Self-Growing Scaffold for Spatial and Border Expansive Regeneration

Liheng Gao,Xinyu Song,Lingxi Meng,Junyi Wang,Hongming Zhang,Xinquan Jiang,Wenjie Zhang

Tissue regeneration and repair techniques approaching personalized treatment are devoted to fabricating high-precision scaffolds that accurately match the size of the defect. However, scaffolds are difficult to implant in situ for obsolete defects with loss of original space, and the size is limited by confined boundary tissue. In nature, the development of fetuses, organs, and even plants all experience a matched growth in volume and border. Inspired by that, this study proposes a space-expanding regeneration model with a self-growing (SG) scaffold, which is then used in refractory alveolar ridge vertical bone augmentation. The SG scaffold contains a multistage hydrophilic polymer network. The initial size can be eliminated for minimally invasive implantation, and gradually increased by orderly absorption of tissue fluid, achieving controlled growth in vivo. The shearing force of the SG scaffold suppresses tissue hematoma and stimulates extracellular matrix remodeling. In addition, macrophages polarize toward M2 and secrete transforming growth factor-β1. Meanwhile, bone regeneration is induced within the expanded space, achieving a ≈5-fold vertical increase of the rat skull, and supporting a 6-mm-long titanium implant. The SG scaffold provides a spatial and border extension model for obsolete injuries.

Enhanced efficacy of 3D architectural glass–ceramic scaffolds in vertical bone augmentation compared with conventional bone-block grafts of different origins

Wenjie Wang,Lingling Zheng,Wei Liu,Chunjuan Wang,Dan Chen,Liya Ai,Antonio Apicella,Chao Wang,Yubo Fan

To address the challenge of vertical bone augmentation in orofacial surgery and implant dentistry, this study focused on additively manufactured 3D architectural glass–ceramic scaffolds (3DP). The 3DP, which mimics the Haversian canal and trabecular porous structure, was innovatively designed and fabricated using Digital Laser Processing (DLP) additive manufacturing. The morphology, microstructure, elemental composition and mechanical characteristics of 3DP were compared with those of bovine Xenogeneic Bone Block (XBB), human Allogeneic Bone Block (ABH), and rabbit Autogenous Bone Block (ABR). Furthermore, the vertical osteogenic capacity using a rabbit calvarium model were compared among them. Micro-CT and histology analyses revealed that the osteogenic height, volume, and area of 3DP and XBB were superior to those of ABH and ABR, and 3DP demonstrated a more rapid vertical osteogenesis compared to XBB. The excellent vertical osteogenic performance of 3DP should be attributed to the ideal pore size of its microporous structure, the spatial maintaining force of its overall architecture, and its microcrystalline chemical composition after sintering. Moreover, the vertical channels in the porous network seemed to promote the rapid vertical growth of bone tissue at the early stage.

3D-printed pre-tapped-hole scaffolds facilitate one-step surgery of predictable alveolar bone augmentation and simultaneous dental implantation

Cuicui Zhang,Zhigang Chen,Juan Liu,Mingming Wu,Jirong Yang,Yaomin Zhu,William Weijia Lu,Changshun Ruan

Despite the convenience of one-step surgery for alveolar bone augmentation and simultaneous dental implantation, achieving a suitable bone graft for predictable alveolar bone augmentation and stable dental implant placement via one-step surgery remains a huge challenge. Herein, a novel personalized scaffold with a pre-tapped-hole that could perfectly fit the dental implant was precisely fabricated by 3D printing technology. The biocompatible organic poly(lactic- co -glycolic acid) (PLGA) and osteoconductive inorganic materials (hydroxyapatite (HA) and β-tricalcium phosphate (β-TCP)) were used as composite materials with an appropriate viscosity for pre-tapped-hole scaffold fabrication (PLGA/HA/β-TCP, PHT). The PHT composite scaffold demonstrated a well-designed structure that could match perfectly with bone defect areas and the titanium screw, and proper mechanical property as high as 67.18 ± 7.40 MPa in Young's modulus and 4.85 ± 0.39 MPa in compression stress. Meanwhile, PHT scaffold exhibited excellent in vitro cellular biocompatibility and proper in vivo osteoconductivity in a rabbit bone augmentation model. In addition, results of micro-CT and histological analysis further confirmed that PHT scaffold was able to obtain adequate primary stability provided by the titanium screw, and offer adequate space maintenance ability for new bone formation. Therefore, this study provided a robust and effective strategy of precise 3D printing technology for one-step surgery of predictable alveolar bone augmentation and simultaneous dental implantation.

A tough injectable self-setting cement-based hydrogel for noninvasive bone augmentation

Peng Jin,Mingjie Xia,Masoud Hasany,Pan Feng,Jing Bai,Jian Gao,Wei Zhang,Mehdi Mehrali,Ruixing Wang

Composite hydrogels with excellent properties can open new opportunities to terminate the need for auto/allografts in bone augmentations. However, their clinical application has been limited by their insufficient mechanical strength and lack of osteoinductivity. Here we report a new strategy to design an injectable bioactive double network hydrogel reinforced by inorganic calcium/magnesium phosphate cement (CMPC) hydrates to meet the mechanical performance requirements for bone regeneration. The engineered CMPC hydration endows the composite hydrogel with an appropriate gelation time and temperature for injection, which shows no harm in the defect site. CMPC hydrates could also provide a lower swelling ratio and higher biodegradation rate fitting the in vivo bone regeneration needs. In vitro and in vivo experiments prove that the ions released from inorganic particles endow biocompatibility, cell migration, adhesion, differentiation, and significantly higher bone regeneration capacity. Taken together, the simple addition of CMPC particles imparts in-demand features that bring us closer to the clinical utilization of hydrogel-based materials for bone regeneration.

Chopped fibers and nano-hydroxyapatite enhanced silk fibroin porous hybrid scaffolds for bone augmentation

Shue Jin,Xiaoxue Fu,Weinan Zeng,Anjing Chen,Zhenyu Luo,Yubao Li,Zongke Zhou,Jidong Li

Chopped fiber (CF)- and nano-hydroxyapatite (n-HA)-enhanced silk fibroin (SF) porous hybrid scaffolds (SHCF) were prepared by freeze-drying for bone augmentation. Compared with pristine SF scaffolds, the incorporation of CF and n-HA can significantly enhance the mechanical properties of the composite scaffold. The results of cell experiments and mouse subcutaneous implantation indicated that the SHCF could alleviate foreign body reactions (FBR) led by macrophages and neutrophils, promote the polarization of RAW264.7 cells to anti-inflammatory M2 macrophages, and inhibit the secretion of pro-inflammatory cytokine TNF-α. A rat femoral defect repair model and bulk-RNA-seq analysis indicated that the CF- and n-HA-enhanced SHCF promoted the proliferation and osteogenic differentiation of bone mesenchymal stem cells (BMSCs) by the upregulation of Capns1 expression and regulated the calcium signaling pathway to mediate osteogenesis-related cell behavior, subsequently promoting bone regeneration.
