MoO3-X nanodots coated suture for combating surgical site infection via antibacterial and anti-inflammatory properties

Jingyu Zhang,Xuexiao Li,Ming Cheng,Kaichen Wan,Shangcheng Yan,Wei Peng,Guangxin Duan,Yongyou Wu,Ling Wen

Surgical site infection (SSI) significantly affects patient recovery time, health outcomes and quality of life which is closely associated with the use of implants or mesh. Sutures are the most frequently used implants that play a significant role in the development of SSI. Studies have demonstrated that the administration of effective bactericidal and anti-inflammatory treatments can significantly decrease the incidence of SSI. To address this concern, a versatile suture was engineered by coating MoO 3-X nanodots in this study. The incorporation of MoO 3-X nanodots endowed the suture with desirable antibacterial and anti-inflammatory properties that were evaluated in in vitro and in vivo experiments. The results showed its remarkable ability to facilitate wound healing and prevent SSI through its dual action of combating bacterial infection and reducing inflammation. These findings highlight the promising potential of this multifunctional surgical suture as a versatile tool to promote better outcomes in surgical procedures.

Aligned nanofibrous collagen membranes from fish swim bladder as a tough and acid-resistant suture for pH-regulated stomach perforation and tendon rupture

Luan Zhaohui,Liu Shuang,Wang Wei,Xu Kaige,Ye Shaosong,Dan Ruijue,Zhang Hong,Shu Zhenzhen,Wang Tongchuan,Fan Chaoqiang,Xing Malcolm,Yang Shiming

Background Wound closure in the complex body environment places higher requirements on suture’s mechanical and biological performance. In the scenario of frequent mechanical gastric motility and extremely low pH, single functional sutures have limitations in dealing with stomach bleeding trauma where the normal healing will get deteriorated in acid. It necessitates to advance suture, which can regulate wounds, resist acid and intelligently sense stomach pH. Methods Based on fish swim bladder, a double-stranded drug-loaded suture was fabricated. Its cytotoxicity, histocompatibility, mechanical properties, acid resistance and multiple functions were verified. Also, suture’s performance suturing gastric wounds and Achilles tendon was verified in an in vivo model. Results By investigating the swim bladder’s multi-scale structure, the aligned tough collagen fibrous membrane can resist high hydrostatic pressure. We report that the multi-functional sutures on the twisted and aligned collagen fibers have acid resistance and low tissue reaction. Working with an implantable “capsule robot”, the smart suture can inhibit gastric acid secretion, curb the prolonged stomach bleeding and monitor real-time pH changes in rabbits and pigs. The suture can promote stomach healing and is strong enough to stitch the fractured Achilles tendon. Conclusions As a drug-loaded absorbable suture, the suture shows excellent performance and good application prospect in clinical work.

Conductive Antibacterial Silk Sutures for Combating Surgical Site Infections via Electrically Controlled Drug Release

Rui Yang,Liqin Tang,Rui Wang,Yongliang Wang,Hui Han,Jifu Mao

Current antibacterial sutures for preventing surgical site infections (SSIs) face challenges including suboptimal drug utilization efficiency and uncontrolled burst release. To address these limitations, an antibacterial conductive suture with an electrically controlled drug release system is developed in this study. Polypyrrole (PPy) doped with tannic acid (TA) is in situ polymerized on the surface of silk sutures precoated with chitosan/gelatin (CS/GE), designated as PCG-SS. The PCG-SS exhibits excellent conductivity, enabling voltage-dependent regulation of TA release. At −0.6 V applied potential, PPy underwent electrochemical reduction with decreased positive charge density, enabling maximal TA release; conversely, at +0.4 V, PPy attained an oxidized state with enhanced positive charges, strengthening electrostatic adsorption of anionic TA and achieving 80% suppression of drug elution. Under −0.6 V stimulation, the antibacterial rates of PCG-SS against S. aureus and E. coli exceeded 90%. This work successfully validated that a PPy-based drug-controlled release system can effectively formulate drug release programs, providing new insights into the study of electronically controlled drug delivery systems.

COCu: A Robust Self-Regenerative Hydrogel with Applicability as Both Hydrated Gel Dressing and Dry Suture for Seamless Tissue Fixation and Repair

Tian Hu,Chuncheung Chan,Min-Zhao Lin,Huaitian Bu,Bin Liu,Gang-Biao Jiang

Self-regenerative hydrogels have recently been developed, and represent a special type of self-healing hydrogels with the ability to restore a dehydrated hydrogel with physical damage. In this study, a self-regenerative hydrogel (COCu) based on two chitosan polymers assembled by slow-released Cu 2+ is developed. The COCu hydrogel displays an excellent regeneration ability after being dehydrated and fractured. By simple hydration at room temperature, the fragments of the dehydrated gel fuse into one seamless whole, thereby preserving the mechanical properties and functionalities of the original hydrogel. The regeneration process can be conducted repeatedly after different methods of dehydration (natural volatilization, heat drying, lyophilization) and various modes of deconstruction (flakes, powder, lumpy sponge, etc.). Furthermore, the COCu hydrogel provides ultra-stretchability, and it can be stretched into thin (0.01–0.1 mm) filaments, which, when dried (dtCOCu), can be used as suture lines. Moreover, when used as a dry suture, it regenerates into the hydrogel in the presence of the tissue fluid, forming an excellent sealant to immobilize tissues and seamlessly seal wounds. The fast self-regeneration allows for its facile application as both a hydrated gel dressing and dry suture, and offers customized strategies for fixing and repair of different wounds in soft tissues.

Oral environment targeting sutures for refractory oral wound

Shuze Wang,Jin Wang,Yue Wang,Xintong Ye,Liyun Zhang,Guangqi Yan,Xue Wang

The use of sutures for stable wound closure in the humid, microbiologically complex oral environment is critical for mucosal healing. However, traditional absorbable sutures often loosen due to oral movements, leading to micro-gaps and accelerated microbial colonization, which disrupts healing. Inspired by spider silk's "expansion-induced supercontraction" in humidity, this study develops tea polyphenols self-contracting sutures to enhance mechanical reinforcement and repair efficiency. This next-generation degradable suture features a spider silk-inspired elastic network: polyvinyl alcohol (PVA) chains provide entropy-driven elastic recovery, while an ion-crosslinked network of sodium alginate and Ca²⁺ serves as a moisture-responsive unit. When dry, the network maintains pre-tension. Upon hydration, water molecules competitively coordinate with alginate and Ca²⁺, reducing crosslink density. This allows the PVA network to actively contract within four hours post-surgery, approximating wound edges, minimizing gaps, and reducing bacterial invasion. This next-generation degradable suture exhibits robust antibacterial activity, with 90.3 % inhibition against E. coli and 95.6 % against S. aureus. It also exerts significant biological regulation: compared to controls, it upregulates anti-inflammatory IL-10 expression by 2.41-fold, enhances oxidative stress scavenging capacity by 1.87-fold, and promotes angiogenesis. In vivo tests on difficult oral wounds show this next-generation degradable suture accelerates healing, reduces scarring, and improves tissue regeneration quality, especially in infected wounds. It holds broad potential for minimally invasive oral and maxillofacial surgeries and infection-prone wound repair.

Preparation and Property Analysis of PPDO Composite Suture With the Release Effect of Nanosilver

Liya Fan,Wenxiang Wang,Xiaoyan Zhao

Poly (p-dioxane) (PPDO) as an absorbable suture exhibits good flexibility and tensile strength. Loading drugs through the immersion method will accelerate the hydrolysis of PPDO and affect its mechanical properties. In order to improve the drug loading performance of the PPDO suture and maintain its excellent mechanical stability, biodegradable material polylactic acid (PLA) was selected as the coating layer. The PLA fiber membrane with a porous structure was prepared by electrospinning, and nanosilver particles (Ag NPs) were loaded onto the membrane by in situ reaction. Subsequently, PLA@Ag and PPDO fibers were woven together to form PLA@Ag/PPDO composite yarn. The sustained-release ability, mechanical property, antibacterial, and cytotoxicity of the composite wire were further investigated. The results showed that the equilibrium release of Ag NPs from PLA@Ag/PPDO in phosphate buffer saline (PBS) was 0.84 mg/g, and the release equilibrium time was 8 h. The knotting strength of the composite wire increased by 17.8% compared to PPDO without a coating layer. Woven sutures presented significant antibacterial effects against S. aureus . The biocompatibility evaluation of human immortalized keratinocytes (HaCaT) demonstrated its safety in use. This work provides an effective technical method for the preparation of mechanically stable and functionalized absorbable sutures.

A bioabsorbable mechanoelectric fiber as electrical stimulation suture

Sun Zhouquan,Jin Yuefan,Luo Jiabei,Li Linpeng,Ding Yue,Luo Yu,Qi Yan,Li Yaogang,Zhang Qinghong,Li Kerui,Shi Haibo,Yin Shankai,Wang Hongzhi,Wang Hui,Hou Chengyi

In surgical medicine, suturing is the standard treatment for large incisions, yet traditional sutures are limited in functionality. Electrical stimulation is a non-pharmacological therapy that promotes wound healing. In this context, we designed a passive and biodegradable mechanoelectric suture. The suture consists of multi-layer coaxial structure composed of (poly(lactic-co-glycolic acid), polycaprolactone) and magnesium to allow safe degradation. In addition to the excellent mechanical properties, the mechanoelectrical nature of the suture grants the generation of electric fields in response to movement and stretching. This is shown to speed up wound healing by 50% and reduce the risk of infection. This work presents an evolution of the conventional wound closure procedures, using a safe and degradable device ready to be translated into clinical practice.

Chitosan/gelatin-tannic acid decorated porous tape suture with multifunctionality for tendon healing

Qian Zhang,Yansha Qiao,Chaojing Li,Jing Lin,Hui Han,Xiaoli Li,Jifu Mao,Fujun Wang,Lu Wang

The inferior tendon healing after surgery is inextricably linked to the surgical suture. Poor load transfer along the suture often results in a high tendon re-tear rate. Besides, the severe inflammation and infection induced by sutures even cause a second surgery. Herein, to alleviate the above-mentioned issues, a multifunctional suture was fabricated by decorating chitosan/gelatin-tannic acid (CS/GE-TA) on the porous tape suture. The porous tape suture ensured the required mechanical properties and sufficient space for tissue integration. Compared to the pristine suture, the CS/GE-TA decorated suture (TA100) presented a 332% increase in pull-out force from the tendon, indicating potentially decreased re-tear rates. Meanwhile, TA100 showed superior anti-inflammatory and antibacterial performances. In vivo experiments further proved that TA100 could not only reduce inflammatory action but also facilitate collagen deposition and blood vessel formation . These results indicate that the multifunctional sutures are promising candidates for accelerating tendon healing.

Biomimetic macroporous hydrogel with a triple-network structure for full-thickness skin regeneration

Xiaojun Long,Xiao Xu,Deshun Sun,Yi Hong,Caining Wen,Yixin Xie,Bing Yan,Huawei Zhang,Qi Ge,Wencui Li,Li Duan,Hongwei Ouyang,Daping Wang

Full-thickness skin repair is still a challenge in clinical practice because it is extremely difficult for dermal reconstruction and calls for a promising strategy to achieve quick sutureless closure and high-quality dermal restoration for large acute skin defects. Inspired by the unique composition, mechanical properties and microscopic architecture of skin tissue, a novel biomimetic hydrogel with a triple-network structure that mimics the extracellular matrix (ECM) composition has been reported. A composite biomimetic hydrogel was prepared using ECM-derived biopolymers, gelatin methacryloyl (GelMA) and N-(2-aminoethyl)-4-(4-(hydroxymethyl)-2‑methoxy-5-nitro-sophenoxy) butanamide (NB)-linked sodium alginate (Alg-NB) and the photoinitiator lithium phenyl-2,4,6-trimethylbenzoylphosphinate (LAP). This biomimetic hydrogel can undergo rapid gelling under UV irradiation (approximately 3 s), has strong mechanical properties (mechanical strength ≈530 kPa) with UV-ionic-crosslinking, and shows strong adhesion to wet tissue. In vitro two-dimensional culture of human skin fibroblasts (HSFs) confirmed that the biomimetic hydrogel has excellent biocompatibility, and HSFs on the surface grow into cell clusters because of the macroporous structures of the hydrogel (pore diameter ≈160 μm). The subcutaneous implantation of the biomimetic hydrogel in Sprague-Dawley (SD) rats confirmed its biocompatibility and low biodegradation in vivo. Moreover, in an SD rat full-thickness skin defect model, this engineered biomimetic hydrogel not only could be used for sutureless wound closure strategies but also could accelerate the reconstitution of dermal tissue with skin appendages. This study provides an effective strategy of quick sutureless wound closure and highly efficient repair for full-thickness skin defects and shows enormous potential for tissue regeneration in clinical applications.

Green and safe preparation of antibacterial sutures composed of PLA ultrafine fibers

Yuhang Wang,Hongyan Fu,Jing Tan,Mahmoud M. Bubakir,Xiaodong Xu,Haoyi Li,Weimin Yang

The electrospun yarn for sutures has gained worldwide attention due to its fine fibers that resemble the extracellular matrix and its abundant functional sites. However, the use of a large number of toxic solvents poses safety risks and environmental pollution during production, making it challenging to directly apply electrospun yarn in the biomedical field. In this study, an environmentally friendly and safe method without toxic solvents was proposed for preparing antibacterial PLA ultrafine fiber sutures. This method involves melt electrospinning, hot-stretching, low-temperature plasma treatment, and chitosan grafting. The PLA ultrafine fiber sutures exhibit a high tensile strength which is 2.04 N before knotting and 1.57 N after knotting. The suture diameter is 118.7 μm and average fiber diameter is 1.72 μm. Chitosan grafted on the fiber surface provides excellent antibacterial properties for the sutures, with antibacterial rates against Escherichia coli and Staphylococcus aureus reaching 99.89% and 99.11%, respectively. The cell survival rate is 97.33%, while hemolysis rate is only 2.35%, demonstrating excellent biocompatibility of PLA ultrafine fiber sutures. This study presents a green and safe method for preparing clinically applicable absorbable antibacterial surgical sutures with high mechanical properties. Highlights A green and safe method for preparing PLA-CS ultrafine fiber sutures. PLA-CS suture with a tensile strength of 2.04 N and a fiber diameter of 1.72 μm. PLA-CS suture with good antibacterial property and biocompatibility.

Tunable Bio-Derived Skin-Like Shape Memory Fibers for Smart Suturing

Huan Liu,Meng Zhang,Hao Xiao,Kaojin Wang,Wen Huang,Gang Wu,Junzhong Yang,Yong-Guang Jia,Xiaoxia Zhu

Materials made from bile acids can benefit from the excellent biocompatibility and rigid skeleton of these natural compounds. To address concerns of the biocompatibility of shape-memory polymers (SMPs) in biomedical applications, a series of polyurethanes (PUs) is developed using bile acid as the hard segment and oligo(ethylene glycol) as the soft segment through a one-pot polymerization. These polymers can be easily processed into skin-like fibers, tubes, strips, etc., with tunable mechanical properties, e.g., elongation at break (483%) and high tensile strength (40.9 MPa). Their glass transition temperatures spread across a wide range, enabling them to exhibit dual, triple, and quadruple shape memory effects (SMEs). Due to intermolecular interactions, these PUs display a maximum self-healing efficiency of 96% in tensile strength for 1.5 h. In vitro cytotoxicity tests, hemolysis evaluations and healing efficiency in vivo all indicate their good biocompatibility and biosafety. These bio-derived PUs offer an option of shape memory polymers easy to make, with good tissue compatibility for applications as surgical sutures, artificial muscles, and soft materials.

Bioinspired, Robust, and Absorbable Cellulose Nanofibrils/Chitosan Filament with Remarkable Cytocompatibility and Wound Healing Properties

Meiyan Wu,Pai Zhang,Mei Li,Rui Xu,Xin Zheng,Qiu Cui,Ruitao Cha,Bin Li

Surgical threads are of great importance to prevent wound infection and accelerate tissue healing in surgical treatment. Cellulose nanofibrils (CNF) and chitosan (CS) are attracting increasing attention to be employed as biomedicine materials due to their nontoxicity, cytocompatibility, and biodegradability. However, a robust and absorbable cellulose-based surgical thread has not been explored. Therefore, in this work, a bioinspired CNF/CS composite thread containing 5% cationic polyacrylamide (CPAM) by the mass of CS was prepared, and the obtained CNF/CS-5C thread exhibited excellent mechanical properties and low swelling ratio in water due to the high cross-link degree. Especially, the tensile strength (1877 ± 107 MPa) of this thread was much higher than that of most reported CNF-based threads. Meanwhile, compared with commercial silk and Vicryl surgical threads, the CNF/CS-5C thread exhibited better in vitro cytocompatibility toward endothelial and fibroblast cells and lower inflammatory response in vivo to subcutaneous tissues of rats. In addition, the obtained thread could be regarded as a promising absorbable suture, which exhibited excellent wound healing performances in vivo. Therefore, the prepared absorbable thread will open a new window to prepare novel and advanced cellulose-based threads for medical applications.
