Background Biological composite scaffolds are increasingly being used in abdominal wall reconstruction but still have certain shortcomings. The present study describes here a novel three-dimensional (3D) scaffold fabricated by combining 3D printing (3DP) and electrospinning (ESP). Methods Biological composite scaffolds are composed of integrated 3DP interconnected macrofiber and random ESP microfiber networks. The 3DP scaffold retains intact 3D architecture and mechanical properties, while the ESP network serves as a cell entrapment system at the extracellular matrix (ECM) scale. Biological composite scaffolds are implanted in a defective rat abdominal wall to detect if it could induce early vascularization and reconstruction of the tissue defect. Results SEM analysis reveals a pore diameter of 424.47 ± 58.49 μm and a porosity of 70.46 ± 2.48% for biological composite scaffolds. In the in vitro test of cell proliferation, biological composite scaffolds do not affect rat dermal fibroblast proliferation in a time- and dose-dependent manner. The animal experiments show tissue remodeling and early angiogenesis as compared to 3DP scaffolds. Conclusions Our experiment prepares a biological scaffold with both a macro- and microscale structure by ESP and 3DP technology. Thus, the integration of 3DP and ESP techniques provides a new set of smart scaffolds for abdominal wall defect and hernia repair.

Emergency repair of complicated full-thickness abdominal wall defects remains one of the most common and challenging surgical emergencies globally. Here, an integrated polydopamine permeating-cross-linking strategy was innovatively proposed to convert porcine acellular dermal matrix (pADM) into versatile, degradation-resistant biopatches (PDA-pADM) for efficiently repairing full-thickness abdominal wall defects. The strategy significantly addresses the challenge that natural-property improvement and biocompatibility of biomaterials are difficult to balance. Molecularly, dopamine (DA) molecules could fully permeate into the collagen fibers of the acellular dermal matrix and then automatically trigger the interfacial in situ polymerization of dopamine monomers among collagen fibers to achieve the efficient cross-linking of pADM. Surprisingly, the enzymatic durability of the biopatch shows significant improvements, extending the original duration from 3 to 20 d. Comprehensive in vivo experiments have shown that PDA-pADM can effectively promote angiogenesis and inhibit inflammatory response, so as to achieve regeneration and repair of abdominal wall damage.

Current mesh materials used for the clinical treatment of abdominal defects struggle to balance mechanical properties and bioactivity to support tissue remodeling. Therefore, a bioactive microgel coated electrospinning membrane was designed with the superiority of cell-instructive and topology in guiding cell behavior and function for abdominal wall defect reconstruction. The electrostatic spinning technique was employed to prepare a bioabsorbable PLCL fiber membrane with effective mechanical support. And decellularized matrix (dECM) derived bioactive microgels were further coated on the fiber membrane through co-precipitation with dopamine, which was expected to endow with cell-instructive hydrophilic interfaces, and also topological morphologies for cell adhesion. Moreover, the introduction of dECM into the microgel was able to promote myogenic proliferation and differentiation of C2C12 cells. Subsequently, in vivo experiments using a rat abdominal wall defect model demonstrated the bioactive microgel coating significantly contributed to the reconstruction of intact abdominal wall structures, highlighting its potential for clinical application in promoting the repair of soft tissue defects associated with abdominal wall damage. This study presented an effective mesh material for facilitating the reconstruction of abdominal wall defects, and also contributed novel design concepts for surface modification of scaffolds with cell-instructive interfaces and topology.

This study aims to employ poly-L-lactic acid (PLLA) and poly(p-dioxanone) (PPDO), loaded with naringin (NAR) to fabricate a functionalized degradable mesh which can promote abdominal wall hernia (AWH) repair. Three meshes named PPDO, PLLA/PPDO, and PLLA/PPDO/NAR were fabricated by electrospinning. The physical and chemical properties of the meshes were evaluated from the aspects of morphology, wettability, chemical composition, mechanical properties, and in vitro degradation. Then, the meshes were implanted into rats to evaluate their repair effects on abdominal wall defect models. The mechanical properties of PLLA/PPDO/NAR mesh were superior to the other two meshes, with a fixed tensile strength of 36.47 ± 2.40 N cm−1 and an elongation at break of 287.98% ± 51.67%, which adequately met the mechanical strength required for the human abdominal wall. The core–shell structure effectively delayed the degradation of PLLA/PPDO as well as PLLA/PPDO/NAR mesh, and drug release of PLLA/PPDO/NAR mesh. On the 7th, 14th, and 28th day after implantation, more neovascularization and tissue formation were observed in the PLLA/PPDO/NAR group and the newborn collagen was arranged in a regular and neat manner compared to the other two groups. The immunohistochemical results showed that the PLLA/PPDO/NAR mesh promoted abdominal wall repair by inhibiting the expression of matrix metalloproteinase2 as well as interleukin-6, and increasing the expression of vascular endothelial growth factor. The PLLA/PPDO/NAR mesh is promising for application in AWH repair.

Abdominal wall defects caused by trauma, congenital rupture, and intra-abdominal infection remain challenging due to the large wound area and complex complications. Herein, an assembled mesenchymal stem cell (MSCs)-laden granular hydrogel (termed assembled GSD@FPs), loaded with basic fibroblast growth factor (bFGF) and platelet-derived growth factor (PDGF), is developed. This hydrogel is constructed through dynamic covalent cross-linking ( via borate ester bonds) among dopamine-grafted gelatin methacrylamide (GelMA-DA), phenylborate-modified hyaluronic acid (HA-PBA), and epigallocatechin-3-gallate (EGCG), serving as multifunctional bulk building blocks for cell delivery and abdominal wall repair. The designed assembled granular hydrogels possess good rheological properties, self-healing, injectability, and tissue-adhesion properties. Detailed in vitro cell experiments are conducted, revealing that the GSD@FPs granular hydrogels can effectively promote cell proliferation, cell migration and angiogenesis. Furthermore, in abdominal wall defects, assembled GSD@FPs significantly accelerates the tissue healing process by simultaneously inhibiting the inflammatory response, promoting collagen deposition, and promoting cell proliferation and angiogenesis. Importantly, the assembled GSD@FPs granular hydrogels can also provide mechanical support and increase the thickness of regenerated tissue (1727.8 ± 169.6 μm for the control group, 3204.2 ± 278.5 μm for the assembled GSD@FPs group at 14 d). Eventually, the GSD granular hydrogels biodegraded, facilitating tissue remodeling and generating new muscle tissues. Therefore, this study provides a promising strategy with great potential for application in abdominal wall repair.

Point-wise suturing is the standard method for ensuring that patches effectively perform mechanically supportive functions in tissue repair. However, stress concentrations around suture holes can compromise the mechanical stability of patches. In this study, we develop a suturable hydrogel patch with flaw-tolerance capabilities by leveraging multiscale stress deconcentration, inspired by natural silk. This design mitigates stress concentration across two scales through the synergistic integration of nanoscale high-energy crystalline domains and intermolecular interactions. The resulting integral hydrogel patch exhibits superior flaw resistance compared to conventional patches and effectively addresses tissue adhesion issues. To validate the efficacy of the patch, we demonstrate successful in vivo repair of abdominal wall defects in rats, comparing the performance of the proposed patch to commercial mesh patches (Prolene). The integral patch design strategy present here offers a valuable approach for developing patches that can be tailored to meet the mechanical support needs of various tissue repair applications.

Background: In this study, a new composite biological mesh named SFP was prepared by combining silk fibroin with polypropylene mesh. The mechanism and clinical application value of the SFP composite mesh were explored. Methods: The fibrous membrane was prepared by electrospinning of silk fibroin. The silk fibrous membrane was adhered to the polypropylene mesh by fibrin hydrogel to make a new composite mesh. The characterizations were verified by structural analysis and in vitro cell experiments. A total of 40 Sprague–Dawley rats were randomly divided into two groups, and 20 rats in each group were implanted with the SFP mesh and pure polypropylene mesh, respectively. The rats were sacrificed in batches on the 3rd, 7th, 14th, and 90th days after surgery. The adhesion degree and adhesion area on the mesh surface were compared, and a histopathological examination was carried out. Results: In vitro cell function experiments confirmed that the SFP mesh had good cell viability. The control group had different degrees of adhesion on the 3rd, 7th, 14th, and 90th days after surgery. However, there was almost no intraperitoneal adhesions on the 3rd and 7th days after surgery, and some rats only had mild adhesions on the 14th and 90th days after surgery in the SFP group. There were statistically significant differences in the postoperative intraperitoneal adhesion area and adhesion degree between the two groups (p < 0.05). Histopathological examination confirmed that the mesenchymal cells were well arranged and continuous, and there were more new capillaries and adipocyte proliferation under the mesenchymal cells in the SFP group. Conclusion: The SFP mesh shows good biocompatibility and biofunction in vitro and in vivo. It can promote the growth of peritoneal mesenchymal cells. The formation of a new mesenchymal cell layer can effectively reduce the extent and scope of adhesion between the mesh and abdominal organs. The SFP mesh will have a good application prospect in the field of abdominal wall hernia repair.

Repairing abdominal wall defects presents significant challenges, due to the high infection risk, poor biocompatibility, and insufficient mechanical strength associated with synthetic materials. To overcome these limitations, we developed a bioinspired multifunctional 3DPF patch by integrating 3D printing and electrospinning technologies. The core material of the patch is 4arm-PLGA-GPO (4A-GPO), synthesized by conjugating the Gly-Pro-Hyp (GPO) peptide sequence with 4arm-PLGA(4A), which significantly enhances bioactivity and mechanical properties. Additionally, the patch encapsulates basic fibroblast growth factor (bFGF) to stimulate cell proliferation and migration, while an antibacterial layer composes of emodin (EMO) and tobramycin to prevent infection. In vivo studies demonstrate the 3DPF patch effectively accelerates tissue repair by reducing fibrosis and adhesions, promoting angiogenesis and collagen deposition, and modulating the immune response. Transcriptomic analysis reveals that the patch downregulates IL-17 mediated inflammatory pathways while upregulating cell adhesion molecule-related pathways, synergistically facilitating microenvironment reconstruction. Furthermore, molecular docking studies suggest the patch interacts with key molecules such as VEGF and COL3, enhancing angiogenesis and matrix remodeling. In summary, this biomimetic patch, composed of bioactive materials with well-defined chemical compositions, integrates mechanical support, immune modulation, and antibacterial protection. by offering a comprehensive solution for abdominal wall repair, it holds significant potential for clinical translation in complex tissue engineering applications.

Application of synthetic or biological meshes is the main therapy for the repair and reconstruction of abdominal wall defects, a common disease in surgery. Currently, no ideal materials are available, and there is an urgent need to find appropriate ones to satisfy clinical needs. Electrospun scaffolds have drawn attention in soft tissue reconstruction. In this study, we developed a novel method to fabricate a composite electrospun scaffold using a thermoresponsive hydrogel, poly (N-isopropylacrylamide)-block-poly (ethylene glycol), and a biodegradable polymer, polylactic acid (PLA). This scaffold provided not only a high surface area/volume ratio and a three-dimensional fibrous matrix but also high biocompatibility and sufficient mechanical strength, and could simulate the native extracellular matrix and accelerate cell adhesion and proliferation. Furthermore, rat adipose-derived stem cells (ADSCs) were seeded in the composite electrospun scaffold to enhance the defect repair and regeneration by directionally inducing ADSCs into endothelial cells. In addition, we found early vascularization in the process was regulated by the hypoxia inducible factor-1α (HIF-1α)/vascular endothelial growth factor (VEGF) pathway. In our study, overexpression of HIF-1α/VEGF in ADSCs using a lentivirus system promoted early vascularization in the electrospun scaffolds. Overall, we expect our composite biomimetic scaffold method will be applicable and useful in abdominal wall defect regeneration and repair in the future.

Healing the wounds of abdominal wall defects characterised by severe complications represents a significant clinical challenge. Mesenchymal stem cells (MSC)-based therapies have emerged as a promising strategy for tissue regeneration. However, the low engraftment and survival rates limit their application owing to the complicated wound microenvironment and large wound areas in abdominal wall defects. Inspired by focal biointerfaces, the present research engineered a spatiotemporal delivery microenvironment-responsive therapeutic platform, termed MSC-GSM@HG, for abdominal wall repair. The constructed platform proved capable of dissolving HA-PBA/Gel-DA (HG) hydrogels in response to in vivo inflammatory responses and subsequently releasing MSC-laden gelatine and silk fibroin microspheres (GSMs). Meanwhile, the HG hydrogels can not only protect the MSCs from excessive reactive oxygen species but also exhibit haemostatic and anti-inflammatory properties. Strikingly, this microsphere-releasing hydrogel platform actively enhanced the repair of abdominal wall defects in vivo , by reducing inflammatory response, facilitating angiogenesis and promoting collagen deposition. Therefore, the microenvironment-responsive platform represents a promising strategy for MSC-based therapies for abdominal wall defects through spatiotemporal delivery mechanisms.

Abdominal wall defect caused by surgical trauma, congenital rupture, or tumor resection may result in hernia formation or even death. Tension-free abdominal wall defect repair by using patches is the gold standard to solve such problems. However, adhesions following patch implantation remain one of the most challenging issues in surgical practice. The development of new kinds of barriers is key to addressing peritoneal adhesions and repairing abdominal wall defects. It is already well recognized that ideal barrier materials need to have good resistance to nonspecific protein adsorption, cell adhesion, and bacterial colonization for preventing the initial development of adhesion. Herein, electrospun poly(4-hydroxybutyrate) (P4HB) membranes infused with perfluorocarbon oil are used as physical barriers. The oil-infused P4HB membranes can greatly prevent protein attachment and reduce blood cell adhesion in vitro. It is further shown that the perfluorocarbon oil-infused P4HB membranes can reduce bacterial colonization. The in vivo study reveals that perfluoro(decahydronaphthalene)-infused P4HB membranes can significantly prevent peritoneal adhesions in the classic abdominal wall defects’ model and accelerate defect repair, as evidenced by gross examination and histological evaluation. This work provides a safe fluorinated lubricant-impregnated P4HB physical barrier to inhibit the formation of postoperative peritoneal adhesions and efficiently repair soft-tissue defects.

Open abdomen (OA), a major advance in modern trauma surgery, enables the treatment of severe abdominal trauma, infections, and hypertension by temporarily closing the abdominal cavity without suturing. Although OA can reduce mortality rates, it also presents challenges such as disruption of the abdominal environment, intestinal exposure, and impaired wound healing. Current temporary abdominal closure (TAC) techniques using polypropylene (PP) mesh are susceptible to abrasion and rupture of the intestine, leading to adhesions and serious complications such as intestinal fistulas and chronic wounds. These complications seriously impact outcomes due to mesh rigidity and its incompatibility with surrounding tissues. In this study, we designed a multifunctional hydrogel composed of GelMA (Gelatin Methacryloyl) and SilMA (Silk Fibroin Methacryloyl) as a promising solution for open abdominal wound repair. In vitro experiments confirmed the biocompatibility, self-healing properties, pro-angiogenic effects, and promotion of macrophage polarization. In vivo studies revealed that the GelMA-SilMA hydrogel could enhance abdominal wall tissue thickness and integrity via isolating the peritoneal cavity, protecting the intestinal tract, polarizing macrophages, promoting collagen formation, inducing neovascularization, inhibiting oxidative stress, and suppressing inflammation. Furthermore, immunofluorescence, immunohistochemistry, and RNA sequencing were employed to evaluate the wound healing mechanisms of GelMA-SilMA hydrogels. The results demonstrated that the hydrogels can promote angiogenesis, inhibit inflammation and immune responses, and enhance wound healing by activating the HIF-a-VEGF signaling pathway, RAP1 signaling pathway, and PI3K-AKT signaling pathway. Hence, the multifunctional GelMA-SilMA hydrogel has tremendous potential for clinical use in open abdominal wound repair.
