overall, this work offers a powerful tactic to fabricate high performance nanotheranostics for clinical application.

Upconversion nanoparticles (UCNPs)-based photodynamic nanotheranostic agents could address the main drawbacks of photosensitizer molecules (PSs) including instability in aqueous solution and rapid clearance. Due to the relatively weak luminescence intensity of UCNPs and insufficient reactive oxygen species (ROSs), UCNPs-based photodynamic therapy (UCNPs-PDT) was discounted for deep-seated tumors. Thus, we proposed a PSs-modulated sensitizing switch strategy. Indocyanine green (ICG) as an NIR organic dye was proved to effectively enhance the luminescence intensity of UCNPs. Herein, four-color UCNPs were coated with a silica layer which loaded ICG and PSs while the thickness of silica layer was controlled to assist the sensitization function of ICG and activation of PSs. Under the drive of mitochondria-targeting ligand, the prepared nanotheranostic agent would accumulate in the mitochondria where ROSs were in-situ produced and then cell apoptosis was induced. Due to the cooperative PDT and high tissue-penetration depth of NIR laser, the prepared upconversion nanotheranostic agent could achieve significant inhibition on the deep-seated tumors.

Fluorine-19 magnetic resonance imaging ( 19 F MRI) offers distinct advantages, including background-free signal detection, quantitative analysis, and deep tissue penetration. However, its application is currently limited by challenges associated with existing 19 F MRI contrast agents, such as short transverse relaxation times (T 2 ), limited imaging sensitivity, and suboptimal biocompatibility. To overcome these limitations, a glutathione (GSH)-responsive triblock copolymer (PB7), featuring self-immolative characteristics, has been developed. In aqueous solution, PB7 can spontaneously self-assemble into a 19 F MRI contrast agent (SPTF), which exhibits a long T 2 relaxation time and GSH-responsive T 2 prolongation. Notably, during the self-assembly process of PB7, the photosensitizer Chlorin e6 (Ce6) can be encapsulated inside of the hydrophobic domain of SPTF, resulting in the formation of a multifunctional nanotheranostic agent (Ce6@SPTF). Ce6@SPTF is able to undergo structural disintegration in response to elevated GSH levels at the tumor site, leading to the dissociation of fluorinated segments and a marked amplification of the 19 F MRI signal. Concurrently, the controlled release of Ce6 generates high levels of reactive oxygen species (ROS) under laser irradiation, enabling effective in vivo ablation of breast tumors. This study presents a promising strategy to effectively combine 19 F MRI with therapeutic interventions.
