Backgrounds Due to the therapeutic needs of inner ear diseases, a simple method of inner ear drug delivery has been under exploration. Postauricular administration, which involves injecting the drug through the middle of the retroauricular groove, is a potential novel approach, but its mechanisms remain unclear. Objectives To investigate threshold shifts in the auditory brainstem response (ABR), as well as the morphology and quantity of hair cells on the bilateral inner ear of guinea pigs after postauricular neomycin administration.Material and Methods Thirty healthy guinea pigs were divided randomly into two groups and administered either neomycin 200 mg/kg or saline 1 ml via daily postauricular injection for 10 days. Auditory threshold-shifts were examined by tests of the ABR. The quantity and morphology of hair cells in the organ of Corti were assessed by confocal laser scanning microscopy. Results The ABR results showed that at a frequency of 16 kHz, the threshold shift was 21.00 ± 14.49 dB in the neomycin injection side, 10.00 ± 16.67 dB in the contralateral side, and 0.50 ± 3.69 dB in the saline group. At a frequency of 24 kHz, the threshold shift was 37.00 ± 16.36 dB in the neomycin injection side, 14.50 ± 9.26 dB in the contralateral side, and 1.00 ± 2.11 dB in the saline group. Histopathological analysis revealed that the outer hair cells (OHCs) in the neomycin injection side suffered significant morphological and numerical loss, while the OHC loss in the contralateral ear was less severe than in the injection side but greater than in the saline group. Conclusions and Significance This study confirms that postauricular administration is a novel method of local drug delivery to the inner ear, which has practical value in future experimental modeling and the treatment of inner ear diseases.

Background: Inner ear disorders, such as EH, commonly lead to hearing loss and vestibular dysfunction. EH is particularly prevalent in various inner ear diseases, including Meniere’s disease. We aimed to evaluate the effects of EH on WAI and OAEs.Objective: This study explores the potential of wideband acoustic immittance (WAI) and otoacoustic emissions (OAEs) for the early detection of changes in acoustic transmission associated with vasopressin-induced endolymphatic hydrops (EH) in a guinea pig model. Methods: We induced EH in guinea pigs via daily intraperitoneal injections of arginine vasopressin over 14 consecutive days. Auditory function was assessed using Auditory Brainstem Responses (ABR), while changes in sound energy transmission were measured using WAI and Distortion Products Otoacoustic Emissions (DPOAE).Results: Increased ABR thresholds in EH models were statistically significant (p < 0.05). After 14 days of EH induction, absorbance at 1 kHz significantly increased, whereas it significantly decreased at 4 kHz and 6 kHz (p < 0.05). DPOAE measures, both magnitude and phase, showed no significant changes (p > 0.05).Conclusion: WAI demonstrates greater sensitivity than DPOAE in the early detection of acoustic transmission alterations in EH models, suggesting its utility as a diagnostic tool in early-stage inner ear disorders.

The motor system controls muscle movement through lower motor neurons in the spinal cord and brainstem. Lower motor neurons are efferent neurons in the central nervous system (CNS) characterized by axonal projections that reach specific targets in the periphery. Lower motor neuron lesions result in the denervation and dysfunction of peripheral skeletal muscle. Great progress has been made to develop therapeutic strategies to transduce lower motor neurons with genes. However, the widespread distribution of lower motor neurons makes their specific, extensive, and efficient transduction a challenge. In this study, we demonstrated that, compared to the other tested recombinant adeno-associated virus (rAAV) serotypes, rAAV2-retro mediated the most efficient retrograde transduction of lower motor neurons in the spinal cord following intramuscular injection in neonatal mice. A single injection of rAAV2-retro in a single muscle enabled the efficient and extensive transduction of lower motor neurons in the spinal cord and brainstem rather than transducing only the lower motor neurons connected to the injected muscle. rAAV2-retro achieved the extensive transduction of lower motor neurons by the cerebrospinal fluid pathway. Our work suggests that gene delivery via the intramuscular injection of rAAV2-retro represents a promising tool in the development of gene therapy strategies for motor neuron diseases.

Overproduction of reactive oxygen species (ROS) and inflammation are two key pathogeneses of noise-induced hearing loss (NIHL), which leads to outer hair cell (OHC) damage and hearing loss. In this work, we successfully developed ROS-responsive nanoparticles as berberine (BBR) carriers (PL-PPS/BBR) for OHC-targeted therapy of NIHL: Prestin-targeting peptide 2 (PrTP2)-modified nanoparticles (PL-PPS/BBR), which effectively accumulated in OHC areas, and poly(propylene sulfide)120 (PPS120), which scavenged ROS and converted to poly(propylene sulfoxide)120 in a ROS environment to disintegrate and provoke the rapid release of BBR with anti-inflammatory and antioxidant effects. In this study, satisfactory anti-inflammatory and antioxidant effects of PL-PPS/BBR were confirmed. Immunofluorescence and scanning electron microscopy (SEM) images showed that PL-PPS/BBR effectively accumulated in OHCs and protected the morphological integrity of OHCs. The auditory brainstem response (ABR) results demonstrated that PL-PPS/BBR significantly improved hearing in NIHL guinea pigs after noise exposure. This work suggested that PL-PPS/BBR may be a new potential treatment for noise-associated injury with clinical application.

Bioelectrical signal is the basis of the rapidly growing field of physiological activity research, clinical diagnosis, therapeutic applications and neural science. However, current bioelectrical signal monitor methods face challenges such as skin mismatch, impedance defect, insufficient cycle performance, tissue damage, and scar/biofilm interface formation. Here, we report a sterilizable bio-adhesive interface based on ion-conducting organohydrogel with skin-compliance for precise and motion-robust epidermal bioelectrical signals monitoring. The interface material is based on silk fibroin-reinforced organohydrogel, which is formed by UV curing. The interface material represents energy storage efficiency up to 97%, extremely low impedance (less than40 Ω) at physiologically relevant frequencies and the electrocardiogram (ECG) signals measurement maintains accurate even after multiple disinfections (100 times) by 75% ethanol. Hence, this interface material can be used for multiple times in long-term bioelectrical signals monitoring during daily life. We further investigated the capabilities of the interface in an auditory brainstem response test and realized its ability to achieve competitive monitoring sensitivity in clinical diagnosis as non-invasive electrodes.

The occurrence of major asthma symptoms is largely attributed to airway vagal hypertonia, of which the central mechanisms remain unclear. This study tests the hypotheses that endothelin-1-mediated brainstem glial activation produces asthmatic airway vagal hypertonia via enhanced action of adenosine 5’-triphosphate on neuronal purinergic P2X4 receptors. A rat model of asthma was prepared using ovalbumin. Airway vagal tone was evaluated by the recurrent laryngeal discharge and plethysmographic measurement of pulmonary function. The changes in the brainstem were examined using ELISA, Western blot, luciferin-luciferase, quantitative reverse transcription-polymerase chain reaction, enzyme activity assay and immunofluorescent staining, respectively. The results showed that in the medulla of rats, endothelin receptor type B and P2X4 receptors were primarily expressed in astrocytes and neurons, respectively, and both of which, along with endothelin-1 content, were significantly increased after ovalbumin sensitization. Ovalbumin sensitization significantly increased recurrent laryngeal discharge, which was blocked by acute intracisternal injection of P2X4 receptor antagonist 5-BDBD, knockdown of brainstem P2X4 receptors, and chronic intraperitoneal injection of endothelin receptor type B antagonist BQ788, respectively. Ovalbumin sensitization activated microglia and astrocytes and significantly decreased ecto-5’-nucleotidase activity in the medulla, and all of which, together with the increase of medullary P2X4 receptor expression and decrease of pulmonary function, were reversed by chronic BQ788 treatment. These results demonstrated that in rats, allergic airway challenge activates both microglia and astrocytes in the medulla via enhanced endothelin-1/endothelin receptor type B signaling, which subsequently causes airway vagal hypertonia via augmented adenosine 5’-triphosphate/P2X4 receptor signaling in central neurons of airway vagal reflex.
