Sprayable surface-adaptive biocompatible membranes for efficient hemostasis via assembly of chitosan and polyphosphate

Chengkun Liu,Chang Liu,Zhuang Shi,Wei Lu,Zhiyuan Liu,Shihai Liu,Xiaojuan Wang,Xiaoqiang Wang,Fang Huang

This work describes a hemostatic membrane system (or surface coating) based on spray-assisted layer-by-layer electrostatic assemblies of oppositely charged polyphosphate (polyP) and chitosan (Cs). The as-prepared membrane formed a robust micro-stratified porous structure with high flexibility. Both blood clotting test and rodent hepatic severe hemorrhage model revealed the excellent hemostatic performance of the membrane system, benefitting from the robust assembly and synergistic effect of polyP/Cs as well as membrane surface chemistry. Compared to Cs-topped membrane surface, polyP-sprayed one exhibited further improved hemostatic effect via promoting fibrin formation . Besides, comprehensive in vitro and in vivo evaluations demonstrated good biocompatibility and biodegradability of the membrane. The present approach that integrated the hemostasis-stimulating capability of polyP/Cs with facile spraying method is highly scalable and flexible, which is envisioned to be adapted readily for other hemostatic polyelectrolytes and surface functionalization of diverse existing hemostatic products on demand.

Effect of cyclodextrins on the structure and functions of blood components in vitro

Yeyun Fu,Xiaoyan Wang,Yu Zhang,Zonghua Liu,Wei Xue

Cyclodextrins have been extensively used in various biomedical and pharmaceutical applications. In these applications, cyclodextrins administered in vivo would inevitably enter blood stream. However, there is not enough information on the hemocompatibility of cyclodextrins until now. In this study, we investigated the influences of cyclodextrins (α-cyclodextrin, β-cyclodextrin, and γ-cyclodextrin) on human blood components and functions in vitro, that is, morphology and lysis of red blood cells, structure and conformation of fibrinogen, complement activation, and blood coagulation. It was found that 10 mg/mL of α- or β-cyclodextrins caused abnormal red blood cell morphology and serious hemolysis, while γ-cyclodextrin at 10 mg/mL did not impair red blood cell membrane morphology and integrity. The three cyclodextrins at up to 10 mg/mL affected the local microstructure but did not change the conformation of fibrinogen. The three cyclodextrins from 0.01 to 1 mg/mL all significantly activated the complement system in a concentration-dependent way. The three cyclodextrins at up to 5 mg/mL in blood plasma did not cause significantly different coagulation times compared with the negative control. In addition, the three cyclodextrins at up to 5 mg/mL in whole blood did not cause abnormal coagulation parameters. These results provide significant information on blood safety of the three cyclodextrins for their biomedical and pharmaceutical applications.

Effects of the gene carrier polyethyleneimines on structure and function of blood components

Dagen Zhong,Yanpeng Jiao,Yi Zhang,Wei Zhang,Nan Li,Qinhua Zuo,Qian Wang,Wei Xue,Zonghua Liu

As a synthetic polycation , polyethylenimine (PEI) is currently one of the most effective non-viral gene carriers. For in vivo applications, PEI will enter systemic circulation and interact with various blood components and then affect their individual bio-functions. Up to now, overall and systematic investigation on the interaction of PEI with multiple blood components at cellular, membrane, and molecular levels is lacking, even though it is critically important for the in vivo safety of PEI. To learn a structure–activity relationship, we investigated the effects of PEI with different molecular weight (MW) and shape (branched or linear) on key blood components and function, specifically, on RBC aggregation and morphological change, platelet activation , conformation change of albumin (as a representative of plasma proteins), and blood coagulation process. Additionally, more proteins from plasma were screened and identified to have associations with PEI by a proteomic analysis. It was found that, the PEIs have severe impact on RBC membrane structure , albumin conformation, and blood coagulation process, but do not significantly activate platelets at low concentrations. Furthermore, 41 plasma proteins were identified to have some interaction with PEI. This indicates that, besides albumin, PEI does interact with a variety of blood plasma proteins, and could have unexplored effects on their structures and bio-functions. The results provide good insight into the molecular design and blood safety of PEI and other polycations for in vivo applications.

Unilateral thermosensitive Janus hemostatic dressing for matching coagulation physiological processes

Chuanjuan Chen,Yicheng Zhang,Siqi Chen,Qinhui Chen

Hemostatic dressings are a simple and effective method for managing bleeding emergencies. Based on the blood coagulation process, hemostatic dressings work optimally by aggregating hemocytes, releasing coagulation factors, and sealing blood in a sequential manner. Janus materials, which have different components compartmentalized, have the ability to independently perform multiple functions. In this study, a Janus nonwoven fabric (BG-PNIPAM) with thermo-sensitivity on one side, designed to consistent with the physiological process of coagulation, was created. When utilized for hemostasis, the poly(N-isopropylacrylamide) (PNIPAM) side of Janus BG-PNIPAM undergoes a phase transition within 1 minute. Prior to this, it adsorbs blood in injured site and aggregate hemocytes to form primary thrombus; Afterwards, it plays a sealing role to decrease blood loss. During this process, the bioactive glass (BG) side continuously releases SiO44- and Ca2+ to activate coagulation factors and accelerate the cascade reaction. The phase transition and hydrophilic/hydrophobic transition are triggered by body temperature and rapidly completed just like a click on the switch. Animal studies have demonstrated that Janus BG-PNIPAM shows superior hemostatic performance compared to nonwoven fabric and commercial QuikClot® gauze. Furthermore, its biosafety profile provides assurance for potential clinical applications in the future.

Additive-free Absorbable Keratin Sponge With Procoagulant Activity for Noncompressible Hemostasis

Yuzhen Wang,Xiao Yang,Ziwei Yang,Hangbin Xia,Xiaoqin Si,Jiahui Hao,Dongxue Yan,Huili Li,Ke Peng,Jie Sun,Changcan Shi,Huaqiong Li,Wenzhong Li

The development of a natural, additive-free, absorbable sponge with procoagulant activity for noncompressible hemostasis remains a challenging task. In this study, we extracted high molecular weight keratin (HK) from human hair and transformed it into a hemostatic sponge with a well-interconnected pore structure using a foaming technique, freeze-drying, and oxidation cross-linking. By controlling the cross-linking degree, the resulting sponge demonstrated excellent liquid absorption ability, shape recovery characteristics, and robust mechanical properties. The HK10 sponge exhibited rapid liquid absorption, expanding up to 600% within 5 s. Moreover, the HK sponge showed superior platelet activation and blood cell adhesion capabilities. In SD rat liver defect models, the sponges demonstrated excellent hemostatic performance by sealing the wound and expediting coagulation, reducing the hemostatic time from 825 to 297 s. Furthermore, HK sponges have excellent biosafety, positioning them as a promising absorbable sponge with the potential for the treatment of noncompressible hemostasis.

Macroporous Adhesive Nano-Enabled Hydrogels Generated from Air-in-Water Emulsions

Manshan Xie,Zhiwen Zheng,Shiheng Pu,Yong-Guang Jia,Lin Wang,Yunhua Chen

Developing nanocomposite hydrogel with multi-functions including adjustable mechanical property, tissue-adhesion, and blood coagulation property to accelerate wound healing is highly desirable in surgical application. Here a macroporous adhesive nano-enabled hydrogel constructed from gelatin methacryloyl stabilized air-in-water emulsions incorporated with dopamine-grafted-gelatin (GelDA) and Laponite nanoclay is reported. The hydrogel exhibits interconnected macroporous structure. The physical/chemical cross-linked network formed among the various components contributes to the good mechanical strength of hydrogel, which could be further regulated by adjusting the concentration of Laponite nanoclay. Furthermore, the nanocomposite macroporous hydrogel is endowed with self-healing properties and tissue adhesion by the intermolecular hydrogen bonds, ionic interactions among Laponite nanoclay and polymers, as well as the catechol functional groups. The in vitro studies demonstrate that the macroporous hydrogel has good biocompatibility and could significantly reduce blood clotting time, which is expected to be applied for the rapid sealing and hemostasis of bleeding wounds.

AA/MA/AMA copolymer and chitosan as self-gelling powders for rapid hemostasis and robust tissue adhesion

Wan Peng,Yefeng Jiang,Yang Zhang,Youjin Lai,Yiming Zhu,Huixia Wang,Zilin Kan,Pingsheng Liu

Bleeding control by traditional hemostatic materials poses a risk of continuing oozing bleeding, while existing self-gelling hemostatic powders still face challenges in simultaneously achieving robust wound sealing and effectively accelerating blood coagulation. Herein, we reported acrylic acid/methacrylic acid/2-aminoethyl methacrylate copolymer (PAMA) and chitosan (CS) as a new type of self-gelling composite powder (PAMA-CS) for acute bleeding wound sealing with reduced blood leakage. Upon rapid blood absorption, PAMA-CS powders could quickly form an adhesive hydrogel that strongly binds to bleeding tissues, exhibiting high burst pressure tolerance (up to 452.6 mmHg) for wound sealing to prevent bleeding. The incorporation of chitosan endowed significantly enhanced liquid uptake ability and accelerated blood coagulation (clot index of 77.92 % within 30 s) to the PAMA-CS powder. Based on the synergistic effect of tight adhesive sealing and accelerating intrinsic blood coagulation, PAMA-CS powders could achieve effective hemostasis within 20 s for acute bleeding liver and stomach on rats, outperforming commercial hemostatic chitosan powders. Combined with rapid hemostasis, tight & stable wound sealing, and favorable biodegradation, PAMA-CS powders may serve as a promising hemostatic candidate for emergency bleeding control and wound closure.

A novel sodium polyacrylate–based stasis dressing to treat lethal hemorrhage in a penetrating trauma swine model

Wang Wei,Han Peng,Yang Lei,Meng Zhiyun,Gan Hui,Wu Zhuona,Zhu Xiaoxia,Sun Wenzhong,Gu Ruolan,Dou Guifang

BACKGROUND Control of massive hemorrhage from penetrating wound sites is difficult in both combat and civilian settings. A new hemostatic dressing, sodium polyacrylate (PAAs)–based bag (PB), based on PAAs is designed for the first aid of massive penetrating hemorrhage. This study aimed to investigate the efficacy of PB in a penetrating trauma model in swine. METHODS A complex groin penetrating injury was produced in swine by completely excising the femoral vessels and surrounding muscles. After 15-second free bleeding, 18 healthy Guizhou female swine were administered PB (n = 6), CELOX-A (n = 6; Medtrade Products, Crewe, United Kingdom), or standard gauze (n = 6) for hemostatic intervention, followed by 3-minute compression if the bleeding persisted, with subsequent observation continuing for 1 hour. The primary outcomes included initial hemostasis, the incidence of applying manual pressure, and application time. RESULTS Sodium polyacrylate could rapidly absorb the liquid to expand, crosslink with a large number of red blood cells, induce cellular morphological alteration, and promote blood coagulation. Sodium polyacrylate–based bag and CELOX-A initiated and sustained hemostasis for 60 minutes, whereas 0% of the standard gauze achieved initial hemostasis. Maximum number of manual compressions were applied in standard gauze (6 of 6 [100%]), followed by CELOX-A (5 of 6 [80%]), while no manual pressure was required in the case of PB (0 of 6 [0%]). Application time for PB (19.0 ± 4.6 seconds) was significantly less than CELOX-A (169.0 ± 73.5 seconds) and standard gauze (187.8 ± 1.7 seconds). CONCLUSION We prepared a type of superabsorbent PAAs and made an original hemostatic dressing, PB. It can rapidly achieve durable hemostasis in the groin-penetrating trauma hemorrhage swine model without any external compression. The packet form makes PB easy to deploy and remove from wounds. Therefore, PB could be a promising hemostatic candidate for controlling penetrating hemorrhage. Export We designed and prepared an original sodium polyacrylate (PAAs)-based stasis dressing, PB, for the first aid of massive penetrating hemorrhage. It allows durable hemostasis in the groin-penetrating trauma hemorrhage swine model without any external compression.

Point-of-Care Assessment of Hemostasis with a Love-Mode Surface Acoustic Wave Sensor

Xi Chen,Meng Wang,Gang Zhao

Monitoring of the hemostasis status is essential for therapeutic anticoagulants, undergoing surgery, cardiovascular diseases, etc. Although the clinical values of conventional blood coagulation tests have been well demonstrated, these devices have limitations such as large and expensive equipment, excessive sample volumes, long turnaround times, and difficulty in miniaturization for point-of-care use. Here, we present a novel strategy to evaluate blood hemostasis using the single-port Love-mode surface acoustic wave (SLSAW) sensor. The SLSAW sensor was designed as a plug-and-play-type unit for disposable use and operated under the harmonic resonant mode to produce frequency response to the blood coagulation cascade. Compared with a quartz crystal microbalance, Lamb wave, and film bulk acoustic resonator, the frequency shift of SLSAW was significantly increased, ranging from approximately 8960 to 10 368 kHz, which indicated enhancement of the signal-to-noise ratio. To demonstrate the feasibility of the SLSAW, studies were carried out to examine the effects of temperature and clotting reagents on coagulation times and kinetics. Activated partial thromboplastin times of plasma were validated by comparing with SYSMEX CA-7000 with the correlation (R2) as 0.996. In terms of coagulation kinetics, reaction time, clot formation time, maximum frequency shift, and clot formation rate of whole blood correlated well with corresponding parameters of the standard thromboelastography (TEG) analyzer (R2 = 0.9942, 0.9868, 0.9712, and 0.9939, respectively). The SLSAW sensor, with the advantages of low cost, small size, little sample consumption (1 μL), disposable use, and simple operation, is a promising tool for point-of-care diagnosis of hemostasis.

Facile synthesis of rapid hemostatic powder based on sodium alginate for promoting hemostasis and wound healing

Aihong Chen,Lixue Wu,Kaiyang Wang,Long Qin,Kexin Zhang,Song Chen,Yijiong Zhang,Wei Song,Qian Zhang,Wei Xia,Yu Luo,Xijian Liu,Jian Wan

Uncontrollable bleeding and excessive blood loss are the primary causes of death on the battlefield and in accidents. However, commercial hemostatic materials possess individual drawbacks for hemostasis. Here, we present a rapid hemostatic powder (SACC-GO) with high biosafety, which is fabricated by a facile ball milling method using inexpensive raw materials (sodium alginate (SA), calcium chloride, cerium nitrate and graphene oxide (GO)). The SACC-GO could immediately become gel when meeting blood, whose strength was enhanced due to specific surface area of GO and high coordination capacity of Ce 3+ ions. In addition, the SACC-GO had a high water absorption rate (~2000 %) allow it to concentrate blood and release Ca 2+ ions to improve blood coagulation. The in vitro and in vivo results, such as hemostasis in tail amputation, femoral hemostasis and hepatic hemostasis, exhibited excellent hemostasis effects by reducing hemostasis time and blood loss (with the majority of reductions exceeding 50 %). The SACC-GO also enhanced wound healing due to Ce ions scavenging reactive oxygen species (ROS). Thus, the SACC-GO hemostatic powder had great potential for rapid hemostasis.

Hemostasis mechanism and applications of N-alkylated chitosan sponge

Yuchen Huang,Longbao Feng,Yi Zhang,Liumin He,Changyong Wang,Jiake Xu,Jianping Wu,Thomas Brett Kirk,Rui Guo,Wei Xue

Chitosan (CHI) is a versatile biological material that is well known for its hemostatic properties. This preliminary study evaluated several self-assembling hydrophobically modified chitosan (HM–CHI) sponges to determine their efficacy on hemostasis . Fourier transform infrared (FT-IR) spectroscopy was used to determine the successful graft of dodecyl groups onto the nitrogen atoms of CHI molecules. A platelet aggregation assay revealed that HM–CHI accelerated the platelet aggregation rate. Fluorescence spectroscopy showed that the HM–CHI changed the structure of fibrinogen in blood. Activated partial thromboplastin time, prothrombin time, fibrinogen time, and thromboelastographic assays were used to explore the effect of HM–CHI on the autologous blood coagulation pathway. Finally, a hemostatic sponge was made with HM–CHI and freeze-dried zeolite composite film and was applied to the rat femoral artery hemostasis model. A hemostasis time of 86 ± 5 sec was achieved, which was significantly better than the one composed with pure CHI. The experimental results of the HM–CHI hemostatic materials are inspiring and will encourage the research and development of such materials. HM–CHI may be a strong candidate as a safe and effective hemostatic material. Copyright © 2017 John Wiley & Sons, Ltd.

Synthesis of cationic carbon dots and their effects on human serum proteins and in vitro blood coagulation

Hua Qiao,Huichao Zhang,Ajab Khan,Bo Li,Lin Ge,Yequn Liu,Wei Bian,Zhizhen Liu,Jun Xie

Cationic carbon dots (CCDs) are a promising alternative to gene-delivery systems, and good biosafety levels are crucial for their in vivo use. In this study, spherical and monodispersed CCDs with an average surface potential of +28.7 mV were prepared using sucrose and glutamate (denoted SG-CCDs) using a one-pot autoclave-assisted method. Molecular interactions between the SG-CCDs and four major human serum proteins (albumin, immunoglobulin G, fibrinogen, and transferrin) were investigated. The results were further verified on human serum, and the effect of the SG-CCDs on in vitro blood coagulation was examined. The results showed that the fluorescence of human serum was clearly quenched by the SG-CCDs through a dynamic collision mechanism. Moreover, SG-CCDs at a concentration of 20 μM exhibited minor effects on the secondary structure of human serum. The activated partial thromboplastin and prothrombin time as well as the fibrinogen concentration were not changed, indicating that the SG-CCDs did not interfere with the coagulation process. This study provided an understandable background on the behaviour of CCDs in clinical applications.
