Ankylosing spondylitis (AS) is a progressively worsening and disabling form of arthritis that primarily affects the axial skeleton. This disease mainly involves the spine and the sacroiliac joint. Fusion of the spine and the sacroiliac joint may occur in the later stage of the disease, resulting in spinal stiffness and kyphosis, as well as difficulty in walking, which seriously affects the quality of work and daily living activities and imposes a heavy burden on the patient, the family, and society. Increasing attention has been paid to non-pharmacotherapy as an alternative therapy for AS. Moxibustion is an ancient therapeutic technique used in Traditional Chinese Medicine (TCM). Du-moxibustion therapy, a unique and innovative external treatment developed on the basis of ordinary moxibustion, has a definite therapeutic effect on AS. Du-moxibustion skillfully combines the compatible techniques of TCM to integrate meridians, acupoints, Chinese herbal medicine, and moxibustion. This paper describes the operation procedures and precautions to be taken during Du-moxibustion in experimental mice in detail to provide an experimental basis for the study of the mechanism of Du-moxibustion in the treatment of AS.

Continuous monitoring of spinal and joint motions in patients allows surgeons to optimize patient rehabilitation. However, the current monitoring approaches are achieved via tethered devices, which are inconvenient and associated with a risk of infections. In this study, we developed and validated a new type of reliable wireless, battery-free, and implantable sensing system (RWBS) for the real-time monitoring of physiological bending of spines and joints. The RWBS exploits the unique movement-sensing capabilities of an implantable flexible magnetic strip, the signals from which are detected with an external receiver. We implanted this magnetic strip beneath the skins of Sprague Dawley rats and human cadavers and demonstrated that in combination with the receiver, the magnetic strip enabled real-time in vivo and wireless monitoring of motions in both rats (anesthetized/awake) and human cadavers. The adaptability of RWBS in other human bones has been verified by sensing the motions of elbow joints and knee joints. The simple design of the RWBS allowed it to continue functioning in vivo after the magnetic strip had been broken into two or more pieces, indicating its high reliability. Thus, the RWBS is biocompatible, exhibits excellent functionalities, and can be conveniently implemented in both rats and humans. This device will help to facilitate the post-surgery monitoring of spinal and joint movements of human patients.

Spinal fusion is an efficient treatment for degenerative disc disease, however, poor osseointegration capacity of spinal cages often cause clinical implant failure. Herein, we proposed an innovative piezoelectric interbody fusion cage with porous structure, load-carrying capacity and enhanced osseointegration effect via 3D printing technology. Firstly, poly(vinylidene fluoride) (PVDF), poly(lactic acid) PLA and ceramic barium titanate (BTO) were compounded to prepare the composite filament with improved 3D printability and good piezoelectric effect. It is found that both BTO and PLA are favored for β -phase transformation and PLA effectively solves the warping issue during the 3D-printing. Then, cages with triangular lattice with varying infill densities are designed and fabricated. It is found that triangular PVDF/PLA/BTO structure with lower infill density exhibits stronger piezoelectric effect, and all the samples exhibited compressive properties analogous to trabecular bone. Furthermore, the in vitro cellular assays and in vivo large animal model sheep experiments validated an obvious piezoelectric effect on osseointegration for PVDF/PLA/BTO cages. After three-month implantation, it presented promoted new bone formation with a bone volume of 76.8 % in contrast with that of 49.3 % in PEEK counterpart. Therefore, the current study highlights a promising material for intervertebral fusion cages with good biomimetic mechanical performance and spinal osseointegration effect.

Objective To explore the role of gut microbiota and plasma metabolites in the therapeutic mechanism of tofacitinib in ankylosing spondylitis (AS). Method Ten AS patients and ten matched healthy controls (HC) were enrolled in this study. 16S rRNA sequencing and LC–MS profiling was conducted to investigate the gut microbiota and plasma metabolite before and after tofacitinib therapy. An AS mouse model was established to validate the effect of tofacitinib in vivo via H&E staining, western blot, and ELISA. Results Tofacitinib improved clinical symptoms in AS patients. Microbiota analysis revealed Microbiota analysis revealed reduced α-diversity (ACE, Chao1) and altered community structure in AS patients compared to HC, which partially normalized post-treatment. LEfSe identified 84 taxa biomarkers; Barnesiella , Coprobacter , Lachnospira , and Lactobacillus tended to return to normal after tofacitinib treatment. Plasma metabolomics uncovered 3 key metabolies, including choline metabolism, glycerophospholipid metabolism, and phenylalanine metabolism. Spearman analysis revealed that the gut microbiota were closely related to the changes in differential plasma metabolites. Combinated tofacitinib and trichostatin therapy attenuated inflammation, restored metabolism caused by AS in mice in vivo . Conclusion AS patients suffer from dysbiosis of gut microbiota, and the mechanism of tofacitinib treatment of AS may be related to the modulation of gut microbiota and alteration of plasma metabolites.
Key Points • Tofacitinib improves clinical symptoms in patients with AS . • Tofacitinib regulates gut microbiota in AS patients . • Tofacitinib regulates plasma metabolites in patients with AS . • Tofacitinib regulates the choline metabolism .

Ankylosing spondylitis (AS) is an immune chronic inflammatory disease, resulting in back pain, stiffness, and thoracolumbar kyphotic deformity. Based on the reported anti-inflammatory and antioxidant capacities of cynarin (Cyn), this study explored its protective role and molecular mechanisms in mesenchymal stem cells (MSCs) from AS. The target pathways and genes were verified using Western blotting, quantitative real-time polymerase chain reaction, and immunofluorescent staining, while molecular docking analysis was conducted. In AS-MSCs, we found that the expression levels of p-NF-κB, IL-6, IL-1β, and TNF-α were higher and IκB-α, Nrf-2, and HO-1 were lower compared with healthy control (HC)-MSCs. With molecular docking analysis, the biding affinities between Cyn and Keap1-Nrf-2 and p65-IκB-α were predicted. The mRNA and protein expression of p-NF-κB, IL-6, IL-1β, and TNF-α and the reactive oxygen species (ROS) generation were downregulated following Cyn administration. Meanwhile, the expression level of IκB-α, Nrf-2, and HO-1 were significantly increased after Cyn pretreatment. The results suggested that the protective mechanisms of Cyn in AS-MSCs were based on enhancing the antioxidation and suppression of excessive inflammatory responses via Nrf-2/ROS/NF-κB axis. Our findings demonstrate that Cyn is a potential candidate for alleviating inflammation in AS.

Low responsiveness to anti-programmed death-1/programmed death-ligand 1 (anti-PD-1/PD-L1) for solid tumors indicates the presence of other immunosuppressive pathways. Siglec15, a newly discovered immune checkpoint, has been reported to repress immune responses in the tumor microenvironment (TME) and regulate osteoclast differentiation. However, the role of Siglec15 in the treatment for bone metastasis remains unclear. Herein, Siglec15 shows significantly higher expression in lung adenocarcinoma spinal metastasis (LUAD-SM) than in para-cancerous spinal tissues and primary LUAD. Subsequently, a TME-responsive hollow MnO 2 nanoplatform (H-M) loaded with Siglec15 siRNA and cisplatin (H-M@siS15/Cis) is developed, and the surface is modified with an aspartic acid octapeptide (Asp 8 ), thus allowing H-M to target spinal metastasis. High drug-loading capacity, good biocompatibility, effective tumor accumulation, and efficient Siglec15 silencing are demonstrated. Furthermore, the nanoparticles could reverse immunosuppression caused by tumor cells and tumor-associated macrophages (TAMs) and inhibit osteoclast differentiation via Siglec15 downregulation in vitro. In a LUAD-SM mouse model, H-M@siS15/Cis-Asp 8 exhibits superior therapeutic efficacy via synergetic immunochemotherapy and osteolysis inhibition. Taken together, this single nanoplatform reveals the therapeutic potential of the new immune checkpoint Siglec15 in LUAD-SM and provides a strategy to treat this disease.

Background Intervertebral disc degeneration (IVDD) is the initiating factor of adult degenerative scoliosis (ADS), and ADS further accelerates IVDD, creating a vicious cycle. Nevertheless, the role of the Wnt/β-Catenin pathway in ADS combined with IVDD (ADS-IVDD) remains a mystery. Accordingly, this study was proposed to investigate the effect of axial stress on the Wnt/β-Catenin pathway in nucleus pulposus cells (NPCs) isolated from DS-IVDD patients. Methods Normal NPCs (N-NPC) were purchased and the NPCs of young (25–30 years; Y-NPC) and old (65–70 years; O-NPC) from ADS-IVDD patients were primary cultured. After treatment of NPC with overloaded axial pressure, CCK-8 and Annexin V-FITC kits were applied for detecting proliferation and apoptosis of N-NPC, Y-NPC and O-NPC, and western blotting was performed to assess the expression of Wnt 3a, β-Catenin, NPC markers and apoptosis markers (Bax, Bcl2 and Caspase 3). Results N-NPC, Y-NPC and O-NPC were mainly oval, polygonal and spindle-shaped with pseudopods, and the cell morphology tended to be flattened with age. N-NPC, Y-NPC and O-NPC were capable of synthesizing proteoglycans and expressing the NPC markers (Collagen II and Aggrecan). Notably, the expression of Wnt 3a, β-Catenin, Collagen II and Aggrecan was reduced in N-NPC, Y-NPC and O-NPC in that order. After overload axial stress treatment, cell viability of N-NPC and Y-NPC was significantly reduced, and the percentage of apoptosis and expression of Wnt 3a and β-Catenin were significantly increased. Conclusions Overloaded axial pressure activates the Wnt/β-Catenin pathway to suppress proliferation and facilitate apoptosis of NPC in ADS-IVDD patients.

Background Alterations in immunoglobulin G (IgG) N -glycosylation have been implicated in various diseases, potentially including ankylosing spondylitis (AS). However, empirical evidence regarding the associations IgG N -glycosylation, inflammatory factors, and AS is sparse. We aimed to investigate these associations and evaluate the diagnostic performance of IgG glycosylation traits as a biomarker for AS. Methods This case–control study recruited 142 AS patients and 142 controls. We used hydrophilic interaction liquid chromatography with ultra-performance liquid chromatography to detect the profiles of plasma IgG N -glycans. An enzyme-linked immunosorbent assay kit was used to measure the levels of serum inflammatory factors. We used multivariate logistic regression, canonical correlation analysis (CCA), and Lasso regression to analyze the data. Results Multivariate logistic analysis identified 14 IgG N -glycan-derived traits significantly associated with AS. The changes were primarily characterized by decreased galactosylation, sialylation, bisgalactosylated fucosylation and sialated fucosylation, alongside increased core fucosylation, agalactosylated fucosylation, neutral fucosylation and sialated bisecting N -acetylglucosamine. Significant differences were observed between cases and controls in levels of C-reactive protein (CRP), IL-17, IL-22, IL-6, and TNF-α. CCA demonstrated a moderate correlation between IgG N -glycan-derived traits and inflammatory factors ( r = 0.476). The combined diagnostic performance of derived traits and inflammatory factors yielded a higher area under the curve (AUC = 0.90) than either alone (AUC derived traits = 0.88, AUC inflammatory factors = 0.78). Conclusion The findings indicate that abnormal IgG N -glycosylation may play an important role in the development of AS through promoting the pro-inflammatory function. The derived traits combined with inflammatory factors may serve as potential biomarkers to distinguish AS.
Key Points • We comprehensively analyzed IgG N-glycan-derived traits in AS by an HILIC-UPLC-based high-throughput method. • Abnormal IgG N-glycosylation plays a role in the development of AS through promoting the proinflammatory. • IgG N-glycosylation combined with inflammatory factors demonstrated strong diagnostic performance for AS.

Objective Spinal fusion is one of the most common surgical interventions for spine reconstruction. Despite the efforts to promote osteogenesis after spinal fusion, osteogenesis after spinal fusion remains a clinical challenge and new methods are still needed. The bone morphogenetic protein-2 (BMP-2) is a widely reported factor that can facilitate the osteogenesis in spinal fusion. In previous research, we found that the delivery of chitosan nanospheres could promote the effects of BMP-2 on osteogenic activity. The coralline hydroxyapatite (CHA) is one of the most frequently used implants in bone fusion. However, up to now no study has focused on the osteogenic efficacy of the CHA composite with recombinant human BMP-2 (rhBMP-2)-loaded chitosan nanospheres. This study aimed to investigate the effects of the CHA implant with rhBMP-2-loaded chitosan nanospheres on osteogenesis in spinal fusion. Methods The rhBMP-2-loaded microspheres and CHA composite (rhBMP-2 microspheres/CHA) were prepared and were used for implantation of the rats. All SD rats were divided into four groups: the rhBMP-2 microspheres/CHA composite group (containing 0.5 mg rhBMP-2), the rhBMP-2-loaded CHA (rhBMP-2/CHA) composite group (containing 0.5 mg rhBMP-2), the blank CHA group, and the negative control group. The microsphere morphology was scanned and analyzed using a scanning electron microscope. Micro-computed tomography examination and three-dimensional reconstruction were performed 4 weeks after the surgery. Hematoxylin and eosin staining was conducted for histological analysis. Both alkaline phosphatase (ALP) and calcium content were measured. Results The rhBMP-2-loaded CHA (rhBMP-2/CHA) composite was successfully prepared. Spherical regularity and a smooth and unwrinkled surface of the spheres were observed in all chitosan (CS)/rhBMP-2 microspheres. No side effects, infections, or abnormal behaviors were found in the animals. After 4 weeks of surgery, obvious new bone formation and bone fusion could be observed around the implant in both the rhBMP-2 microspheres/CHA composite group and the rhBMP-2/CHA composite group. No ectopic osteogenesis was found in the vertebral canal or other muscle tissues. After 4 weeks of implantation, in both the rhBMP-2 microspheres/CHA composite group and the rhBMP-2/CHA composite group, osteoid tissues could be found, and bone cells, bone marrow, and trabecular bone turned into mature sclerotin, obvious bone tissue formation could be also seen. Both ALP activity and calcium content in the rhBMP-2 microspheres/CHA composite group (6.52 ± 0.50 kat/g and 17.54 ± 2.49 μg/mg) were significantly higher than in all other groups. Conclusion The composite with rhBMP-2-loaded CS nanospheres could enhance osteogenic efficacy and increase the ALP activity and calcium content. These results might provide a novel method for osteogenesis in spinal fusion and offer new insight into the role of BMP-2 in osteogenesis.

The widespread use of veterinary antibiotics in animal husbandry, such as tilmicosin (TIL), poses potential threats to environment pollution and human health. In this study, we conducted acute TIL exposure experiment using zebrafish model to explore the toxicological effects. In the concentration range of 20–80 mg/L, TIL did not affect zebrafish body length development but induced idiopathic scoliosis (IS)-like symptoms accompanied by impaired mobility. IS predominantly occurs in adolescents, while the etiology remains unclear. Further studies showed that the combination of quercetin and luteolin effectively alleviated TIL-induced spinal curvature and motor suppression. To explore the underlying mechanism, we performed network pharmacology analysis and enriched the PI3K-Akt pathway. We validated that the expression of key target genes in the PI3K-Akt pathway were significantly activated after TIL exposure, while quercetin and luteolin treatment effectively inhibited the PI3K-Akt pathway activation. Further experiments revealed that TIL exposure decreased brain neural cell apoptosis, thereby causing an increase in neurocytes, interfering with motor ability, and promoting IS-like vertebral malformations, which were especially noticeable during movement. This study expands our understanding that TIL exposure may be a predisposing factor for IS occurrence and progression, while the combination of quercetin and luteolin combination could protect against IS-like symptom by regulating PI3K-Akt pathway. Graphical The experiments revealed that exposure to tilmicosin (TIL) induces the development of scoliosis-like deformities, which could be effectively alleviated by the quercetin and luteolin combination. Further mechanistic studies found that TIL exposure activates the PI3K-Akt pathway, resulting in reduced apoptosis of brain neurons and increased neural cells, which could be inhibited by quercetin and luteolin. As disturbed brain development is a predisposing factor for scoliosis, the TIL-induced abnormalities in brain neurodevelopment exacerbate the progression of scoliosis-like deformities and dyskinesia.

Triclosan (TCS) is a prevalent anthropogenic contaminant in aquatic environments and its chronic exposure can lead to a series of neurotoxic effects in zebrafish. Both qRT-PCR and W-ISH identified that TCS exposure resulted in significant up-regulation of miR-137, but downregulation of its regulatory genes ( bcl11aa , MAPK6 and Runx1 ). These target genes are mainly associated with neurodevelopment and the MAPK signaling pathway, and showed especially high expression in the brain. After overexpression or knockdown treatments by manual intervention of miR-137, a series of abnormalities were induced, such as ventricular abnormality, bent spine, yolk cyst, closure of swim sac and venous sinus hemorrhage. The most sensitive larval toxicological endpoint from intervened miR-137 expression was impairment of the central nervous system (CNS), ventricular abnormalities and notochord curvature. Microinjection of microRNA mimics or inhibitors of miR-137 both caused zebrafish malformations. The posterior lateral line neuromasts became obscured and decreased in number in intervened miR-137 groups and TCS-exposure groups. Up-regulation of miR-137 led to more severe neurotoxic effects than its down-regulation. Behavioral observations demonstrated that both TCS exposure and miR-137 over-expression led to inhibited hearing or vision sensitivity. HE staining indicated that hearing and vision abnormalities induced by long-term TCS exposure originated from CNS injury, such as reduced glial cells and loose and hollow fiber structures. The findings of this study enhance our mechanistic understanding of neurotoxicity in aquatic animals in response to TCS exposure. These observations provide theoretical guidance for development of early intervention treatments for nervous system diseases.
