Suture-Mediated Delivery System Reduces the Incidence of Uterine Scarring Through the TGF-β Pathway

He Bai,Wei Zhang,Xuanxuan Yan,Lin Qiu,Pengfei Cui,Weiyang Chen

In recent years, factors such as the postponement of childbearing and the relaxation of the childbearing policy have led to an increase in the proportion of cesarean sections and other intrauterine surgeries among pregnant women, further increasing the incidence of uterine scars. Currently, there is a lack of effective clinical treatment methods for uterine scars. In this study, a suture loaded with gene medicine was designed for the repair of uterine scars. Specifically, the non-viral vector Lipo8000 was first used to form a complex solution with the plasmid TGF-β3. Then, it was mixed and adsorbed with the surgical sutures pretreated with recombinant human type III collagen (RhCol III). In vitro experiments confirmed that RhCol III and the plasmid were successfully loaded onto the sutures and could be released and expressed. In vivo experiments were carried out using a rat model simulating uterine scars. The section results showed that compared with the scar model group, the expression level of TGF-β3 in the RhCol III+TGF-β3 group increased by 39%, the expression level of TGF-β1 decreased by 62.8%, and the fibrosis rate decreased by 16.8%, which has a positive effect on the prevention of uterine scars. This study integrates the therapeutic medicine into the sutures, ensuring that the medicine can come into contact with the wound site after suturing. Moreover, RhCol III and the gene medicine work synergistically to promote the repair of uterine wounds.

Guanidine-containing double-network silks with enhanced tensile and antibacterial property

Chang Liu,Xin Hu,Xiang Zhou,Yan Ma,Polly H.M. Leung,John H. Xin,Bin Fei

The bacterial infection of surgical wounds results in prolonged hospitalization and even death of patients, calling for antibacterial function in modern suture products. To tackle this challenge, cationic guanidine-containing copolymer was synthesized, exhibiting antibacterial potency over 5 log reduction against both Gram-positive S. aureus and Gram-negative E. coli . Furthermore, we developed a double-network silk suture by integrating a guanidine-containing copolymer network into the silk fibroin network. This suture exhibited biocidal activity against S. aureus and E. coli , and superior strength compared to the commercial product in both dry and wet conditions. These results may bring general benefits to public health and medical equipment sustainability.

A suturable biohydrogel with mechanical matched property based on coating chitosan and polyethylene glycol shell for tissue patching

Yimei Yang,Tian Hu,Hongyan Zhang,Bu-Ning Liu,Jinwen Liu,Wei-Xiong Li,Kui Song,Gang-Biao Jiang

The mechanical mismatch between soft hydrated tissues and sutures has become a common negative impact on wound healing process. A novel method of coating multilayer polymer shells is thus reported to improve the mechanical performance of hydrogel sutures. It is suitable for tissue patching and shows advantages of convenient, efficient, and biosafety. Specifically, a precursor hydrogel (Cu@CMC) consisted of carboxymethyl chitosan and copper modified by carbon dots was used as the inner sheath, and then bonding the precursor hydrogel sheath with toughening polyethylene glycol network by anchoring sites composited from rigid chitosan shell integrated a whole structure. Subsequently, the whole system was soaked with EtOH, and rapid dehydration of EtOH was used to accelerate the entanglement process between the two coatings by constricting the molecular chains. Finally, an ideal suture (Cu-fiber) with both toughness and rigidness was obtained. The data showed that the tensile strength and biosafety of the hydrogel sutures prepared by the new strategy were significantly improved, and the skin, liver and vessel of rodents can be sutured without secondary damage. Moreover, it can inhibit inflammation response and promote the healing process of skin wound, indicating that the Cu-fiber will become a great candidate for tissue patching.

Antibacterial Surgical Sutures Developed Using Electrostatic Yarn Wrapping Technology

Ching-Wen Lou,Chun-Yu Hung,Mengdan Wei,Tingting Li,Bing-Chiuan Shiu,Jia-Horng Lin

A significant amount of research has been conducted on applying functional materials as surgical sutures. Therefore, research on how to solve the shortcomings of surgical sutures through available materials has been given increasing attention. In this study, hydroxypropyl cellulose (HPC)/PVP/zinc acetate nanofibers were coated on absorbable collagen sutures using an electrostatic yarn winding technique. The metal disk of an electrostatic yarn spinning machine gathers nanofibers between two needles with positive and negative charges. By adjusting the positive and negative voltage, the liquid in the spinneret is stretched into fibers. The selected materials are toxicity free and have high biocompatibility. Test results indicate that the nanofiber membrane comprises evenly formed nanofibers despite the presence of zinc acetate. In addition, zinc acetate can effectively kill 99.9% ofE. coliandS. aureus.Cell assay results indicate that HPC/PVP/Zn nanofiber membranes are not toxic; moreover, they improve cell adhesion, suggesting that the absorbable collagen surgical suture is profoundly wrapped in a nanofiber membrane that exerts antibacterial efficacy and reduces inflammation, thus providing a suitable environment for cell growth. The employment of electrostatic yarn wrapping technology is proven effective in providing surgical sutures with antibacterial efficacy and a more flexible range of functions. Keywords:HPC;zinc acetate;electrostatic wrapping yarns;surgical sutures

A braided surgical silk suture with controllable biodegradability via enzymatic hydrolysis

Xuchen Wang,Hongqiang Shen,Dahua Shou,Yu Liu,Tianyi Wang,Zhaozhu Zheng,Zeyu Zhao,Xiaoqin Wang,Gang Li

Surgical sutures are essential in wound closure, with different wound sites demanding sutures with specific mechanical properties and biodegradation rates. While silk sutures have a rich history of clinical use, they are typically regarded as nonabsorbable due to their slow degradation in the human body. In this study, we present the development of an absorbable surgical silk suture (ASS) that features controllable biodegradability and mechanical properties, along with excellent biocompatibility and antibacterial attributes. The ASS was created through a straightforward enzymatic hydrolysis process using proteinase K on degummed braided silk sutures. Additionally, we enhance the mechanical properties of the ASS by applying a regenerated silk fibroin (RSF) coating technology, which uses vapor treatment under water and methanol atmospheres. This combination of enzymatic hydrolysis and RSF coating technology allows for the customization of ASS's biodegradation rates while ensuring that the suture diameter and tensile strength comply with the United States Pharmacopeia (USP) standards for absorbable sutures. This innovative approach addresses the dynamic needs of medical professionals and patients alike, providing a novel solution for wound closure.

Microstructural and Tribological Properties of a Dopamine Hydrochloride and Graphene Oxide Coating Applied to Multifilament Surgical Sutures

Gangqiang Zhang,Jiewen Hu,Tianhui Ren,Ping Zhu

With the development of fine surgery and desire for low-injury methods, the frictional properties of surgical sutures are one of the crucial factors that can cause damage to tissue, especially for some fragile and sensitive human tissues such as the eyeball. In this study, dopamine hydrochloride and graphene oxide were used as external application agents to prepare a biological coating for the surface of multifilament surgical sutures. The effects of this biocoating on the surface morphology, chemical properties, mechanical properties, and tribological properties of surgical sutures were studied. The friction force and the coefficient of friction of surgical sutures penetrating through a skin substitute were evaluated using a penetration friction apparatus and a linear elastic model. The tribological mechanism of the coating on the multifilament surgical sutures was investigated according to the results of the tribological test. The results showed that there were uniform dopamine and graphene oxide films on the surface of the surgical sutures, and that the fracture strength and yield stress of the coated sutures both increased. The surface wettability of the surgical sutures was improved after the coating treatment. The friction force and the coefficient of friction of the multifilament surgical sutures with the dopamine hydrochloride and graphene oxide coating changed little compared to those of the untreated multifilament surgical sutures.

Clinical Grade Fibroin Sutures with Bioactive Gold Clusters Enhance Surgical Wound Healing via Inflammation Modulation

Fangzhen Tian,Shu Zhang,Miaoyu Wang,Yuxing Yan,Yiyao Cao,Yili Wang,Kelong Fan,Hao Wang,Jianning Zhang,Xiao-Dong Zhang

Silk sutures are common in surgeries, and silk-based textiles are widely used in clinical medicine on account of their great mechanical properties and biodegradability. However, due to the lack of biocatalytic activity, silk sutures show unsatisfactory anti-inflammatory properties and healing speed. To address this constraint, we construct clinical grade bioactive gold cluster–sutures through a heterojunction. The antioxidant activity of bioactive gold cluster–sutures is ∼160 times more than that of clinical sutures. Meanwhile, the suture displays superb reactive oxygen species (ROS) scavenging, superoxide dismutase-like (SOD-like, 5 times more than the silk suture), and catalase-like (CAT-like) activities. The clusters assemble on the surface of silk through hydrogen bonding, leading to a durable catalytic and structural stability for 15 months without decay. Subsequently, the suture significantly accelerates wound healing by exerting excellent anti-inflammatory effects, improving neovascularization and collagen deposition. Clinical grade bioactive gold clusters with high bioactivity, stability, and biocompatibility hold promise for clinical translation and pave the way for other implanted biomaterials from wound healing to intelligent textiles.

Preparation of biodegradable, antibacterial core-spun yarns with braided structures using QAS/PLA micro/nanocomposites

Jun Xu,Xingyu Zhao,Zhenzhen Lei,Huimin Jia,Haolong He,Genghao Gong,Jing Wang,Tiegang Wang

Medical antibacterial textiles play a vital role in tackling the issue of bacterial infection. Traditional surgical sutures face significant challenges due to wound infection caused by bacteria and breakage and scars caused by poor suture strength. Therefore, a new antibacterial and high-strength suture preparation strategy with wide clinical applicability was highly desired. In this study, a biodegradable quaternary ammonium salt (QAS)/polylactic acid (PLA) core-spun yarn with excellent antibacterial and mechanical properties was prepared by conjugated electrospinning technology combined with the braiding process. The antibacterial test results revealed the best overall performance of the PLA micro/nanofiber core-spun yarn with 0.3 wt% QAS antibacterial agent. The antibacterial rate against Escherichia coli and Staphylococcus aureus was 94.49% and 94.00%, respectively, which could effectively solve the problem of wound infection caused by bacteria. In addition, we used the diamond-braided structure to address the poor strength and fragility of the traditional suture strength. The braiding angle of 30° and 45° could effectively enhance the mechanical properties of the yarn, and the breaking strength was also in line with the industry standard. The study proposed that the degradable QAS/PLA micro/nanofiber core-spun yarn, due to its excellent antibacterial and mechanical properties, could find application in medical protection. This provided a new avenue for research into new antibacterial surgical sutures.

Self-reinforced silk nanofibrils networks enable ultrafine fibroin monofilament sutures applied in minimally invasive surgery

Xin Zhang,Xin Yang,Zhenbei Yang,Kejing Zhan,Yuxi Liu,Jiacheng Chen,Dan Wu,Yuqiu Zhang,Shidong Ma,Zhijuan Pan

Superfine recycled regenerated silk fibroin (RRSF) monofilament sutures are designed to minimize surgical incisions, prevent scar formation, and facilitate precise suturing in cosmetic and plastic surgeries. However, balancing fineness and strength remains a challenge because the RRSF extraction process disrupts the hierarchical structure of natural silk fibers, leading to suboptimal mechanical properties. This study employed mechanical peeling and grinding to exfoliate β-sheets-rich silk fibroin nanofibrils (SFNF), which served as homologous reinforcing materials uniformly dispersed in the RRSF spinning system. Water-soluble, biocompatible polyvinyl alcohol (PVA) was introduced to enhance ductility. The fiber underwent extensive water immersion stretching to produce ultrafine fibers and foster a highly oriented internal self-reinforcing SFNF network structure. Monofilament surgical sutures were successfully fabricated following surface coating with chitosan. Our results showed that with 0.1 wt% SFNF and a stretching multiplier of 2.5, the tensile strength of the fibers increased by 33 % compared to those without SFNF. Fluorescence staining confirmed the presence of highly oriented SFNF networks within the fibers. Mechanical simulations validated the pronounced reinforcement effect of this network structure. The sutures, measuring 39.38 μm in diameter and exhibiting a tensile strength of 0.31 N, met USP standards for 9-0 surgical sutures, making them suitable for microsurgery. Additionally, they demonstrated antibacterial properties, biocompatibility, and degraded at a rate of 43.09 % within 30 days. In animal trials, the sutures facilitated wound closure, reduced inflammatory responses, and minimized scar formation.

Sustainable Antibacterial Surgical Suture Based on Recycled Silk Resource by an Internal Combination of Inorganic Nanomaterials

Xin Zhang,Zhenbei Yang,Xin Yang,Feng Zhang,Zhijuan Pan

The current antibacterial treatment methods of silk sutures can only be finished by surface modification, leading to problems of short antibacterial effects, easy slow-release consumption, prominent toxicity, and susceptibility to drug resistance. Speculatively, surgical sutures combining antibacterial material internally will possess a more promising efficacy. Hence, we extracted recycled regenerated silk fibroin (RRSF) from waste silk resources to make RRSF solutions. Internally combining with inorganic titanium dioxide (TiO2) nanoparticles, we fabricated antibacterial RRSF-based surgical sutures. The morphologies, mechanical and antibacterial properties, biocompatibility tests, and in vivo experiments were carried out. The results showed that the surgical sutures with 1.25 wt % TiO2 acquired 2.40 N knot strength (143 μm diameter) and achieved a sustainable antibacterial effect of 93.58%. Surprisingly, the sutures significantly reduced inflammatory reactions and promoted wound healing. Surgical sutures in this paper realize high-value recovery of waste silk fibers and provide a novel approach to preparing multifunctional sutures.

Biomimicking Antibacterial Opto-Electro Sensing Sutures Made of Regenerated Silk Proteins

Mengwei Liu,Yujia Zhang,Keyin Liu,Gai Zhang,Ying Mao,Liang Chen,Yinbo Peng,Tiger H. Tao

Surgical sutures play an important role across a wide range of medical treatments and a wide variety exist, differing in strength, size, composition, and performance. Recently, increasing interest has been paid to bioactive and electronic sutures made of synthetic polymers, owing to their ability to reduce inflammation as well as medically and/or electronically facilitate wound healing. However, integrating sensing capabilities into bioactive sutures without adversely affecting their mechanical strength, biocompatibility, and/or bioactivity remains challenging. In this work, a set of biomimicking, antibacterial, and sensing sutures based on the regenerated silk fibroin is designed and fabricated. These sensing sutures, inspired by the “core–shell” multilayered structure of natural spider-silk fibers, are hierarchically structured and heterogeneously functionalized to allow for the integration of multiple, clinically favorable functions into one suture device. These functions included: reducing inflammation and bacterial infection in wound sites, measuring tension of both the tissue and suture, and aiding tissue healing via multi-modal controlled drug and growth factor release. Critically, these functions are coupled with real-time optical and electronic monitoring capabilities. This approach provides greater insight into multifunctional sutures with inherent sensing capabilities and offers enormous potential in both therapeutic and diagnostic applications.

Yongxin Tan,Muhammad Shahid Riaz Rajoka,Zekai Ke,Hafiza Mahreen Mehwish,Wenjing Deng,Jiaying Li,Wenqian Qin,Liqing Zhao,Yiguang Wu

Suture is an important part of surgery, and wounds closing after surgery remains a challenge for postoperative care. Currently, silk, linen fiber, and cotton are available in the market as non-absorbable suture biomaterials. So, there is an urgent need to develop a novel suture with advantageous characteristics compared to the ones available on the market. In present study, a series of ultra-high molecular weight chitosan with different DD and MVwere prepared from squid cartilage by alkaline treatment and ultrasonic degradation. The corresponding chitosan monofilaments were prepared by a wet spinning process and were characterized as sutures. The effects of the DD and MVof chitosan on the properties of its monofilament were studied, including surface morphology, mechanical property, swelling ratio, ash content, in vitro enzymatic degradation, and in vitro cytotoxicity. According to the results, AS-85 was chosen to be the best suitable as an absorbable surgical suture, which was spun from squid cartilage chitosan with DD~85% and MV~1.2 × 106. The outcome of the present study might derive tremendous possibilities for the utilization of squid cartilage β-chitin for biomedical applications. Keywords:squid cartilage chitosan;absorbable surgical suture;wet spinning;degree of deacetylation;viscosity average molecular weight
