Helicobacter pylori ( H. pylori ) infection has ≈75% probability of causing gastric cancer, so it is considered to be the strongest single risk factor for gastric malignancies. However, the harsh gastric acid environment has created obstacles to medical treatment. This work reports a nanomotor with a bottle-shaped container that can be loaded with small molecules of clarithromycin, nano calcium peroxide (CaO 2 ), and Pt nanoparticles (Pt NPs) by ultrasound. Nanomotors can quickly consume gastric acid through the chemical reaction of CaO 2 to temporarily neutralize gastric acid. The product hydrogen peroxide (H 2 O 2 ) is catalytically decomposed into a large amount of oxygen (O 2 ) by Pt NPs. The local concentration gradient of O 2 bubbles causes it to be expelled from the nanobottles through a narrow opening, and then push the nanobottles forward to provide maximum release and prodrug efficacy. Experiments in animal models show that 15 mg nanomotors can safely and quickly neutralize gastric acid in the stomach and simultaneously release prodrugs to achieve good therapeutic effects without causing acute toxicity. H. pylori burden in mice was 2.6 orders of magnitude lower than that in the control group. The stomach returns to normal pH within 1 d after administration.

Stress response pathways are critical for probiotic survival, promoting survival through adaptive changes in genes and proteins. However, the underlying stress response mechanisms of probiotics, especially Weissella confusa , are poorly understood. We have employed whole genome sequencing to assemble and annotate the W. confusa ZJU.2 genome, probing in parallel the proteome and transcriptome changes under the treatment of gastric acid and bile salts through a multi-omics approach. Stringent filtering reveals that significant upregulation of alanine, aspartate and glutamate metabolism and glycolysis/gluconeogenesis pathway are critical for gastric acid and bile salt tolerance in W. confusa ZJU.2. Concomitantly, the conjoint transcriptome and proteome analysis showed that the polysaccharide biosynthesis protein WP_039968605.1 plays an important role in gastric acid and bile salt tolerance. Parallel reaction monitoring (PRM) and Real-time quantitative polymerase chain reaction (RT-qPCR) analysis were used to demonstrate the reliability of the results. Moreover, we found that W. confusa ZJU.2 may have hypoglycemic effects through an in vivo zebrafish chip model and in vitro α-glucosidase activity assay. Collectively, this study provides a valuable multi-omics resource for the study of stress response mechanisms of Weissella and lays a solid foundation for its broader functional activity applications.

In this work, an edible cellulose-based antibacterial material was prepared by cross-linking α-cellulose and kanamycin sulfate via glutaraldehyde to form kanamycin sulfate-glutaraldehyde-cellulose. Fourier transform infrared spectroscopy, X-ray photoelectron spectroscopy and X-ray diffraction results indicated that the kanamycin sulfate molecule was cross-linked with the molecular chain of cellulose. The optimal mass ratio of kanamycin sulfate to α-cellulose was 1:100 and the degree of substitution reached 1.11%. The optimal kanamycin sulfate-glutaraldehyde-cellulose material showed an excellent inhabitation against both Gram-positive and Gram-negative bacteria. Meantime, the optimal kanamycin sulfate-glutaraldehyde-cellulose had a marked resistance to gastric acid and had low cell cytotoxicity. To promote the application of the kanamycin sulfate-glutaraldehyde-cellulose material, the porous microspheres were prepared via the sol-gel method. The particle size of the homogeneous porous microspheres is mainly distributed between 1.5 and 2.0 μm. Therefore, the kanamycin sulfate-glutaraldehyde-cellulose described herein is a potential edible, eco-friendly, potent, stable, inexpensive, and antibacterial carrier material for delivering drugs, proteins, or vaccines.

Surgical sealants suitable for wounds with non-flat complex geometries are still a challenge to fulfill clinical requirements. Herein, a novel fit-to-shape sealant enhanced by photo-initiated crosslinking was developed utilizing maleic anhydride modified chitosan (MCS), benzaldehyde-terminated PEG (PEGDF) and polyethylene glycol diacrylate (PEGDA). Initially, the shear-thinning hydrogel prepared through the Schiff-base linkage between MCS and PEGDF could be injected into target sites, remolded to conform to a wound with non-flat complex geometry, and remain on the wound, avoiding adverse liquid leakage. Under illumination with ultra-violet (UV) light, the hydrogel was solidified in situ rapidly to adopt the wound contour and enhanced in adhesive strength to seal defects of the tissue. In addition, the hydrogel exhibits stability in extreme pH environments (pH = 1) and has potential to treat wounds inside the stomach with the existence of gastric acid. Moreover, the hydrogel can be applied as adhesive wound dressings through in situ 3D printing. Taken together, the fit-to-shape sealant enhanced by photo-initiated crosslinking can be considered as promising tissue adhesives for wound closure and other biomedical applications.

Infection with Helicobacter pylori (Hp) is one of the leading causes of stomach cancer. The ability to treat Hp infection is hampered by a lack of stomach gastric acid environment. This work introduces a nanoliposome that can rapidly adjust the gastric acid environment to ensure a drug’s optimal efficacy. We introduce CaCO3@Fe–TP@EggPC nanoliposomes (CTE NLs) that are composed of Fe3+ and tea polyphenols (TPs) forming complexes on the surface of internal CaCO3 and then with lecithin producing a phospholipid bilayer on the polyphenols’ outer surface. Through the action of iron–TP chelate, the phospholipid layer can fuse with the bacterial membrane to eliminate Hp. Furthermore, CaCO3 can promptly consume the excessive gastric acid, ensuring an ideal operating environment for the chelate. TPs, on the other hand, can improve the inflammation and gut microbes in the body. The experimental results show that CTE NLs can quickly consume protons in the stomach and reduce the bacterial burden by 1.2 orders of magnitude while reducing the inflammatory factors in the body. The biosafety evaluation revealed that nanoliposomes have good biocompatibility and provide a new strategy for treating Hp infection.

Fucoxanthin (FX) has various excellent biological properties but suffers from poor bioavailability. In this work, we build a bionic cell wall model using TEMPO (2,2,6,6-tetramethyl-1-piperidinyloxy)-oxidized cellulose. The bionic cell wall enhances the environmental stability of the liposomes and serves as a pH-responsive mechanism. The coating processes protect the structure of liposomes and fucoxanthin under the acidic conditions of the stomach. The bionic cell wall disperses and releases the fucoxanthin in simulated intestinal fluid (SIF). Overall, the protective and release capabilities highlight the potential of cellulose in a bionic cell wall model and provide diversity for the structural design of carriers for delivering functional bioactive components.

Ulcerative arterial bleeding is characterized by sudden onset, rapid disease development, and high mortality, which is a great challenge for clinicians to treat, specially bleeding in areas where endoscopic operation is difficult, or in the case of diffuse bleeding, tumor bleeding, and recurrent bleeding. Herein, we proposed a novel treatment strategy using biomaterials to protect the wound and isolate the erosion of digestive tract contents to prevent arterial bleeding in advance. By introducing chitosan to construct multihydrogen-bonding and an electrostatic interaction system, we developed polyethyleneimine/polyacrylic acid/chitosan (PEI/PAA/CS) multifunctional hydrogel. The new hydrogel is characterized by ultrafast gelation, strong tissue adhesion, gastric acid resistance, burst resistance, biocompatibility, hemostasis, and tissue repair. The addition of CS significantly improved the tissue adhesion, biocompatibility, hemostasis, and tissue repair ability of the hydrogel. The PEI/PAA/CS hydrogel could adhere to the ulcer surface and form a protective layer on the wound to prevent arterial bleeding. Importantly, the PEI/PAA/CS hydrogel also has the ability to stop bleeding and promote wound repair, which has been demonstrated in a variety of hemorrhage models in rats and rabbits. All of these factors indicate that the PEI/PAA/CS hydrogel is a promising biomaterial for reducing the risk of ulcerative arterial bleeding.

1-(5-(1H-indol-5-yl)-1-(pyridin-3-ylsulfonyl)-1H-pyrrol-3-yl)-N-methylmethanamine (KFP-H008),a novel and potent potassium-competitive acid blocker for the treatment of acid secretion related diseases, has not been reported previously. In this study, we demonstrated that KFP-H008 inhibits basal acid secretion, 2-deoxy- D -glucose- (2DG-) stimulated gastric acid secretion in rats. KFP-H008 blocked histamine-stimulated acid secretion in rats and heidenhain pouch dogs and reversed acid output in isolated gastric perfusion under histamine stimulation. In all the animal experiments, KFP-H008 exerted a more effective, potent and longer-lasting inhibitory action in comparison with lansoprazole , a proton pump inhibitor (PPI) commonly used in clinic. KFP-H008 inhibited H + -K + -ATPase activity both at pH 6.5 and pH 7.5, and was unaffected by pH. The inhibitory action was reversible and was achieved in a K + -competitive manner. Furthermore, KFP-H008 did not affect Na + -K + -ATPase activity, thus exhibiting high selectivity, which is different from PPIs. In all, KFP-H008, a novel potassium-competitive acid blocker, may provide new option for the patients with acid-related diseases and provide longer-lasting inhibitory action than drugs commonly used in clinical treatment.

Gastric ulcer (GU) is currently one of the most serious and common gastrointestinal diseases worldwide, severely affects the quality of life of patients. However, treatment strategies have limitations, such as drug safety and lack of repair specificity. In this work, we developed a polyethyleneimine/polyacrylic acid/lactose-modified chitosan (PPCL) hydrogel to repair gastric ulcers. This hydrogel was characterized by robust tissue adhesion, low swelling rate, gastric acid fluid resistance, potent antioxidants and good biocompatibility, which could act as a physical barrier to prevent gastric acid and digestive enzymes from further destroying ulcers. In vivo data from a rat GU model confirmed that this hydrogel effectively promoted gastric ulcer healing, and the therapeutic effect was better than that of commercial gastric mucosal protectants. Further mechanistic research identified that the hydrogel accelerated GU healing by alleviating oxidative stress, promoting reepithelization, angiogenesis and mucosal repair. Our findings revealed that the PPCL hydrogel provides a promising therapeutic approach for healing gastric ulcer.

Oxidized high-amylose starch macrogels (OHASMs) were successfully prepared and, for the first time, used as novel delivery vehicles for nutraceuticals. Oxidized high-amylose starch (OHAS) fabricated by 2,2,6,6-tetramethyl-1-piperidinyloxy (TEMPO) oxidation could crosslink Ca 2+ , Al 3+ , Fe 2+ , Fe 3+ , Cu 2+ , Zn 2+ and Ba 2+ . For OHASM, the oxidation degree (DO) of OHAS had great effects on its morphology and controlled release properties except for the variety and concentration of metal ions. OHASM, formed by Zn 2+ crosslinking OHAS (DO~90%), was suitable for encapsulating various nutraceuticals, such as hydrophobic and hydrophilic bioactive substances as well as probiotics, evidenced by 62.4%, 77.9% and 88.8% encapsulation efficiency for β -carotene, tea polyphenols and Lactobacillus paracasei , respectively. Furthermore, the outstanding pH-responsiveness and resistance of OHASM against gastric acid, bile salt, and digestive enzymes were demonstrated by swelling and simulative digestive experiments in vitro . The development of OHASM in this study suggests an attractive strategy for designing potential vehicles to encapsulate a variety of nutrients in food industries.

The oral delivery of protein-based drugs is of great significance, but faces various obstacles, including the deactivation of proteins by the low pH in the stomach and the high concentration of protease, poor transport through intestinal bio-barriers, etc. Herein, we present an acid-resistant metal–organic framework (MOF), NU-1000, in which insulin (Ins, a model protein) was loaded with high capacity (Ins@NU-1000) through the pseudo second-order kinetic model and Langmuir isotherm model. Ins@NU-1000 protects Ins from deactivation in the stomach acid environment and releases it in the intestine through the transformation of the micro-sized rod particles into spherical nanoparticles. Interestingly, the rod particles exhibit long-term retention in the intestine, and Ins is efficiently transported by the shrunk nanoparticles through intestinal bio-barriers and released into the blood, resulting in significant oral hypoglycemic effects (lasting more than 16 h after a single oral administration). Our findings demonstrate that switching the physical properties of the delivery vehicle, such as the shape and size, can contribute to the success of oral protein administration.

Probiotics offer numerous beneficial functions for human bodies, while the low survival rate under gastric acid and short retention time in the intestine are the major obstacles to their utilization. To address these issues, we designed a novel dual-network hydrogel microsphere that combines gastric acid resistance with enhanced mucoadhesion, aiming for the targeted delivery of probiotics. Thiolated oxidized guar gum (SOGG) was disulfide-linked to form the first network, and sodium alginate (SA) was cross-linked with Ca 2+ to form the second network. Under the protection of the interpenetrating dual network microspheres, a much higher viability of Lactobacillus rhamnosus (LGG) (8.73 log CFU/mL) was achieved in simulated gastric fluid, compared to the zero-survival rate of free LGG. Mucoadhesion tests showed that the adhesion rate of SOGG/SA microspheres to the intestinal mucosa was 1.75 times higher than that of thiol-free microspheres. In vivo studies revealed that LGG-loaded microspheres significantly enhanced intestinal barrier function, remodeled the gut microbiome, and alleviated DSS-induced colitis in mice. Overall, SOGG/SA microspheres provide an effective strategy to the challenges of probiotic reduction in the stomach and rapid expulsion from the intestines, enhancing their health benefits.
