Poly(dopamine)-inspired surface functionalization of polypropylene tissue mesh for prevention of intra-peritoneal adhesion formation

Wanjun Hu,Shenglin Lu,Ying Ma,Pengfei Ren,Xiaoe Ma,Naizhen Zhou,Tianzhu Zhang,Zhenling Ji

Polypropylene (PP), as one of the most common prosthetic materials, has been widely used in intra-peritoneal repair. However, its adhesion to viscera has severely limited its application. Therefore it is critical to improve the PP surface with an anti-adhesion property. In this work, based on dopamine-inspired chemistry, virgin PP (V-PP) mesh was first pretreated with O2 plasma, subsequently dipped in dopamine aqueous solution for 24 h, and then chitosan (CS) was grafted onto it. Finally the anti-adhesion mesh (O-PP/PDA/CS) was obtained. The formation procedure of a PDA/CS ad-layer was characterized by water contact angle measurements, ATR-FTIR, SEM, and XPS. The results show that a PDA/CS ad-layer could be coated on the PP surface efficiently. NIH/3T3 cells were first cultured on O-PP/PDA/CS meshes to evaluate the availability of anti-adhesion and biocompatibility in vitro, and then the efficacy of the PDA/CS-coating as a barrier for reducing postsurgical adhesions was evaluated using a rat abdominal wall defect model. Compared with the V-PP group, NIH/3T3 cells exhibited higher viability in the O-PP/PDA/CS groups as evaluated by the CCK-8 method. In addition, NIH/3T3 cells grow into round-shapes on the O-PP/PDA/CS surface. This indicates that the modification strategy can facilely lead to excellent properties of anti-adhesion. In vivo tests further indicate that O-PP/PDA/CS meshes were effective in reducing adhesion formation.

A Single-Component Janus Zwitterionic Hydrogel Patch with a Bionic Microstructure for Postoperative Adhesion Prevention

Rui Liu,Zhongming Zhao,Qi Yang,Shuang Chen,Zhuojun Yan,Xiuqiang Li,Lei Liang,Bingyan Guo,Baoqun Wang,Hong Zhang,Fanglian Yao,Junjie Li

The development of anti-adhesion hydrogels for preventing postoperative adhesions is an ongoing challenge, particularly in achieving a balance between exceptional antifouling properties and effective in situ tissue retention. In this study, we propose a unique approach with the design of a single-component Janus zwitterionic hydrogel patch featuring a bionic microstructure. The Janus patches were prepared through free radical polymerization of sulfobetaine methacrylate with N, N′-methylenebis(2-propenamide) as the cross-linker. The incorporation of hexagonal facets separated by interconnecting grooves on one side imparts durable and reliable in situ retention capabilities to the Janus hydrogel patch when it is applied to traumatized tissues. The opposing flat surface exhibits outstanding resistance to bacteria, proteins, and cell adhesion, due to the superhydrophilicity and excellent antifouling characteristics of zwitterionic polymers. This dual functionality empowers the Janus hydrogel patch to mitigate adhesions between traumatized and surrounding tissues. The hexagonal and groove bionic microstructures facilitate rapid drainage, promoting swift contact with the tissue for increased adhesion strength, while independent hexagonal microfacets enhance the peeling energy. In an in vivo setting, Janus zwitterionic hydrogel patches with surface microstructures form mutually embedded structures with the cecum surface, minimizing the likelihood of slippage and detachment. Remarkably, in vivo experiments involving abdominal wall cecum injuries illustrate the Janus zwitterionic hydrogel patch’s superior anti-adhesion effectiveness compared to commercial controls. Thus, the Janus hydrogel patch, distinguished by its bionic microstructure surface, presents substantial potential in the biomedical field for averting postoperative adhesions.

In situ manufacture of hydrogel lubrication coating on damaged tissue to resist adhesion

Renjie Li,Cewen Hu,Luyao Gao,Yunlei Zhang,Pei Zhang,Zengjie Fan,Ji Liu,Xiaoduo Zhao,Shuanhong Ma,Feng Zhou

The human body involves many soft interfaces with excellent hydration and lubrication properties to resist biological adhesion. The abdominal cavity, one of the most important parts of soft tissues, commonly generates severe wear and adhesion-related complication upon encountering lubrication failure between the internal organs of the abdominal cavity and the abdominal wall. Here, we report a method for the manufacture of UV-cured hydrogel lubrication coating on damaged cecum tissue. The hydrogel coating consists of methacrylated carboxymethyl chitosan (MA-CMC) and o -nitrobenzene-grafted hyaluronic acid (HA-NB). The coating not only exhibits fast curing time, good bonding strength, as well as matching mechanics with organs but also shows good lubricity and anti-fouling properties. The anti-adhesion performance of the coating is verified in a constructed rat cecum-abdominal wall postoperative adhesion model. The current concept of hydrogel lubrication coating may help accelerate the development and clinical application of soft anti-adhesion materials.

Antibacterial and anti-adhesion effects of the silver nanoparticles-loaded poly(l-lactide) fibrous membrane

Shen Liu,Jingwen Zhao,Hongjiang Ruan,Wei Wang,Tianyi Wu,Wenguo Cui,Cunyi Fan

The complications of tendon injury are frequently compromised by peritendinous adhesions and tendon sheath infection. Physical barriers for anti-adhesion may increase the incidence of postoperative infection. This study was designed to evaluate the potential of silver nanoparticles (AgNPs)-loaded poly( l -lactide) (PLLA) electrospun fibrous membranes to prevent adhesion formation and infection. Results of an in vitro drug release study showed that a burst release was followed by sustained release from electrospun fibrous membranes with a high initial silver content. Fewer fibroblasts adhered to and proliferated on the AgNP-loaded PLLA electrospun fibrous membranes compared with pure PLLA electrospun fibrous membrane. In the antibacterial test, the AgNP-loaded PLLA electrospun fibrous membranes can prevent the adhesion of Gram-positive Staphylococcus aureus and Staphylococcus epidermidis and Gram-negative Pseudomonas aeruginosa . Taken together, these results demonstrate that AgNP-loaded PLLA electrospun fibrous membranes have the convenient practical medical potential of reduction of infection and adhesion formation after tendon injury.

Injectable and self-fused hydrogels with antifouling capability based on amino acid derivatives for postoperative anti-adhesion application

Li Meng,Zhang Mengyuan,Liu Zheng,Xie Ruilin,Yang Yuxuan,Shen Kaixiang,Yang Aimin,Cheng Yilong

The design and fabrication of injectable barriers with promising antifouling capability to prevent the formation of adhesions after surgery remains challenging. Herein, we introduce amino acid derivative with hydroxyl group ( N -acryloyl serine (ASer)) into the preparation of hydrogels to develop a novel hydrogen bond-crosslinked hydrogel (PASer) by free radical polymerization. It is found that one substituted methyl group can cause considerable disruption to the hydrogen bonding interaction and result in increased initial gelling concentration and significant decrease in the mechanical properties of poly( N -acryloyl threonine) (PAThr) hydrogel compared with PASer hydrogel. Moreover, it is worth noting that the hydrophilic hydroxyl groups enable PASer hydrogel to efficiently resist protein adsorption and cell adhesion. The PASer hydrogel possesses excellent mechanical performance, injectability, self-fusion, good biocompatibility and appropriate in vivo residence time, which are consistent with the requirements of postoperative anti-adhesion materials in clinical practice. In vivo studies reveal that the advanced hydrogel shows satisfactory preventive effect on postoperative peritoneal adhesions in the established mice abdominal wall defect model.

Preparation of fluorinated chitosan-based multifunctional materials with rapid hemostasis, anti-infection, and non-adhesive properties for wounds

Yufang Ren,Zeyuan Sun,Yong Huang,Xiaoyan An,Xiaona Bian,Zhenhao Cao,Yifan Liu,Kanwal Javed,Tetiana Derkach,Xue Li

Excessive bleeding can give rise to a range of pathologies, encompassing hypothermia, coagulation disorders, infection, and hemorrhagic shock. Uncontrolled massive blood loss stands as one of the foremost contributors to mortality. Consequently, higher requirements are put forward for the promptness and efficacy of hemostasis. At present, the hemostatic materials available on the market still have the problem that the hemostatic effect is not ideal, the antibacterial effect is poor, which is easy to cause wound infection, and the removal of hemostatic materials will cause secondary bleeding. The present study describes the development of a novel multifunctional hemostatic material based on carboxymethyl chitosan (CMC). First, the fluorinated CMC (CMC-FDA) is successfully synthesized via the Michael addition reaction utilizing 1H, 1H, 2H, 2H -fluorodecyl alcohol (FDA), followed by the preparation of CMC-FDA/PHMB materials through electrostatic interactions with polyhexamethylene biguanide (PHMB). The successful synthesis of CMC-FDA/PHMB was characterized by XPS, FT-IR, NMR, and so forth. The CMC-FDA/PHMB coating exhibits excellent hemostatic properties through in vivo liver hemostasis and in vitro caudation hemostasis experiments. The hemostatic effect of CMC-FDA/PHMB is highly pronounced, as evidenced by the results obtained for hemostasis time and blood loss. The experiment of simulating dynamic blood loss further confirmed that the gauze coated with CMC-FDA/PHMB has excellent hemostatic performance. Moreover, the synthesized CMC-FDA/PHMB coating demonstrates significant anti-adhesion and antibacterial properties of Staphylococcus aureus and Escherichia coli , which could prevent of wound infection. Furthermore, the CMC-FDA/PHMB coating exhibits non-wettability due to its low surface energy of FDA, which can prevent blood wetting and thus play a hemostatic effect, synergistically with CMC. Additionally, the CMC-FDA/PHMB coating demonstrates easy separation from the wound site attributed to it hydrophobic property. The results of the hemolysis and cytotoxicity tests verified the excellent biocompatibility of the CMC-FDA/PHMB coating. This CMC-FDA/PHMB material hold promising potential for rapid hemostasis and prevention of wound infection. Graphical

Wet Tissue Interfacial Water Triggers Ultrafast In Situ Gelation of Sprayable Dry Powder for Postoperative Anti-Adhesion

Chaoyao Wen,Yihang Gong,Haoyan Chen,Yu Tian,Hui Yu,Xudan Xin,Le Wang,Chuyang Xiang,Yisheng Chen,Tianhua Xiao,Chengyun Ning,Xuebin Yang,Lei Zhou,Guoxin Tan

Postoperative adhesions complicate the majority of surgical procedures, imposing an excess medical burden. The segregation of injured tissue from healthy tissue is considered the optimal therapeutic strategy. However, achieving hemostasis while preventing adhesions in the immediate postoperative setting continues to pose a significant challenge. This paper presents the in situ formation of an asymmetrically adhesive hydrogel barrier via a sprayable dry powder (SDP) that enables interfacial water-induced, ultra-rapid gelation. Owing to SDP hydrophilicity and the interstitial spaces between particles, it can rapidly and efficiently absorb interfacial water, thereby facilitating smooth cross-linking between material functional groups and tissue functional groups. In addition, SDP becomes less adhesive after gelation as a layer of interfacial water forms on the surface of the SDP hydrogel, enhancing its anti-adhesion properties. Animal experiment outcomes demonstrate that SDP exhibits significant asymmetric adhesive properties before and after gelation, successfully preventing postoperative adhesions. As a result, SDP heralds a novel approach to the development of adhesives that can simultaneously mitigate adhesions and promptly arrest bleeding.

Bio-inspired multiple composite film with anisotropic surface wettability and adhesion for tissue repair

Minhan Zou,Xin Zhao,Xiaoxuan Zhang,Yuanjin Zhao,Chunwu Zhang,Keqing Shi

The ideal patch for tissue repair can induce host cells to migrate directionally to defected area and proliferate rapidly to form healthy granulation, while avoiding the adhesion and abrasion with adjacent tissues. However, existing patches hardly achieve this goal because of the contradictory processes during tissue repair, the single property of biomaterials as well as the absence of effective surface microstructures. Herein, we present a bio-inspired composite patch film that simultaneously integrate opposite properties of adhesion and anti-adhesion. Based on a double-layered inverse opal scaffold, the liquid paraffin is infused into one surface to form an anti-adhesive Nepenthes-like slippery liquid-infused porous surface (SLIPS) for isolating defected area with adjacent viscera; while methacrylated gelatin (GelMA) hydrogel is infiltrated into the opposite surface with design of anisotropic microgrooved structures, aiming to facilitate cell adhesion, promote division growth, and induce oriented migration. We have demonstrated that the patch film could promote tissue repair without adhesion-related complications after being implanted into a rat model with abdominal wall defects. These features make such patch film highly versatile for application in regenerative medicine.

Fluorinated poly(aryl ether)/polypropylene composite patch for prevention of abdominal adhesions after hernia repairs

Wenqing Zhang,Huawei Yang,Xu Zhang,Zhaoyang Wang,Shifang Luan

Hernia typically does not heal spontaneously. Large-pore patches, most notably polypropylene patches (PP patches), are the gold standard in hernia repair surgery. However, a single patch is insufficient for both anti-adhesion and tissue fusion, leading to complications such as organ adhesions. In this study, a chemically stable and biocompatible modified fluorinated poly(aryl ether) (FPAE-F) was prepared by grafting perfluoroalkyl groups onto a fluorinated poly(aryl ether) via nucleophilic aromatic substitution. A porous FPAE-F fiber film (eFPAE-F) was fabricated by electrospinning and combined with a PP patch to produce a modified fluorinated poly(aryl ether)/polypropylene (FPAE-F/PP) composite patch. The eFPAE-F layer of the composite patch, which faces the abdominal viscera, exhibits a water contact angle of 151.3 ± 1.2°. This superhydrophobic surface prevents protein adhesion, thereby inhibiting rapid fibroblast proliferation. The small pore size (3.22 ± 1.25 μm) of the eFPAE-F layer effectively impedes fibroblast infiltration while permitting the transport and metabolism of nutrients. In vivo experiments have demonstrated that the composite patch is a viable anti-adhesion material, resulting in no adhesions and low inflammation levels after 2 weeks. Due to its outstanding anti-adhesion properties, eFPAE-F/PP is expected to be applied in the field of hernia repair.

An enzyme cross-linked hydrogel as a minimally invasive arterial tissue sealing and anti-adhesion barrier

Di Sun,Han Wang,Jie Liu,Xinlian Wang,Hongbo Guo,Lijuan Xue,Libing Li,Jubo Li,Baojie Zhang,Yueguang Xue,Shilin Li,Ying Liu

Uncontrollable bleeding is the main cause of death during surgical trauma and accidental injuries. To halt the bleeding caused by arterial rupture is still challenging due to the high pressure and velocity of arterial blood flow . Enzyme-responsive hydrogels have emerged as promising solutions for hemostasis after arterial rupture, largely due to their high hemostatic efficiency and specificity. However, the existing enzyme-responsive hydrogels suffer from weak cross-linking. Here, we report the design of a hemostatic material with nanoporous , combining a transglutaminase enzyme reaction and Schiff base reaction, for percutaneous injection. Here, we show that the resulting burst pressure of the hemostatic material was 134 ± 10 mmHg, which is significantly greater than the normal systolic blood pressure . Most importantly, the material was able to efficiently stop the bleeding a pig and rat arterial vessel models, with a ~10 s gelling time and without the need for the hemostatic clamps on both ends of the vessel injury. Furthermore, the hemostatic material functioned as an anti-adhesion barrier in a rat liver model. The present study describes an effective, injectable hydrogel solution for the rapid and minimally invasive hemostatic treatment of bleeding diseases in parenchymal organs.

Water triggered injectable polylactic acid hydrogel based on zwitterionic sulfobetaine modification for incompressible bleeding and tissue anti-adhesion

Xinran Yang,Xiudan Wang,Lizong Tang,Zhiguang Sun,Xing Gao,Yanmei Zhao,Shike Hou,Jie Shi,Qi Lv

Massive blood loss is the main cause of prehospital trauma-related death, the development of rapid and effective hemostatic materials is imminent. Injectable hydrogels have the advantages of covering irregular bleeding sites and quickly closing the wound. However, its inherent viscosity can easily precipitate tissue adhesion in vivo and other complications. Based on the anti-protein properties of zwitterion and our previous work about in situ hemostatic/anti-adhesion hydrogel material, we have synthesized a series of injectable hydrogel composed of sulfobetaine-modified polylactic acid (PLA) and gelatin (Gel). These hydrogels could form a smooth film structure by simple water triggering, thereby conferring anti-adhesive properties. We visualized the changes in surface hydrophobicity using fluorescent probes and demonstrated tissue adhesion, rapid hydrophobic interface response, as well as rapid hemostasis for incompressible wounds through in vivo and in vitro experiments. Additionally, we explored the application of hydrogel materials in the scenario of postoperative bleeding, which can effectively prevent unnecessary adhesion through rapid film formation and the anti-protein property of sulfobetaine. We believe that this multifunctional hemostatic hydrogel has the potential to serve as a prehospital emergency treatment of incompressible bleeding and benefit to the postoperative recovery of patients.

Construction and properties of the silk fibroin and polypropylene composite biological mesh for abdominal incisional hernia repair

Luan Fengming,Cao Wangbei,Cao Chunhui,Li Baizhou,Shi Xiaoyu,Gao Changyou

Background: In this study, a new composite biological mesh named SFP was prepared by combining silk fibroin with polypropylene mesh. The mechanism and clinical application value of the SFP composite mesh were explored. Methods: The fibrous membrane was prepared by electrospinning of silk fibroin. The silk fibrous membrane was adhered to the polypropylene mesh by fibrin hydrogel to make a new composite mesh. The characterizations were verified by structural analysis and in vitro cell experiments. A total of 40 Sprague–Dawley rats were randomly divided into two groups, and 20 rats in each group were implanted with the SFP mesh and pure polypropylene mesh, respectively. The rats were sacrificed in batches on the 3rd, 7th, 14th, and 90th days after surgery. The adhesion degree and adhesion area on the mesh surface were compared, and a histopathological examination was carried out. Results: In vitro cell function experiments confirmed that the SFP mesh had good cell viability. The control group had different degrees of adhesion on the 3rd, 7th, 14th, and 90th days after surgery. However, there was almost no intraperitoneal adhesions on the 3rd and 7th days after surgery, and some rats only had mild adhesions on the 14th and 90th days after surgery in the SFP group. There were statistically significant differences in the postoperative intraperitoneal adhesion area and adhesion degree between the two groups (p < 0.05). Histopathological examination confirmed that the mesenchymal cells were well arranged and continuous, and there were more new capillaries and adipocyte proliferation under the mesenchymal cells in the SFP group. Conclusion: The SFP mesh shows good biocompatibility and biofunction in vitro and in vivo. It can promote the growth of peritoneal mesenchymal cells. The formation of a new mesenchymal cell layer can effectively reduce the extent and scope of adhesion between the mesh and abdominal organs. The SFP mesh will have a good application prospect in the field of abdominal wall hernia repair.
