Diatoms, the typical marine algae with autotrophic oxygen generation and siliceous frustules, are anticipated to address the current obstacle of oxygen deprivation, cellular dysfunction, and repair imperfection in most intricate damaged tissues. Here, we are motivated to apply the Chaetoceros species , an ancient diatom, to an engineered cross-species domesticator for challenging tendon-to-bone injuries. This construct augmented the osteotendinous differentiation of tendon stem/progenitor cells, stamped by the silicon ions released from frustules and the up-regulated oxygen through photosynthetic behavior. The biocompatibility was at the forefront without adverse effects on rat subcutaneous models. The construct promoted hypoxia alleviation and locomotion recovery of rotator-cuff-torn (RCT) rat models. Pre-differentiated constructs induced by sacrificial diatoms displayed superb interface maturation in RCT rabbit models. The photosynthesis and inorganic ion interactive regeneration within a cross-species construct permits the creation of such a diatom-derived artificial domesticator, promising a paradigm shift towards the accomplishment of state-of-the-art regeneration comparable to natural tissues.

Accumulation of senescent cells in the elderly impairs bone homeostasis. It is important to alleviate cell senescence and scavenge excessive oxidative stress for enhanced bone fracture healing in elderly patients. In this study, resveratrol (RSV), an antioxidant drug, was encapsulated in a biocompatible zeolitic imidazolate framework-8 (ZIF-8) nanoparticle to protect it from oxidation and improve its bioavailability. Cells responsible for bone healing, including osteoblasts, bone marrow-derived mesenchymal stem cells (BMSCs), macrophages, and endothelial cells, were used to evaluate the regulatory role of the nanoformulation in the alleviation of cellular senescence and promotion of cell functions. It was proved that the nanoformulation sustainably released RSV with well-preserved bioactivity and improved bioavailability. Cell experiments confirmed that ZIF-8/RSV was capable of alleviating the senescence of cells [human osteoblasts (HOBs), BMSCs, H2O2-induced senescent vascular endothelial cells (HUVECs)] and scavenging excessive intracellular reactive oxygen species (ROS). Excitingly, the ZIF-8/RSV improved the osteogenic ability of senescent osteoblasts and promoted macrophage M2 polarization. In addition, the ZIF-8/RSV also enhanced the angiogenic function of senescent HUVECs. More importantly, the ZIF-8/RSV nanoformulation outperformed the REV alone, indicating the critical role of encapsulation using ZIF-8. These findings suggest that the ZIF-8/RSV nanoformulation exhibits potential for bone fracture treatment in elderly patients.

Bone tissue engineering is a critical area of research focused on enhancing the regeneration of bone tissue, particularly in cases of complex defects. Despite inherent self-healing capabilities of bone, irregularly-shaped defects pose significant challenges for complete regeneration, thereby necessitating innovative therapeutic strategies. This study addresses these challenges by exploring the development of advanced tissue regeneration scaffolds. Here, tricalcium phosphate (TCP) is integrated with short silica (SiO 2 ) fibers to develop 3D cryogel scaffolds, designated as SSFx@TCP. These cryogel scaffolds exhibit low density (<2 mg cm − 3 ), high water absorption (>3500%), and favorable sustained release properties, enabling effective cellular interactions. Notably, the SSFx@TCP cryogels support cell attachment, proliferation, and differentiation, while also regulate gene expression associated with angiogenesis and osteogenesis. Furthermore, in vivo assays demonstrated that these scaffolds can effectively promote de novo bone production in a rat calvarial defect model 8 weeks post-operatively, thereby indicating their potential to mimic the natural extracellular matrix. The successful integration of bioactive components in these cryogels may be beneficial for improved clinical outcomes in bone regeneration therapies and ultimately enhancing patient care in reconstructive surgery.

Amorphous calcium phosphate (CaP) is the precursor of minerals in bone, and this precursor phase has been suggested to exist in the form of nanoclusters . In vitro experiments have demonstrated a strong ability of these CaP nanoclusters in mineralizing collagen matrix, suggesting their potential applications in bone fracture healing. It is however difficult to obtain stable CaP nanoclusters and control their release to bone fracture sites in vivo . In this work, we designed an organic-inorganic polyvinyl alcohol-arginine-calcium phosphate (PVA/Arg/CaP) biofiber. The as-prepared biofiber displays good mechanical performance and can continuously release CaP nanoclusters to mouse tibial fracture sites. These released CaP nanoclusters display good in vivo stability and can be directly utilized for collagen mineralization and bone regeneration, thus accelerating the healing process. The bone healing ability is confirmed by in vivo experiments, and the regenerated bones possess satisfactory mineral content and mechanical performance comparable with that of normal ones. The discovery not displays a promising avenue of bone fracture healing, but also provides insight into the bone mineralization process.

Diabetes-induced oxidative stress can lead to poor bone defect healing, severely affecting the quality of life for patients. Studies show that improving the microenvironment and promoting bone formation can effectively accelerate the healing of bone defects. However, traditional local drug delivery methods face various challenges during the treatment process. Therefore, this study develops a novel hydrogel (HLBP) loaded with natural protein polysaccharides (LBP) extracted from goji berries, aiming to enhance the healing of diabetic bone defects. The hydrogel is composed of freeze-dried polyvinyl alcohol (PVA) and photocrosslinked poly (ethylene glycol) diacrylate (PEGDA). This hydrogel exhibits excellent biocompatibility. Additionally, it demonstrates effective loading capacity for the LBP. LBP's bioactivity enables ROS scavenging and promotes bone regeneration at defect sites. In vitro, experimental results show that HLBP significantly reduces ROS levels and enhances osteogenic differentiation ability and cell viability of human bone marrow mesenchymal stem cells. In vivo studies using BKS-db diabetic mice show that HLBP implantation at bone defects achieves over 80% healing, highlighting its strong healing potential. This method effectively avoids potential toxicity from systemic drug administration and significantly promotes regeneration at the bone defect site, providing a new strategy for treating diabetic bone defects.

Piezoelectric composites have shown great potential in constructing electrical microenvironment for bone healing since their integration of polymer flexibility and ceramic piezoelectric coefficient. Herein, core-shell structured BaTiO 3 @carbon (BT@C) hybrid nanoparticles were prepared by in situ oxidative self-polymerization and template carbonization. Then the BT@C was introduced into polyvinylidene fluoride (PVDF) scaffolds manufactured by selective laser sintering. On one hand, the carbon shell could strengthen the local electric field loaded on BT in poling process owing to it served as a diffusion layer to provide space for charge transfer and accumulation. In this case, more electric domain within BT would be aligned along the polarization field direction and thus promoted the paly of BT's piezoelectric activity. On the other hand, the carbon shell could induce the formation of β phase due to the sp 2 hybrid-bonded carbon atoms in carbon shell forming electrostatic interaction with hydrogen atoms in PVDF chains, which further enhanced the piezoelectric response of the scaffolds. Results showed that the scaffold presented augmented piezoelectric performance with output voltage of 5.7 V and current of 79.8 nA. The improved electrical signals effectively accelerated cell proliferation and differentiation. Furthermore, the scaffold displayed improved mechanical performance due to rigid particle strengthen effect.

Rotator cuff injuries often result in chronic pain and functional limitations due to retears and scar formation at the enthesis. This study assess the efficacy of electrohydrodynamic-printed microfibrous dual-triphase scaffolds (DTSs), designed to optimize enthesis repair. These scaffolds, composed of polycaprolactone enhanced with nanohydroxyapatite, nano-magnesium-oxide, and kartogenin demonstrate significant biological advantages. In vitro, the scaffolds support over 95% stem cell viability and promote enhanced expression of critical markers such as tenomodulin (TNMD), sex-determining region Y-Box transcription factor 9 (SOX-9), and runt-related transcription factor 2 (RUNX-2). Enhanced expressions of tendon markers tenomodulin and scleraxis (SCX) are noted, alongside significant upregulation of chondrocyte and osteoblast markers. In vivo, these scaffolds significantly improve the biomechanical properties of the repaired enthesis, with a maximum failure load of 27.0 ± 4.2 N and ultimate stress of 5.5 ± 1.0 MPa at 6 weeks postimplantation. Lipidomic analysis indicates substantial regulation of phospholipids such as phosphatidylcholine and phosphatidylserine, highlighting the scaffold's capacity to modulate biochemical pathways critical for tissue repair and regeneration. This study underscores the potential of DTS to improve clinical outcomes in rotator cuff injury treatment by enhancing cellular differentiation, biomechanical properties, and biochemical environment, setting a foundation for personalized treatment strategies in tendon–bone repair.

The macrophage-dominated bone immune response plays an important role in osteogenesis of bone defects. Generally, M2 macrophages are beneficial to promote osteogenesis. However, recent studies have confirmed that M1 also plays an important role in early angiogenesis, osteointegration. Therefore, achieving sequential polarization of macrophages from M1 to M2 may be more beneficial for osteogenesis. In this study, a new type of bone immunomodulatory IFN-γ/Sr-dropped bioactive glass composite scaffold (IFN-γ/SrBG) was successfully prepared which integrates multiple biological functions at different stages of the bone healing process. The scaffold can polarize macrophages into pro-inflammatory M1 type at the early stage of implantation by releasing IFN-γ within the first day, and then polarize macrophages into anti-inflammatory M2 type at a later stage by releasing Sr2+ from SrBG, which promoted mature bone formation in bone defects to a greater extent. Therefore, IFN-γ/SrBG scaffolds are expected to become excellent bone tissue engineering materials by sequential regulation of macrophage polarization.

The aging population is closely associated with chronic metabolic diseases, including diabetes, hypertension, and other metabolic disorders. Metabolic dysfunction is a primary characteristic of diabetic patients, which can impair cellular response to insulin and lead to increased production of reactive oxygen species (ROS), hindering bone healing. Therefore, the development of multifunctional titanium (Ti) implants with the ability to eliminate reactive oxygen and promote bone formation is the optimal strategy to address this issue. In this study, we designed a thioctic acid(Ta) coating loaded with strontium(Sr), forming a strong bond with the Ti implant (Ti-Ta@Sr). During treatment, sulfuric acid demonstrated the ability to release monomers in the early stages, effectively eliminating excess reactive oxygen produced in high-glucose environments and regulating the diabetic microenvironment. The delayed in-situ release of strontium ions also enhanced osteogenic differentiation efficiency. Both in vivo and in vitro experiments confirmed that Ti-Ta@Sr significantly restored the osteogenic capacity and proliferation of osteoblasts under high-glucose conditions. These advantages make Ti-Ta@Sr a promising candidate material for orthopedic implants, suitable for repairing bone defects in diabetic patients.

The management of open bone defects poses a formidable challenge in clinical practice, primarily due to issues such as profuse bleeding, inflammation, bacterial infections, and compromised bone fracture healing. To tackle these complexities, we have devised a novel hemostatic powder, namely the compound strontium-based chitosan quaternary ammonium salt hemostatic powder (QCS@SrT-TA). This innovative powder leverages the self-assembly of tripolyphosphate and strontium ions, facilitated by a positively charged core cross-linker. Furthermore, its surface has been strategically modified with polyphenol and positively charged macromolecules, imparting unique properties. The mesoporous architecture of QCS@SrT-TA facilitates rapid moisture absorption, enhancing its efficacy. In preclinical studies using rat tail artery amputation and liver bleeding models, QCS@SrT-TA exhibited remarkable performance, significantly reducing both bleeding time and volume (62.39 ± 2.89 mg, 35.33 ± 4.16 s in tail amputation, 63.7 ± 5.19 mg, 62.33 ± 9.61 s in liver bleeding). Notably, the positively charged strontium ions and quaternary ammonium salts within the powder were effective in removing bacteria, minimizing the risk of infection. Beyond hemostasis, QCS@SrT-TA demonstrates additional therapeutic benefits. It polarizes M2 macrophage phenotypes and promotes angiogenesis within bone defect sites, accelerating bone healing processes. Ultimately, a substantially enhanced and notable functional recovery is achieved. In summary, the rapid hemostatic, potent antibacterial, anti-inflammatory, and angiogenesis-promoting characteristics of QCS@SrT-TA hold immense promise as a groundbreaking clinical treatment strategy for open bone defects.

Background. Tendon-bone healing is a reconstructive procedure which requires a tendon graft healing to a bone tunnel or to the surface of bone after the junction injury between tendon, ligament, and bone. The surgical reattachment of tendon to bone often fails due to regeneration failure of the specialized tendon-bone junction. Materials and Methods. An extra-articular tendon-bone healing rat model was established to discuss the effect of the baicalein 10 mg/(kg·d) in accelerating tendon-bone healing progress. Also, tendon-derived stem cells (TDSCs) were treated with various concentrations of baicalein or dickkopf-1 (DKK-1) to stimulate differentiation for 14 days. Results. In vivo, tendon-bone healing strength of experiment group was obviously stronger than the control group in 3 weeks as well as in 6 weeks. And there were more mature fibroblasts, more Sharpey fibers, and larger new bone formation area treated intragastrically with baicalein compared with rats that were treated with vehicle for 3 weeks and 6 weeks. In vitro, after induction for 14 days, the expressions of osteoblast differentiation markers, that is, alkaline phosphatase (ALP), runt-related transcription factor 2 (Runx2), osteocalcin (OCN), osterix (OSX), and collagen I, were upregulated and Wnt/β-catenin signaling pathway was enhanced in TDSCs. The effect of DKK-1 significantly reduced the effect of baicalein on the osteogenic differentiation. Conclusion. These data suggest that baicalein may stimulate TDSCs osteogenic differentiation via activation of Wnt/β-catenin signaling pathway to accelerate tendon-bone healing.

Background:The common fresh-frozen allografts that are used for anterior cruciate ligament (ACL) reconstructions behave slower during the remodeling process and produce weaker tendon-bone integrations than do autografts. Decellularization of allogenic tendons results in a clean and porous collagen scaffold with low antigenicity and high compatibility, which may be more suitable for ACL reconstructions.Hypothesis:Allograft decellularization will result in a tissue structure with suitable mechanical characteristics for ACL reconstruction, thereby promoting graft remodeling and enhancing tendon-bone healing.Study Design:Controlled laboratory study. Methods:Decellularized allograft tissues were prepared with a pH-modified decellularization process and evaluated for their biocompatibility and biomechanical character in vitro. Eighty New Zealand White rabbits were divided into 2 groups, with 40 in each group, to receive ACL reconstruction with either fresh-frozen (common) allografts or decellularized allografts on both knees. At 2, 4, 8, and 12 weeks postoperatively, the rabbits were euthanized for biomechanical testing, micro–computed tomography analysis, and histologic analysis. Results:The pH-modified decellularized allograft tissues kept excellent biocompatibility and biomechanical character during the in vitro study. Biomechanical testing indicated that the decellularized allograft had significantly higher ultimate load (P = .02) and stiffness (P = .01) levels than the common allograft at 12 weeks, and there was no significant difference between the 2 groups at any other time point. The micro-CT evaluation determined significantly higher bone mineral density (P < .01) in the decellularized allograft group than that in the common allograft group at 12 weeks, but no difference between the 2 groups was observed at any other time point. Regarding bone volume/total volume, there was no difference between the 2 groups at any time point. Fibroblast ingrowths, vascular formation, and connective tissue formation in the tendon-bone interface were better in the decellularized group within 8 weeks. New bone formation was more common in the decellularized allograft group. The collagen birefringence was restored more quickly in the decellularized allograft group than in the common allograft group at all time points. Conclusion:The use of pH-modified decellularized allografts compared with the common allografts resulted in better cellularity, vascularity, collagen matrix remolding, new bone formation around the graft, enhanced tendon-bone healing, and higher ultimate failure load and stiffness of the graft after ACL reconstruction in the rabbit model.Clinical Relevance:The pH-modified decellularized allograft may be a better graft option than the common fresh-frozen allograft for knee ligament reconstructions.
