Choline Phosphate Surface-Activated 3D-Printed Porous Titanium Scaffold Combined with Stem Cell Exosomes for Enhancing Bone Defects Repair

Xuezhong Cui,Jing Li,Yuemin Wang,Tong Sun,Jie Weng,Zhiqiang Li,Jianshu Li,Xingyu Chen

In recent decades, porous titanium (Ti) bone-engineered scaffolds have emerged as a promising biomaterial for bone defect repair due to their excellent biocompatibility and mechanical properties. However, the limited bioactivity on the surface of the porous scaffold hinders osteogenesis and osseointegration, thereby restricting its further application. In this study, we utilized surface-initiated atom transfer radical polymerization to prepare a zwitterionic poly[2-(methacryloyloxy)ethyl choline phosphate] (PMCP) modified bioactive coating on the surface of a 3D-printed porous Ti scaffold. Additionally, exosomes derived from bone mesenchymal stem cells (BMSCs) were introduced into the scaffold surface via specific interactions between choline phosphate and phosphatidylcholine (CP-PC) on exosomes. In vitro studies for ossification and transcriptome analysis have shown that the exosome bioactive coating on a Ti scaffold enhances the proliferation of BMSCs, their osteogenic activity, and the expression of osteogenic-related genes. Furthermore, in vivo study results from hard tissue sectioning and microcomputed tomography indicate that the bioactive Ti scaffold significantly promotes new bone formation after 4 and 12 weeks of implantation in rabbit femoral defects. Overall, this study showcases the potential of the exosome-based Ti scaffold to enhance osteogenic activity, offering a novel strategy for cell-free bone tissue regeneration with significant therapeutic implications.

Mechanical and biological properties of 3D-printed porous titanium scaffolds coated with composite growth factors

Jiang Chunwen,Gong Guojia,Xiao Shan,Zhang Shengxiang,Chen Diansheng,Song Shuqing,Dai Honglin,Wu Chongxue,Zou Qiaoru,Li Jianping,Wen Bing

Background Osseointegration is considered a prerequisite for predicting implant success, and structure, biocompatibility, and properties of the implant are important parts of the factors that influence osseointegration. The focus of current research is on how to increase the strength of osseointegration on the implant and shorten the osseointegration time. Methods In this research, different porous scaffolds, including uniform, radial-gradient, and axial-gradient porous, were designed and fabricated. Their mechanical properties and biocompatibility were comprehensively evaluated through mechanical tests and in vitro cellular experiments. A porous scaffold exhibiting optimal properties was identified through preliminary experiments. Subsequently, three different sets of composite scaffolds were developed, consisting of the selected scaffold modified with chitosan microspheres loaded with Bone Morphogenetic Protein-2 (BMP-2), Platelet-Derived Growth Factor-BB (PDGF-BB), or a combination of both. The biological responses to composite scaffolds were systematically examined through in vitro and in vivo experiments. Results Finite element analysis indicated that the maximum equivalent stress of all three porous implants was lower than that of solid implants, while the maximum equivalent stress in the cortical bone of the porous group was higher than in the solid group. Compression tests confirmed that the elastic modulus of all three porous scaffold structures fall within the range of natural human bone. In vitro cell experiments showed that the radial gradient porous group scaffolds had the highest cell count and Alkaline phosphatase activity. The composite scaffolds exhibited superior wettability and water absorption properties compared to the non-coated scaffolds. Cell and animal experiments demonstrated that the titanium scaffolds co-modified with BMP-2 and PDGF-BB showed greater cell proliferation and new bone formation compared to scaffolds with single-factor coatings and uncoated scaffolds. Conclusions Radial-gradient porous scaffolds exhibit compatible elastic modulus, excellent cell compatibility, and osteogenic potential, making them promising candidates for bone tissue engineering applications in dentistry. Furthermore, the composite scaffolds incorporating BMP-2 and PDGF-BB-loaded chitosan microspheres demonstrated enhanced osteogenic differentiation compared to single-factor modified porous scaffolds, providing experimental evidence for the clinical application of novel implants.

Modified multilayered porous titanium scaffolds with silicon-doped coating surface-loaded BMP-2 prepared by microarc oxidation for bone defect repair

Xue'e Zhang,Xu Zhang,Wuchao Zhou,MengZhen Tang,Tiebiao Wang,Ketong Le,Weihong Xi

As is generally known, the lack of osteoinductive properties in titanium scaffolds has been a major barrier to bone defects repair. In our previous studies, we found that silicon-doped porous coatings prepared via micro-arc oxidation (MAO) had the potential application of Si-doped coatings in dental and orthopedic fields. We are currently focused on developing more advanced surface coatings to maximize their osteoinductive efficacy. Bone morphogenetic protein-2 (BMP-2), known for its ability to induce bone and cartilage development, has also been extensively studied. Therefore, in this study, macroporous titanium alloy scaffolds were fabricated using 3D printing technology, silicon ions were introduced onto the surface of customized macroporous titanium alloy scaffolds through MAO to enhance the bioactivity of the scaffolds, and BMP-2 was loaded onto the Si-doped coating to further improve the osseointegration of the titanium alloy scaffolds post-implantation. Results demonstrated that silicon-doped coating-loaded BMP2-modified multi-level porous titanium scaffolds exhibited improved hydrophilicity and biocompatibility. In-vivo and vitro studies further confirmed the superior bone regeneration capabilities of these scaffolds. This study demonstrates an optional strategy to combine two surface modification treatments, offering substantial potential as an advanced bone defect repair material with improved and accelerated bone regeneration.

A Magnetic Iron Oxide/Polydopamine Coating Can Improve Osteogenesis of 3D-Printed Porous Titanium Scaffolds with a Static Magnetic Field by Upregulating the TGFβ-Smads Pathway

Zhenfei Huang,Yu He,Xiao Chang,Jieying Liu,Lingjia Yu,Yuanhao Wu,Yaqian Li,Jingjing Tian,Lin Kang,Di Wu,Hai Wang,Zhihong Wu,Guixing Qiu

3D-printed porous titanium–aluminum–vanadium (Ti6Al4V, pTi) scaffolds offer surgeons a good option for the reconstruction of large bone defects, especially at the load-bearing sites. However, poor osteogenesis limits its application in clinic. In this study, a new magnetic coating is successfully fabricated by codepositing of Fe 3 O 4 nanoparticles and polydopamine (PDA) on the surface of 3D-printed pTi scaffolds, which enhances cell attachment, proliferation, and osteogenic differentiation of hBMSCs in vitro and new bone formation of rabbit femoral bone defects in vivo with/without a static magnetic field (SMF). Furthermore, through proteomic analysis, the enhanced osteogenic effect of the magnetic Fe 3 O 4 /PDA coating with the SMF is found to be related to upregulate the TGF β -Smads signaling pathway. Therefore, this work provides a simple protocol to improve the osteogenesis of 3D-printed porous pTi scaffolds, which will help their application in clinic.
