Achieving instant adhesion on wet tissues and preventing the concurrently occurred postoperative tissue adhesion remain tough challenges but urgently demanded. And the deteriorated cohesion and adhesion caused by undesired swelling of hydrogel could not be ignored. Herein, we developed a Janus hydrogel with instant wet adhesion and anti-swelling behavior, based on poly (acrylic acid) (PAA), gelatin (GT) and hyperbranched polymers modified with catechol (HBPC). The asymmetric structure is achieved through complexed one-side of the hydrogel with cationic polymers. The strong physical interaction between PAA and GT formed during immersing in water enhanced the mechanical strength of the gels. And the hydrogel exhibits good anti-swelling behavior, which circumvents the limitations of conventional hydrogel caused by swelling. Water drainage induced by hydrophobic component and the following removal of water residuals achieved by hydrophilic PAA synergistically enhanced the instant wet adhesion of the hydrogel. Besides, pH-responsive protonation of carboxyl groups in PAA equipped the adhesive with extreme acid-tolerance. The in vivo evaluation results demonstrated that the asymmetric adhesive hydrogel could promote the repair of gastric perforation while reducing the risk of postoperative tissue adhesion. The hydrogel with asymmetric wet adhesion might provide valuable insights into the establishment of bioadhesives for gastric sealing and repair.

In recent years, remarkable progress has been made in the research of injectable hydrogel for internal tissue healing. However, the therapeutic outcome is usually limited when the hydrogel is used for the treatment of gastric perforation due to the high acidic gastric juice and violent deformation of the gastric wall. Regarding these challenges, we proposed an ionic nano-reservoir (INR)-based dual-network hydrogel, which has excellent adhesion and mechanical properties, and can be easily applied to the perforation site to block the perforation while promoting tissue repairing. The results showed that the first network made of polyacrylamide had cross-linked on the stomach tissue within 5 s under blue light, and enhanced the adhesion performance through mechanical interlock. The nano-hydroxyapatite acted as ionic INR, which can gradually release Ca2+ under acid environments to form the second network with sodium alginate and inhibit the swelling of hydrogel in gastric juice. Meanwhile, the adhesion was further enhanced through amide covalent bonds at the hydrogel-tissue interface with the presence of 1- ethyl-3-(3-dimethylaminopropyl)carbodiimide/N-hydroxysuccinimide (EDC/NHS). The dual network hydrogels obtained by the INR strategy could be employed as a potential therapeutic option for gastric perforation and other similar biomedical prolems.

Adhesive hydrogels have become an ideal alternative to sutures and staples for sealing tissue defects and anchoring devices. However, most existing adhesive hydrogels are designed for two-sided adhesion and suffer from irreversible swelling in the in vivo environment. In this work, we report a nanoscale phase separation induced self-reinforced PABA-EGCG-ALC Janus hydrogel as a sutureless therapeutic solution for gastrointestinal (GI) defect sealing and gastric perforation repair. The Janus hydrogel is prepared by a one-step method and exhibits excellent mechanical compliance, one-sided adhesion behavior, and ROS scavenging ability. Notably, the nanoscale phase separation structures formed by polymer chain entanglements effectively inhibit the swelling behavior of the hydrogel (60.2 % at equilibrium) and induce self-reinforced mechanical strength and interfacial robustness under extreme acidity. After soaking the hydrogel in simulated gastric fluid (SGF, pH 2.0) for 72 h, its tensile stress, elongation at break, toughness, Young's modulus, and burst pressure are increased by 2.9, 1.7, 6.4, 2.4, and 2.1 folds, respectively. Subsequent in vivo animal experiments demonstrate that the PABA-EGCG-ALC Janus hydrogel provides a convenient, fluid-tight, gastric fluid-resistant, robust and suture-free sealing for GI defects and effectively preventing postoperative adhesions while addressing the key limitations of commercially available tissue adhesives and Janus hydrogels.

Hydrogel adhesives are hot spots due to their ubiquity and practical relevance. However, achieving a robust wet adhesion is still a challenge due to the preferential formation of hydrogen bonds between interfacial fluids and bulk hydrogel, as well as targeted substrates. Herein, a half-dry adhesive consisting of a silk fibroin (SF) semi-interpenetrating network and poly(acrylic acid) covalent network, which can allow a rapid liquid adsorption and repulsion process encountering a wet tissue, is reported. The remaining water enables excellent hydrogel flexibility to a dynamic surface, while the β -sheet fold endows its tough bulk strength under the peeling-off process. Notably, the wet adhesion energy versus porcine skin is 1440 J m −2 due to the combination of hydrogen bonds, electrostatic interactions, and chain entanglement derived from SF. In particular, both in vitro and in vivo outcomes indicate excellent hemostatic effects and result in incision closure of skin, artery, gastric perforation, and lung. After the first-stage closure, polyacrylic-silk fibroin adhesive (PSA) sealants can detach from the lung surface, fitting well to the healing period. By virtue of the reliable adhesion and good noncytotoxicity, PSA may be a prospective candidate for tissue sealant and drug carrier applications.

Background Wound closure in the complex body environment places higher requirements on suture’s mechanical and biological performance. In the scenario of frequent mechanical gastric motility and extremely low pH, single functional sutures have limitations in dealing with stomach bleeding trauma where the normal healing will get deteriorated in acid. It necessitates to advance suture, which can regulate wounds, resist acid and intelligently sense stomach pH. Methods Based on fish swim bladder, a double-stranded drug-loaded suture was fabricated. Its cytotoxicity, histocompatibility, mechanical properties, acid resistance and multiple functions were verified. Also, suture’s performance suturing gastric wounds and Achilles tendon was verified in an in vivo model. Results By investigating the swim bladder’s multi-scale structure, the aligned tough collagen fibrous membrane can resist high hydrostatic pressure. We report that the multi-functional sutures on the twisted and aligned collagen fibers have acid resistance and low tissue reaction. Working with an implantable “capsule robot”, the smart suture can inhibit gastric acid secretion, curb the prolonged stomach bleeding and monitor real-time pH changes in rabbits and pigs. The suture can promote stomach healing and is strong enough to stitch the fractured Achilles tendon. Conclusions As a drug-loaded absorbable suture, the suture shows excellent performance and good application prospect in clinical work.

Bioadhesives or sealants have received considerable attention for suture-less wound sealing. However, most adhesive biomaterials involve complex molecular designs which lack dynamic mechanical properties , inherent hemostasis , as well as the capability of preventing postsurgical tissue adhesion. Herein, a nano-enabled supramolecular hydrogel sealant based on complementary DNA duplexes combined with multiple physicochemical crosslinks is designed and demonstrated. This supramolecular sealant exhibits excellent dynamic reversibility , low swelling, and wet adhesive properties . It can achieve rapid sealing of damaged tissues, blood coagulation , as well as hemostasis. Furthermore, DNA molecules endow the sealant with promising capabilities of preventing protein absorption, cell adhesion and postsurgical tissue adhesion. The in vivo evaluation based on the gastric perforation repair model shows that the sealant can improve granulation tissue growth, collagen deposition, and vascularization and facilitate gastric perforation closure. The nano-enabled DNA supramolecular hydrogel sealant represents a promising alternative for gastric perforation repair and has large clinical potential pertaining to suture-less repair of soft tissues.
