Repositioning and securing the cranial bone flap after craniotomy remain significant challenges in neurosurgery. Traditional fixation methods often suffer from weak mechanical strength, bioinertness, limited osteogenic capacity, and a lack of antibacterial properties, complicating clinical outcomes. Recent medical adhesives offer superior fixation but face significant limitations in cranial bone applications. In this study, we explored the application of PAH (Poly (allylamine) hydrochloride)-TPP (Tripolyphosphate) coacervate (PT) as a bone adhesive. The PT coacervate demonstrated excellent anti-swelling (anti-swelling ratio less than 1 %), self-healing, and injectable properties, as well as exceptional shape adaptability and cytocompatibility. Adhesion tests revealed its outstanding adhesion (99.06 ± 11.76 kPa for lap shear and 121.42 ± 16.73 kPa for end to end), long-term durability, and tunable adhesion strength. Furthermore, the coacervate demonstrated broad-spectrum antibacterial activity against both Gram-positive and Gram-negative bacteria (antibacterial rate more than 90 %), with mechanistic studies revealing promising strategies to address localized and systemic drug-resistant infections. Additionally, the coacervate's self-mineralizing properties significantly enhanced its osteogenic performance. In vivo studies confirmed its effective fixation, robust antibacterial activity, and improved osteogenesis, underscoring its suitability for cranial bone flap repositioning and fixation after craniotomy. In summary, this coacervate adhesive offers a promising therapeutic solution for addressing the challenges of cranial flap fixation in neurosurgery.

Bone glue with robust adhesion is crucial for treating complicated bone fractures, but it remains a formidable challenge to develop a “true” bone glue with high adhesion strength, degradability, bioactivity, and satisfactory operation time in clinical scenarios. Herein, inspired by the hydroxyapatite and collagen matrix composition of natural bone, we constructed a nanohydroxyapatite (nHAP) reinforced osteogenic backbone-degradable superglue (O-BDSG) by in situ radical ring-opening polymerization. nHAP significantly enhances adhesive cohesion by synergistically acting as noncovalent connectors between polymer chains and increasing the molecular weight of the polymer matrix. Moreover, nHAP endows the glue with bioactivity to promote osteogenesis. The as-prepared glue presented a 9.79 MPa flexural adhesion strength for bone, 4.7 times that without nHAP, and significantly surpassed commercial cyanoacrylate (0.64 MPa). O-BDSG exhibited degradability with 51% mass loss after 6 months of implantation. In vivo critical defect and tibia fracture models demonstrated the promoted osteogenesis of the O-BDSG, with a regenerated bone volume of 75% and mechanical function restoration to 94% of the native tibia after 8 weeks. The glue can be flexibly adapted to clinical scenarios with a curing time window of about 3 min. This work shows promising prospects for clinical application in orthopedic surgery and may inspire the design and development of bone adhesives.

The unique properties of self-healing materials hold great potential in the field of biomedical engineering. Although previous studies have focused on the design and synthesis of self-healing materials, their application in in vivo settings remains limited. Here, we design a series of biodegradable and biocompatible self-healing elastomers (SHEs) with tunable mechanical properties, and apply them to various disease models in vivo, in order to test their reparative potential in multiple tissues and at physiological conditions. We validate the effectiveness of SHEs as promising therapies for aortic aneurysm, nerve coaptation and bone immobilization in three animal models. The data presented here support the translation potential of SHEs in diverse settings, and pave the way for the development of self-healing materials in clinical contexts.

Achilles tendon sleeve avulsions often involve tissue defects. Standard bone tunnel fixation risks tunnel enlargement, inflammation, and graft failure. Native tendon and bone tissue exhibit inherent bioelectrical properties whose disruption impedes healing. To address these, the study developed electroactive, anti-inflammatory sutures featuring a strong polylactic acid (PLA) micron yarn core and a conductive poly(3,4-ethylenedioxythiophene)/hydroxyethyl cellulose/glycerol/chondroitin sulfate/PLA (PEDOT/HEC/glycerol/CS/PLA) nanofibrous shell. A novel synthesis strategy enabled EDOT polymerization templated by HEC, eliminating the need for poly(styrene sulfonate) (PSS), oxidants, or catalysts and allowing direct electrospinning. In vitro, it can regulate macrophage polarization and enhance tenocytes and bone marrow mesenchymal stem cell (BMSCs) proliferation and differentiation. In a rabbit Achilles tendon avulsion model, the suture enables secure tendon-to-bone fixation via calcaneal bone tunnels. At 12 weeks post-operation, the suture exerts anti-inflammatory effects by modulating CD206/CD86 expression. Electroactive suture significantly enhances tendon regeneration and bone formation within the tunnels, and achieves robust tendon-to-bone integration at the interface, markedly improving fixation stability. In summary, this electroactive, anti-inflammatory suture effectively promotes tendon-bone regeneration for repairing Achilles tendon sleeve avulsions.
