Highly Efficient Antibacterial Hydrogel-based Multifunctional Coatings for Ureteral Stent to Inhibit Severe Infectious Encrustation

Jinxia Zhai,Lin Shi,Yucong Wu,Jiawei Li,Senwei Liu,Peng Yu,Ziying Feng,Jiawen Zhan,Ye Tian,Haoyu Jin,Chengyun Ning

Ureteral stents are prone to encrustation due to urinary crystallization and bacterial infection, leading to severe complications and restricting long-term use. In this study, a multifunctional hydrogel-based coating was developed, which integrates the crosslinking of polyacrylic acid coupled with zinc ions (PAA(Zn 2+ )) possessing antibacterial properties and polyvinylpyrrolidone (PVP) with hydrophilic lubrication properties on the PU stent (PPAZ/PU). This novel PPAZ/PU coating was devised to mitigate the challenges of bacterial infectious encrustation associated with stent placement in clinical settings. The coating was synthesized via complex decomposition and addition polymerization, forming a stable zinc-ion-rich crosslinked network. The intertwining of PVP and PAA(Zn²⁺) molecular chains, along with hydrogen bond formation, enhanced the hydrogel's cohesion and peel strength up to 1000 N/m. The hydrophilicity and lubricity of the PPAZ/PU-coated stent was markedly enhanced for clinical applications, with a reduction in contact angle and friction coefficient by a factor of 3.25 and 2.62, respectively. In vitro antibacterial assays and simulated infectious encrustation experiments revealed that the PPAZ/PU-coated stent had an antibacterial efficacy of over 99% and halved the surface encrustation per unit area. This PPAZ/PU hydrogel-based coating strategy presents an efficacious solution to the anti-encrustation demands of ureteral stents, particularly in severe urinary tract infection scenarios.

A stable dual functional superhydrophobic coating to inhibit Proteus mirabilis colonization, migration, and encrustation formation for urinary catheter applications

Jiru Miao,Haiyang Chai,Longxing Niu,Mi Ouyang,Rong Wang

Catheter-associated urinary tract infection is one of the most common hospital-acquired infections. Proteus mirabilis, a common pathogen associated with urinary tract infections, has swarming motility and has pili on its surface for adhesion and flagella for upward movement. Migration of P. mirabilis along the catheter surface can lead to ascending urinary tract infection. However, there is currently a lack of effective strategies to inhibit or delay the colonization, migration, and encrustation formation of P. mirabilis in urinary catheters. This study developed a method for constructing a stable superhydrophobic coating on the surface of urinary catheters using a layer-by-layer approach. The adhesion and deposition of polydopamine were enhanced by pre-coating a liquid bandage film on the polydimethylsiloxane surface, resulting in a multilayer micro-nano composite structure on the catheter surface. This structure was combined with copper ions and superhydrophobic modifiers, ultimately achieving a highly stable superhydrophobic coating. The coating retains its superhydrophobic properties after prolonged incubation, friction tests, and tape peeling tests. Importantly, the coating demonstrates excellent efficacy in inhibiting P. mirabilis colonization, migration, and encrustation formation. This study offers novel insights into developing biomedical superhydrophobic coatings with enhanced stability and efficacy in inhibiting urinary tract bacterial infections.

Trimethylamine N-Oxide–Derived Zwitterion Coating for Polyurethane Ureteral Stents Prevents Encrustation Formation

Kaiguo Xia,Guang Chen,Bingbing Hou,Zhe Wang,Yaqi Zhu,Yuexian Xu,Shanfu Zhang,Qiang Xuan,Yezi You,Zongyao Hao

A ureteral stent with strong resistance to proteins, bacteria, and multivalent ions is crucial for the safe treatment of urologic diseases. Generally, the proteins, bacteria, and multivalent ions present in urine tend to bind to the stent surface, leading to aggregation, nucleation, and subsequent stent encrustation. Stent encrustation can induce or exacerbate urinary tract infections and obstructions, thereby seriously harming kidney function. Although hydrophilic coatings on ureteral stents can reduce the binding of proteins, bacteria, and multivalent ions, encrustation still occurs. To date, preventing stent encrustation formation remains a significant challenge. Here, we grafted dense trimethylamine oxide (TMAO)-derived zwitterionic polymers onto the stent surface via a branched amplification strategy. These zwitterions can strongly bind water molecules, forming a stable hydration shell that repels proteins, bacteria, and multivalent ions from adhering to the surface of the polyurethane ureteral stent, thus rendering the stent anti-encrustation. The results showed that the TMAO-derived zwitterion-coated stents exhibited a significantly reduced encrustation weight (13.8% of the original polyurethane stent) and demonstrated good safety. This approach offers a promising method for enhancing stent encrustation resistance. Statement of Significance This study successfully developed a TMAO-derived zwitterionic coating on the surface of a polyurethane stent, creating a superhydrophilic surface with a minimal contact angle of 5.2 o . This surface effectively shields the stent from interactions with proteins, bacteria, and multivalent ions in urine, demonstrating favorable anti-protein adsorption and antibacterial adhesion properties. The superhydrophilic surface formed by the TMAO-derived zwitterionic coating on the stents (PTMAO-s) provides strong anti-fouling resistance and enhanced anti-encrustation properties. Under identical conditions, the encrustation resistance of PTMAO-s is approximately 7.2-fold greater than that of original polyurethane stents (PU), 3.6-fold greater than Bard commercial stents, and 2.1-fold greater than betaine-coated stents (PSBG-s).

Bio-inspired antibacterial coatings on urinary stents for encrustation prevention

Qin Yao,Binghai Chen,Jiaxiang Bai,Wenbo He,Xu Chen,Dechun Geng,Guoqing Pan

Urinary tract infection (UTI) represents one of the most common nosocomial infections, which is mainly related to indwelling catheters or stents. In addition to the formation of biofilms to reduce antibiotic sensitivity, the urease-producing bacteria can also increase urine pH, causing Ca2+ and Mg2+ deposition and finally catheter obstruction. The prevention of UTIs and its complication (i.e., encrustation) thus is a great challenge in design of catheters and ureteral stents. In this work, a metal-catechol-assisted mussel chemistry (i.e., dopamine self-polymerization) was employed for surface functionalization of commercially available catheters with antimicrobial peptides (AMP), for the purpose of long-term anti-infection and encrustation prevention. To improve the stability of the polydopamine coating on polymeric stents, we used Cu2+-coordinated dopamine self-polymerization. Then, a cysteine-terminated AMP was introduced on the polydopamine coating through Michael addition. We found that the Cu2+-coordinated polydopamine coating showed improved stability and antibacterial effect. The cytotoxicity test confirmed that the bioinspired antibacterial coating showed good biocompatibility and no obvious toxicity. The results confirmed that the stents with AMP could in situ inhibit bacterial growth and biofilm formation, and finally reduce the deposition of struvite and hydroxyapatite crystals both in vitro and in vivo. We anticipate that this bioinspired strategy would develop a safe, stable and effective antibacterial coating on urinary tract medical devices for long-term bacterial inhibition and encrustation prevention.

Crystallization ripening and erosion of calcium oxalate under the effect of bacteria and a polymer materials surface

Xiran Shen,Wentao Chen,Li Guo,Huayu Li,Hong Chen,Fan Liu

As typical examples of pathological biomineralization, urinary stones and stent encrustation have been associated with bacteria, yet the underlying mechanisms remain unclear. In this study, the effect of Gram-negative Escherichia coli and Gram-positive Staphylococcus aureus on the nucleation and growth of calcium oxalate crystals both in solution and on material surfaces in vitro was investigated. Both bacteria can promote calcium oxalate crystallization, and E. coli shows a prominent ability to boost the nucleation and growth rate. Interestingly, we discovered an Ostwald ripening phenomenon after the initial nucleation on the material surfaces, where larger particles emerge upon the disappearance of small nuclei particles, evident in the case of S. aureas. Over an extended period of time, erosion and disintegration of the crystals was observed when bacteria were involved. Based on these understandings, we developed a new functional surface by synthesizing an antibacterial polypeptoid in-house and utilizing polyurethane as the substrate material. This surface exhibits a synergistic effect that inhibits the formation of calcium oxalate crystals. This study helps to elucidate the role of bacteria in calcium oxalate biomineralization and supports further development of treatment approaches such as anti-encrustation polymer materials.

Enhanced long-term performance of ureteral stents with functional coating: Combating bacterial adhesion and inhibiting encrustation

Senwei Liu,Lei Qian,Yucong Wu,Lei Zhou,Xinggang Xu,Jiapeng Yang,Chengli Liu,Zurong Zhu,Jie Shu,Zhengao Wang,Jinxia Zhai,Chengyun Ning

Long-term indwelling ureteral stents in urology surgeries often lead to severe encrustation and challenging clinical complications, necessitating an effective medical response. Although many researchers have started with antibacterial, anti-fouling and other aspects, the lack of direct urinary pertinence or develop drug resistance have not really improved the current situation of crust. Therefore, while giving full play to the excellent properties of coated anti-bacterial adhesion, it is still a challenging problem to ensure that it can be effectively applied to ureteral stents and urinary tract environment. Herein, this work proposes a practical functional coating to inhibit encrustation, achieved by incorporating the urinary crystal inhibitor citric acid to crosslink the polyvinylpyrrolidone with hydrophilic lubrication properties (PVP/CA). The coating based on charge interaction has excellent stability (peel strength up to 600 N/m) excellent wettability (contact angle reduced by 60°), lubricity (friction coefficient reduced by 4 times) and biocompatibility . More importantly, the synergistic anti-bacteria adhesion (>90 %) and inhibition of urinary tract crystallization properties of the coated ureteral stent demonstrate superior performance in preventing encrustation over a 4-week period in an in vitro model. This innovative coating strategy holds promising prospects for ureteral stents, offering a potential solution to alleviate pain in urological patients.

Preparation and antifouling property of polyurethane film modified by chondroitin sulfate

Huihui Yuan,Jing Xue,Bin Qian,Huaying Chen,Yonggang Zhu,Minbo Lan

An antifouling polyurethane film modified by chondroitin sulfate (PU-CS) was prepared by chemical grafting with N-Boc-1,3-propanediamine as a spacer. The different mass fraction of N-Boc-1,3-propanediamine was investigated to obtain PU-CS films with different CS grafting density. The surface properties of PU-CS films were comprehensively characterized. Proteins adsorption and glycosaminoglycans adhesion on films were evaluated. Moreover, inorganic salt deposition on film with highest CS grafting density (3.70   μg/cm 2 ) was briefly investigated. The results showed that the increase of CS grafting density improved not only the hydrophilicity but the antifouling performance of films. The best antifouling film reduced the adsorption of fibrinogen (BFG), human serum albumin (HSA) and lysozyme (LYS) by 81.4%, 95.0% and 76.5%, respectively, and the adhesion of chondroitin (CS), heparin (HP) and hyaluronic acid (HA) by 70.6%, 87.4% and 81.3%, respectively. In addition, the co-adsorption of proteins and glycosaminoglycans reduced up to 86.9% and 75.5%, respectively. Changes in inorganic salt deposition after co-adsorption of proteins and glycosaminoglycans on PU-CS(3) suggested that the proteins promoted the inorganic salt deposition, while glycosaminoglycans inhibited the crystal growth. The negatively charged polysaccharides might promote the generation of smaller crystals which could be conducive to provide theoretical and practical guide to develop novel urinary stents with significant anti-encrustation properties.

One-step coordination of metal–phenolic networks as antibacterial coatings with sustainable and controllable copper release for urinary catheter applications

Zhimao Huang,Dawei Zhang,Qinwei Gu,Jiru Miao,Xiao Cen,Robert Petrovich Golodok,Vadim Victorovich Savich,Alexander Phyodorovich Ilyushchenko,Zhansong Zhou,Rong Wang

Catheter-associated urinary tract infections (CAUTIs) draw great concern due to increased demand for urinary catheters in hospitalization. Encrustation caused by urinary pathogens, especially Proteus mirabilis, results in blocking of the catheter lumen and further infections. In this study, a facile and low-cost surface modification strategy of urinary catheters was developed using one-step coordination of tannic acid (TA) and copper ions. The copper content of the coating could be manipulated by the number of TA–Cu (TC) layers, and the coating released copper in a pH-responsive manner. The coating exhibited high antibacterial efficiency (killed >99% of planktonic bacteria, and reduced biofilm coverage to <1% after 24 h) due to the synergistic antimicrobial effect of TA and copper ions. In vivo study with a rabbit model indicated that with two TC layers, the coated catheter could effectively inhibit bacterial growth in urine and colonization on the surface, and reduce encrustation formation. In addition, the TC-coated catheter exhibited better tissue compatibility compared to the unmodified catheter, probably due to the antibacterial performance of the coating. Such a straightforward coating strategy with good in vitro and in vivo antibacterial properties and biocompatibility holds great promise for combating CAUTIs in clinical practice.

Multifunctional hydrogel coatings with high antimicrobial loading efficiency and pH-responsive properties for urinary catheter applications

Jiru Miao,Xiang Wu,Yue Fang,Mingzhu Zeng,Zhimao Huang,Mi Ouyang,Rong Wang

Catheter-associated urinary tract infections are one of the most common hospital-acquired infections. Encrustation formation results from infection of urease-producing bacteria and further complicates the situation. A typical sign of the initial onset of encrustation formation is the alkalization of the urine (pH up to 9–10). However, effective antibacterial strategies with high antimicrobial loading efficiency and pH-responsiveness of antimicrobial release are still lacking. In this study, we developed a poly(sulfobetaine methacrylate)-tannic acid (polySBMA-TA) hydrogel coating, which served as a universal, efficient, and responsive carrier for antimicrobials on urinary catheters. Common antimicrobials, including poly(vinylpyrrolidone)-iodine, copper ions, and nitrofurazone were loaded into the polySBMA-TA coating in high efficiency (several fold higher than that of the polySBMA coating), via the formation of multiple non-covalent interactions between the antimicrobials and hydrogel coating. The hydrogel coatings maintained good antibacterial properties under neutral conditions. More importantly, the pH-responsive release of antibacterial agents under alkaline conditions further enhanced the antibacterial activity of the coatings, which was advantageous for killing the urease-producing bacteria and preventing encrustation. In vitro and in vivo tests confirmed that the hydrogel coating has good biocompatibility, and could effectively inhibit bacterial colonization and encrustation formation. This study offers new opportunities for the utilization of a simple and universal antimicrobial-loaded hydrogel coating with smart pH-responsive properties to combat bacterial colonization and encrustation formation in urinary catheters.
