The development of a scaffold that can be quickly prepared and used to repair maxillofacial bone defects is an urgent clinical need. 3D printing technology can prepare personalized bone defect substitutes according to the computed tomography (CT) data of the patient's defect location. Magnesium is a bioactive molecule that plays an important role in the process of bone repair. In this study, different contents of magnesium phosphate (Mg 3 (PO 4 ) 2 ) were incorporated into polycaprolactone (PCL), and a bone defect repair scaffold was prepared using 3D printing technology. In vitro results showed that PCL scaffolds containing 20 % Mg 3 (PO 4 ) 2 (PCL#20MgP) had the strongest ability to promote osteogenic differentiation. Micro-CT and histological staining results of the repair of tibial defects in rats also further proved that the PCL#20MgP scaffold had strong bone formation ability in vivo . The PCL#20MgP scaffold was used to repair the rabbit maxillofacial bone defect. The Micro-CT results also confirmed that PCL#20MgP had a better osteogenesis effect than the PCL scaffold. The PCL#20MgP scaffold has good clinical application prospects in the field of maxillofacial bone defect repair.

Maxillofacial trauma is an infection and oxidative stress-induced damage to maxillofacial area related cells. Increasing evidences have suggested that therapeutic strategies with controllable anti-infective and anti-inflammatory capabilities hold great potentials in the clinical treatment of maxillofacial trauma. Herein, we reported an infectious microenvironment responsive bimetallic sulfide (FeCuS x , FCS) combining with near infrared (NIR) irradiation for maxillofacial trauma therapy. In cellular levels, FCS exhibited the most efficient antioxidant and anti-inflammatory, cell migration, and angiogenesis behaviors through down-regulating the inflammatory factor (iNOS) expression level, up-regulating anti-inflammatory factors (Arg-1 and TGF-β) expression level, inducing macrophages M2 polarization (CD86 down-regulation and CD206 up-regulation), and promoting tissue repair factors (CD31) expression level. Besides, it presented the excellent antibacterial ability of Escherichia coli (99.74%) and MRSA (99.78%) for FCS + NIR. Significantly, it also confirmed that FCS + NIR presented the excellent functions of inhibiting wound infections, decreasing wound inflammation, as well as promoting trauma tissue repairing. Altogether, it had developed the strategy of FCS + NIR for the synergistic enhancement of infectious maxillofacial trauma healing, could also serve as a promising therapeutic strategy for the treatment of infectious diseases. Bone substitutes are widely used in maxillofacial and oral surgeries. However, in clinical practice, bone substitutes with various forms, including separated particulates, powders, and blocks, have exhibited poor handling properties and space maintenance characteristics, resulting in long surgery procedures and unstable volume of the newly formed bone. Movable separated particulates with high stiffness have induced local inflammatory responses that hinder bone regeneration. The present study aimed to develop a new method to enhance the stability and operability of bone substitutes commonly used in dentistry by premixing with photocurable hydrogel GelMA. The GelMA-encapsulated particulate had a strong capacity to aggregate separated particulates and firmly attach to the host bone defect after photocuring compared to particulates alone. Additionally, macrophages at the surface of the GelMA-stabilized particulates tended to present a more M2-like phenotype than those at the surface of Bio-Oss®, leading to more MMR+ multinucleated giant cell formation and the induction of blood vessel invasion and new bone formation. In conclusion, this hydrogel-coated bone substitute strategy facilitates bone regeneration with increased operability, a stable volume of osteogenic space, and a favorable osteogenic microenvironment, indicating its potential value in the field of maxillofacial and oral surgeries when bone substitutes are needed.

Craniofacial bone defects, particularly those of substantial size, require a significant quantity of cells for effective regeneration. Although developmental engineering strategies using bone mesenchymal stem cells (BMSCs) aggregates have demonstrated potential, they do not fully replicate natural cellular processes. In contrast, periosteal mesenchymal stem cells (PMSCs) exhibit superior efficacy for craniofacial bone regeneration compared to BMSCs. This study aims to engineer periosteum-derived organoids (PDOs) by integrating PMSCs with osteogenic induction centers in 3D-printed polycaprolactone (PCL) scaffolds. Cell condensation was initiated with fibroblast growth factor 8b (FGF8b) and Vitamin C (Vc) to develop a high-density bioink possessing strong osteogenic differentiation capabilities, thereby mimicking osteoblast condensation integral to craniofacial bone repair. Composite scaffolds comprising PCL/β-TCP and naringin-loaded GelMA hydrogel bio-ink were fabricated, successfully forming calcification centers. Transcriptomic analysis revealed the molecular mechanisms underlying the formation and osteogenic differentiation of PDOs in vitro. This study presents a novel approach to biomimetic periosteum-based bone regeneration in vitro and highlights a promising strategy for scaffold fabrication incorporating calcified centers. These innovations contribute to the ongoing efforts to address the clinical challenge of repairing large craniofacial bone defects, offer valuable insights into the potential of 3D bioprinting for creating biomimetic bone structures, and explore the incorporation of traditional Chinese medicine compounds into bone tissue engineering.

The human mandible exhibits significant variation among individuals, necessitating customised restoration. This study proposes a stress distribution-driven modelling algorithm for gradient triply periodic minimal surface (TPMS) structures, specifically Gyroid-type double TPMS (GTDT) structures. The design process determines stress distribution in the macroscopic repair region through finite element method (FEM) mechanical analysis. In addition, designed structures were fabricated using laser powder bed fusion (LPBF). FEM analysis, mechanical testing, permeability experiments, and in vitro experiments were performed and identified an optimal Iso Value of 1.25 for bone regeneration. The results showed that the structures produced by the stress-driven design method had higher yield strength and more uniform stress distribution and double the strength in three-point bending tests, all at comparable relative density. Compared to the solid structure, the customized prostheses designed for maxillofacial repairs using this method exhibit stress distribution and displacement behaviour more closely aligned with native bone during mastication, thereby enhancing post-operative comfort.

Guided bone regeneration (GBR) stands as an essential modality for craniomaxillofacial bone defect repair, yet challenges like mechanical weakness, inappropriate degradability, limited bioactivity, and intricate manufacturing of GBR membranes hindered the clinical efficacy. Herein, we developed a Janus bacterial cellulose(BC)/MXene membrane through a facile vacuum filtration and etching strategy. This Janus membrane displayed an asymmetric bilayer structure with interfacial compatibility, where the dense layer impeded cell invasion and the porous layer maintained stable space for osteogenesis. Incorporating BC with Ti 3 C 2 Tx MXene significantly enhanced the mechanical robustness and flexibility of the material, enabling clinical operability and lasting GBR membrane supports. It also contributed to a suitable biodegradation rate, which aligned with the long-term bone repair period. After demonstrating the desirable biocompatibility, barrier role, and osteogenic capability in vitro , the membrane's regenerative potential was also confirmed in a rat cranial defect model. The excellent bone repair performance could be attributed to the osteogenic capability of MXene nanosheets, the morphological cues of the porous layer, as well as the long-lasting, stable regeneration space provided by the GBR membrane. Thus, our work presented a facile, robust, long-lasting, and biodegradable BC/MXene GBR membrane, offering a practical solution to craniomaxillofacial bone defect repair.

Repair of periodontal and maxillofacial bone defects is a major challenge in clinical. Guided bone regeneration (GBR) is considered one of the most effective methods. However, the efficacy of currently available GBR membranes for repair is frequently limited by their poor osteogenic potential and lack of antibacterial activity. The first step in this investigation was to construct a zinc-based zeolite-imidazolate framework loaded with copper ions ( [email protected] ). Following that, a novel polycaprolactone/polylactic acid/nano-hydroxyapatite/ [email protected] (PCL/PLA/n-HA/ [email protected] ) GBR membrane was developed using a simple porogen with nonsolvent-induced phase separation (NIPS) approach. The produced membrane with asymmetric porous structure (one smooth side and one rough side) possesses hydrophilicity corresponding to the roughness of its two sides. The superior mechanical property, stability of degradation, and ion release capability of the membrane all contribute to the clinical feasibility. Additionally, in vitro biological experiments demonstrated that the PCL/PLA/n-HA/ [email protected] membrane had favorable osteogenic and antibacterial properties, which suggests the high potential for application in the GBR procedure.

Purpose Maxillofacial infection is a common disease in stomatology and is difficult to treat owing to its high potential to spread to vital anatomical structures. Excessive levels of reactive oxygen species (ROS) in infected tissues lead to cellular damage and impede tissue regeneration. However, uncontrollable strategies to remove ROS have limited therapeutic efficacy. Nanoparticle systems for scavenging ROS and remodeling the inflammatory microenvironment offer much promise in the treatment of maxillofacial inflammation. Methods Here, a novel microenvironment-stimuli-responsive drug delivery nanoplatform (HMPB@Cur@PDA) based on a polydopamine (PDA)-functionalized hollow mesoporous Prussian blue (HMPB) nanozyme was developed for the delivery of curcumin (Cur) in the treatment of maxillofacial infection. Low pH and excess ROS in the inflammatory microenvironment cause degradation of the outer PDA layer of the nanocomplex, exposing the HMPB nanozyme and loaded Cur, which synergistically act as a ROS scavenger and anti-inflammatory agent, respectively, and induce macrophage polarization from the pro-inflammatory M1 to the anti-inflammatory M2 phenotype. Results Experiments in vitro provided strong evidence for the application of novel nanocomplexes in scavenging multiple ROS and inhibiting lipopolysaccharide-induced inflammation. In addition, in vivo results obtained using a mouse maxillofacial infection model demonstrated that HMPB@Cur@PDA had excellent biocompatibility, significantly attenuated the inflammatory response in periodontal tissue, and improved the repair of damaged tissue. Conclusion Our results indicate that HMPB@Cur@PDA nanocomposites have great potential for ROS regulation as well as having anti-inflammatory effects, providing new insights for the development of dual-response maxillofacial infection treatments.

Treating maxillofacial infections is hindered by severe inflammation and bacterial biofilms, which delay healing and often result in significant scarring. Photothermal therapy (PTT), which utilizes a photothermal agent (PTA) to generate localized heat, presents a promising approach that effectively eliminates pathogens while minimizing tissue damage, thereby enhancing treatment safety and efficacy. However, challenges such as low photothermal conversion efficiency and biocompatibility issues persist. In this study, we developed hemin chloride-modified organic small molecule-based nanoparticles (PTA@Hemin NPs). This material demonstrates exceptional photothermal effects with 81 % photothermal conversion efficiency and excellent biocompatibility. Under near-infrared (NIR) laser irradiation, the PTA@Hemin NPs generate strong photothermal effects, which synergize with the inherent antibacterial properties of hemin to significantly enhance the eradication of free bacteria and biofilms. Furthermore, PTA@Hemin NPs effectively scavenge reactive oxygen species (ROS) and alleviate inflammation by downregulating the expression of pro-inflammatory factors such as Cd86 and Il-1α, reducing M1 macrophage polarization. Both in vitro and in vivo experiments confirmed that PTA@Hemin NPs significantly enhance antibacterial and anti-inflammatory effects, while facilitating scarless healing of maxillofacial wounds in mice. These results highlight the potential of PTA@Hemin NPs as a highly effective strategy for treating bacterial infections, particularly those involving biofilms, in future clinical applications.

The clinical treatments of maxillofacial bone defects pose significant challenges due to complex microenvironments, including severe inflammation, high levels of reactive oxygen species (ROS), and potential bacterial infection. Herein, we propose the de novo design of an efficient, versatile, and precise electron-donable heterojunction with synergetic Ru-Cu pair sites (Ru-Cu/EDHJ) for superior biocatalytic regeneration of inflammatory mandible defects and pH-controlled antibacterial therapies. Our studies demonstrate that the unique structure of Ru-Cu/EDHJ enhances the electron density of Ru atoms and optimizes the binding strength of oxygen species, thus improving enzyme-like catalytic performance. Strikingly, this biocompatible Ru-Cu/EDHJ can efficiently switch between ROS scavenging in neutral media and ROS generation in acidic media, thus simultaneously exhibiting superior repair functions and bioadaptive antibacterial properties in treating mandible defects in male mice. We believe synthesizing such biocatalytic heterojunctions with exceptional enzyme-like capabilities will offer a promising pathway for engineering ROS biocatalytic materials to treat trauma, tumors, or infection-caused maxillofacial bone defects.

Obtaining a safe and reliable scaffold that can be rapidly fabricated and used for clinical bone defect repair has always been a challenge. In this study, polycaprolactone (PCL) composite scaffolds with various MXene (Ti 2 AlN) contents were prepared using 3D printing technology. The effects of different contents of Ti 2 AlN on the mechanical properties, hydrophilicity, cytocompatibility, and osteogenic differentiation ability were systematically studied and analysed. In vitro experiments showed that scaffolds containing 5% Ti 2 AlN ( [email protected] #Ti 2 AlN) obtained the best cell adhesion and proliferation ability and significantly upregulated the alkaline phosphatase (ALP) level. In vivo experiments of tibial defect repair in rats showed that the PCL scaffold containing 5% Ti 2 AlN ( [email protected] #Ti 2 AlN) could significantly promote the formation of new bone, and the experimental results of rabbit maxillofacial bone defect repair further proved that the [email protected] #Ti 2 AlN scaffold could effectively promote the repair of bone defects. Transcriptome analysis indicated that Ti 2 AlN may promote osteogenic differentiation by the Wnt/β-catenin signaling pathway and calcium-binding proteins. These findings suggest that Ti 2 AlN/PCL composite scaffolds with improved in situ bone repair ability represent an intelligent strategy for bone defect repair.

Hard tissue engineering scaffolds especially 3D printed scaffolds were considered an excellent strategy for craniomaxillofacial hard tissue regeneration, involving crania and facial bones and teeth. Porcine treated dentin matrix (pTDM) as xenogeneic extracellular matrix has the potential to promote the stem cell differentiation and mineralization as it contains plenty of bioactive factors similar with human-derived dentin tissue. However, its application might be impeded by the foreign body response induced by the damage-associated molecular patterns of pTDM, which would cause strong inflammation and hinder the regeneration. Ceria nanoparticles (CNPs) show a great promise at protecting tissue from oxidative stress and influence the macrophages polarization. Using 3D-bioprinting technology, we fabricated a xenogeneic hard tissue scaffold based on pTDM xenogeneic TDM-polycaprolactone (xTDM/PCL) and we modified the scaffolds by CNPs (xTDM/PCL/CNPs). Through series of in vitro verification, we found xTDM/PCL/CNPs scaffolds held promise at up-regulating the expression of osteogenesis and odontogenesis related genes including collagen type 1, Runt-related transcription factor 2 (RUNX2), bone morphogenetic protein-2, osteoprotegerin, alkaline phosphatase (ALP) and DMP1 and inducing macrophages to polarize to M2 phenotype. Regeneration of bone tissues was further evaluated in rats by conducting the models of mandibular and skull bone defects. The in vivo evaluation showed that xTDM/PCL/CNPs scaffolds could promote the bone tissue regeneration by up-regulating the expression of osteogenic genes involving ALP, RUNX2 and bone sialoprotein 2 and macrophage polarization into M2. Regeneration of teeth evaluated on beagles demonstrated that xTDM/PCL/CNPs scaffolds expedited the calcification inside the scaffolds and helped form periodontal ligament-like tissues surrounding the scaffolds.
