Background Cryptococcosis is a global invasive mycosis associated with significant morbidity and mortality. Cryptococcal antigen (CrAg) testing from serum and cerebrospinal fluid (CSF) has been regarded as a gold standard for early diagnosis. This study aimed to develop and validate a rapid and sensitive sandwich chemiluminescent magnetic microparticle immunoassay (CMIA) for quantitative detection of CrAg in sera.Research design and methods CMIA is based on magnetic beads modified with capture antibodies and biotinylated antibodies and Streptavidin-polyHRP, where biotinylated antibodies functioned as the recognition element and Streptavidin-polyHRP as the signal component. Assay parameters were first optimized, and then assay performances were evaluated. Results Under optimized conditions, the total runtime of the CMIA was 22 min. The assay had a wide linear range (2 –10,000 ng/mL) and high analytical sensitivity (0.24 ng/mL), together with acceptable reproducibility, accuracy, and stability. Besides, it exhibited no cross-reactivity with other pathogens. Importantly, the assay showed 92.91% (95% CI, 80.97–93.02%) overall qualitative agreement with a commercial ELISA kit in a retrospective cohort of 55 cases with confirmed cryptococcal infection, and 72 controls without evidence of invasive fungal disease (IFD).Conclusion These results demonstrated that the present study paved a novel strategy for reliable quantitative detection of CrAg in sera.

Rapid identification and accurate diagnosis are critical for individuals with acute leukemia (AL). Here, we propose a combined deep learning and surface-enhanced Raman scattering (DL-SERS) classification strategy to achieve rapid and sensitive identification of AL with various subtypes and genetic abnormalities. More than 390 of cerebrospinal fluid (CSF) samples are collected as targets, encompassing healthy control, AL patients, and individuals with other diseases. Sensitive SERS detection could be achieved within 5 min, using only 0.5 μL volume of CSF. Through integrated feature fusion (1D spectra and 2D image) with a transformer model, the classification method is developed to screen and diagnose AL patients, demonstrating exceptional classification performances of accuracy, sensitivity, specificity, or reliability. Also, this approach demonstrates remarkable versatility and could be extended to the classifications of meningitis diseases. The sensitive DL-SERS classification platform has the potential to be a powerful auxiliary in vitro diagnostic tool.

Lumbar puncture is a minimally invasive technique for delivering drugs into cerebrospinal fluid (CSF). However, its effectiveness is limited by the spine’s extensive length and the narrow, complex CSF spaces, which hinder efficient passive diffusion of therapeutic agents. Here, we have engineered a differentiable stem cell-assembled soft robot (SCASR) by using a three-dimensional self-assembly process in liquid marbles. These biologically viable robots can interface with the tissues inside the CSF, adapting to the complex surroundings while navigating. With X-ray imaging and magnetic actuation, the SCASRs can be guided toward targeted regions. The SCASRs autonomously disassemble into individual cells, which then differentiate into neural cells to facilitate the restoration of neural connections, thereby aiding the recovery of paralyzed lower limbs. The proposed strategy provides an imaging-based therapeutic system for nerve injury, enabling accessibility of hard-to-reach spinal regions and facilitating efficient therapy in minimally invasive manner, by means of soft microrobots.

In view of the importance of quantification of beta-amyloid (Aβ), an “aptamer -target- peptide” sandwich electrochemical biosensor to assay Aβ42 was fabricated by using gold–palladium bimetallic nanoparticles covalent organic framework-carbon cloth (Au–Pd@COF TTF-Thi -CC) and prion protein (PrP C ) labeled with horseradish peroxidase (HRP) nanocomposites. The concentration of Aβ42 showed a positive correlation with the peak current resulting from the oxidation of hydroquinone (HQ) by HRP. In contrast, the reference current signal from thionin (Thi), derived from covalent organic framework (COF), showed a negative correlation. The current response of the electrochemical biosensor was proportional to the Aβ concentrations ranging from 0.01 to 2.5 nM with a detection limit (LOD) of 2.17 fM ( S / N = 3). The sensitivity of the sandwich electrochemical biosensor could be primarily amplified based on the substrates of Au–Pd@COF TTF-Thi -CC. The AuNPs and PdNPs not only facilitated the bonding of the linker based on the Au–S bond but also improved the performance of COF TTF-Thi -CC. Secondary sensitivity amplification of the biosensor could be achieved by using PrP C -labeled HRP to catalyze hydroquinone in solution. Furthermore, the biosensor showed excellent selectivity, anti-interference performance, and stability. The Aβ concentrations in cerebrospinal fluid (CSF) were successfully evaluated using the constructed biosensor. This assay provides a potential tool for early-stage Alzheimer's disease diagnosis.
